Surgical Outcome of Adult Idiopathic Chiari Malformation Type 1

  • Objective : The pathophysiology of idiopathic Chiari malformation (CM) type 1 is disturbance of free cerebrospinal fluid (CSF) flow and restoration of normal CSF flow is the mainstay of treatment. Additional migration of the medulla oblongata in pediatric patients is referred to as CM type 1.5, but its significance in adult patients is unknown. This study is to compare surgical outcomes of adult idiopathic CM type 1.5 with that of type 1. Methods : Thirty-eight consecutive adult patients (M : F=11 : 27; median, 33.5; range, 18-63) with syringomyelia due to idiopathic CM type 1 were reviewed. Migration of the medulla oblongata was noted in 13 patients. The modified McCormick scale (MMS) was used to evaluate functional status before and one year after surgery. All patients underwent foramen magnum decompression and duroplasty. Factors related to radiological success (${\geq}50%$ decrease in the diameter of the syrinx) were investigated. The follow-up period was $72.7{\pm}55.6$ months. Results : Preoperative functional status were MMS I in 11 patients and MMS II in 14 of CM type 1 and MMS I in 8 and II in 5 of CM type 1.5. Of patients with MMS II, 5/14 patients in group A and 3/5 patients in group B showed improvement and there was no case of deterioration. Radiological success was achieved in 32 (84%) patients and restoration of the cisterna magna (p=0.01; OR, 46.5) was the only significant factor. Conclusion : Migration of the medulla oblongata did not make a difference in the surgical outcome when the cisterna magna was restored.

  • Background: The prevalence of extended-spectrum ${\beta}$-lactamase (ESBL)-producing Escherichia coli is increasing rapidly worldwide. Treatment options for ESBL-producing E. coli are limited, and infections caused by this organism are associated with improper antibiotic use, a long hospital stay, and increased mortality. Thus, the assessment and early recognition of the risk factors of nosocomial infections due to ESBL-producing E. coli are important for the infection control and proper treatment. Methods: A case-control study was performed that included nosocomial episodes of ESBL-producing E. coli bacteremia at a tertiary care hospital from January 2004 to December 2007. For each case patient, three controls were randomly selected and data on predisposing factors were collected. Results: Fifty-five cases of nosocomial ESBL-producing E. coli bacteremia were studied. Carbapenem usage (OR: 11.3, 95% CI: 1.1-115.9, p=0.041), quinolone usage (OR: 4.5, 95% CI: 1.1-18.8, p=0.042), biliary obstructive disease (OR: 11.8, 95% CI: 3.0-46.7, p<0.001) and the APACHE II score (OR: 1.3, 95% CI: 1.2- 1.5, p<0.001) were analyzed as independent risk factors of nosocomial ESBL-producing E. coli bacteremia. Conclusion: Our results showed that physicians caring for patients with risk factors of nosocomial bacteremia should consider ESBL-producing E. coli as the causative organisms of the disease.

설악산 국립공원 지역 토석류 발생가능성 평가 기법의 개발 (Development of a Prediction Technique for Debris Flow Susceptibility in the Seoraksan National Park, Korea)

  • 최근의 기후변화는 산지토사재해의 발생을 가속시키고 있다. 산지토사재해 중 토석류는 전파길이가 길고 매우 빠른 거동 특성을 갖기 때문에 매우 위협적으로 인식된다. 본 연구는 설악산 국립공원내 토석류 발생지 263개소를 대상으로 발생 길이(m)에 미치는 영향인자를 구명하고, 수량화이론(I)을 사용하여 발생 길이에 대한 각 산림환경 인자의 기여도 분석을 하여 예방적인 측면에서 국립공원내 토석류 발생 가능성지역에 대한 예측기법을 개발하였다. 각 산림환경 인자의 Range를 추정한 결과, 종단사면(0.9676)이 가장 높게 나타나 설악산 국립공원의 토석류 발생 가능성에 큰 영향을 미치는 것으로 추정되었으며, 다음으로는 횡단사면(0.6876), 고도(0.2356), 사면경사(0.1590), 방위(0.1364) 순으로 나타났다. 설악산 국립공원 산악지 토석류 발생 발생가능성 판정표를 기준으로 5개의 산림환경 인자의 category별 점수를 계산한 추정치 범위는 0점에서 2.1864점 사이에 분포하고 있으며, 중앙값은 1.0932점으로 토석류 발생가능성을 예측을 작성한 결과 I등급은 1.6399 이상, II등급 1.0932~1.6398, III등급 0.5466~1.0931, IV등급 0.5465 이하로 나타나 1등급, 2등급에서 토석류 발생 비율이 86.3%로서 높은 적중률을 보였다.

