제주마 주파기록에 대한 연도별 추세 및 환경효과 분석 (Estimation of Annual Trends and Environmental Effects on the Racing Records of Jeju Horses)

  • 본 연구는 제주마의 연도별 주파기록 특성을 파악하고 경주기록에 대한 환경효과 분석을 위해 수행되었다. 한국마사회가 2002년부터 2019년까지 수집한 제주마 2,167두의 48,645개 관측치 정보가 분석에 사용되었다. 환경효과 분석을 위하여 관측치 빈도가 가장 높은 800 m 기준으로 주파기록은 보정되었다. 거리별 경주기록은 동일거리 기준으로 변환되었으며 연도별 비교가 가능했다. 그 결과 표현형에서 매년 주파기록이 0.242초(R2=0.66) 단축됨을 알 수 있었다. 보정된 주파기록에 대한 환경효과의 유의성 검증 위해 일원분산분석을 수행하였고 모형에서 설정한 모든 변수에서 고도의 유의성을 나타내었다(p<0.001). 이들 변수에 대해 단계적 변수 선발법을 적용, AIC 값 산출을 통해 모형 적합도를 평가하였으며 연도, 부담구분, 기수 순위, 조교사 순위, 주로상태, 날씨, 마령, 성별 순으로 변수 선택 시 가장 낮은 AIC 값을 갖는 모형이 수립되었다. 최종적으로 주파기록에 영향하는 환경효과 분석을 위하여 기수, 조교사 순위 및 마령 3가지 변수를 임의효과로 가정하였다. 그 결과 기수와 마령을 임의효과로 설정하였을 때 주파기록에 영향하는 환경효과 분석에 가장 적합한 모형인 것으로 나타났다. 본 연구에서 도출된 결과는 제주마 유전능력 평가를 위한 모형설정 시 기초자료로 활용 가능할 것으로 사료된다.

청보리 사일리지 TMR 또는 청호밀 사일리지 TME 급여 및 급여기간이 거세 한우의 증체, 사료비 및 육질특성에 미치는 효과 (Effect of Feeding Whole Crop Barley Silage- or Whole Crop Rye Silage based-TMR and Duration of TMR Feeding on Growth, Feed Cost and Meat Characteristics of Hanwoo Steers)

