라이프 케어를 위한 언어장애인의 언어치료 이용여부에 따른 특성 및 보건의료서비스 인식 비교 : 2017년 장애인 실태조사를 이용하여 (Comparisons of Awareness of Health Care Services and Characteristics in Persons with Speech-Language Disorder Related to Speech Therapy Use for Life Care : From National Survey of the Disabled Person of 2017)

  • 보건의료서비스는 장애인을 포함한 국민의 건강을 위해 제공되는 가장 기초적인 사회제도이다. 그러나 언어장애가 있는 사람 중 언어치료 이용여부에 따른 보건의료서비스의 차이를 알아본 연구는 미흡하였다. 본 연구는 언어장애가 있는 장애인을 대상으로 언어치료 이용여부에 따른 특성과 보건의료서비스의 인식을 비교하고자 하였다. 연구자는 2017년 장애인 실태조사의 원시자료를 이용하여 언어장애를 가진 사람 229명을 선정하였다. 이 중 언어치료를 이용하는 자와 이용하지 않는 자를 구분하여 그들의 특성(사회인구학적 특성, 장애특성)과 보건의료서비스 인식(미충족 의료, 정부 또는 사회에서 강화해야할 보건의료 서비스)을 비교하였다. 229명의 언어장애가 있는 사람 중 언어치료를 이용하고 있는 사람은 37명(16.2%)이었다. 언어치료 이용자는 비이용자와 비교하여 나이가 젊었고, 미취학인 아동이 많았으며, 월평균 총 가구 소득이 높았으며, 장애등록유형에서 지적장애와 자폐성장애가 더 높은 비중을 차지했다. 언어치료 이용자는 비이용자에 비해 미충족 의료 경험의 비율이 낮았다. 미충족 의료 이유에서 언어치료 비이용자 49명의 분포를 살펴보면 '경제적인 이유'와 '의사소통에 어려움'이 13명(6.8%)과 12명(6.3%)로 1순위와 2순위를 차지하였다. 언어치료 비이용자와 이용자 모두는 정부(사회)에서 강화해야 할 보건의료서비스로 '장애관리'를 요구하였다. 본 연구의 결과는 언어장애가 있는 사람의 언어치료 이용의 장벽을 낮추기 위해서 보건의료서비스의 접근성을 증대시킬 필요가 있음을 시사한다.

  • 부부간 의사소통은 결혼 기간이 축적될수록 '공유된 경험세계'가 상대적으로 많은 반면 '친밀성'으로 인해 다양한 의사소통 문제들이 창출되는 산실이기도 하다. 따라서 부부간 의사소통은 표면적으로 매우 용이한 의사 소통 상황으로 비추어질 수 있지만 심층적으로 살펴보면 다양한 대인 커뮤니케이션(interpersonal communication)의 문제가 산재해 있음을 볼 수 있다. 이 연구는 한국사회의 부부간 의사소통 행태를 각 세대별로 나누어 그 특성을 탐색해보고, 결혼생활이 축적됨에 따라 나타날 수 있는 부부간 의사소통 행태의 변화 추이를 살펴보는데 중점을 두었다. 이러한 연구목적을 달성하기 위하여 20대, 30대, 40대, 50대 부부, 즉 연령 동기자 그룹을 대상으로 대인 커뮤니케이션과정의 5가지 차원-개방적 태도 차원, 문제 해결 차원, 의사 표현 차원, 경청(傾聽) 차원, 피드백 차원에서 설문조사를 진행하여 그 결과를 분석해보고자 하였다. 분석 결과 부부간 의사소통 행위의 특성이 각 세대별로 파악 되었으며, 다시 의사 종단 분석을 통하여 개방적 태도 차원, 문제 해결 차원, 의사 표현 차원, 경청 차원, 피드백 차원의 부부간 의사소통 행위가 변천하는 과정을 추적해볼 수 있었다.

당뇨병 노인의 mHealth 이용에 영향을 미치는 요인 (Factors Influencing mHealth Use in Older Adults with Diabetes)

