골임파종의 치료효과판정을 위한 핵의학적 골스캔과 갈륨스캔의 비교 (Comparison of Radionuclide Bone and Gallium Scans in the Therapeutic Evaluation of Bone Lymphoma)

  • 완전 치유되었던 골임파종 환자에서 치료효과를 판정하기 위해 치료후 $^{99m}Tc$-MDP 스캔과 $^{67}Ga$ 스캔으로 추적검사한 35례를 후향적으로 분석하였다. $^{99m}Tc$-MDP 스캔은 35례중 29례에서 치료중, 33례에서 치료후 추적검사하였고, $^{67}Ga$ 스캔은 16 례중 13례에서 치료중, 15례에서 치료후 추적검사를 하였다. 핵의학적검사의 병적섭취농도의 분류는 4단계로 나누었는데 $^{99m}Tc$-MDP 스캔의 경우 정상 천장골관절 섭취농도와 같은 병변의 섭취농도를 3으로, $^{67}Ga$ 스캔의 경우 정상간의 섭취농도와 같은 병변의 섭취농도를 3으로 기준하였다. 치료중과 치료후 치료효과를 나타내는 소견은 $^{99m}Tc$-MDP 스캔의 경우 각각 66.0%(19/29), 72.7%(24/33)였으며 $^{67}Ga$ 스캔의 경우 각각 84.6%(l1/13), 86.7%(13/15)였다. 치료전, 치료중, 치료후의 병변 섭취농도는 $^{99m}Tc$-MDP 스캔의 경우 각각 3.06, 2.34, 1.75였으며, $^{67}Ga$ 스캔의 경우 각각 3.22, 1.42, 1.30이었다. 따라서 골임파종에서 치료효과를 판정하는 추적검사로는 $^{67}Ga$ 스캔이 $^{99m}Tc$-MDP 골 스캔보다 더 예민한 결과를 나타내었다.

악성 골 및 연부조직 종양에서 Tl-201 SCAN의 진단적 효능 - Tc-99m-MIBI 및 Tc-99m-MDP scan과의 비교 - (Thallium-201 Scan in Bone and Softtissue Sarcoma - Comparison with Tc-99m-MIBI and Tc-99m-MDP Scan -)

  • PURPOSE : The purpose of this study is to know the ability of detecting malignant tumor tissue by Tl-201 scan, and to compare with that of Tc-99m-MIBI and Tc-99m-MDP scan. MATERIAL AND METHODS : Between February 1994 and December 1995,38 unselected patients with various bone pathologies were studied prospectively. Eighteen had malignant bone and soft tissue pathologies, while twenty had benign. All patients were studied with Tl-201, Tc-99mMIBI and Tc-99m-MDP scan prior to surgical biopsy. PICKER Prism 2000 gamma camera with high resolution parallel hole collimator was used for scanning. To avoid the interaction of isotope, the early(30min.) and delayed phase(3hrs.) of Tl-20l scan was performed first and Tc-99m-MIBI scan was performed after 30 minutes, and then Tc-99m-MDP scan 48 hours later. The scan images were visually evaluated by a blinded nuclear medicine physician. We could find true positive, true negative, false positive and false negative by the comparison of results with those of biopsy. We calculated positive and negative predictive value(%), sensitivity(%), specificity(%) and diagnostic accuracy(%) of each scan. RESULT : The results of each scan were 85.7, 100, 100, 85, 92.1% in Tl-201, 81, 94.1, 94.4, 80, 86.8% in Tc-99m-MIBI and 50, 66.7, 88.9, 20, 52.6% in Tc-99m-MDP scan. As a conclusion, Tl-201 scan was the most specific and accurate method for detecting malignant tumor tissue. Tc-99m-MIBI scan was also good for malignant tumor searching. CONCLUSION : With our results, we can use Tl-201 scan to differentiate benign from malignant tumor, and to evaluate the response of preoperative chemotherapy or radiotherapy, and to determine the residual tumor or local recurrence. For the better result, we need to have a more detail information about false positive cases and a more objective and quantitative reading technique.

$^{99m}Tc-MDP$를 이용한 Bone scan 시간(時間)에 관(關)한 고찰(考察) (A Study of Bone Scan Time by Use of $^{99m}Tc-MDP$)

  • The bone scan reflects the metabolic reaction of bone to a disease process, whether neoplastic, traumatic or inflammatory. In normal bone tissue, the labeled diphosphate complexes accumulate in the perivascular fluid next to the marrow cavity at the interface between uncalcified and calcified bone matrix. HMDP has the most rapid plasma clearance among the commonly used diphosphonate followed by MDP and then HEDP. I have studied about bone scan time by use of $^{99m}Tc-MDP$, and got 336 images from 112 patients. The result obtained as follow; 1. Accumulation rate of $^{99m}Tc-MDP$ is higher than other age groups in below 30-year old group. 2. Accumulation rate, in 10year old group, is 75.85% on 120min. but other groups are most high on 180 min. 3. The density differants between bone and soft tissue increased with time. 4. Image contrast is good in younger group than old.