  • 본 연구는 주요 동계사료작물인 청보리와 청호밀을 이용한 TMR 급여가 한우 거세우의 생산성과 사료비, 그리고 도체특성과 등급판정 및 배최장근의 이화학적 특성에 미치는 효과를 조사하기 위해 한우 거세우 송아지 80두를 대상으로 육성기에서 비육후기까지 총 680일에 걸쳐 실시되었다. 시험사료는 배합사료와 볏짚을 분리 급여하는 관행사양구(Control), 청보리 사일리지 TMR을 비육 중기까지 자유채식토록 하고 비육 후기에는 관행사양구와 동일한 사료를 급여하는 처리구(BS-TMR I), 청보리 사일리지 TMR을 비육 후기까지 자유채식토록 하는 처리구(BS-TMR II), 청호밀 사일리지 TMR을 비육 중기까지 자유채식토록 하고 비육 후기에는 관행 사양구와 동일한 사료를 급여하는 처리구(RS-TMR I), 청호밀 사일리지 TMR을 비육 후기까지 자유채식토록 하는 처리구(RS-TMR II)로 구성하였다. 각 처리 당 거세된 송아지 16두 씩을 배치하였고, 처리 당 거세한우 송아지를 4두씩 4개의 pen에 수용하였으며, 완전임의배치 방법으로 각 처리구의 pen을 배치하였다. 한우 거세우의 증체량은 관행사양구에 비해 육성기 및 비육전기에서 청보리 및 청호밀 사일리지 TMR을 섭취한 처리구에서 다소 높았고 비육 중기에서 현저히 높았으나(P<0.0001) 비육 후기에서는 RS-TMR II를 제외하고는 관행사양구의 한우에서 다소 높은 것으로 나타났다. 한우 두당 kg 증체 당 사료비는 관행사양구의 한우에 비해 청보리 및 청호밀 사일리지 TMR을 섭취한 한우에서 다소 낮았다. 또한 비육 후기까지 사일리지 TMR을 섭취한 처리구(BS-TMR II 및 RS-TMR II)에 비하여 비육 중기까지 사일리지TMR을 섭취하고 비육 후기에는 관행사료를 섭취한 처리구(BS-TMRI 및 RS-TMR I)에서 두당 kg 증체 당 사료비가 다소 감소 되었다. 사일리지 TMR이 관행사료 급여에 비해 한우 거세우의 도체중, 등지방 두께 및 배최장근 면적을 다소 증가시켰으나 육량지수를 낮추었다(P<0.047). 다른 시험사료에 비해 청보리 사일리지 TMR이 근내지방도를 다소 낮추었으나 육질 1등급 이상 출현두수에는 처리 간 차이가 없었다. 한우 거세우 내 배최장근의 화학적성분, 도체특성, 대부분의 육질 특성, 육색 및 지방산 조성은 시험사료에 의한 영향을 받지 않았다. 그러나 전단력은 사일리지 TMR을 섭취한 한우에 비해 관행사양 방법으로 비육된 한우에서 낮았으나(P<0.046) 사일리지 TMR을 섭취한 처리구 간에는 차이가 없었다. 본 시험의 결과로 미루어 보아 배합사료와 볏짚 중심의 관행사양방법에 비하여 BS 또는 RS를 이용한 TMR 급여가 거세한우의 육질에 크게 영향하지 않은 반면 증체를 개선하고 사료비를 절감할수 있을 것이라 여겨진다. 또한 전체적으로는 청보리 및 청호밀 사일리지 TMR의 사료적 가치는 거의 비슷한 것으로 여겨진다.

자아조절자원 및 해석수준이 공짜대안 선택에 미치는 영향 (The Effects of Self-regulatory Resources and Construal Levels on the Choices of Zero-cost Products)

  • 사람들이 돈을 지불하지 않고 무료로 얻을 수 있는 공짜제품을 과다하게 선호하는 현상을 '공짜효과'라 한다. 기존 연구들에 의하면 공짜제품에 주어지는 특별한 가치 때문에 이와 같은 효과가 발생한다. 본 연구는 공짜효과가 항상 나타나는 것이 아니라 심리적 변수에 의하여 조절될 수 있다는 것을 보이기 위하여 자아조절자원과 해석수준의 조절효과를 살펴보았다. 자아조절자원이 고갈되면 통제의 힘이 약해져서 가격에 대한 민감도가 감소할 뿐만 아니라 직관적이고 노력을 별로 기울이지 않는 정보처리과정을 통해 의사결정을 수행한다. 또한, 주어진 정보를 어떤 해석수준에서 처리하는가에 따라 선택이 달라진다. 고차원 해석수준에서 중심기능을 바탕으로 대안의 바람직성에 따라서 선택하는 반면, 저차원 해석수준에서 부가기능을 바탕으로 대안의 실행가능성에 초점을 두어 선택한다. 이와 같은 특성이 공짜효과의 크기에 미치는 영향을 살펴보는 것이 본 연구의 가장 중요한 목적이다. 자아조절자원과 해석수준에 의해서 공짜효과의 크기가 조절될 수 있다는 사실을 검증하기 위해 2개의 실험설계를 채용하였다. 두 실험 모두에서 기존연구에서 사용한 실험재(키세스와 페레로로쉐 초콜릿)를 이용했다. 실험 1은 자아조절자원 고갈 여부가 공짜효과에 미치는 영향을 검증했다. 자아조절자원 고갈과 비고갈 집단으로 나누어 공짜대안이 있는 선택과업과 그렇지 않은 과업에 할당했다. 자아조절자원이 고갈되지 않은 집단에서 공짜효과가 확실하지만, 자아조절자원이 고갈된 집단에서 공짜효과가 약해진다는 것을 밝혔다. 실험 2는 해석수준이 공짜효과에 미치는 영향을 검증했다. 실험 2는 '왜(why)'와 '어떻게(how)'를 이용해 해석수준을 조작했으며, 실험 1과 유사하게 공짜대안이 존재하는 의사결정과업과 존재하지 않는 과업에 할당한 뒤 공짜대안 선택에 미치는 영향을 확인하였다. 고차원 해석수준의 집단은 저차원 해석수준의 집단에 비하여 공짜제품 선택비율이 낮았다.

