;Dentistry has benefited from tremendous advances in technology with the introduction of new techniques and materials, and patients are aware that esthetic approaches in dentistry can change one's appearance. Increasingly. tooth-colored restorative materials have been used for restoration of posterior teeth. Tooth-colored restoration for posterior teeth can be divided into three categories: 1) the direct techniques that can be made in a single appointment and are an intraoral procedure utilizing composites: 2) the semidirect techniques that require both an intraoral and an extraoral procedure and are luted chairside utilizing composites: and 3) the indirect techniques that require several appointments and the expertise of a dental technician working with either composites or ceramics. But, resin restoration has inherent drawbacks of microleakage. polymerization shrinkage, thermal cycling problems. and wear in stress-bearing areas. On the other hand, Ceramic restorations have many advantages over resin restorations. Ceramic inlays are reported to have less leakage than resin restoration and to fit better. although marginal fidelity depends on technique and is laboratory dependent. Adhesion of luting resin is more reliable and durable to etched ceramic material than to treated resin composite. In view of color matching, periodontal health. resistance to abrasion, ceramic restoration is superior to resin restorationl. Materials which have been used for the fabrication of ceramic restorations are various. Conventional powder slurry ceramics are also available. Castable ceramics are produced by centrifugal casting of heat-treated glass ceramics. and machinable ceramics are feldspathic porcelains or cast glass ceramics which are milled using a CAD/CAM apparatus to produce inlays (for example, Cered. They may also be copy milled using the Celay apparatus. Pressable ceramics are produced from feldspathic porcelain which is supplied in ingot form and heated and moulded under pressure to produce a restoration. Infiltrated ceramics are another class of material which are available for use as ceramic inlays. An example is
$In-Ceram^{\circledR}$ (Vident. California, USA) which consists of a porous aluminum oxide or spinell core infiltrated with glass and subsequently veneered with feldspathic porcelain. In the 1980s. the development of compatible refractory materials made fabrication easier. and the development of adhesive resin cements greatly improved clinical success rates. This case report presents esthetic ceramic inlays for posterior teeth.teeth. -

Tooth resorption os perplexing problem for all dental practitioners. The etiology factors and diagnosis are vague, chosen treatment dose not prevent the rapid disappearance of the calcified dental tissues. Since the etiologic factors, diagnosis, treatment and prognosis differ for these various types of resorptive defects, it is important to diagnose resorption radiographically or clinicall and distinguish internal from external resorption. Following these cases are internal root resorption, external root resorption of serious complication subsequent to avulsion and traumatic injury.
-
Root resorption is conventionally divided into internal and external varieties. Internal resorption occurs where there is loss of the internal wall of the root canal. and is usually associated with a localized area of necrotic pulp(Trope '||'&'||' Chivian 1984). External resorption occurs where there is loss of the external surface of the root and the resorption may be either transient or progressive(Tronstad 1988). External inflammatory resorption usually occurs following damage to the periodontal ligament or where there is communication between the periodontal ligament and a necrotic pulp. via open dentinal tubules or accessory canals. This type of resorption is usually progressive until root canal treatment is instituted (Barclay 1993).

;A dental developmental anomaly is defined as an isolated aberration in tooth form, caused by a disturbance or abnormality which occurred during tooth development. There are numerous types of dental anomalies, and a considerable variation in the extent of the defects occurs with each type. Teeth with these anomalies pose unique challenges. Since the defects are not always apparent clinically, they can confuse diagnosticians investigating the etiology of pulpal pathosis. When endodontic treatment is required, the defects often hinder access cavity preparation and canal instrumentation. Treatment planning also becomes more challenging, since the defects can create complicated periodontal problems, and the malformed teeth can be difficult to restore, particularly those weakened by endodontic therapy. Fusion is defined as the joining of two developing tooth germs resulting in a single large tooth structure. The incidence of fusion is < 1% in the Caucasian population, and it is believed that physical force or pressure produces contact of the developing teeth. Clinically and radiographically, a fused tooth usually appears as one large crown with at least partially separated roots and root canals. There may be a vertical groove in the tooth crown delineating the originally separate crowns. Dens invaginatus is a deep surface invagination of the crown or root that is lined by enamel. Teeth in both maxillary and mandibular arches may be affected, but the permanent maxillary lateral incisor is the tooth most commonly involved. Studies have revealed an incidence ranging from 0.25% to as high as 10%. The invagination ranges from a slight pitting to an anomaly occupying most of the crown and root. The invagination frequently communicates with the oral cavity, allowing the entry of irritants and microorganism either directly into pulpal tissues or into an area that is deparated from pulpal tissues by only a thin layer of enamel and dentin. This continuous ingress of irritants and the subsequent inflammation usually lead to necrosis of the adjacent pulp tissue and then to periapical or periodontal abscesses. If the invagination extends from the crown to the periradicular tissue and has no communication with the root canal system, the pulp may remain vital. Recommended treatment of fused tooth and dens invaginatus has been reported in the endodontic literature. This case report describes the endodontic treatment of a maxillary laterl incisors having fused crown and dens invaginatus.natus.

