Clinical experience with amitriptyline for management of children with cyclic vomiting syndrome

Purpose : To report the clinical experience with amitriptyline for managing children with cyclic vomiting syndrome (CVS). Methods : Totally, eleven children (eight males) were diagnosed with CVS. Of these, medical records were reviewed for eight children treated with amitriptyline; three children were not treated because one was not followed up and two were kept under observation to study the natural course without medication, because the frequency of vomiting had recently decreased to less than one episode per year. The eight amitriptyline-treated children were divided into three groups by the therapeutic response: (1) complete remission (CR), with no recurrence of symptoms after treatment; 2) partial remission (PR), in which the frequency of symptoms decreased by almost 50% after treatment; and (3) no response. Results : For the eight amitriptyline-treated children, the mean age of symptom onset was 4.7 (0.3-7) yr and mean age at diagnosis was 8.9 (6.0-11.3) yr. The mean number of vomiting episodes was 8.8 (2-25) per year, and the duration of vomiting in each episode ranged from 3 hours to 5 days. CR was achieved in five (62.5%) of the eight amitriptyline-treated children (0.2-0.8 mg/kg/day orally, at night) and PR was achieved in three children (37.5%). Two children relapsed on discontinuation of amitriptyline by their parents decision but showed improvement on remedication. No adverse effects were noted throughout the study period. Conclusion : Amitriptyline can be used in long-term prophylaxis for the management of children with CVS aged over 6 yr. However, a large-scale study is needed to ascertain its effects.

주기성 구토 증후군 소아에서 구토 예방을 위한 amitriptyline의 임상 경험

목 적 : 뇌신경계 질환이나 소화기계 질환을 포함한 기질적 질환 없이 주기적으로 발작적 구토를 보이는 주기성 구토 증후군(CVS, cyclic vomiting syndrome)의 증상 예방 요법에 관한 국내 연구가 거의 없는 실정이다. 본 연구는 저용량의 삼환계 항우울제인 amitriptyline을 투여하여 CVS 환자에서 구토 발생의 장기적인 예방 효과가 있는지 알아보고자 하였다. 방 법 : 총 11명(남아 8명)의 CVS 환자 중 8명(남아 6명)의 환자들에게 amitriptyline을 투약했고 이들의 의무기록을 후향적으로 분석하였다. 11명 중 구토의 빈도가 연 1회로 감소 중인 2명은 투약 없이 경과 관찰 중에 있고 한 명은 CVS 진단 후 외래 추적이 중단되어 분석에서 제외하였다. Amitriptyline 투여 군을 반응도에 따라 세 군으로 분류하고 특성을 분석하였다. 1) 완전 관해-치료 후 증상의 재발이 2주기 이상 없음. 2) 부분 관해-1주기 간 증상 재발이 없거나 증상의 빈도, 정도가 50% 이상 감소한 군. 3) 무반응-호전 정도가 50% 미만이거나 호전 없음. 결 과 : 총 8명의 환자들의 증상이 발현된 시작 연령은 평균 4.7세(0.3-7), 진단 당시 평균 나이는 8.9세(6-11.3), 증상 발현 후 진단될 때까지 걸린 기간은 평균 4.3년(2-10.5), 증상 발현 후 amitriptyline 투여를 시작할 때까지 걸린 기간은 평균 4.9년(2-10.5)이었다. 연간 발작적 구토의 에피소드는 평균 8.8회(2-25)이었으며 구토 발생시 구토 지속 시간은 3시간에서 5일이었다. 발작적 구토가 있을 때 동반되는 증상으로는 편두통이 2명(25%), 복통이 6명(75%), 어지러움이 2명(25%)에서 있었고 편두통 가족력이 있는 환자가 2명 있었는데 이들은 환자 본인도 구토 에피소드 동안 두통과 복통이 동반되었다. 검사상 위배출 지연이 있는 환자는 3명이었고 이들은 저용량 erythromycin을 함께 단기간 투약하였으며, 요소 호기 검사에서 양성을 보인 환자 1명과 부비동염이 있던 환자 1명은 항생제 치료를 단기간 병행하였다. 총 8명의 환자들 중 amitriptyline 예방 요법 후 증상의 완전 관해를 보인 환자는 5명(62.5%)이었고 부분 관해를 보인 환자는 3명(37.5%)이었으며 무반응자는 없었다. 8명 중 2명은 보호자가 임의로 약물을 중단하였다가 증상 재발을 경험하였으며 재투약 후 각각 완전 관해 및 부분 관해를 보였다. 8명 중 부분 관해군 환자 1명과 완전 관해군 환자 1명은 외래 추적이 중단되었고 1명은 17개월 투약 후 증상이 없어 투약을 중단하였으며 현재까지 경과가 양호하다. 나머지 5명은 현재까지 평균 23.6개월(12-31) 동안 예방 요법 지속 중에 있다. 특별한 부작용이 나타난 환자는 없었다. 결 론 : 본 연구의 대상이었던 CVS 환자 모두에서 amitriptyline은 구토 증상의 장기적인 예방 효과를 보여 amitriptyline을 6세 이상 CVS 환자에서 예방 요법으로 시도해 볼 수 있을 것으로 생각되나 대상 환자군이 적고 대조군이 없는 관계로 향후 더 큰 규모의 무작위 연구가 필요할 것으로 생각한다.