선발 3세대 북방전복의 성장형질에 대한 가계변이 및 유전모수 추정 (Estimation of Family Variation and Genetic Parameter for Growth Traits of Pacific Abalone, Haliotis discus hannai on the 3th Generation of Selection)

  • 본 연구에서는 전복의 성장형질에 대한 가계변이를 비교분석하기 위한 목적으로 2011년에 생산된 선발 3세대 북방전복 5,334마리에 대한 18개월령 성장형질인 각장, 각폭 및 중량의 전체 계측자료를 이용하여 유전모수와 육종가를 추정하였으며, 그 중 개체수가 많은 상위 10가계의 865마리 대한 가계변이를 조사하였다. 가계효과를 추정하기 위해 개체모형에 근거한 선형모형을 이용하였고, 유전모수 및 육종가는 생산시기를 고정 효과로 처리하고, EM-REML algorithm을 전산 프로그램화한 REMLF90을 이용하여 최적선형불편예측법에 의해 추정하였다. 본 연구에서 조사된 18개월령 북방전복의 표현형에 있어 각장, 각폭 및 중량의 전체평균은 각각 54.5 mm, 36.8 mm 및 21.3 g로 나타났고, 중량의 변이계수가 51.0%로 나타나 각장의 21.1% 및 각폭의 20.7% 보다 자료의 변동성이 크게 나타났다. 개체수가 많은 상위 10가계를 대상으로 각장과 중량의 관계를 산점도로 표시한 결과 상관관계식이 TW = $0.0002SL^{2.8796}$ ($R^2$ = 0.9864) 과 같이 지수곡선식으로 추정되었다. 가계효과에 있어서는 각 가계별로 유의적인 차이를 보였으며 (p < 0.05), 각장, 각폭 및 중량의 유전력은 각각 0.370, 0.382 및 0.367로 중도의 유전력을 보여 개체선발 보다는 가계선발이 유리할 것으로 사료된다. 또한 각 형질 간 상관계수는 매우 높은 정의 상관관계를 보여 한 형질만의 개량으로 다른 형질의 개량효과를 기대할 수 있을 것으로 판단된다. 추정된 각장 및 중량의 육종가를 토대로 개체수가 많은 상위 10가계에 대한 각 개체별 분포와 순위를 조사하기 위해 표준화육종가로 변환하여 이용하였으며, 각장을 기준으로 상위 5.4%의 개체 수는 152마리, 하위 5.4%의 개체 수는 8마리로 조사되었고, 중량의 경우 상위 5.4%의 개체 수는 164마리, 하위 5.4%의 개체 수는 1마리로 조사되었다. 이와 같이 가계간의 표현형 및 유전적인 다양한 변이를 확인 할 수 있었고, 다음 세대 생산을 위한 모집단의 유전모수와 육종가를 추정하여 선발과 교배에 적절히 이용한다면 보다 나은 육종효과를 기대할 수 있을 것으로 사료된다.

