영양과 환경과의 관계 (Nutritional Effects on the Environmental Health)

  • The effects of environmental agents on health are great concern for all. It was recognized that each human has differential susceptibility to environmental effects. Susceptibility are changed by many factors includin gdevelopmpntal processes, genetic factors, nutritional stratus, preexisting disease conditions, life style and personal habits. Of all factors nutritional factors seem to be the area most modifiable. Consequently, It is an area that must be more thoroughly evaluated. In this paper, nutrient and environment interactions are reviewed briefly with published literatures. This paper deals with the influence of micronutrients(energy, protein and fat), Vitamins (vitamin 4, vitamin B-complex, vitamin C, vitamin D and vitamin I) and Minerals(calcium, iron, selenium, zinc and other minerls) on environmental effects. The role of arch nutrient was assessed in modifyine the expression of environmental pollutant toxicity with available litertures. In each nutrient section, the effect of environment was considered in following agents : heavy metals(lead, cadmium, mercury, silver and etc), inorganic agents(nitwits, sulfite, fluoride and etc), organic agents(benzene, carbon tatra-chloride, aflatoxin, auto dye, dialbrin etc), Irritant gas(ozone, carbon monooxide and etc), physical agents(X-irradiation, ultra violet, temperature and noise) and insectcides. The extent to which nutritional status modifies environmental effects 3nd its converse, how envirollments affects nutritional status is very complex. In deed, at the present time there are more than 50 chelnical/phycical agents that affect the nutrient metabolism and/or have their toxicity either directly diminished or enhanced by nutrients of those agents, small number of agents for each nutrients have sufficient evidence to warrant any reasonable degree of confidence in their hypothesized associtation. With these information at this present time it is hard to conclude that the recommended dietary allowance for each nutrient should be reconsidered.

The Effect of Reducing Dietary Phytate Intake and Zinc Supplementation on the Iron Status of Elderly Korean Women

  • To investigate the effects of dietary phytate reduction and zinc supplementation on biochemical iron parameters in elderly Korean women consuming inadequate iron, fifteen healthy women aged 64-75 years were recruited for a feeding study. A high-phytate diet (27.8 phytate:zinc molar ratio) was provided for 9 days, followed by a nine-day low-phytate diet(12.3) and a subsequent 28-day period of unregulated meals with zinc supplementation (22 mg/d as zinc gluconate). Serum iron increased significantly with the low-phytate diet (130.4 $\mu g$/L) but returned to the level observed during the high-phytate diet (113.0 $\mu g$ /L) period when subjects were taking zinc supplements (105.8 $\mu g$ /L). However, serum ferritin in the subjects decreased significantly with the low-phytate diet (73.8 $\mu g$ /L) as well as with zinc supplementation (57.2 $\mu g$ /L), compared to levels following consumption of the high-phytate diet (89.6 $\mu g$ /L). Transferrin receptor and transferrin saturation were unchanged with the treatments. In summary, zinc supplementation might result in deteriorated iron status in elderly Korean women who consume inadequate iron, while there was no significant effect from reducing dietary phytate.

Association between nutritional status and cognitive functions of the Korean elderly

  • Nutrition is one of the factors influencing cognitive functions. But, the role of nutrition on cognitive functions within the elderly is recognized to a lesser degree. The aim of this academic endeavor was to analyze the affiliation between nutritional status and cognitive functions of the elderly in Korea. 316 subjects, of 114 male and 201 female aged above 65 years, were gathered from university hospital clinics, Elderly Welfare Centers, and Health Welfare Centers located in the district of Seoul, Gyeonggi province, and Incheon in Korea. The cognitive function was tested by the Korean-Mini Mental State Examination (K-MMSE) questionnaire. Nutrition intake was assessed by a 24-hour recall, a food frequency questionnaire (FFQ). Nutrition adequacy and quality were assessed by the Nutrition Screening Initiative (NSI) checklist, the nutritional adequacy ratio (NAR), and the mean adequacy ratio (MAR). The elderly with normal cognitive function status displayed significantly higher levels of calcium, riboflavin, thiamin, folate, vitamin C, vitamin E, vitamin B6, phosphorous, potassium, iron, niacin, zinc, animal fat, animal protein, polyunsaturated fatty acids, total protein, vegetable oil, vegetable protein, fiber than levels of the elderly with moderate cognitive impairment. The K-MMSE score positively correlated with of high consumption of pork, white radish, sea mustard, tomato, tangerine, grape, apple, and ice cream, and K-MMSE was negatively associated with high intake of potato, anchovy, fish cake, and mushroom. The findings suggest that nutritional status is affiliated with cognitive function within the elderly of Korea. The consumption of variety of foods and nutrients ensures adequate cognitive function in the Korean elderly.

