  • PET of the cerebral metabolic rate of glucose is increasingly used to support the clinical diagnosis in the examination of patients with suspected major neurodegenerative disorders, such as Alzheimer's disease. $^{18}F-FDG$ PET has been reported to have high diagnostic performance, especially, very high sensitivity in the diagnosis and clinical assessment of therapeutic efficacy. According to clinical research data hitherto, $^{18}F-FDG$ PET is expected to be an effective diagnostic tool in early and differential diagnosis of Alzheimer's disease. Since 2004, Medicare covers $^{18}F-FDG$ PET scans for the differential diagnosis of fronto-temporal dementia (FTD) and Alzheimer's disease (AD) under specific requirements; or, its use in a CMS approved practical clinical trial focused on the utility of $^{18}F-FDG$ PET in the diagnosis or treatment of dementing neurodegenerative diseases.

  • Adrenal tumors are increasingly detected by widespread use of anatomical imaging such as a, MRI, etc. For these adrenal tumors, differentiation between malignancy and benignancy is very important. In diagnostic assessment of adrenal tumor, $^{18}F-FDG$ PET and PET-CT have been reported to have high diagnostic performance, especially, very excellent performance in evaluation of adrenal metastasis in the oncologic patient. In cases of adrenal incidentalomas, $^{18}F-FDG$ PET or PET-CT is helpful if a or chemical-shift MRI is inconclusive. $^{18}F-FDG$ PET and PET-CT may be applied to the patients with MIBG-negative pheochromocytomas. In summary, $^{18}F-FDG$ PET and PET-CT are expected to be effective diagnostic tools in the management of adrenal tumor.

  • $^{18}F-FDG$ PET showed a high sensitivity and specificity in the initial staging of malignant melanoma, and it also predicted the prognosis correctly. In addition, it had a higher sensitivity and specificity in the detection of recurrence and restaging than conventional imaging modalities. Meanwhile, there's less clinical evidence to support the use of $^{18}F-FDG$ PET in the response evaluation for chemotherapy and the diagnosis/differential diagnosis of malignant melanoma.

  • Salivary gland tumors are relatively rare, constituting 3% of all head and neck neoplasms. In patients with salivary gland malignancies, $^{18}F-FDG$ PET is clinically useful in initial staging, histologic grading, and monitoring after treatment. According to clinical research data hitherto, $^{18}F-FDG$ PET is expected to be an effective diagnostic tool in the management of salivary gland tumors.

  • Nonmelanomatous skin cancer includes basal cell carcinoma, squamous cell carcinoma, merkel cell carcinoma and dermatofibrosarcoma protuberance. So far, there have been a few reports that $^{18}F-FDG$ PET was useful in the evaluation of metastasis and therapeutic response in nonmelanomatous skin cancer, however, those are very weak evidences. Therefore, further studies on the usefulness of $^{18}F-FDG$ PET in nonmelanomatous skin cancer are required.

  • Diagnosis of primary origin site in the management of malignancy of unknown origin (MUO) is the most important issue. According to the histopathologic subtype of primary lesion, specialized treatment can be given and survival gain is expected. F-18 FOG PET (PET/CT) has been estimated as useful in detection of primary lesion with high sensitivity and moderate specificity. F-18 FDG PET (PET/CT) study before conventional studies is also recommended because it has high diagnostic performance compared to conventional studies. Although there has few data, F-18 FDG PET (PET/CT) is expected to be useful in diagnosis of recurrence, restaging, evaluation of treatment effect, considering that PET (PET/CT) has been reported as useful in other malignancies.

두경부 편평상피암 환자에서 수술 전 경부림프절 전이 평가에 대한 F-18 FDG PET/CT의 정확도: CT/MRI와의 비교 (Accuracy of F-18 FDG PET/CT in Preoperative Assessment of Cervical Lymph Nodes in Head and Neck Squamous Cell Cancer: Comparison with CT/MRI)

  • 목적: 두경부 편평상피암 환자에서 경부림프절전이에 대한 F-18 FDG PET/CT의 진단성적을 평가하고 CT/MRI와 비교하였다. 대상 및 방법: 수술전 조직검사로 편평상피암으로 진단받은 17명(남: 15명, 여: 2명, 평균나이: $59{\pm}11$세)의 환자를 대상으로 F-18 FDG PET/CT를 시행하였다. 경부림프절의 F-18 FDG섭취 정도는 세 등급(1=배후방사능, 2=간방사능보다 낮거나 같은 경우, 3=간방사능보다 높은 경우)으로 나누었고, 3등급일 때 전이로 진단하였다. 경부림프절전이에 대한 F-18 FDG PET/CT의 예민도와 특이도를 CT/MRI와 비교하였다. 결과: 17명의 환자에서 123개의 경부림프절이 절제되었고 이 중 29개가 전이로 확진되었다. F-18 FDG PET/CT의 예민도는 69%(20/29), 특이도는 99%(93/94), CT/MRI의 예민도는 62%(18/29), 특이도는 96%(90/94)로 경부림프절전이에 대한 두 검사의 진단성적은 유의한 차이가 없었다. 그러나 F-18 FDG PET/CT는 각각의 환자에서 간에 생긴 이차성 원발암과 늑골전이를 발견할 수 있었다. 결론: 두경부 편평상피암에서 경부림프절 전이에 대한 F-18 FDG PET/CT의 특이도는 매우 높았지만 예민도는 비교적 낮았고 CT/MRI와 유의하게 다르지 않았다. F-18 FDG PET/CT의 비교적 낮은 예민도의 원인은 최근 많이 절제되고 있는 잠재 전이의 가능성이 있는 작은 경부림프절을 발견하지 못하는 F-18 FDG PET/CT 의 한계 때문이었다. 그러나 F-18 FDG PET/CT는 이차성 원발암과 원격전이를 부가적으로 평가할 수 있어 환자의 치료 방침 결정에 유용하였다.

