Recent Advancements in Smart Bandages for Wound Healing

  • Wound healing is a complex and dynamic process, making the accurate and timely assessment of skin wounds a crucial aspect of effective wound care management, especially for chronic wounds. Unlike conventional wound dressings that simply cover the wound area once some form of medicine is administered onto the wound, recent studies have introduced versatile approaches to smart wound dressings capable of interacting with wound fluids to monitor physicochemical and pathological parameters to determine the wound healing status. Such electrochemical wound dressings can be integrated with on-demand, closed-loop drug delivery or stimulation systems and ultimately expanded into an ideal technological platform for the prevention, treatment, and management of skin wounds or illnesses. This article briefly reviews the wound healing mechanism and recent strategies for effective wound care management. Specifically, this review discusses the following aspects of smart wound dressings: sensor-integrated smart bandages to detect wound biomarkers, smart bandages developed to accelerate wound healing, and wireless, closed-loop automatic (on-demand) wound healing systems. This review concludes by providing future perspectives on effective wound care management.

일반외과 입원 환자에 적용되는 간호진단-간호결과-간호중재 연계 확인 (Identification of Nursing Diagnosis-Outcome-Intervention (NANDA-NOC-NIC) Linkages in Surgical Nursing Unit)

  • Purpose: This study was to identify nursing diagnosis-outcome-intervention (NANDA- NOC-NIC: NNN) linkages applied to inpatients in general surgical nursing units. Methods: We developed the NNN linkage computerized nursing process program, which consisted of the 107 nursing outcomes and the 190 nursing interventions linked to the 39 nursing diagnoses. This program was applied to 324 patients who admitted to those nursing units from July, 2004 to February, 2005. Results: First, nursing outcomes of each nursing diagnosis were identified as follows: for 'acute pain', pain control, pain level, and comfort level; for 'risk for infection', wound healing: primary intention, wound healing: secondary intention, and infection status; for 'nausea', nutritional status: food & fluid intake, comfort level, symptom severity and hydration. Second, major nursing interventions for each nursing outcome were analyzed as follows: for pain control or comfort level, pain management and medication management; for pain level, pain management and analgesic administration; for wound healing: primary intention, incision site care and wound care; for Wound healing: secondary intention or infection status, infection control; for nutritional status: food & fluid intake, fluid monitoring; for comfort level, nausea management; for symptom severity, nausea management and vomiting management; for hydration, fluid/electrolyte management. Conclusion: This identified NNN linkages will facilitate the use of nursing process in surgical nursing practice and documentation systems.

전신성 혈관염 환자의 족부 만성 창상의 치험례 (Treatment of Chronic Wound in a Patient with Systemic Vasculitis)

  • Management of chronic wound has been a great problem to many surgeons because the wound is usually associated with an underlying disease of the patient. Without accurate diagnosis and treatment of the disease, the wound can not be healed. Systemic vasculitis is a rare systemic disease which causes inflammation and obstruction of the vessels. This autoimmune disease involves multiple organs and may inflict skin wound spontaneously without traumas. It would improve or aggravate the wound in proportion to the activities of the disease. Our experience is a case of 28-year-old female who has had chronic ulcers on her right foot, especially on the great toe for 1 year. Although she had several operations of sympathectomy, debridement and artificial dermal graft, her wound was not improved. She has been diagnosed as systemic vasculitis during the evaluation for histopathology and cause of fever and pancytopenia. After medical treatments, she had the operation of adipofascial turnover flap coverage and skin graft, and the wound was improved without any complication or relapse. The diagnosis and treatment of the underlying disease should be ahead of the management of chronic wound.

  • NPWT는 90년대 이후 많은 연구가 이루어지며 발전해왔다. 이 기술은 사용하기에 간단하고 다양한 상처에 적용할 수 있고 효용성과 비용-효과가 뛰어나며 부작용이 적다. 따라서 NPWT를 일반적인 드레싱 대신에 사용하면 육아조직의 빠른 성장과 상처 수축을 통해 피부 이식이나 피판이 필요한 부위를 줄일 수 있고 봉합이 불가했던 부위를 가능하게 하므로 창상 환자에 대한 의료 비용을 감소시키고 만성 창상 환자가 입원하지 않고 움직이면서 창상 관리가 가능하도록 할 수 있는 유용한 방법이다.

New techniques for wound management: A systematic review of their role in the management of chronic wounds

  • Debridement is a crucial component of wound management. Recent technologies such as hydrosurgery (Versajet), ultrasound therapy (the MIST therapy device), or plasma-mediated bipolar radio-frequency ablation therapy (Coblation) seem to represent interesting alternatives for wound debridement. The purpose of this systematic review was to describe, evaluate, and compare these three recently developed methods for the management of chronic wounds. In January 2016, an electronic database search was conducted of MEDLINE, PubMed Central, and Embase for articles concerning these three innovative methods for the management of chronic wounds. A total of 389 references were identified by our search strategy, and 15 articles were included. We extracted data regarding the number and age of patients, indications, operating time, number of procedures, costs, wound healing time, decrease in exudation, perioperative blood loss, bacterial load, and the occurrence of complications. The 15 articles included studies that involved 563 patients who underwent hydrosurgery (7 studies), ultrasound therapy (6 studies), or Coblation (2 studies). Six randomized controlled trials were included that compared the use of a scalpel or curette to hydrosurgery (2 studies) or ultrasound therapy (6 studies). Hydrosurgery, in addition to being a very precise and selective tool, allows significantly faster debridement. Ultrasound therapy provides a significant reduction of exudation, and improves the wound healing time. No comparative study dedicated to Coblation was identified. Despite the obvious clinical interest of the topic, our review of the current literature revealed a lack of prospective randomized studies comparing these devices with each other or with standard techniques, particularly for Coblation and hydrosurgery.

