A Case of a Collision Tumor in the Ampulla of Vater with an Adenocarcinoma and a Large Cell Neuroendocrine Carcinoma

초록

바터 팽대부에 발생하는 악성 종양은 대부분 선암 (adenocarcinoma)이며 다른 조직학적 형태의 암 발생은 극히 드물다. 특히 선암과 신경내분비암이 동시에 발생하는 복합종양은 매우 드문 것으로 알려져 있다. 저자들은 최근 상복부불쾌감과 황달을 주소로 내원한 60세 여자에서 바터 팽대부암 의심 하에 유문보존 췌십이지장절제술 후 조직학적으로 선암과 대세포 신경내분비암이 병발한 바터팽대부의 충돌종양을 진단하였다. 복합종양 중 대세포 신경내분비암이 췌장 주위 지방조직으로의 침윤 및 림프절 전이가 있어 수술 후 보조항암요법을 받았던 예를 경험하여 문헌고찰과 함께 보고한다.

Most tumors affecting Vater's ampulla are adenocarcinomas, but a neuroendocrine carcinoma in the ampulla of Vater is extremely rare. The coexistence of these two tumors has been reported in only a few cases. Here, we report a rare case of a collision tumor of the ampulla of Vater with an adenocarcinoma and a large cell neuroendocrine carcinoma.
