A Case Report of Sjögren's Syndrome with Influenza A Virus Infection Treated with Korean Medicine (A형 독감을 동반한 쇼그렌 증후군 환자에 대한 한의치료 1례)

  • Objectives: The purpose of this study was to report the treatment of a patient suffering from $Sj{\ddot{o}}gren^{\prime}s$ Syndrome with influenza A virus infection with Korean medicine. Methods: We used herbal medicine, acupuncture, and moxibustion to treat a patient during hospitalization. We observed the changes in symptoms using the European League against Rheumatism $Sj{\ddot{o}}gren^{\prime}s$ Syndrome Patient Reports Index (ESSPRI) and a visual analogue scale (VAS). Results: After treatment for 17 days, the patient's symptoms showed improvement in joint pain, dry eye, and dry mouth. The ESSPRI score was decreased from 10 to 5.3. The VAS for dry mouth and dry eye were decreased from 10 to 6 and from 10 to 5, respectively. Conclusion: This clinical case study suggests that Korean medicine treatment that includes Insamyangyoung-tang-gami could be effective in the treatment of $Sj{\ddot{o}}gren^{\prime}s$ Syndrome.

  • Objectives This study was designed to evaluate the effects of Korean medicine therapy(Cheongsimyeonja-tang) on Taeeumin patients diagnosed with Sj$\ddot{o}$gren's syndrome with arthritis. Methods The Sj$\ddot{o}$gren's syndrome with arthritis patient was treated with Cheongsimyeonja-tang. The progress was evaluated with the visual analogue scale and erythrocyte sedimentation rate on hematologic examination. Results There was improvement in the patient's Sj$\ddot{o}$gren's syndrome symptoms. The patient's symptoms of the alimentary system, dried fever and arthralgia evaluated with the visual analogue scale improved. Erythrocyte sedimentation rate was decreased on hematologic examination. Conclusions This study suggests that Korean medicine(Cheongsimyeonja-tang) is effective in the treatment of Sj$\ddot{o}$gren's syndrome on Taeumin patients.

  • Sj${\ddot{o}}$gren's syndrome is a chronic autoimmune exocrinopathy that destroys salivary and lacrimal gland tissue. We report an unusual case of this disease in a 54-year-old woman who presented with multiple and bilateral parotid cystic masses. The multiple, small, bead-like cysts were clearly evident in the computed tomography sections in this patient, a visible reminder that this may be the initial presentation in a patient with Sj${\ddot{o}}$gren's syndrome. As the case illustrates, Sj${\ddot{o}}$gren's syndrome should be included in the differential diagnosis of multiple and bilateral cystic parotid lesions.

  • Sj$\ddot{o}$gren's syndrome (SS) is an autoimmune disease characterized by keratoconjunctivitis sicca and xerostomia. About thirty percent of patients with Sj$\ddot{o}$gren's syndrome experience nervous system involvement such as myelopaty, optic neuropathy, and peripheral neuropathy. The most common pattern of peripheral nerve involvement is axonal polyneuropathy. We present a case of demyelinating polyneuropathy in a patient with Sj$\ddot{o}$gren's syndrome, which had been proven by electrophysiology and pathology.

  • Sj$\ddot{o}$gren's syndrome (SS) is an immunologic disorder characterized by progressive destruction of the exocrine glands due to infiltration of lymphocyte resulting in mucosal and conjunctival dryness accompanied by a variety of autoimmune phenomena. And SS is divided into primary and secondary by accompanying collagen vascular diseases. In Korea, only a few cases of primary SS and primary SS with interstitial pneumonitis have been documented. Recently we experinced a suspected case of primary SS with interstitial pneumonitis diagnosed by sublabial and open lung biopsy, Schirmer's test and slit lamp test.

  • Sj$\ddot{o}$gren's syndrome(SS) is a chronic inflammatory disorder characterized by lymphocytic infiltration of lacrimal and sailvary glands, which results in dry eyes and dry mouth. SS may exist as a primary condition or as a secondary condition in association with connective tissue disease such as rheumatoid arthritis, systemic lupus erythematosus, or progressive systemic sclerosis. We experienced a patient with primary SS who developed multiple bullae, nodular type of pulmonary amyloidosis and lymphocytic interstitial peumonitis. We believe this to be the first reported case of SS acompanied by these three types pulmonary manifestations at the same time.