Nutritional and antioxidant status by skin types among female adults

  • This study was performed to analyze the relationship among sebum hydration content of the skin and nutritional intake, serum antioxidant minerals and antioxidant enzymes, and lipid peroxide concentration in 50 female subjects in their 20s. The skin type was divided into Dry Skin, Mixed Skin, and Oily Skin, and the dry skin group was 14%, the mixed skin group was 56%, and the oily skin group was 30% of all subjects. The average age of the subjects was $20.54{\pm}1.43$ years and BMI was 20.66. The average sebum content in each group was in the order of T-zone>forehead>chin>cheek. In case of the T-zone, a significant difference between the dry skin group and the oily skin group was observed, suggesting that the area is most sensitive to sebum content by skin type. Significant differences were not observed in energy and nutrient intakes by skin type. Serum concentrations of antioxidant minerals such as copper, manganese, zinc and selenium were not significantly different among the groups, but the dry skin group tended to be higher than the oily skin group. Serum catalase was significantly higher in the oily skin group (P < 0.05), and MDA was significantly higher in the mixed skin group (P < 0.05). The hydration of the cheek and serum zinc showed a negative correlation, and the sebum content of the cheek and GPx showed a significant negative correlation. The hydration of the forehead and serum copper showed a significant negative correlation, and the hydration of the forehead and GPx showed a significant positive correlation. The hydration of the chin and serum SOD showed a significant positive correlation. With these results, it is considered that the basic condition of nutritional status can affect the skin health.

Serum Levels of Minerals, Ceruloplasmin, and Ferroxidase Activity in Female Adolescent Smokers

  • The purpose of this study was to investigate the nutritional status of serum minerals, ceruloplasmin, and ferroxidase activity in female adolescent students according to their smoking status. The subjects were 87 non-smokers and 88 smokers, who were female high school students. The smokers were divided into tow groups by smoking status, 35 light smokers(pack-year<1) and 53 smokers(pack-year$\geq$1). The serum concentrations of 6 minerals were determined by ICP emission spectroanalyzer. The serum concentration of ceruloplasmin and ceruloplasmin ferroxidase activity were determined. All data were statistically analyzed by SAS PC package program. The serum minerals concentrations of all subjects were in a normal range. There were significantly differences in the concentrations of serum copper, zinc, magnesium, and phosphorus by smoking status while there were no significant differences in the concentrations of serum iron and manganese by smoking status. Furthermore there were no significant differences in the ceruloplasmin concentration and ferroxidase activity by smoking status. In conclusion, smoking status altered the serum levels of some minerals in healthy young women. The minerals levels in light smokers with relatively short pack-year(pack-year<1) were altered compared to those in non-smokers or smokers. This finding seemed to be consistent with the results of previously published data related to antioxidant vitamin and lipid peroxide levels. However further research is needed to clarify these findings in female adolescent smokers.

여성 유방암 환자의 철, 구리, 아연 영양상태에 관한 연구 (A Study on Zinc, Copper, Iron Nutritional Status of Women Patients with Breast Cancer in Korea)

  • This study was performed to compare nutrients intake and levels of three elements (iron, copper and zinc) in the plasma of breast cancer patients to those in healthy controls. Patients (n = 97) and their age-matched healthy control groups (n = 97) participated in the study. A semi-quantitative food frequency questionnaire specifically formulated to evaluate the consumption of nutrients was used to estimate dietary intake. Only plasma was taken from the 20 patients and their age-matched healthy control group. Plasma concentrations of iron, copper, zinc, ratio of Cu/Zn were measured in both groups. Results showed that patients consumed significantly lower protein, fat, iron. Plasma level of copper and ratio of Cu/Zn were significantly higher in patients compared to those of controls. In conclusion, change of trace elements in plasma might be useful and significant as biomakers involving the initial process. And these results imply that trace elements status is a factor related to breast cancer risk and suggest that proper intake of those nutrients is required for cancer prevention.

A Study on the Dietary Intake and the Nutritional Status among the Pancreatic Cancer Surgical Patients

  • The adequate dietary intake is important to maintain the nutritional status of the patients after pancreatic cancer surgery. This prospective study was designed to investigate the dietary intake and the nutritional status of the patients who had pancreatic cancer surgery. Thirty-one patients (15 men, 16 women) were enrolled and measured body weight, body mass index (BMI), nutritional risk index (NRI), and Malnutrition Universal Screening Tool (MUST). Actual oral intake with nutritional impact symptoms recorded on the clinical research foam at every meal and medical information were collected from electronic medical charts. The rates of malnutrition at admission were 45.1% (14/31) and 28.9% (9/31) by NRI and MUST method, respectively, but those were increased to 87% (27/31) and 86.6% (26/31) after operation on discharge. The median values of daily intake of energy, carbohydrates, fat, and protein were 588.1 kcal, 96.0 g, 11.8 g, and 27.0 g, respectively. Most patients (n = 20, 64.5%) experienced two or more symptoms such as anorexia, abdominal bloating and early satiety. There were negative correlations between C-reactive protein (CRP) levels and the intake of total energy, protein, fat, and zinc. The rates of malnutrition were increased sharply after surgery and the dietary intake also influenced the inflammatory indicators. The results suggested that need of considering special therapeutic diets for the patients who received pancreatic surgery.

