Cow`s Milk Protein-specific IgE Concentrations in Two Age Groups of Children with Cow`s Milk Allergy

Background : About 70-$80{\%}$ of children with cow`s milk allergy(CMA) become outgrown clinically by the age of 3 years. Casein, one of the three major cow`s milk proteins(casein,$\beta$-lactoglobulin(BLG), $\alpha$-lactoalbumin(ALA)) has been reported to play an important role in the persistence of CMA. The aim of this study was to determine different effects of causative milk proteins on the persistence of CMA between two age groups. Methods : A total of 65 patients with CMA were enrolled in this study. Their cow`s milk-specific IgEs were positive(≥0.7 U/ml by Pharmacia CAP). After dividing 65 patients into two age groups, under the age of 3 years and over 3 years(persistent CMA), we compared the levels of casein-, BLG- and ALA-specific IgE antibodies between the two groups. Results : There were 44 patients in the group of less than 3 years of age and 21 patients in the group of more than 3 years of age. The concentrations of the specific IgE antibodies to casein, BLG and ALA were not significantly different between the two groups. However, although statistically insignificant, those more than 3 years of age had higher mean values of casein-specific IgE antibodies and lower mean values of whey protein(BLG and ALA)-specific IgE antibodies compared with those less than 3 years of age. A single dominant allergenic milk protein was not identified within either of the two age groups, but the concentrations of the casein-specific IgE antibodies in children with more than 3 years of age tended to be higher than those of whey protein-specific IgE antibodies. Conclusion : Although statistically insignificant, the concentrations of the casein-specific IgE antibodies were higher in the group of more than 3 years of age than in the younger group. Moreover, the concentrations of the casein-specific IgE antibodies in children more than 3 years of age tended to be higher than those of whey proteins. These findings implicate that casein plays a certain role in the persistence of CMA.

나이에 따른 우유 알레르기 환아의 우유단백 특이 IgE 항체 비교

목 적 : 우유 알레르기는 2세 미만 소아에서 1-$3{\%}$의 발생률을 보이는 질환으로 3세경까지 약 70-$80{\%}$가 호전을 보이는 것으로 알려져 있다. 카제인과 $\beta$-lactoglobulin(BLG), $\alpha$-lacto-albumin(ALA)이 주요 원인 우유단백으로 알려져 있고 그 중 카제인은 우유 알레르기의 지속에 중요한 것으로 보고되고 있다. 이에 대한 연구가 국내에서는 미미한 상태이다. 저자는 우유 알레르기 환아를 3세 미만(일시적 우유 알레르기의 가능성이 높은 환아)과 3세 이상(지속적 우유 알레르기 환아) 두 군으로 나누어 원인 우유단백의 차이를 알아보고자 하였다. 방 법 : 우유 특이IgE항체 검사(Pharmacia CAP FEIA)상 양성(≥0.7 U/ml)인 18세 이하의 우유 알레르기 환아 65명을 대상으로 하였다. 대상 환아를 3세 미만과 3세 이상 두 군으로 나누어 이들의 임상 양상과 3가지 우유단백(casein, BLG, ALA) 특이IgE항체가를 비교하였다. 결 과 : 3세 미만군은 44명(평균 나이 : 11개월)으로 남아가 23명이였으며. 3세 이상군은 21명(평균 나이 : 5년 4개월)으로 남아가 17명이였다. 총 IgE는 3세 이상 군에서 높게 나타났으나 우유 특이IgE항체가는 두 군 간에 차이를 보이지 않았다. 3가지 우유단백(casein, BLG, ALA) 특이IgE항체가와 양성률을 비교 시 모두 두 군간에 유의한 차이를 보이지 않았다. 하지만 평균값만 비교 시 유청 단백(BLG와 ALA) 특이IgE 항체가는 3세 이상군에서 3세 미만군보다 낮은반면, 카제인 특이IgE항체가는 3세 이상군에서 더 높은 경향을 보였다. 두 군 모두에서 통계적으로 유의한 단일 주요 원인 우유단백은 없었으나 3세 이상군에서 카제인 특이IgE항체가가 유청 단백에 비해 높은 경향을 보였다. 두 군 모두 대부분에서 1가지 이상의 우유단백 특이IgE항체가 존재하였으며 약 50%의 환아는 3가지 우유단백 모두에 대한 특이IgE항체가 존재하였다. 결 론 : 3가지 우유단백 특이IgE항체 모두 두군 간에 유의한 차이를 보이지 않았고 두 군 모두에서 유의한 단일 주요 원인 단백은 없었다. 하지만 카제인 특이IgE항체 농도는 유청 단백 특이IgE항체와 달리 지속적 3세 이상군에서 3세미만군보다 더 높았으며, 3세 이상군에서는 카제인 특이IgE항체가가 유청 단백에 비해 높은 경향을 보였다. 이는 카제인이 우유 알레르기의 지속에 어느 정도 관여하고 있을 가능성을 시사하는 소견이라고 할 수 있겠다.