Sjögren's Syndrome: an Update on Diagnostic, Clinical, and Basic Aspects for Oral Medicine Specialists

  • $Sj{\ddot{o}}gren^{\prime}s$ syndrome (SjS) is a common autoimmune disorder characterized by lymphocytic infiltration in the salivary and lacrimal glands, resulting in severe dry mouth or eyes. As a result, most of SjS patients suffer from oral dryness and can visit the department of oral medicine with or without diagnosis of SjS. Therefore, oral medicine specialists should know clues, which may indicate the diagnosis of SjS from the clinical and laboratory investigations. By the recent SjS criteria, SjS can be diagnosed by focus score, ocular staining, Schirmer's test, unstimulated whole saliva flow rate, and anti-SSA/Ro antibodies. The aim of this article is to review the diagnostic criteria, clinical investigation, and basic aspect related to SjS and to make oral medicine specialists play an important role in the detection of emerging SjS.

  • A 47-years-old woman presented with a 2-month history of a dry mouth and dry cough. The patient had been taking medication for Sj$\ddot{o}$gren's syndrome for approximately 7 years. The chest radiography showed multiple cystic lesions and a hazy density in both lower lung fields. The HRCT showed a diffuse ground glass like appearance and multiple variable sized cystic lesions in both lung fields. After medication, the symptoms were aggravated. Bronchoscopy was preformed with a transbronchial lung biopsy. The biopsies showed an infiltration of lymphocytes, neutrophils, monocytes and histiocytes through the interstitial space of the alveola and a widening of the alveolar septa. However, the histological findings of the cysts were not obtained. Sj$\ddot{o}$gren's syndrome is a slowly progressive inflammatory autoimmune disease, which is characterized by lymphocyte mediated destruction of the exocrine glands, with pulmonary involvement in approximately 19-65%, High-resolution CT is a sensitive technique for assessing the pulmonary involvement in patients with Sj$\ddot{o}$gren's syndrome. Although a lung biopsy is not always necessary for establishing a diagnosis of an interstitial lung disease in Sj$\ddot{o}$gren's syndrome. A lung biopsy may reveal a wide spectrum of changes ranging from a mild inflammatory response to end stage fibrosis with honeycombing. Because of the predominantly peribronchiolar inflammatory infiltration and inspissated secretions the cysts were suspected to have been formed by the ball-valve phenomen. However, no definite evidence was obtained.

  • Patients with $Sj{\ddot{o}}gren^{\prime}s$ syndrome usually suffer from teeth discoloration and attrition due to xerostomia. If the anterior teeth are badly worn, problems such as loss of anterior guidance, occlusal disharmony, and limited space for restoration may occur. However, ideal occlusion is obtained in both centric and eccentric relation by regaining the disocclusion of the posterior teeth through the anterior and lateral guidance using twin-stage method. In this case, rehabilitation was performed for a $Sj{\ddot{o}}gren^{\prime}s$ syndrome patient with maxillary and mandibular incisor's severe attrition and teeth discoloration by using twin-stage method.

  • A bronchus-associated lymphoid tissue(BALT) lymphoma of the lung is a rare disorder of patients with Sj$\ddot{o}$gren's syndrome. A 49-year-old woman was admitted for an evaluation of exertional dyspnea and general weakness which had persisted for two years. The patient had suffered from dry mouth and dry eyes for five years. The physical examinations showed a coarse breath sound with inspiratory crackles on the whole lung field, particularly on the both basal lungs. The laboratory data disclosed high titers of anti-nuclear antibodies, and anti-SSA (Ro), and anti-SSB (La) antibodies. Chest radiographs demonstrated the presence of bilateral, diffuse, reticulonodular densities in both lungs. Thin-section CT scans showed diffusely distributed mosaic pattern of an inhomogeneous attenuation extending over the entire lung zone. The histological findings from an open-lung biopsy specimen revealed an accumulation of lymphoid cells around the bronchioles and an extension of malignant lymphoma cells from the bronchiolar epithelium toward the alveolar space. Immunohistochemically, the neoplastic cells reacted positively to the CD 20 antigen and were focally positive for the UCHL 1 antigen. The histological diagnosis was consistent with a low grade marginal zone B-cell lymphoma originating in the BALT. Here, we present a case of a histologically proven BALT lymphoma of the lung in a patient with primary Sj$\ddot{o}$gren's Syndrome.
