Aim & Scope

KJM aims to contribute to the cure of diseases and to promote human health through sharing scientific information on the latest developments in internal medicine among the society members and researchers from other international societies. Its focus covers clinical and basic research, which bridges the laboratory and clinical practice in internal medicine. Its regional focus is mainly Korea, but it also welcomes submissions from physicians all over the world. The language of the main text is in Korean or English; while, the abstract is provided in English. The publication type includes original articles, case reports, reviews, editorials, and specialized serial articles. It is published bimonthly, and supplementary issues may be published.

    The relief system for adverse drug reactions is a reimbursement system for non-fault injury from drugs that started with the intent of relieving victims of adverse drug reactions despite normal use of the medicines. In Korea, the relief system for adverse drug reactions started on December 19, 2014. To date, the deliberation process for 100 cases of adverse drug reactions has ended, of which 78 cases received relief reimbursement. As this is the early phase of system implementation, efforts to stabilize the system are needed. It is very important for clinicians to participate actively in mediating between the victims of adverse drug reactions and the Korea Institute of Drug Safety & Risk Management, to establish an effective relief system. It is also important to implement the most favorable relief system considering the socioeconomic and medical environment in Korea.

    Despite advances in cancer therapy, gastric cancer has a poor prognosis and high cancer-related mortality. Based on the molecular characteristics of cancer, specific targeted therapies have shown clinical benefits for various tumors. In addition, immunotherapy using immune checkpoint inhibitors has led to a paradigm shift in cancer treatment and shown remarkable results in some solid tumors. Although immunotherapy has been actively applied to gastric cancer, the efficacy is unsatisfactory compared with other solid tumors, such as melanoma and lung cancers. This is because of the complex mechanism of gastric cancer, tumor heterogeneity, heterogeneity among patients, and the absence of appropriate biomarkers to predict response. An effective new cancer treatment strategy that combines targeted therapies and various immunotherapies based on biological markers such as tumor mutation burden and microsatellite instability is urgently needed. Furthermore, customized treatment is necessary to overcome tumor heterogeneity.

    The 2017 Clinical Guidelines for ST-segment elevation myocardial infarction of the European Society of Cardiology updated many important aspects of the pre-hospital phase, the selection of a reperfusion strategy, interventional and pharmacological treatment, and patient quality assessment. The principal changes are a clear definition of the first medical contact, elimination of the door-to-balloon time from consideration, recommended radial access intervention, a recommendation that drug-eluting stents should be preferred to bare metal stents, recommended complete revascularization during hospitalization, and early discharge of selected patients. Routine manual thrombus aspiration and oxygen supplementation for patients who are not hypoxemic are now discouraged.

    비알콜성 지방간은 최근에 우리나라에서도 매우 급격하게 유병률이 증가하고 있으며, 이 중 진행된 비알코올성 지방간염 및 섬유화가 동반되어 있을 가능성이 높은 환자를 선별하여 정밀하게 검사하고 치료를 적용하는 것이 요구된다. 현재까지 비알코올성 지방간염을 진단할 수 있는 가장 정확한 표준 검사법은 간 조직검사이지만 환자 및 의사 모두에게 쉽게 선택할 수 있는 검사법이 아니기 때문에, 간 조직검사를 대신하여 비침습적으로 비알코올성 지방간염의 진단 및 조직학적 중증도를 평가하고자 하는 노력이 영상의학 검사 및 생화학 검사 분야를 중심으로 진행되고 있다. 간과해서는 안 되는 중요한 문제는 지방간염 진단기준이 서양에서 도입된 방식을 따라가다 보니 진단에 요구되는 여러 가지 구성요소를 유전적, 환경적으로 다른 우리나라와 같은 동양인에도 획일적으로 적용되고 있다는 점이며 조직검사의 적절한 시기 및 병리 판독자 간의 일치도, 치료의 올바른 선택에 대하여 보다 적절한 근거가 필요한 실정이다.

