Comparison of Quality of Life for Oral Health between Dental Hygiene Students and Non-Dental Hygiene Students in Some Areas by Using Oral Health Impact Profile-14

구강건강영향지수-14를 이용한 일부 지역 치위생 계열과 비치위생 계열 대학생들의 구강건강 삶의 질 비교

Abstract

The purpose of this study is to compare the quality of life for oral health between dental hygiene students and non-dental hygiene students in some areas and to analyze the factors affecting on it by using oral health impact profile-14 (OHIP-14). This study was carried out by surveys. Subjects for study were 248 undergraduate students, subjectively recognized oral health condition by subjects were periodontal disease and bad breath, and non-dental hygiene students recognized statistically significantly higher than dental hygiene students (p<0.001). Among the detail factors of OHIP-14, factor of decreased social ability was the lowest impact factor (p<0.01). The most influential factor on OHIP-14 was gender and age, grade of school and alcohol intake were influential in order (p<0.01).

본 연구는 치위생 계열과 비치위생 계열 대학생들을 대상으로 주관적으로 인지하는 구강상태와 OHIP-14를 활용한 삶의 질을 비교 분석할 목적으로 대학생 248명을 대상으로 자기기입식 설문지를 작성하여 다음과 같은 결론을 얻었다. 비치위생계 학생들은 치주질환증상, 구취증상에서 치위생계 학생들보다 구강상태에 대하여 통계적으로 유의하게 높게 인식하는 것으로 나타났다(p<0.001). 치위생계 학생과 비치위생계 학생들은 심리적 불편요인에서 높은 구강 내 문제를 경험한 것으로 나타났지만 통계적으로 유의한 차이를 나타내지 못하였고, 사회적 능력저하 요인은 두 계열 학생들에게 가장 낮은 영향을 미치는 요인으로 나타났고 치위생계 학생들이 비치위생계 학생들보다 통계적으로 높게 문제를 경험한 것으로 나타났다(p<0.01). 여성이 남성보다 심리적 능력저하(p<0.01), 사회적 능력저하(p<0.05)에서 구강 내 불편을 높게 경험하고, 4학년보다 3학년이 심리적 불편(p<0.01), 심리적 능력저하(p<0.05)에서 높게 경험하는 것으로 나타났다. 또한 흡연량은 신체적 동통(p<0.05), 심리적 불편(p<0.05), 심리적 능력저하(p<0.05)에서 높게 나타났다. 연구대상자가 인식하는 TMJ기능장애가 있으며 신체적 동통, 심리적 불편이 가중되며, 치주질환증상은 OHIP-14의 세부영역에 영향을 미치는 것으로 나타났으며 정적 상관관계를 나타냈다. 또한 구취증상은 신체적 동통, 심리적 능력저하, 사회적 불리에서 정적 상관관계를 나타냈다.흡연량은 주관적인 구강상태에 미치는 영향이 큰 것으로 나타났고(p<0.01), OHIP-14에 가장 영향을 많이 미치는 변수는 성별인 것으로 나타났으며 연령, 학년, 음주량 순으로 영향력을 미치는 것으로 나타났다(p<0.01). 이상의 결과를 통해 구강보건교육과 관리지침 및 계획에 있어서 치위생 전공자와 비전공자의 교육내용을 차별화하여 교육효과를 올릴 수 있는 방안이 필요하다. 또한 성별에 따른 교육프로그램 개발 역시 필요한 사안이며 구강건강관련 삶의 질을 평가함에 있어서 연령에 따른 구강 내 문제 경험은 다르다는 것을 인지하고 그에 따른 교육내용도 보완되어야 할 것으로 생각된다.