외과적 손씻기 및 외과용 장갑의 천공율에 대한 연구 (A Study on the Surgical Hand Scrub and Surgical Glove Perforation)

  • Post - operative wound infections have been a serious problem in nursing care in the operating room and appear to be strongly related to the infection occurring during the performance of operation. The purpose of this study is to identify patterns in duration of surgical hand scrub (SHS), to evaluate the method of SHS and to examine the rate of glove perforation. Subjects for this study include 244 doctors and 169 nurses working in the operative theatre of a hospital in Seoul area. Test samples and related data were collected from this medical facility between April 1, through 15, and July 1, through 5, 1995 by the author and a staff member working in the operating room. For the study, data on the SHS of doctors and nurses were obtained at the time of operation and multiple batches of surgical gloves worn by the operating doctors were collected after each operation. The duration of SHS was measured with a stop watch and the method of SHS was evaluated according to Scoring Hand Scrub Criteria (SHS Criteria) and expressed as SHS scores. For the analysis of the data, t-test was used to compare the differences in the duration and the SHS scores of doctors and nurses, and Pearson's correlation coefficient was used to examine the relationship between the SHS duration and the SHS scores. The results of the study are summarized as follows. 1) The mean time spent in each SHS was 167 seconds in nurses, and 127 seconds in doctors. The data comparing nurses and doctors indicated that there were significant differences in Our ation of SH S between these two groups (t=5.58, p=.000). 2) The mean time spent in the first SHS was 145 seconds and that in the End SHS, 135 seconds, and there was not a significant difference in the duration of the SHS between doctors and nurses (t=1.44, P=.156). 3) The mean time spent in the SHS by OS (Orthopaedic surgery) doctors was 162 seconds, 150 seconds by NS(Neurologic surgery), 121 seconds by GS(General surgery), 94 seconds by OPH(Opthalmology) and DS(Dental surgery), 82 seconds by URO(Urology), 78 seconds by PS(Plastic surgery) and 40 seconds by ENT(Ear, Nose & Throat) These also showed a significant difference in the duration of the SHS among the medical specialities (t=4.8, P=.0001). 4) The average SHS score of the nurses was 15.2, while that of doctors was 13.1. The statistical analysis showed that t-value was 3.66, p was. 000. This indicates that the nurses actually clean their hands more thoroughly than the doctors do. 5) The average SHS score of NS doctors was 15.5, 15.3 for doctors for OPH,14.3 for OS,12.7 for GS, 12.0 for DS, 11.7 for URO, 10.1 for PS, 7.5 for ENT. Comparison of the average SHS scores from 8 specialties showed that there was a significant differences in the patterns of the SHS (F=5.08, P=.000) among medical specialties. 6) It appears that the operating personnel scrub the palms and dorsum of their hand relatively well, however, less thorough the nails and fingers. 7) The more the operating personnel spend their time in hand scrubbing, the more correctly they clean their hands(r=.6427, P<.001). 8) The overall frequencies of perforation in all post-operative gloves tested was 38 out of 389 gloves (10.3%). The perforation rate for PS was 13%, 12.1% for GS,8.8% for 05, and 3.3% for NS.

Potential Benefits of Intercropping Corn with Runner Bean for Small-sized Farming System