    목적: 과학적 근거 부족과 비표준화된 프로토콜로 인하여, 의료인이 개인 보호구를 정확하게 사용하는 것이 쉽지 않다. 개인 보호구 탈의 과정에서 발생하는 오염을 줄이기 위한 실험적 근거를 마련하기 위하여 본 시뮬레이션 연구를 진행하였다. 대상 및 방법: 서울대학교 생명윤리위원회 승인 하에 10명의 감염관리팀장급 실무자에게 자문을 구하여 확인이 필요한 개인 보호구 사용 프로토콜 차이 항목을 결정한 후, 4명의 표준화된 의료인을 선발하여 시뮬레이션을 통해 방법과 단계별 개인 보호구 사용에 있어 오염 정도와 경위를 확인하였다. 개인 보호구 사용상의 오염 확인은 형광 파우더가 뿌려진 환자모형을 상대로, 1분 이상의 직접적인 접촉이 요구되는 간호 행위를 하게 한 후, 각 탈의 단계마다 소등 후 자외선 전등으로 확인하였다. 형광 파우더로 오염된 부위는 디지털 카메라로 사진을 찍고, 모든 개인 보호구 착탈의 과정은 태블릿 컴퓨터로 녹화하였다. 수집된 모든 데이터는 부여된 고유한 연구실험번호로 연동하여 각 사례 분석을 실시하였다. 결과: 2016년 12월 14-20일 동안 4명의 표준의료인이 총 38개의 시뮬레이션 실험을 실시한 결과, 프로토콜 간의 상이한 착탈의 방법에 따른 오염 발생 결과에 큰 차이는 없었다. 대신, 신체 사정과 같이 1분 이상 환자 모형과 직접적인 접촉을 하게 한 직후, 보호복 앞면과 소매 아랫 부분이 심하게 오염된 것을 확인하였다. 또한, 소독 티슈로 겉장갑을 닦은 후에도 오염이 완전하게 제거되지 않았으며, 손가락 사이, 마디, 끝 부분 등에 주로 오염이 남아 있었다. 여러 N95 마스크 종류 중에서는, 접이형 N95 마스크가 벗는 끈 순서와 관계없이 안정적인 제거가 가능하여 오염 가능성이 가장 적은 것으로 나타났다. 결론: 개인 보호구 착탈의시 오염 발생에 대한 시각적 증거를 제공한 본 연구 결과에 근거하여, 전신 보호복 위에 1회용 긴팔 앞치마 사용을 권고하는 등 몇 가지 지침의 제시가 가능해졌다. 이러한 권고사항에 대한 후속 연구가 필요하다.

    Gallbladder (GB) cancer is relatively rare and has a poor prognosis, with a median survival time of less than 3 months. It is resistant to chemotherapy. Therefore, the role of systemic chemotherapy is limited. However, administering the anticancer agent directly into the hepatic artery can result in a higher drug concentration in the cancer tissue. In this paper, we report a case of advanced GB cancer treated with hepatic arterial infusion chemotherapy (HAIC) using the port system. The patient received six cycles of HAIC with 5-fluorouracil ($750mg/m^2$) and cisplatin ($25mg/m^2$); each cycle lasted for 4 days every month. The tumor showed objective response during HAIC, and the patient survived for 15 months from the first therapy. HAIC using the port system might be a promising therapeutic modality for treating locally advanced GB cancer.

    간문맥 및 상장간막정맥의 광범위한 혈전증으로 인해 복막염 및 장경색의 징후를 보이는 환자에서 요골동맥을 통한 카테터를 이용하여 상장간막동맥으로 혈전 용해제를 주입하고 경피경간 문맥조영술을 이용하여 간문맥 및 상장간막정맥의 혈전제거술을 시행하여 광범위한 간문맥 및 상장간막정맥 혈전증을 성공적으로 치료한 1예를 경험하였다. 경색 및 복막염이 동반되어 수술 위험도가 클 경우 나이, 전신상태, 진단 시기, 혈전증의 원인 등을 고려해서 방사선 중재법 및 내과적 보존적 치료를 시도해 볼 수 있겠다.

    난원공개존에 걸린 이행성 혈전을 동반한 광범위 폐색전증은 드물게 발생하지만 발생시 높은 사망률을 보인다. 본 증례는 정맥혈전증의 고위험군 환자에서 난원공개존에 걸린 이행성 혈전을 동반한 폐색전증이 발생하였을 때 기이성 색전증 발생 없이 전신성 혈전 용해술로 성공적으로 치료할 수 있음을 보여주는 1예로 문헌고찰과 함께 보고하는 바이다.

    Chryseobacterium indologenes (C. indologenes) is a nonmotile, gram-negative bacillus that is widely distributed in nature. Generally considered nonpathogenic, C. indologenes rarely infects humans and is not normally present in the human microflora. C. indologenes infections have been observed in cases of peritoneal dialysis (PD)-associated peritonitis, although the incidence of these infections is low. Although C. indologenes is generally susceptible to trimethoprim-sulfamethoxazole, levofloxacin, ciprofloxacin, piperacillin-tazobactam, and cefepime, no guidelines have been established for the treatment of PD-associated peritonitis. Here we report the first case of PD-associated peritonitis in Korea with C. indologenes identified as the sole etiologic agent. The patient recovered after intraperitoneal antibiotic treatment without the need for Tenckhoff catheter removal.