대전지역 학령기 아동의 체위, 혈액성상 및 영양실태에 관한 연구 (A Study of Dietary Intake, Physical Status and Biochemical Status of Children in Taejon)

  • This study was conducted to investigate the relationship among Rohrer index, and nutritional intake and biochemical status. We examined physical status, dietary intakes, serum cholesterol, blood pressure and other biochemical of children in Taejon. Thin survey was carried out in October, 1995. The subjects were 362 children, aged from 6 to 11 years old. The results were summarized as follows. By Rohrer index, 4.5% of the subjects were lean, 70.2% were normal, 17.0% were overweight and 8.6% were obese. Systolic and diastolic blood pressures of the subjects were 111.73 and 69.88mmHg, respectively. The systolic blood pressure of obese subjects(119.93mmHg) was higher than any other groups. Total cholesterol level was 160.87mg/dl, cholesterol levels in each group were not significantly different. Hemoglobin level of total subjects was 13.10mg/dl, especially hemoglobin levels of 11 year-old girls was lower than that of the same aged boys suggesting that the girls should be supplied with more protein, iron and other nutrients. Energy and protein intakes wee 86.6% and 94.5% of RDA, respectively. The nutrients above the RDA were only two, phosphorus and-vitamin C. The intakes of iron, zinc and vitamin A were lower than 50% of the RDA's. The levels of zinc and niacin were highest in obese group than any other groups. The subjects in overweight and obese groups had eaten more calories from snack and less calories from breakfast than other groups. Rohrer index was correlated with cholesterol, systolic blood pressure and creatinine levels.

도시 여대생에 있어 식품분석에 의한 아연, 구리섭취량과 혈액, 머리카락, 소변의 아연, 구리 함량에 관한 연구 (Zinc and Copper Intake with Food Analysis and Levels of Zinc and Copper in Serum, Hair and Urine of Female College Students)

  • The purpose of this study was to assess the zinc and copper nutritional status of 102 college women by measuring zinc and copper intake, hematological parameters of zinc and copper, hair zinc and urinary excretion of zinc and copper. The mean zinc intake was 5.5mg(45.8% RDA) with food analysis and 4.5mg(37.8% RDA) with computation from food composition table. The copper intake with food analysis was 2.3mg and 1.2mg with computation. Mean serum zinc concentration was 77.02ug/dl and the proportion of subjects with zinc deficiency estimated by serum zinc(<70ug/dl)was 23.0%. Mean serum copper concentration was 121.80ug/dl and 4.1% of subjects showed serum copper less than 70ug/dl, The mean ceruloplasmin concentration was 22.63mg/dl and the proportion of subjects whose ceruloplasmin was lower than 18-40mg/dl was 6.6%. The mean hair zinc of subjects was 143.8ppm and the mean hair copper was 11.2ppm. The mean urinary excretion of zinc was 0.43mg/day and the proportion of subjects with marginal deficiency estimated by urinary zinc excretion( <0.3mg/day) was 23.3%. The mean urinary copper excretion was 0.044mg/day which was within the normal range(0.01-0.06mg/day). Assessing by zinc content in hair, urine and serum, 22.9-23.3% of college women had bordeline zinc deficiency or zinc deficiency. Whereas 4.1-6.6% of college women was assessed copper deficiency estimated by serum copper and ceruloplasmin.

Nutritional status of Korean elderly with dementia in a long-term care facility in Hongseong

  • BACKGOUND/OBJECTIVES: This study investigated nutritional status of the elderly with dementia in a care facility with the aim of improving the meal quality of the facility. SUBJECTS/METHODS: Data were collected from 30 dementia patients aged more than 65 years in a long-term care facility in Hongseong. The data were obtained from questionnaires and medical records. The food intake data was obtained using food photographs and the nutrient intakes were calculated using the CAN-Pro 5.0. The data were compared with the dietary reference intakes for Koreans (KDRIs). The nutrient density, diet quality such as nutrient adequacy ratio (NAR), mean adequacy ratio (MAR), and index of nutritional quality (INQ), as well as dietary diversity score (DDS) were evaluated. The data were analyzed using SPSS statistical programs. RESULTS: The average daily energy intakes for men and women were much lower than the estimated energy requirements of the KDRIs. The average intakes of energy and most nutrients in the general diet group were significantly higher than those of the other two groups. Significant differences in diet quality and diet diversity were observed according to the meal type groups. The NARs of some minerals (calcium, iron, and zinc) and vitamins (vitamin $B_6$ and folic acid) were less than 0.5 in all study groups. The NARs of protein, iron and MAR of the general diet group were significantly higher than those of the liquid diet group. The DDS scores of meats, fruits and diary food group were very low in all meal type groups, meaning that the diet qualities of the study subjects were not appropriate in all meal type groups. CONCLUSIONS: The food intakes of the study groups showed some limitations by a direct comparison with KDRIs because of the very low physical activities of the study subjects. The diet quality and diet diversity indices suggest the need for improvements in the nutritional quality in all types of diet. Overall, new intervention strategies targeting facility residents with dementia in Korea are needed as soon as possible.