  • The objectives of this study were to evaluate potential benefits of intercropping of corn with runner bean for a smallsized farming system, based on land equivalent ratio (LER) and silage yield and quality of corn intercropped with runner bean (Phaseolus vulgaris L.), in arid conditions of Turkey under an irrigation system. This experiment was established as a split-plot design in a randomized complete block, with three replications and carried out over two (consecutive) years in 2006 and 2007. Seven different mixtures (runner bean, B and silage corn sole crop, C, 10% B+90% C, 20% B+80% C, 30% B+70% C, 40% B+60%C, and 50% B+50%C) of silage corn-runner bean were intercropped. All of the mixtures were grown under irrigation. The corn-runner bean fields were planted in the second week of May and harvested in the first week of September in both years. Green beans were harvested three times each year and green bean yields were recorded each time. After the 3rd harvest of green bean, residues of bean and corn together were randomly harvested from a 1 $m^{2}$ area by hand using a clipper when the bean started to dry and corn was at the dough stage. Green mass yields of each plot were recorded. Silages were prepared from each plot (triplicate) in 1 L mini-silos. After 60 d ensiling, subsamples were taken from this material for determination of dry matter (DM), pH, organic acids, chemical composition, and in vitro DM digestibility of silages. The LER index was also calculated to evaluate intercrop efficiencies with respect to sole crops. Average pH, acetic, propionic and butyric acid concentrations were similar but lactic acid and ammonia-N levels were significantly different (p<0.05) among different mixtures of bean intercropped with corn. Ammonia-N levels linearly increased from 0.90% to 2.218 as the percentage of bean increased in the mixtures up to a 50:50 seeding ratio. While average CP content increased linearly from 6.47 to 12.45%, and average NDF and ADF contents decreased linearly from 56.17 to 44.88 and from 34.92 to 33.51%, respectively, (p<0.05) as the percentage of bean increased in the mixtures up to a 50:50 seeding ratio, but DM and OM contents did not differ among different mixtures of bean intercropped with corn (p>0.05). In vitro OM digestibility values differed significantly among bean-corn mixture silages (p<0.05). Fresh bean, herbage DM, IVOMD, ME yields, and LER index were significantly influenced by percentage of bean in the mixtures (p<0.01). As the percentage of bean increased in the mixtures up to a 50:50 seeding ratio, yields of fresh bean (from 0 to 24,380 kg/ha) and CP (from 1,258.0 to 1,563.0 kg/ha) and LER values (from 1.0 to 1.775) linearly increased, but yields of herbage DM (from 19,670 to 12,550 kg/ha), IVOMD (from 12,790 to 8,020 kg/ha) and ME (46,230 to 29,000 Mcal/ha) yields decreased (p<0.05). In conclusion, all of the bean-corn mixtures provided a good silage and better CP concentrations. Even though forage yields decreased, the LER index linearly increased as the percentage of bean increased in the mixture up to a 50:50 seeding ratio, which indicates a greater utilization of land. Therefore, a 50:50 seeding ratio seemed to be best for optimal utilization of land in this study and to provide greater financial stability for labor-intensive, small farmers.

Effects of Dietary Fatty Acids on fatty Acid Pattern in Development Rat Brain Phospholipids - Effects on P/M/S and $\omega$3/$\omega$6 Fatty Acid Ratios -

  • Docosahexaenoic acid(DHA), a $\omega$3 series fatty acid and arachidonic acid(AA). a $\omega$6 series fatty acid were found in relatively high concentrations in the phospholipids(PLs) of cell membranes of nerve tissues, and they can be affected by various factors. The present study examined the effects of dietary $\omega$6 and $\omega$3 fatty acid composition on P/M/S and on $\omega$3/$\omega$6 fatty acid ratios in brain PLs of 2nd generation rats. The expeimental diets consisted of 10% fat(by wt), which were computer- searched mixed oil('M') with P/M/S ratio, 1 : 1.4 : 1 and $\omega$6/$\omega$3 ratio, 6 : 1 and safflower oil('S') poor in $\omega$3 fatty acids. The experimental diets were started 3-4 wks prior to conception. During the lactation period, the feeding mothers were switched 1 wk after birth and provided the pups for 2 wks with milk which had compositions different from that of their natural mother. The same diet as their mothers was provided from weaning to 9 wks of age. The 'M'and 'S' rats were again subdivided into MM, MS, SS, SM rats according to diet which their lactating mothers were fed from the begining of the experiment. The relative percentage of P/M/S fatty acids in brain PLs in all experimental groups converged to a very similar value at 9 wks of age, indicating the existence of a control mechanism for the degree of fatty acids, unsaturation. The $\omega$3/$\omega$6 fatty acid ratios of brain PLs converged to about 1.0 in MM & SM groups and to 0.7 in SS & MS groups, suggesting also the existence of some balance between $\omega$3 and $\omega$6 fatty acids in developing rat brain. The concentrations of $\omega$3 fatty acids, especially DHA, in the SM group were increased and became similar to those in MM group at 9 wks of age. The increase in DHA of brain PLs was counterbalanced b)r a decrease in 22 5$\omega$6. Therefore, the ratios of 22 : 6$\omega$6/22 : 5$\omega$6 were higher in both MM & SM groups than those of SS & MS groups at 9 wak of age. Although dietary $\omega$3 and $\omega$6 fatty acids affected 22 : 6$\omega$S and 22 : 5$\omega$6 contained in rat brain PLs reciprocally, the relative percentage of AA did not appear to be significantly influenced by the diet in all groups at 9 wks of age, suggesting that a mechanism for the maintenance of a certain level of AA in brain PLs exists. In conclusion, the $\omega$3/$\omega$6 fatty acid and 22 : 6$\omega$3/22 : 5$\omega$6 ratios, but not P/M/S ratio, of rat brain PLs were affected by the postnatal dietary changes. Futher studies are required to clarify the mechanism(S) of ensuring a certain level of DHA and of maintaining a similar level of AA in rat brain PLs after. weaning(9 wk) regardless of prenatal and postnatal dietary changes. ATHEROSCLEROSIS, CHOLESTEROL AND EGG - REVIEW -

