The Comparative Study on the Effect of Trigger Point Treatment, Self-Exercise Treatment, Trigger Point - Self-Exercise Cooperative Treatment for Neck Pain Caused by Traffic Accidents (교통사고 후 경항통 발생환자 중 Trigger Point 치료군, 자가운동 치료군, Trigger Point 및 자가운동 병행 치료군 비교연구)

  • Objectives This study was aimed at comparing clinical effectiveness among trigger point treatment, Self-exercise treatment, trigger point - self-exercise cooperative treatment for neck pain caused by traffic accidents. Methods 63 patients with neck pain by traffic accidents were divided into 3 groups. Different types of treatments were carried out for each groups ; group A with trigger point, group B with self-exercise, group C with trigger point and self-exercise cooperative treatment. 3 groups were also treated with general acupuncture and herb-medicine treatment. Pain threshold, visual analogue scale (VAS), and neck disability index (NDI) were checked on a daily basis. SPSS ver. 18.0 for Windows was used for analysis of data. The effectiveness of treatment of each groups were verified by using paired test and one-way repeated-measures ANOVA test, and the comparison of VAS, NDI and pain threshold were demonstrated by independent samples t-test. Results 3 groups all showed significant difference in pain threshold, VAS and NDI after 8 days later. Group C showed more improvement in pain threshold, VAS and NDI than shown in group A and B. In the first period (from pre-treatment to 4th day treatment) group C showed significant difference in NDI and pain threshold but not in VAS as compared to group A and B. In the second period (from 4th treatment to 8th day treatment), Group C showed significant difference in pain threshold, VAS and NDI as compared to group A and B. Conclusions Trigger point - self-exercise cooperative treatment is proved to be more helpful to improve the unstability and reduce neck pain than trigger point treatment or self-exercise treatment only, therefore eventually leading to better satisfaction for patients with neck pain.

The Clinical Study on the Effect of Acupotomy on Chronic Arthritis of Ankle (침도침 시술을 시행한 만성 족관절 염좌 환자 2례에 대한 증례보고)

  • Objectives : The purpose of this study is to report the effect of acupotomy for patients with chronic arthritis of ankle. Methods : We treated Two patients who have chronic arthritis of ankle with acupotomy. To check the effect and satisfaction of acupotomy we used visual analog scale(VAS) and Five-point Likert scale. At the same time, we measured Change of ankle range of motion(ROM). Results : As a result of evaluation by using VAS, after treatment VAS score decreased(VAS3, VAS5). ROM of ankle widened. And the Likert scale point were checked 3 and 4. Conclusions : This study shows acupotomy has useful effect on chronic arthritis of ankle. An acupotomy is an alternative effective treatment modality for chronic arthritis of ankle.

  • Objectives : The Purpose of this report is to examine the effect of Remote Acupuncture Point Needling in the Low Back Pain patient of Acute Stage. Methods : From October 1st 2005 to May 31th 2006, 30 cases of low back pain patients at acute stage were divided into 2 groups; one group(control group) took near acupuncture point needling, and the other group(test group) took near acupuncture point needling with remote acupuncture point needling. For evaluating change of pain, Visual Analog Scale(VAS) was checked before and after Treatment. Results : Both acupuncture therapy showed good effect on low back pain of acute stage. And test group showed better effect on decreasing pain than control group. It was proved by the difference between VAS checked before treatment and what checked after treatment. But it was not significant statistically. Conclusion : Near acupuncture point needling and remote acupuncture point needling can be recommended as a useful therapy to treat low back pain of acute stage.

  • Purpose ; Iension-type headache is the most common headache. The effect of acupuncture for headache is already known, but the methods and technique of acupuncture is varied. Choice of acupuncture points is mostly relied on experience. Moreover, objective evaluation of acupuncture effects is lacking. Objective ; The objective of this study is to assess the difference of effects between dry needling on trigger point and dry needling on remote acupuncture point. The patients who were diagnosed as tension-type headache and administrated herb-medication were divided into two groups: Trigger point dry needling group and remote acupuncture point dry needling group, matching in sex, age. The degree of pain was measured by visual analogue scale(VAS) two times, before and after dry needling. Conclusion ; VAS showed significant decrement after dry needling on tension-type headache in both groups, but there was no statistically significant difference between two groups according to sex, age and duration. In addition, it is not statistically approved that there exists the difference of VAS between two groups about the treatment effects.

  • Objectives : The purpose of this study is to report the effect of acupotomy for patients with Lumbar intervertebral disc Herniation. Methods : We treated Four patients who have HIVD of L-spine with acupotomy. Visual analog scores(VAS) and Oswestry Low-back pain Disabiliby Index(ODI) were compared before-treatment with after-tretment. When the patients is discharged, the satisfaction of acupotomy were graded by Five-point Likert scale. Results : VAS and ODI were decreased at all case. Likert scale point were cheked as grade 4 at all case. Conclusions : This study shows acupotomy has useful effect on HIVD of L-spine.

