Esosinophilic Gastroenteritis with Clostridium difficile-associated Colitis: A Case Report

Eosinophilic gastroenteritis is an uncommon disease characterized by eosinophilic infiltration in the bowel wall and presents various symptoms depending on the affected site and bowel layer. Pseudomembranous colitis is an antibiotic-associated infection caused by abnormal overgrowth of the toxin-producing Clostridium difficile in the large bowel. A 16-year-old boy was admitted with abdominal pain for 6 days. On admission, we performed an endoscopy and diagnosed gastroduodenitis. Then, we prescribed gastritis medication but he still presented with diffuse abdominal pain and fever above $38.0^{\circ}C$ after admission. We considered infectious enterocolitis, so we prescribed an antibiotic. The next day, he presented with bloody diarrhea. A diagnosis of pseudomembranous colitis was confirmed by a colonoscopic examination with a biopsy. We also obtained a diagnosis of eosinophilic gastroenteritis through a histological diagnosis. This is the first case of the simultaneous occurrence of eosinophilic gastroenteritis and pseudomembranous colitis in the Korean medical literature. We report this case with a brief review of the literature.

위막성 대장염을 동반한 호산구성 위장관염 1예

호산구성 위장관염은 드물게 발생하는 질환으로 호산구가 위장관의 장벽 침습을 특징으로 하고 침범 부위와 장벽의 층에 따라 다양한 임상 양상을 보인다. 위막성 대장염은 항생제와 관련되어 발생하는 감염성 질환으로 독소를 분비하는 Clostridium difficile의 과도한 증식으로 발생한다. 16세 남아가 약 6일간의 복통으로 본원에 내원하였다. 상부위장관내시경 검사를 시행하여 급성 위장관염으로 진단 후 약물치료를 했으나, 내원 당일 증상은 호전되지 않고 추가적으로 열이 $38.0{^{\circ}C}$ 이상으로 관찰되어 경험적 항생제를 한차례 사용한 수시간 후 혈성 설사가 발생하였다. 상부위장관내시경 검사시 시행한 조직검사 결과 호산구성 위장관염이 진단되었으며, 대장내시경검사 및 조직 검사를 통해 위막성 위장관염이 진단되었다. 아직 호산구성 위장관염에 위막성 장염이 동반된 증례 는 보고되지 않았다. 이에 문헌 고찰과 함께 증례를 보고한다.