F-18 FDG PET에서 높은 포도당 섭취를 보인 식도 평활근종 예 (A Case of Esophageal Leiomyoma Showing High FDG Uptake on F-18 FDG PET)

  • An esophageal leiomyoma is the most common benign tumor of the esophagus mainly occurred in intramural portion. Occasionaly, it is difficult to discriminate esophageal malignancy from large leiomyoma. Although F-18 FDG PET has been used for differentiating malignant from benign disease, false-positive cases have been reported. Recently, uterine leiomyoma has been reported to have relatively high F-18 FDG uptake in some patients but little is known about how an esophageal leiomyoma might be showed on F-18 FDG PET. We report a case of esophageal leiomyoma that showed high FDG uptake on PET images.

Case Report of Prostate Cancer Patient with Only Lymph Node Involvement on F-18 FDG PET/CT

  • We report a case of a patient with locally advanced prostate cancer who had only lymph node involvement without bone metastasis on F-18 FDG PET/CT. A 62-year-old Korean male was admitted to our hospital due to dysuria. His PSA level on admission was 79.35 ng/mL. A transrectal ultrasound-guided prostate biopsy confirmed prostate cancer and his Gleason score was 10 (5+5). F-18 FDG PET/CT demonstrated a hypermetabolic mass lesion with SUVmax 7.0 in the prostate and hypermetabolism with SUVmax 4.7 of the abdominal and pelvic lymph nodes. Tc-99m HDP bone scan showed no significant bone metastasis. The patient underwent hormonal therapy for 9 months. Follow-up F-18 FDG PET/CT showed significantly reduced size and FDG uptake in the prostate and abdominal and pelvic lymph nodes. In this case, treatment monitoring with F-18 FDG PET/CT showed decreased mass size and FDG uptake in the prostate and abdominal and pelvic lymph nodes.

18F-FDG PET/CT검사에서 지연영상을 이용한 갑상선암 진단의 유용성 평가 (Evaluate Utility of Thyroid Cancer Discrimination by 18F-FDG PET/CT Delay Scan Images)

  • 목적: 본 연구는 $^{18}F$-FDG PET/CT검사 시 우연히 발견된 갑상선병변의 악성여부를 평가하기 위하여 위양성으로 판독되어지는 양성종양이나 염증질환과 악성 종양의 감별이 가능한 검사법으로 지연영상(Delay scan)을 적용하고 결과 값(SUV)의 기준안을 마련하며 이 연구에서 제시되는 검사법의 유용성을 평가하는데 있다. 재료 및 방법 : 2008년 1월부터 2008년 4월 까지 E대학병원에 내원하여 $^{18}F$-FDG PET/CT를 시행한 환자 25명(1명 제외)을 대상으로 후향적 분석을 실시하였다. $^{18}F$-FDG PET/CT영상 촬영은 SIEMENS사에서 만든 Biograph-Duo에서 시행하였으며, $^{18}F$-FDG PET/CT 정상영상(1hr)을 취득한 후 갑상선이상섭취가 있고 SUV 2.0이상인 환자를 대상으로 1시간후에 갑상선부위를 1bed 지연영상을 촬영하였고, 한 bed에 2분 동안 영상을 얻었다. 갑상선이상섭취가 있고 SUV 2.0이상인 환자를 대상으로 이상섭취가 있는 부위의 1시간영상과 2시간지연영상의 maxSUV를 측정하여 비교분석하였다. 결과: $^{18}F$-FDG PET/CT 검사 시 우연히 갑상선에 $^{18}F$-FDG 섭취를 보인 환자 24명중 갑상선 암(Thyroid cancer)이 5명으로 20.8%로 나타났으며 모두 양성병변을 시사하는 소견을 나타냈다. 갑상선기능이상이나 갑상선염 등 단순 갑상선 이상섭취로 판독된 19명의 1시간영상과 2시간 영상의 표준섭취계수(SUV)값은 나타낸 표로 1시간영상의 $^{18}F$-FDG 표준섭취 평균값은 5.06maxSUV이었고, 2시간 평균값은 5.23maxSUV였다. 측정된 두 값의 차이는 0.19maxSUV이었다. 갑상선암으로 판독된 5명의 1시간 PET/CT검사에서 갑상선국소부위의 $^{18}F$-FDG 표준섭취 평균값은 9.63maxSUV 이었고 2시간 평균값은 10.65maxSUV이였다. 두 값의 차이는 1.02maxSUV로 나타났다. 결론 : 본 연구는 우연히 발견된 갑상선 국소부위에 5.0이상 maxSUV의 이상섭취병변이 발견된 경우에는 $^{18}F$-FDG PET/CT 지연영상을 추가로 검사한다면 병소의 악성 여부 감별에 도움을 줄 수 있는 유용한 촬영방법임을 확인할 수 있었다.
