Factors Influencing Health Promotion Behavior in Patients with Pulmonary Tuberculosis

This study was conducted to identify the health promotion behavior of patients with pulmonary tuberculosis and examine the factors influencing this behavior. The subjects were 152 outpatients who were treated for pulmonary tuberculosis at the national tuberculosis hospital located in M City. Data were collected from July to September, 2016 and analyzed using the t-test, ANOVA, Duncan's test, Pearson's correlation coefficient, and multiple regression analysis. Health promotion behavior was negatively correlated with physical symptoms, perceived barriers, and depression, but positively correlated with self-efficacy. The most powerful variable influencing health promotion behavior was depression and the model showed an explanatory power of 63.5%. The results of this study suggest that it is necessary to employ a variety of strategies to practice and maintain the health promotion behavior of pulmonary tuberculosis patients to decrease their depression and provide psychological support.

폐결핵 환자의 건강증진행위와 관련요인

본 연구는 폐결핵 환자의 건강증진행위에 대한 관련요인으로서 신체적 증상, 지각된 장애성, 자기효능감, 우울 간의 상관관계를 확인하고 대상자의 건강증진행위에 미치는 영향을 파악하기 위해 수행되었다. 연구대상자는 전라남도 M시에 소재한 일개 국립병원 외래를 통해 치료를 받고 있는 폐결핵 환자 152명이었으며, 자료수집은 2016년 7월부터 9월까지 실시하였다. 자료분석은 기술통계 분석과 독립표본 t검정, 일원배치분산분석, Duncan test, Pearson's correlation analysis, 다중회귀 분석을 실시하였다. 연구결과, 대상자의 건강증진행위 점수는 총 5점 만점에 평균 $3.29{\pm}0.87$점이었고 영양, 운동, 휴식/스트레스 관리, 대인관계, 건강관리, 자아실현의 6개 하위영역에 따라 분석한 결과, 자아실현 점수가 $3.50{\pm}0.88$점으로 가장 높았고 건강관리 $3.35{\pm}0.84$점, 대인관계 $3.30{\pm}0.95$점, 영양 $3.19{\pm}0.98$점, 휴식/스트레스 관리 $3.18{\pm}0.95$점, 운동 $3.14{\pm}1.00$점 순이었다. 신체적 증상 점수는 총 4점 만점에 평균 $1.91{\pm}0.65$점이었고, 지각된 장애성 점수는 총 5점 만점에 평균 $2.90{\pm}1.20$점, 자기효능감 점수는 총 5점 만점에 $3.11{\pm}1.02$점, 우울 점수는 총 5점 만점에 평균 $2.85{\pm}0.70$점이었다. 신체적 증상과 지각된 장애성, 우울은 건강증진행위와 유의한 부정적 상관관계를 보였고, 자기효능감은 건강증진행위와 정적 상관관계를 보였다. 대상자의 건강증진행위에 영향을 미치는 주요 변인은 우울로 63.5%의 설명력을 나타냈다. 따라서 폐결핵 환자의 건강증진행위를 향상시키기 위해서는 대상자의 우울수준을 파악하고 우울을 감소시킬 수 있는 중재프로그램의 개발이 요구된다.