  • Purpose: Wound healing is a result of complex processes whose components, such as cells, extracellular matrix, proteolytic enzymes, and their inhibitors receive effects from immune compartments, cytokines, chemokines, and growth factors. Impairment of normal physiologic response to wounding makes nonhealing chronic wounds. Wound infection and exacerbated proteolytic process may induce uncontrolled tissue degradation or exudates formation, which may result in the development of a nonhealing chronic wound. Thus proper management of wound infection and exudates is critical to prevent and treat nonhealing wound. The aim of this study is to evaluate effects of Aquacel AG, silver-containing carboxymethylcellulose dressing on treatment for exudative infected wound. Methods: The study included 31 patients with nonhealing wound. Wound was dressed with Aquacel AG. The effect of dressing was investigated by serial bacterial culture and wound exudates assessment. Each infection and exudates control time was determined and statistically analyzed. Results: Wound infection and exudates were effectively managed using Aquacel AG dressing. Mean infection and exudates control time were $3.4{\pm}1.2$ and $5.7{\pm}1.4$ weeks, respectively. Statistical analysis of the data indicated that infection control time correlated positively to age and exudates control time (p<0.05). Conclusion: There is as yet no ideal dressing for the topical treatment of chronic nonhealing wound. But silver-containing carboxymethylcellulose dressing can be used effectively for exudative, infected nonhealing wound.

Adaptation of Evidence-based Surgical Wound Care Algorithm

  • Purpose: This study was designed to adapt a surgical wound care algorithm that is used to provide evidence-based surgical wound care in a critical care unit. Methods: This study used, the 'ADAPTE process', an international clinical practice guideline development method. The -'Bonnie Sue wound care algorithm' - was used as a draft for the new algorithm. A content validity index (CVI) targeting 135 critical care nurses was conducted. A 5-point Likert scale was applied to the CVI test using a statistical criterion of .75. Results: A surgical wound care algorithm comprised 9 components: wound assessment, infection control, necrotic tissue management, wound classification by exudates and depths, dressing selection, consideration of systemic factors, wound expected outcome, reevaluate non-healing wounds, and special treatment for non-healing wounds. All of the CVI tests were ${\leq}$.75. Compared to existing wound care guidelines, the new wound care algorithm provides precise wound assessment, reliabilities of wound care, expands applicability of wound care to critically ill patients, and provides evidence and strength of recommendations. Conclusion: The new surgical wound care algorithm will contribute to the advancement of evidence-based nursing care, and its use is expected as a nursing intervention in critical care.

Aureobasidium 유래 ${\beta}$-Glucan의 Nude Mouse 감염 피부에 대한 창상 치유 효과 (Effect of ${\beta}$-Glucan Originated from Aureobasidium on Infected Dermal Wound Healing of the Normal Nude Mouse)

  • Nude mouse의 세균 감염 피부에서 Aureobasidium 유래 ${\beta}$-glucan이 창상 치유에 미치는 영향을 평가하기 위해 본 실험을 실시하였다. 실험 결과, ${\beta}$-glucan은 S. aureus, S. pyogenes 및 P. aeruginosa 에 대해 살균 작용을 나타내지는 않았으나 대조군에 비해 감염 창상 크기를 현저히 감소시켰다. 따라서 ${\beta}$-glucan을 새로운 창상 치유 촉진제로서 고려해볼 수 있을 것이다.

  • The management of penetrating zone II neck injuries without hard signs of vascular injury has been controversial. The controversy lies between mandatory exploration and a selective approach to the management of theses injuries. Authors that advocate mandatory exploration state its low complication rate and high sensitivity in support of this approach. Surgeons in support of selective management argue selective management has comparable efficacy with lower morbidity in comparison with mandatory exploration. Recently we experienced a case of stab wound near vertebral artery and operatively explored, therefore we report a case along with review of literature.

개심술 후 발생한 심부 흉골 창상감염의 세척-흡입법을 이용한 치험 -4예 보고- (Irrigation-suction System for Management of Deep Sternal Wound Infection after Open Heart Surgery -Four cases-)

  • 개심술 후 발생하는 심부 흉골 창상감염(급성 종격동염)은 아주 심각한 합병증으로 높은 이환율, 사망률, 그리고 입원기간의 연장을 보인다 치료 결과는 주로 흉골 창상감염의 조기 진단과 적절한 외과적 조치에 의존한다. 개심술 후 심부 흉골 창상감염을 보였던 4예에서 광범위한 변연절제술, 항생제 용액의 높은 압력 하 세척, Robicsek방식에 의한 흉골의 재 고정, 항생제 용액을 사용한 세척-흡입법을 통해 성공적인 치료를 하였기에 문헌 고찰과 함께 보고한다.