반폐쇄식 순환여과 사육시스템에서의 넙치 (Paralichthys olivaceus) 양식 (Culture of the Olive Flounder (Paralichthys olivaceus) in a Semi-closed Recirculating Seawater System)

  • 현재 유수식 방법으로 양식되고 있는 넙치(Paralichthys olivaceus)의 육상수조 양식에서 환수율, 월동시가온 및 양식배수 등의 문제를 해결하기 위하여, 실제 양식장에 설치한 반폐쇄식 순환여과 사육시스템을 이용하여 1992년 2월부터 1994년 1월까지 2년 동안(실험 I , 실험II) 넘치를 사육하면서 그 실용성을 연구하였다. 실험 I에서는 평균전장 7.5cm, 평균체중 3.4g, 실험II에서는 각각 5.0cm, 1.8g의 종묘를 사용하였다. 실험 I의 사육수 용존 무기태질소량은 $NH_4-N\;0.247-0.512ppm,\;NO_2-N\;0.010-0.043ppm,\;NO_3-N\;0.108-0.342ppm$ 실험II에서는 각각 0.091-0.715ppm, 0.002-0.045 ppm, 0,007-0.277 ppm 범위에서 변화하였다. 일간섭식률은 실험 I $0.67-2.41\%$, 실험II $0.69-2.22\%$, 사료효율은 각각 $34.8-59.8\%,\;40.5-88.4\%$였다. 실험 I에서의 어체는 사육 340일만에 전장 40.0-42.8cm, 체중 695.0-852.6g으로 성장하였고, 실험II에서는 사육 365일후에 전장 36.7-39.7cm, 체중 552.4-706.4g으로 자라났다. 사육종료시 어체의 평균 생존율은 실험 $192.0\%$, 실험II $96.0\%$였다. 어체의 체표면적이 수조바닥을 덮는율(covering rate)은 넙치의 수용밀도의 지표로 활용 가능하였다. 사육시 어체의 최대 수용량은 실험 I에서 덮는율 2.2, 중량 $34.1kg/m^2$였고, 실험II에서는 각각 $2.6,\;36.3kg/m^2$였다. 순환여과식 양식시스템은 현행의 유수식 양식시스템에 비해 양식 생산성 및 연안 환경오염 방지에 기여도가 높은 양식방법으로 평가되었다.

추계 광양만의 유기물 기원과 분포 특성 (Origin and Spatial Distribution of Organic Matter at Gwangyang Bay in the Fall)

  • 유기물 증가에 미치는 환경인자를 중심으로 그 수평분포 특성, 원인, 주요오염원의 영향권에 대하여 검토하기 위해 표층 해수와 표층 퇴적물을 조사하였으며, 그 결과를 요약하면 다음과 같다. 유기물변동에 영향을 미치는 환경인자에 대한 주요 오염원은 크게 섬진강과 동천 등의 담수, 광양시 생활하수, 여수산업단지로 크게 나누어볼 수 있었다. 해수와 퇴적물의 환경인자에 대한 수평분포 특성과 해수의 흐름 등을 고려하여 이들 주요 오염원의 영향권을 구분한 결과, (I) 섬진강 담수의 영향을 많이 받는 해역, (II) 광양시와 동천의 영향이 큰 해역, (III) 여수 산업단지의 영향을 많이 받는 해역으로 나누어졌다. 그리고, 오염원의 영향권 별 수질환경인자의 특성으로는 섬진강 담수의 영향을 많이 받는 해역은 낮은 염분, 높은 농도의 $NO_3-N$$SiO_2-Si$, 담수와 생환하수의 영향이 큰 해역은 낮은 염분, 높은 농도의 $NO_3-N,\;NH_4-N,\;SiO_2-Si$, 여수 산업단지의 영향을 많이 받는 해역은 표층해수의 경우 높은 수온, 높은 농도의 $NH_4-N$$PO_4-P$, 퇴적물의 경우 높은 농도의 $NH_4-N,\;PO_4-P,\;SiO_2-Si$로 특징 지울 수 있을 것으로 보인다.

