Subjectivity toward Death among HIV-Positive Men

Abstract

Purpose: This study was to identify the attitude of Korean HIV (Human Immunodeficiency Virus)-positive men toward death. Methods: A Q-methodology was performed with 20 HIV-positive male individuals. Participants were asked to select and answer questions among a set of 40 Q-statements using a 9-point scale. The collected data were analyzed using the PC QUANL program. Results: Participants' attitudes toward death were categorized into four types. Type I was characterized by respect for life, type II by reality orientation, type III by pain evasion and type IV religious beliefs. Conclusion: It is necessary to develop an assessment tool and an intervention program for HIV-positive individuals.

목적: 본 연구는 Q 방법론으로 HIV 감염인 남성의 죽음에 대한 주관성을 파악하고 그 구조와 유형을 분류하고 분석하여 유형별 특성에 따른 중재를 개발하는데 기초자료를 마련하고자 하였다. 방법: 김분한 등(1997)이 선정한 죽음의 의미에 대한 40개 진술문으로 Q 표본의 분포도를 작성하였다. P 표본은 대한에이즈예방협회에 등록되어 있는 성인 남성 20명을 편의추출하였으며, Q 진술문을 자유의사에 따라 선택하도록 하여 9점 척도상에 분류하도록 하였고 양극단에 진술문에 대해서는 선택한 이유를 직접 기록하도록 하였다. 수집된 자료는 PC QUANL program으로 처리하여 분석하였다. 결과: HIV 감염인 남성의 죽음에 대한 주관성은 모두 4개 유형으로 나타났으며, 이들의 전체 변량은 51.09%였다. 제1유형인 '생명 존중형'은 생명존중과 타인에 대한 배려가 있었으나 죽음에 대해서는 생각하지 않고 현재에 최선을 다하려는 태도를 가지고 있었다. 제2유형인 '현세 중시형'은 살아있는 한 열심히 살기를 원했으며, 황금기에 죽지 않고 죽을 때까지 모든 치료를 받고자 하였고 사형제도에 찬성하였으며, 죽을 때 고통없이 죽기를 희망하였다. 제3유형인 '고통 회피형'은 경제적 능력이 없거나 식물인간 상태로 사는 것보다는 고통없이 죽기를 원했으며, 죽음에 대하여 준비하기를 원하였다. 제4유형인 '종교 수용형'은 종교인으로서 자신의 질환 때문에 장기기증을 반대하였으며, 죽음에 대해 성직자나 의사와 솔직하게 대화하고자 하였다. 결론: 본 연구의 의의는 HIV 감염인 남성을 대상으로 죽음에 대한 다양한 태도를 파악했다는 것이다. 본 연구의 결과는 HIV 감염인의 죽음에 대한 인식 유형별로 적합한 죽음준비 교육 프로그램을 개발하는데 기초자료로 활용될 것이다.