  • The pathogenesis of atherosclerosis can not be summarized as a single process. Lipid infiltration hypothesis and endothelial injury hypothesis have been proposed and investigated. Recent developments show that there are many points of potential interactions between them and that they can actually be regarded as two phases of a single, unifying hypothesis. Among the many risk factors of atherosclerosis, plasma homocysteine and lipoprotein(a) draw a considerable interest because they are independent indicators of atherogenicity. Triglyceride (TG)-rich lipoproteins (chylomicron and VLDL) are not considered to be atherogenic but they are related to the metabolism of HDL cholesterol and indirectly related to coronary heart disease (CHD). LDL can of itself be atherogenic but the oxidative products of this lipoprotein are more detrimental. HDL cholesterol has been considered to be a favorable cholesterol. The so-called 'causalist view' claims that HDL traps excess cholesterol from cellular membranes and transfers it to TG-rich lipoproteins that are subsequently removed by hepatic receptors. In the so-called 'noncausalist view', HDL does not interfere directly with cholesterol deposition in the arterial wall but instead reflects he metabolism of TG-rich lipoproteins and their conversion to atherogenic remnants. Approximately 70-80% of the human population shows an effective feedback control mechanism in cholesterol homeostasis. Type of dietary fat has a significant effect on the lipoprotein cholesterol metabolism and atherosclerosis. Generally, saturated fatty acids elevate and PUFA lower serum cholesterol, whereas MUFA have no specific effect. EPA and DHA inhibit the synthesis of TG, VLDL and LDL, and may have favourable effects on some of the risk factors. Phospholipids, particularly lecithin, have an antiatherosclerotic effect. Essential phospholipids (EPL) may enhance the formation of polyunsaturated cholesteryl ester (CE) which is less sclerotic and more easily dispersed via enhanced hydrolysis of CE in the arterial wall. Also, neutral fecal steroid elimination may be enhanced and cholesterol absorption reduced following EPL treatment. Antioxidants protect lipoproteins from oxidation, and cells from the injury of toxic, oxidized LDL. The rationale for lowering of serum cholesterol is the strong association between elevation of plasma or serum cholesterol and CHD. Cholesterol-lowing, especially LDL cholesterol, to the target level could be achieved using diet and combination of drug therapy. Information on the link between cholesterol and CHD has decreased egg consumption by 16-25%. Some clinical studies have indicated that dietary cholesterol and egg have a significant hypercholesterolemic effect, while others have indicated no effect. These studies differed in the use of purified cholesterol or cholesterol in eggs, in the range of baseline and challenge cholesterol levels, in the quality and quantity of concomitant dietary fat, in the study population demographics and initial serum cholesterol levels, and clinical settings. Cholesterol content of eggs varies to a certain extent depending on the age, breed and diet of hens. However, egg yolk cholesterol level is very resistant to change because of the particular mechanism involved in yolk formation. Egg yolk contains a factor of factors responsible for accelerated cholesterol metabolism and excretion compared with crystalline cholesterol. One of these factors could be egg lecithin. Egg lecithin may not be as effective as soybean lecithin in lowering serum cholesterol level due probably to the differences of fatty acid composition. However, egg lecithin may have positive effects in hypercholesterolemia by increasing serum HDL level and excretion of fecal cholesterol. The association of serum cholesterol with egg consumption has been widely studied. When the basal or control diet contained little or no cholesterol, consumption of 1 or 2 eggs daily increased the concentration of plasma cholesterol, whereas that of the normolipemic persons on a normal diet was not significantly influenced by consuming 2 to 3 eggs daily. At higher levels of egg consumption, the concentration of HDL tends to increase as well as LDL. There exist hyper-and hypo-responders to dietary (egg) cholesterol. Identifying individuals in both categories would be useful from the point of view of nutrition guidelines. Dietary modification of fatty acid composition has been pursued as a viable method of modifying fat composition of eggs and adding value to eggs. In many cases beneficial effects of PUFA enriched eggs have been demonstrated. Generally, consumption of n-3 fatty acids enriched eggs lowered the concentration of plasma TG and total cholesterol compared to the consumption of regular eggs. Due to the highly oxidative nature of PUFA, stability of this fat is essential. The implication of hepatic lipid accumulation which was observed in hens fed on fish oils should be explored. Nutritional manipulations, such as supplementation with iodine, inhibitors of cholesterol biosynthesis, garlic products, amino acids and high fibre ingredients, have met a limited success in lowering egg cholesterol.