  • European side impact analysis of an electric vehicle was done using the robust design method. In order to minimize VC as well as rib deflection, the injury response table which consists of rib deflection and VC response table has been introduced. The sensitivities and interactions are almost the same when it was compared with those of rib deflection and VC response table. Using internal energy of the factors, the starting time of dummy rib deflection and the contact average velocity, the internal energy and time-velocity response table were introduced. It is shown that the results of the new response tables have the similar characteristics to those of the Injury response table. It is suggested that the internal energy and time-velocity response table should be utilized to minimize injuries.

  • This study examined blends of styrene butadiene rubber(SBR) and chloroprene rubber(CR) prepared from an open 2-roll mill following the conventional polymer blend method for a wide range of the blend composition. Rubber vulcanizates were manufactured by hot press and then mechanical properties, heat and ozone resistance of the specimens were examined. Due to the post cure during the aging test, hardness of vulcanizates was increased. It was found that the undesirable characteristics of heat and ozone resistance of pure SBR was significantly improved through the blending of SBR with CR.

산업체 급식소에서 제공되는 고등어조림의 미생물적 품질 관리에 관한 연구(II) (A Study on Microbiological Quality & Safty Control of Hard-boiled Mackerel served by a Industry Foodservice Establishment (II))

  • This study was written to assess microbilogical quality by passage of time and holding method after making foods, by means of evaluating time, temperature and microbiological quality during vairous phases in product flow of hard-boiled mackerel served by an industry feeding operation for 500 persons a day, measuring pH & Aw and analyzing factors affecting nicrobiological growth conditions. The results were as fallows: 1. According to phases in product flow of hard-boiled mackerel, it showed 4.9 hours of mean of needed time, $27.5^{\circ}C$ of room temperature, 4.8~5.7 of pH value and 0.95~0.98 of Aw. these conditions were suitable for microbiological growth, and the phases with potential hygienic danger were pre-preparation and assembly & service. 2. As for holding methods and passage of time, holding as steam table was more effective than holding at room temperature as time past. 3. Food poisoning bacteria were not dectected from phases in product flow of hard-boiled mackerel.

디젤기관의 실린더내 유동 및 분무액적 거동의 수치적 연구(I) (A Study on the Numerical Analysis of Behavior of Spray Droplets and Internal Flow Field of Cylinder in Diesel Engine)

  • In this study, we calculated gas flow fields and distribution of fuel droplet and mass fraction using the CONCHAS-SPRAY code which modified to execute in IBM PC and changed three important factors, injection rate pattern (BASIC, I, II, III), different bowl shape and spray type. Especially vortices which be influenced by fuel-air mixing process, evaporation and flame propagation are generated more strongly in the bowl-piston type combustion chamber than in the flat-piston type. As the spray type changes, it is found that conical type produced large and strong vortices and fuel droplets are effictively diffused into the entire combustion chamber. As the injection rate pattern changes I, II, III based on BASIC type, we confirmed that End-of-Injection Effect strongly influence on droplets life time.

  • Optimum reactons for the preparation of extra-light magnesium carbonate from magnesium chloride and sodium carbonate solutions were found by observing the difference of crystalline shapes under an electromicroscope. Reaction temperature the and washing temperature were main factors affecting the crystalline shapes, and drying temperature was found to be of secondary importance. Optimum temperatures for reaction and washing ranged from $20^{\circ}C$ to $30^{\circ}C$ and the temperature over $40^{\circ}C$ should be avoided for the reaction and washing. It was found that the higher the drying temperature, the lighter the crsytal of the produced magnesium carbonate. Reaction time, molar ratio (Mg$^{2+}/CO_{3}^{2-}$ ) and the concentrations of magnesium chloride and sodium carbonate solutions have only a slight effect on the form of the product.