Comparison of Visual Analogue Scale, Categorical Scale and Satisfaction for Postoperative Pain (수술 후 통증 평가를 위한 Visual Analogue Scale, Categorical Scale 그리고 환자 만족도와의 비교)

  • Background: The categorical scales and visual analogue scales (VAS) are methods used for evaluating variations of postoperative pain intensity. Several studies have introduced the idea that there is a clear correlation between visual scales and categorical scales. However, when VAS is the only pain measure in the study, we do not know what point on the VAS represents a category on the categorical scale and their degree of correlation with satisfaction for postoperative pain. Methods: 252 patients who had undergone elective surgery were studied. A 5-point categorical scale (none, mild, moderate, severe, worst possible pain), a 0-100 mm VAS (no pain to worst possible pain) and patient satisfaction score were checked 24 hours after surgery using a pain questionnaire and VAS tool. Results: The mean VAS score of the 14 patients reporting 'no-pain' was $1.9{\pm}0.9$, $23.9{\pm}1.0$ for the 132 patients reporting 'mild-pain', $47.2{\pm}1.1$ for the 82 patients reporting 'moderate-pain' and $67.5{\pm}2.8$ for the 24 patients reporting 'severe-pain'. Of the patients reporting moderate pain, 85% scored over 45.6 mm on the corresponding VAS, with a mean score 47.2 mm. The mean satisfaction scores were $90.6{\pm}2.7$ for the 'no pain', patients, $75.1{\pm}1.3$ for ‘mild pain', $58.3{\pm}1.5$ for 'moderate pain', and $55.1{\pm}4.0$ for 'severe pain' patients. The categorical scale was significantly correlated with VAS (P < 0.01). The satisfaction score was significantly inversely correlated with VAS (P < 0.01). Conclusions: Our results indicate that if a patient records a VAS score in excess of 45.6 mm they would probably have recorded at least moderate pain on a 5-point categorical scale. The categorical scale can be used properly for postoperative pain measurement with VAS. More research is required for the development of suitable pain descriptor for a categorical scale and pain questionnaire in Korean.

  • Objective: The objective of this study is to observe the effect of anti-inflammatory herbal acupuncture on the Complex Regional Pain Syndrome Methods : Anti-inflammatory herbal acupuncture, A-Shi Point, Sa-am acupunture were used to treat shoulder pain & ROM(range of movement)disorder, chest pain, finger's causalgic pain. We evaluated the patient through VAS(Visual Analog Scale) daily and Physical Examinations Results & Conclusions : After 42days of treatment, shoulder pain was decreased from VAS10 to VAS3, chest pain was decreased from VAS10 to VAS1.5, finger's causalgic pain was decreased from VAS10 to VAS2.5. and the patient showed that the ROM(range of movement) of shoulder was better, oriental treatment is good method for pain relief and better movement.

  • Objective : The purpose of this study was to investigate the effects of acupuncture treatment on frequently adopted acupuncture point and self exercise therapy for lumbar sprain patient Methods : For this study, a retrospective chart review was conducted. Data are derived from lumbar sprain patients who have received hospital treatment in Daejeon Univ. Cheon-Ahn Korean Hospital. All patients were treated with acupuncture treatment on high frequency acupuncture point (BL25, BL23, BL24, GB30, BL40, BL26, BL60, GB34), physical therapy and herbal medicine and self exercise therapy(Swiss ball exercise, Lumbar stabilization exercise.) We measured visual analogue scale(VAS), Oswestry disability index(ODI), Pain threshold twice(pre-treatment and post-treatment) on 1st day and 5 days later. Results : After being treated, lumbar sprain patients were meaningful improved in VAS, ODI, pain threshold The VAS score, ODI score were statistically decreased after treatment and the pain threshold was statistically increased after treatment. Conclusion : In this study, treatment on frequently adopted acupuncture point and self exercise therapy were effective on lumbar sprain patient.

  • Objectives : The purpose of this research was to compare remote and near acupuncture point needling with near acupuncture point needling acupuncture on posterior neck pain treatment. Methods : We divided 36 patients with posterior neck pain patient into two groups and treated Group A with near acupuncture and remote acupuncture point needling and Group B with near acupuncture point needling. The efficacy of treatment was measured by VAS scores and NDI scores of before and after 5 days to start treatment. Results : Both group A and B represented effective improvement in VAS and NDI of neck pain. And group A is more effective than B group statistically. Conclusions : It was suggested that near acupuncture and remote acupuncture point needling might have more effect compared with near acupuncture point needling acupuncture on posterior neck pain treatment.

Comparative Study on the Effect of Acupuncture on Local Acupoints and Distal Acupoints for Patients with Whiplash Injury (교통사고로 인한 편타성 손상 환자에 대한 원위취혈 및 근위취혈의 치료효과 비교 연구)

  • Objective : The purpose of this study is to compare the Effect of Acupuncture on Local Acupoints and Distal Acupoints for Patients with Whiplash Injury. Method : From January 1st 2009 to September 31th 2009, 10 patient were divided into 2 groups. One group was taken Local Acupoints, and the other group was taken Distal Acupoints. To check pain and treatment satisfaction, visual analog scale(VAS) and five-point likert scale were used before and after treatment. Result : Both acupuncture theraphy showed improvement in VAS and five-point likert scale. And Local Acupoints group showed more effective than Distal Acupoints group on five-point likert scale. Conclusion : This Study suggests that Acupuncture on Local Acupoints and Distal Acupoints can be applicable to improve symptom in Patients with Whiplash Injury.