만성 폐쇄성 폐질환에서 급성 호흡부전 발생시 Simplifed Acute Physiology Score에 따른 단기예후의 평가 (Determination of Short Term Prognosis Among Chronic Obstructive Lung Disease with Acute Respiratory Failure According to Simplified Acute Physiology Score)

  • 임상에서 환자의 생존율을 평가하는 것은 순간순간 당면하는 치료를 결정함에 있어 중요하다. 특히 만성 폐쇄성 폐질환에서 급성 호흡부전증은 대부분의 환자들이 궁극적으로 겪게 되는 질병의 경과로 여겨지고 있고 이들을 치료하는 과정에서 질병 자체의 비가역적인 특성과 기계호흡 자체의 부작용으로 인하여 중환자실에서 인공호흡기의 부착여부에 대한 논란이 계속되고 있으나, 아직 이에 대한 뚜렷한 기준이 없는 상태이다. 이에 본 연구에서는 입원 첫 24시간이내에 측정가능한 14개의 임상지표를 이용하여(Simplified Acute Physiology Score, 이하 SAPS) 환자의 중증도와 사망률을 예측해 보는 것이었다. 대상환자는 1980년부터 1992년까지 중앙대학 부속병원에서 입원치료를 받은 74명의 만성 폐쇄성 폐질환에 동반된 급성 호흡부전증 환자들에 대한 후향성 연구로 시행하였고, 결과는 다음과 같다. 1) 사망율은 34%(25/74), 생존율은 66%(49/74)였으며, 나이가 많을수록 유의하게 사망율이 높았다. 2) 기저 폐기능 검사치에 따른 사망율의 차이는 없었다. 3) SAPS는사망군에서 10.8, 생존군에서 6.5로 유의한 차이가 있었고, SAPS와 사망율 사이에는 유의한 상관관계가 있었다. 4) SAPS에 포함되는 14개의 임상지표 이외에 입원당시 악액질, 뇌병증, 혈중 Phosphate와 Creatinine 농도에 따라 유의한 사망율의 차이가 있었다. 결론적으로, SAPS는 만성 폐쇄성 폐질환에 동반된 급성 호흡부전증 환자의 예후와 중증도 평가에 유용하며, 이러한 환자군에서 SAPS에 포함되는 14개의 임상지표 이외에도 혈중 Creatinine 농도, 혈중 Phosphate 농도, 입원 당시 악액질유무, 뇌병변에 의한 신경학적 증상의 유무가 예후에 중요한 영향을 미친다고 사료되며, 적극적인 치료와 인공호흡기 치료의 결정에서 SAPS의 사용이 타당하리라 여겨지나, 향후 대단위 전향적 연구가 필요하리라 여겨진다.