  • 정보통신기술의 발전과 일상 중심으로의 의료서비스 변화는 스마트폰 건강관리 앱(mHealth) 활용을 통한 자가관리 시대를 열었다. mHealth는 이용자의 건강 자가관리에 도움을 주며 서비스 제공자의 주요 수익원이므로 mHealth 이용 요인을 이해하는 것은 중요하다. 그러나 mHealth의 주요 이용자가 될 수 있는 당뇨병 노인을 대상으로 이용 요인을 규명한 연구는 제한적으로 이루어졌다. 따라서 본 연구는 당뇨병 노인의 mHealth 이용에 영향을 미치는 요인을 파악하는 것을 목표로 하였다. 구체적으로 당뇨병 노인의 주관적 건강상태와 e헬스리터러시가 mHealth 이용에 미치는 영향이 사회적 지지 수준에 따라 어떻게 달라지는지 살펴보는 것에 초점을 두었다. 65세 이상 당뇨병 노인 252명을 대상으로 한 온/오프라인 설문조사를 통해 자료를 수집했으며, 주관적 건강상태 및 e헬스리터러시와 사회적 지지의 상호작용항을 투입하는 위계적 다중회귀분석을 실시하여 조절효과를 검증하였다. 본 연구의 주요 결과는 다음과 같다. 첫째, 당뇨병 노인이 남성, 저연령일수록 mHealth 이용이 높았다. 둘째, 당뇨병 노인의 e헬스리터러시가 높을수록 mHealth 이용이 높았다. 셋째, 주관적 건강상태와 mHealth의 관계에서 사회적 지지의 완충효과가 나타났다. 이는 사회적 지지가 높을수록 주관적 건강상태가 mHealth에 미치는 긍정적 영향이 완화되는 것을 의미한다. 본 연구는 mHealth 이용에 관한 정보시스템 및 의료 분야의 지식에 기여하는 한편, mHealth 서비스 제공자의 고객 확보 전략 수립, 정부와 의료진의 당뇨병 노인의 mHealth이용을 통한 자가관리 증진 방안 마련에 유용한 실무적 시사점을 제공할 것으로 기대된다. 나아가 mHealth 이용 활성화를 통한 당뇨병 노인이 삶의 질 개선 및 디지털 포용 사회 구축에 기여할 수 있을 것이다.

여론조사보도에 대한 제3자효과 검증: 온라인 여론조사를 주목하며 (Measuring the Third-Person Effects of Public Opinion Polls: Focusing On Online Polls)

  • 이 연구는 사람들이 전통적인 여론조사와 온라인 여론조사를 통해 얻어진 결과를 어떻게 다르게 인식하는가를 제3자효과론 (Third Person Effects)을 중심으로 살펴보았다. 미디어의 영향이 본인보다 다른 사람에게 더 클 것이라는 제 3자효과론은 그 동안 미디어효과 연구분야에서 폭넓게 적용되어 왔다. 하지만 최근 인터넷을 이용한 온라인 여론조사의 광범위한 사용으로 과연 이러한 여론조사에서, 특히 우리가 기존에 사용해왔던 전화를 통한 무작위 표본조사와 대표성이 약한 온라인 여론조사의 차이를 고려할 때, 실질적으로 미디어 이용자 입장에서는 얼마나 그 차이를 인식하는지는 매우 중요한 연구과제라 할 수 있다. 뿐만아니라, 서로 다른 두 가지 유형의 여론조사의 결과가 수용자에게 미치는 영향 측면에서 서로 다른 크기의 제 3자 효과가 나타나는지에 대한 관심도 커질 수밖에 없다. 이를 위해서 서베이와 실험연구 두 방법을 병용하여 이 문제에 대한 좀더 심층적인 분석을 시도하였다.

우리나라 간호연구의 현황과 문제 (Nursing research issues and trends : views from Korea)

  • The purposes of study was: 1. To identify the direction and scope of research activities in Korean Nursing. 2. To discuss the unique problems present in Korea that warrant nursing research. 3. To delineate the factors that facilitate and/or hinder nursing research in Korea. This study was conducted at six colleges of nursing in Korea. The schools which were selected had doctoral programs at the time, the data were collected in 1990. Four of the schools were located in the metropolitan area of the capital city, Seoul, and two were in two other cities similar in size. The total population of the study was 283 nurses. The instrument for the research was the English version of Research Profile Questionnaire which was translated into Korean by the researchers and was validated in its translation by two professors. A pretest procedure was done before the data collection process. Of the population of 283, 210 subjects received the questionnaire and 150 subjects responded(71.4%). Excluding incomplete questionnaires, 141 questionnaires were utilized for data analysis. Data were analyzed using descriptive statistics. Responses to open-ended questions were content analyzed for themes and categories. Results of the study were as follows : Respondents currently , involved in nursing research numbered 108(76.6%), but 33(23.4%) did not have any research experience. The inexperienced respondenLs tended to be younger than the experienced subjects and most( 60.6%) were employed in hospitals, while 82.4 percent of the experienced nurses were employed in nursing schools. Among the respondents with research experience, 68.5 percent were involved in one to three projects and 31.5 percent were engaged in four or more projects. The experienced nurses indicated that nursing research in Korea is active. On the other hand, the responses of the inexperienced were that nursing research is not active. The most frequently selected research subjects were patients(76.9%, 83 respondents) followed by nurses, healthy persons and nursing students. The relatively high percentage of healthy persons as study subjects, could be explained as a concern of nurses for healthy persons as well as for ill patients. The aforememioned literature review showed a dramatic change in the kinds of study subjects; approximately 33 percent were patients but 25 percent were healthy subjects of various ages. The hospital was the prevailing research setting(67.6%) but at the same time various community settings were used. This is a changing phenomena in nursing research of Korea. Current research designed to build on previous studies amounted to 75 percent; about 39.8 percem were theory and hypothesis generated. Over 65 recommended additional research focusing on hypothesis-testing 0:' theory building. The previously quoted literature review found that only 4.0 percent of the study quescions were developed from a theoretical framework. The most frequently listed current major focus of nursmg research was stress and adaptation. Patients with cancer, pain, social support and care of the elderly were the next most frequently reported. These concerns may reflect problems associated with a technological and industrializing societyl. The most frequently identified problem was lack of clinical research. The need for replication research and research that contributes to the accumulation of nursing knowledge were found to be rare in Korean research. A need for theory testing and theory construction research was also identified. Although advanced statistical methods were often utilized in nursing research, the results were frequently considered by the interviewers not to be applicable in practice, and readers had difficulty in comprehending the findings. Even though the number of clinical nurses involved in research is increasing, it is still considered inadequate. Among 108 respondents, 83(76.9%) gave lack of time as a barrier to conducting nursing research. Over fifty percent of the respondents reported lack of funding or, personally, low confidence in research skills as barriers. The respondents offered 183 suggestions for future emphasis in nursing research. Among the 51 areas of emphasis, clinical research received the highest number and nursing intervention was next. The findings suggested that nursing research should be based on theory, be related to practice and relevant to the situation of a unique Korean culture.

