노인틀니 급여화에 대한 공급자의 인식
Awareness of Dental Service Providers on the Inclusion of Denture in the Coverage of the National Health Insurance

연구목적 : 노인인구 증가, 노인의 건강에 대한 관심 증가 등의 일환으로 노인의 구강건강에 대한 관심과 증진을 위한 노력은 계속되어 왔다. 또한 보험의 보장성 강화의 측면에서도 노인틀니의 보험급여화는 지속적으로 추진되어왔다. 이에 본 연구는 노인의 구강건강 증진과 보험의 보장성 강화라는 목적을 가지고 시작되는 노인틀니 급여화에 대한 공급자의 인식을 조사함을 통해 제도의 안정적 정착과 발전에 발판을 마련하고자 하였다. 연구방법 : 공급자(치과의사)가 근무(또는 개원)하는 치과의 특성을 반영하기 위해 지역적 안배를 하고, 병·의원을 적절히 포함하는 표본을 설계하여 자기기입식 설문지를 배포하여 완전틀니 급여방안 및 관리방안 대한 의견을 수렴하였다. 수집된 자료는 STATA 11.0을 이용하여 교차분석, t-test, ANOVA를 실시하였다. 연구결과 : 노인틀니 급여화에 대해서는 전반적으로 찬성(93.08%)의 의견을 보였으며, 수련여부, 전공과목에 따라서는 통계적으로 유의한 차이를 나타내었다(p<0.05). 급여적용 대상에 대해서는 치과보철과를 수련한 경우 현행 '75세 이상'이 적절하다에 60.30%가 찬성, 비수련의 경우에는 '65세 이상'이 적절하다에 68.60%가 응답하여 상이한 결과를 나타냈다(p<0.05). 급여적용법위에 대한 의견 역시 비금속, 금속상에 모두 급여 적용해야 한다는 의견이 나타났으나, 세부의견에서는 수련여부, 전공과목에 따라 차이를 기록하였다(p<0.05) 틀니서비스 술전, 술후술식에 대한 급여화 역시 대부분 찬성하는 것으로 나타났으나, 이를 수련여부 전공과목에 따라 구분해서 살펴 본 결과 통계적으로 유의한 차이가 있는 것을 확인할 수 있었다(p<0.05). 결 론 : 완전틀니 급여화에 대해서는 대부분의 긍정적인 인식을 가지고 적극적 참여의지를 나타냈으나 수련여부 및 전공과목에 따라 그 인식에 차이가 있는 것으로 나타났다.
Objective There are growing concern and efforts for the promotion of the elderly's oral health along with an increase in the elderly population and concern for the health of that population, and there are continuing efforts to insert denture into insurance coverage in a bid to increase insurance coverage. The purpose of this study was to examine the awareness of dental service providers on the inclusion of denture into the coverage of the national health insurance in an attempt to step up the oral health promotion of the elderly and the reinforcement of insurance coverage. It's ultimately meant to lay the foundation for the successful entrenchment and development of the health care system. Method The dental service providers(dentists) who worked or run the selected dental hospitals or clinics were surveyed, and the selected dental hospitals and clinics were located in different regions and similar in number, as it's meant in this study to consider the characteristics of the dental institutions. A self-administered survey was conducted to gather their opinions on how to insert denture into full insurance coverage and how to manage it. The collected data were analyzed by the statistical package STATA 11.0, and crosstabs, t-test and ANOVA were utilized. Results The dental service providers consented to the inclusion of denture in health insurance coverage on the whole, and there were statistically significant differences according to training experience and major(p<0.05). As for eligibility, 60.30 percent of the dental service providers who took a prosthetic dentistry course replied that elderly people aged 70 and over should benefit from the health insurance in case of using denture according to the current practices, and 68.60 percent of those who didn't take any prosthetic dentistry course replied that senior citizens aged 65 and over should be eligible for that. Thus, there was a difference between the two groups(p<0.05). Concerning insurance coverage, they answered that denture should be included in it, no matter whether it would be made of metal or not, but there were some difference in the details of the insurance coverage according to training experience and major(p<0.05). The majority consented to the inclusion of 틀니서비스 술전,술후술식 as well, but there were statistically significant differences in that regard when their opinions were analyzed by training experience and major(p<0.05). Conclusion The majority took a positive view of the inclusion of denture into full health insurance coverage, and they had an active will to comply with it, but there were differences in their awareness according to training experience and major.