임신 중 체중변화와 임부 및 신생아 상태에 관한 연구 (The studies about the weight-changes during pregnancy and the condition of mother and infant)

  • This research is to study about the weight-change of a pregnant woman, conditions of the woman and an infant. The weight-change of a mother during pregnancy was observed and that was expressed as the basis on the body mass index of a mother before pregnancy. The effects of weight-changes on both the discomfort, complications of pregnant mother and the condition of an infant were also investigated. Thus we set a purpose that this study would help pregnant woman and an infant to maintain and enhance their health conditions by proper weight control through nursing mediation. This study was performed in a certain hospital of university in seoul from Feb. 1. 2000 to Mar. 31. 2000. We explained the purpose of this study to the hospital institution and obtained consent of investigation. 152 inpatients who were in condition from PA 37 weeks to PA 42 weeks were the subject of this study. The research materials were made through of question paper that inpatients make answer by themselves and investigation paper. The question paper was about general background, weight and height before pregnancy and discomfort of the physical degree. And the investigation paper was about parity, maternal weight(late pregnancy), high pregnancy, delivery method, hemoglobin level, Apgar score, fetal weight. Physical discomfort was measured using the implement made by Kim hae won(1996) (chronbach's ${\alpha}=0.85$). SPSS was used to do statistics for managing and analyzing data. The results of this study were like followings. 1. The mean value of gained weight during pregnancy was about 13.8kg within from 3 kg to 26 kg. Among 152 research candidates, the gained weight of 80(52.6%) candidates remained within an ideal range. But that of 37 candidates(24.3%) became less than the ideal range. Also that of 35 candidates(23.0%) became over than the ideal range. 2. In the investigation of the relation between the weight change of a pregnant woman and her condition, the scores to represent physical discomfort were middle in all candidates. And the physical discomfort of over weight-gained group was more than that of low weight-gained group, but there was no difference in statistics(F=0.234, p=0.791). The weight-changes of pregnant woman didn't have an influence with the high risk of pregnancy(F=0.509, p=0.477). Also, the weight-changes didn't have an influence on delivery method($x^2=3.825$, p=0.148). However, in the investigation of the relation between weight-change and hemoglobin level, the change of hemoglobin level was highest in over weight gained group(F=3.062, p=0.05). 3. In the investigation of the weight-change of pregnant woman and the condition of infant. the weight changes didn't have an influence on both 1 min Apgar score(F=0.157, p=0.855) and 5 min Apgar score(F=0.030, p=0.970) of infant. Also, in the investigation of weight-change of a pregnant woman and weight difference of a infant with Pearson Correlation Coefficient, the weight-change of a pregnant woman affected vastly the weight of a infant. It was also found that the more pregnant woman gained in weight, the more did gain weighty infants. This relation was in net proportion(r=0.256, p=0.001). In conclusion, these results suggest that the weight-changes during pregnancy in Korea women of these days are more increased than that of the past days and individual variation in weight-changes is very high. Also, these results suggest that the changed hemoglobin level of a mother and weight of an infant were meaningfully affected by the weight-changes of a pregnant woman during pregnancy. However, the physical discomfort of a pregnant woman, the high risks of pregnancy, the delivery method and Apgar score of an infant were not affected by the weight-changes during pregnancy. Because the recommendation suggesting the ideal weight-change, used this study, is basis on the subject of American women, therefore, these results also suggest the necessity of such recommendation which is subject to Korean women.