;The Varicella zoster virus can cause two infectious diseases. The primary infection of this virus leads to Chicken pox and it goes into a latent period until it reveals itself again. When secondarily shown. it is called Herpes Zoster. Herpes Zoster can usually be seen between T-3 and L-2 of immunodepressant patients but in 13%. it is seen in the trigeminal nerve branch. During the predrominal stage. burning sensation, itching. and sharp pain can be experienced unilaterally where the nerve branch is distributed. Patients can also express tooth pain in this stage. After 2-4 days the acute stage begins with its characteristic unilateral macule showing vesicle formation. In this stage fever and fatigue can be experienced. Continuous pain even after the acute stage is called postherpetic neuralgia; deep pain and burning sensation can be expressed by the patient. The sequelae of this disease consists of rare osteomyelitis. necrosis of the jaws. or loss of teeth.teeth.

Failures occur in dentistry as a result of manny factors some of which can be controlled by the operator whilst others are unavoidable. The long-term success rate of endodontic treatment has often been thought to be very high although studies reported in the literature do not support this perception. The number of failure can be reduced by adhereing to accepted treatment procedures and by avoiding 'short cut'. Endodontic disaters are usually related to operator errors and they mat have detrimental effects on the outcome of treatment in the long term, eventually becoming catastrophes. Endodontic disasters will require special techniques to salvage them whereas catastrophes usually result in loss of the tooth and every effort should be made to prevent such problems from occurring. This presentation will cover common errors in endodontic procedures especially access opening, canal negotiation, canal irrigation, canal preparation, canal filling, post preparation.ration.

Fiber-reinforced materials have highly favorable mechanical properties. and their strength-to-weight ratios are superior to those of most alloys. When compared to metals they offer many other advantages as well. including non-corrosiveness. translucency. good bonding properties. and ease ofrepair. Fiber-reinforced materials can be categorized to pre-impregnated. impregnation required. dental laboratory products. chairside products and prefabricated posts. so it is not suprising that fiber-reinforced composites have potential for use in many applications in dentistry. Fiber-reinforced materials can be utilized in frameworks for crowns. anterior or posterior fixed prostheses. chairs ide tooth replacements. periodontal splints. customized posts. prefabricated posts. orthodontic retention. denture reinforcements and in implants dentistry. To realize the full potential of using fiber-reinforced composite restorations. it is essential that the clinician and laboratory technician understand concepts of tooth preparation and framework design. Also practitioner may appreciate the background information and other details about the materials themselves so that identify the rationale for their use in various clinical situations. select well-suited materials. and carry out related procedures. Understanding the material properties and take many attentions. fiber-reinforced materials will give more esthetic. more easy. more strong and more reliable restorations.ations.

;A new root canal instrument and instrumentation technique: a preliminary report. Cleaning and shaping the root canal system has been and continues to be a challenge for even the most experienced endodontist. Curved. narrow canals. in particular. cause difficulties for the beginner as well as the specialist. A new instrument designed to incorporate new concepts was developed to ameliorate the problems in cleaning and shaping root canal systems. This new instrument has been given the name of SW (Senia and Wildey). The new SW instrument uses controlled right and left rotational forces. This motion was used to clean and shape simulated root canals in plastic blocks and root canals in extracted teeth. Instrumentation appeared to be easier. faster. and more precise than with conventional instruments. especially in curved canals where there was remarkable reduction of canal transportation. A mechanical version of the SW instrument was also developed. It was used to flare the coronal portion of the root canal system. Wildey WL. Senia ES., Oral Surg Oral Med Oral Pathol1989 Feb:67(2):198-207 Another look at root canal instrumentation. Several aspects of root canal instrumentation need additional research. Various factors must be considered in an analysis of instrumentation of the root canal system: the dentin that is cut: the technique used to cut it: the design of the instruments: the material and manufacturing process used to make the instruments: the irrigant used during the procedure: and the anatomic configuration of the root canal system. An analysis of these factors clearly indicates that existing root canal instruments and techniques are less than ideal and. in fact. do not accomplish what is expected of them. Root canals must be properly. but. at the same time. destructive and unnecessary removal of dentin should be kept to a minimum. The Flex-R and Canal Master instruments were developed to address some of the shortcomings of existing instruments and techniques. More scientifically based research is needed to fully evaluate these new instruments and techniques and to develop future instruments. Wildey WL; Senia ES.