$^ {99m}Tc-MDP$를 이용한 개의 요골 골절 치유 과정의 평가 (Evaluation of Fracture Healing in Canine Radius by Bone Scan with $^ {99m}Tc-MDP$)

  • Bone scintigraphy with $^{99m}Tc-methylene$ diphosphonate(MDP) performed for 20 weeks per two weeks interval respectively after fracture in seven(male 3 heads, female 4 heads) canine radius were analysed. All of bone scans performed 2 weeks to 20 weeks after fracture showed increase in generalized tracer uptake and showed localized increase in tracer uptake at the fracture site. Bone scans and ratio performed 6 weeks after fracture showed the most outstanding increase in generalized and localized tracer uptake. New bone formation had been observed from 2 weeks and they were incorporated completely on 18 weeks, they showed most activity during 6-10 weeks after fracture. It was recognized that the bone scan with $^{99m}Tc-methylene$ was quite sensitivity but low specificity on the fracture healing in canine radius.

  • Elbow hygroma and hypertrophic osteodystrophy were diagnosed in Doberman referred to Veterinary Teaching Hosptial of Chungbuk national University. Physical examination, plain radiography, and bone scan were performed in the patient. The radiography revealed soft tissue swelling on the caudal region of the elbow and irregular radiolucent zone involving the metaphysis subjacent and parallel to the distal radial and ulnar physes. The bone scan of the forelimbs revealed increased uptake in region where the soft, fluid-filled mass on the elbow was present. But the radiolucent areas (the distal radial and ulnar physes) were seen normal bone uptake. Therefore, hypertrophic osteodystrophy ruled out the diagnosis. $^{99m}Tc-MDP$ scan is suitable to routine clinical use for soft tissue inflammation and bone lesions detection.

  • Purpose: Tc-99m MDP bone scan was performed to evaluate a generalized bone pain in a 24-year-old male chronic myelogenous leukemia patient who received bone marrow transplantation at 7 months ago. The patient had received large amounts of blood transfusion for managing symptoms related to anemia. Bone scan revealed substantial splenic tracer uptake. Magnetic resonance image and laboratory evidence of hemochromatosis suggests that the presence of large quantities of iron in the spleen of this patient may have been responsible for the splenic uptake of the bone scanning agent. The authors report a case of splenic hemochromatosis incidentally found on Tc-99m MDP bone scan.

  • A 24 year-old female presented for a $^{99m}Tc$-methylene diphosphonatae (MDP) whole body bone scan due to chronic pain in the bilateral lower extremities that has aggravated since 2002. She was diagnosed with Camurati-Engelmann disease (CED) based on the clinical and radiological findings in 2002, and she re-visited our institute to evaluate disease status at this time. CED is a rare autosomal dominant type of bone dysplasia characterized by progressive cortical thickening of long bones, and narrowing of medullary cavity, and thus presents with typical clinical symptoms and signs such as chronic pain in the extremities, muscle weakness, and waddling gait. On the $^{99m}Tc$-MDP bone scan performed to evaluate disease status, intense increased uptake was seen in the skull, facial bones, bilateral scapulae, bilateral long bones, and bilateral pelvic bones, which clearly demonstrated the extent of CED involvement.

  • Although the exact mechanism is not clearly understood yet, there were examples of the uptake of radiopharmaceuticals to the soft tissue lesions with $^{99m}Technetium$ methylene diphosphonate bone scan. Recently, we experienced two cases of neuroblastoma of which primary sites were imaged with $^{99m}Tc-MDP$ bone scan preoperatively and could make the diagnosis. So, we report here that $^{99m}Tc-MDP$ bone scan is the reasonable method not only to find out the bone metastasis, but also to diagnose the neuroblastoma preoperatively.

  • The $^{99m}Tc-phosphate$ compounds, of which methylene diphosphonate is most widely used, were originally introduced as bone scanning agents intended primarily to detect metastatic bone deposit. They may also accumulate in extraskeletal sites including breast. Recently we had the opportunity to observe one case of cystosarcoma phyllodes that was confirmed by tissue biopsy and demonstrated diffuse extraskeletal uptake of $^{99m}Tc-MDP$ in the breast on $^{99m}Tc-MDP$ bone scan in the breast on $^{99m}Tc-MDP$ bone scan. So we present this case with the brief review of literatures.

  • A case of extraosseous uptake of ureteral transitional cell Cancer on $^{99m}Tc-MDP$ bone scan was experienced. The Hot spot seemed to be caused by the tumor uptake of $^{99m}Tc-MDP$ ifself.
