본태성 고혈압에 대한 Manidipine 장기투여의 강압효과와 안전성 검토

Hypotensive Efficacy and Safety of Long-term Treatment with Manidipine on the Patients with Essential Hypertension

연구배경: 고혈압은 평생치료를 요하는 만성병중의 하나이다. 평생치료를 위해서는 항 고혈압제의 선행요건으로 효율성, 안전성, 편리성, 비용 등이 매우 중요하다. 방법: 경증 및 중등증의 본태성고혈압환자 22명에게 새로운 칼슘길항제인 manidipine을 하루에 10mg 에서 20mg을 12개월동안 투여하여 강압효과의 지속유지, 내약성 및 안전성을 검토하였다. 결과: 1) 관찰기 혈압 163/98mmHg에서 12개월말 투약종료기에 141/88mmHg로 절대치가 22/10mmHg하강하여 82%의 강압유효율을 보였다. 2) 투약종료시 혈압 정상화율은 73%이었다(150/90mmHg미만) 3) 주요부작용은 안면부종, 하지부종, 심계항진과 안면홍조등 총 6건이 4명(18%)에서 관찰되었는데 치료없이 소실되었다. 4) 투약전과 투약후에서 환자들의 흉부X선, 심전도검사, 안저검사 및 기본적인 생화학검사가 변화없이 정상이었다. 5) 전반유용도는 73%이었다. 결론: Manidipine 1일 1회 $10{\sim}20mg$의 장기투약은 경증 및 중등도 고혈압환자에서 안전하고도 우수한 강압효과 및 순응도를 가진 강압제라 생각된다.

Background : Hypertension is one of the chronic diseases which require lift-long treatment. To assure life-long treatment. the prerequisites of antihypertensive agents such as efficacy, safety, convenience and cost are very important. Method : Twenty-two patients with mild to moderate essential hypertension were administered manidipine-a new calcium antagonist-10 to 20mg once daily for 12 months to evaluate its hypotensive efficacy, safety, and patient's compliance. The following results were obtained. Results : 1) Blood pressure dropped from initial 163/98mmHg to 141/88mmHg at the end of 12 months trial resulting 22/10mmHg decrease with 82% effectiveness. 2) Normalization of blood pressure(less than 150/90mmHg) was obtained in 73% 3) Major side reactions were observed in 4 cases(18% ), namely facial edema, pitting leg edema. eyeball Pain. and facial flushing which subsided without Preventing further medication4) Most of routine laboratory parameters were within normal limits and unchanged before and after trial. 5) Overall rating of usefulness was 73%. Conclusion : Long-term administration of manidifine$ 10{\sim}20mg$ once daily was well tolerated and effective in the treatment of mild to moderate essential hypertension.