농촌(農村)에 있어서 분만개조요원(分娩介助要員)의 봉사(奉仕)에 의(依)한 모자보건(母子保健)rhk 가족계획(家族計劃)에 관(關) 연구(硏究) (A Study on Maternity Aids Utilization in the Maternal and Child Health and Family Planning)

  • This study was conducted to assess the effectiveness of service by maternity aids concerning maternal and child health in improving simultaneously infant mortality, contraception and vital registration among expectant mothers in rural Korea, where there is less apportunity for maternal and child health care. It is unrealistic to expect to solve this problem in rural Korea through professional persons considering the situation of medical facilities and the socioeconomic condition of residents. So, we intended to adopt a system of services by maternity aids who were educated formally among indigenous women. After the women were trained in maternal and child health, contraception, and registration for a short period, they were assigned as a maternity aids to each village to help with various activities concerning maternal and child health, for example, registration of pregnant women, home visiting to check for complications, supplying of delivery kits, attendance at delivery, persuasion of contraception, and invitation for registration and so on. Mean-while, four researchers called on the maternity aids to collect materials concerning vital events, maternal child health, contraception and registration, and to give further instruction and supervision as the program proceeded. A. Changes of women's attitude by services of maternity aid. Now, we examined to what extent' such a service system to expectant mothers affected a change in attitude of women residing in the study area as compared to women of the control area. 1) In the birth and death places, there were no changes between last and present infants, in study or control area. 2) In regard to attendants at delivery, there were no changes except for a small percentage of attendance (8%) by maternity aid in study area. But, I expect that more maternity sids could be used as attendants at delivery if they would be trained further and if there was more explanation to the residents about such a service. 3) Considering the rate of utilization of sterilized delivery kit, I am sure that more than 90 percent would be used if the delivery kit were supplied in the proper time. There were significant differences in rates between the study and the control areas. 4) Taking into consideration the utilization rate of the clinic for prenatal care and well baby care, if suck facilities were installed, it would probably be well utilized. 5) In the contraception, the rate of approval was as high as 89 percent in study area as compared to 82 percent in the control area. 6) Considering the rate of pre-and post-partum acceptance on contraception were as much as 70 percent or more, if motivation to use contraception was given to them adequately, the government could reach the goals for family planning as planned. 7) In the vital registration, the rate of birth registration in the study area was some what improved compared to that of the control area, while the rate of death registration was not changed at all. Taking into account the fact that the rate of confirmation of vital events by maternity aids was remarkably high, if the registration system changed to a 'notification' system instead of formal registration ststem, it would be improved significantly compared to present system. B. Effect of the project Thus, with changes in the residents' attitude, was there a reduction in the infant death rate? 1) It is very difficult problem to compare the mortality of infants between last and present infants, because many women don't want to answer accurately about their dead children especially the infants that died within a few days after birth. In this study the data of present death comes from the maternity aides who followed up every pregnancy they had recorded to see what had happened. They seem to have very reliable information on what happened in first few weeks with follow up visitits to check out later changes. From these calculaton, when we compared the rate of infant death between last and present infant, there was remarkable reduction of death rate for present infant compare to that of last children, namely, the former was 30, while the latter 42. The figure is the lowest rate that I have ever heard. As the quality of data we could assess by comparing the causes of death. In the current death rate by communicable disease was much lower compare to the last child especially, tetanus cases and pneumonia. 2) Next, how many respondents used contraception after birth because of frequent contact with the maternity aid. In the registered cases, the respondents showed a tendency to practice contraception at an earlier age and with a small number of children. In a comparison of the rate of contraception between the study and the control area, the rate in the former was significantly higher than that of the latter. What is more, the proportion favoring smaller numbers of children and younger women rose in the study area as compared to the control area. 3) Regarding vital registration, though the rate of registration was gradually improved by efforts of maternity aid, it would be better to change the registration system. 4) In the crude birth rate, the rate in the study area was 22.2 while in the control area was 26.5. Natural increase rate showed 15.4 in the study area, while control area was 19.1. 5) In assessment of the efficiency of the maternity aids judging by the cost-effect viewpoint, the workers in the Medium area seemed to be more efficiency than those of other areas.
