  • Background: Laryngeal malignancy is a common malignancy of the head and neck region. Affected patients usually present with features that are characteristic of certain subsites. The larynx is oncologically divided into three: supraglottis, glottis and subglottis. Studies from Western countries have shown that the glottis is the commonest subsite to harbour laryngeal malignancy. However, the supraglottis has been reported to be the commonest subsite in developing countries, including examples in the Indian subcontinent. To our knowledge, no study has been carried out in western Nepal about the epidemiology of laryngeal cancer to date. The purpose of this study was to analyse the epidemiology of laryngeal cancer in relation to its risk factors, age distribution, and preferred subsites and to see if there is any recent change in the subsite wise distribution of laryngeal malignancy in western Nepal. Materials and Methods: Patients of all ages and both sexes with suspected laryngeal malignancy were enrolled in the initial study. Detailed history taking and clinical examination was performed to find out the involved subsite in relation to the clinical features. Direct laryngoscopy was performed to further confirm the subsite and to take biopsy from the growth under general anesthesia. After confirmation of malignancy from the biopsy report, patients were finally included in the study. Data were analysed and observations were made to find out the distribution of laryngeal malignancy in different subsites. Results: The supraglottic larynx was the commonest subsite to harbor laryngeal malignancy. Smoking and alcohol were found to be the common risk factors. The mean age of the patients was in their sixties. Conclusions: Laryngeal malignancy is common in elderly individuals. Supraglottic laryngeal malignancy is the commonest laryngeal malignancy in people who smoke and drink alcohol in Nepal. Avoidance of alcohol use and smoking will be a milestone to reduce the incidence of laryngeal cancers and associated mortality.

  • Being latecomers to the industrialization, Pacific rim countries except the U.S.A. are doing very little in developing the universal certification system. One should note that Asian Pacific region enjoys the highest economic growth rate, and there is a good possibility that it will be the center of economic activities of the world in the next century. It appears that ISO and CEN share similar specifications in numerous areas. ASNT is being changed to cope with the trend with the introduction of ACCP. Although we may have to adopt the new system someday, we should pay attention and make concerted effort to include our own practice in the global standards. We are the countries in the Far East Region with similar culture and tradition, which are quite different from those of western countries. We hope that Japan should play an important role in this matter, who has been chaired the ISO TC135 since 1993. In the beginning of this decade, there has been a trivial but troublesome matter that who is representing the NDT Society or member countries in Asia-Pacific Conference on NDT. The problem appears to be solved since the establishment of FENDT For the next decade, actually next centry, FENDT will have to accomplish another important role for countries in the Far East Region to form a strong coalition for its survival in the western dominant world. Immediate issue is to establish Far East Working Group for Harmonization and Mutual Recognition of Qualification and Certification of NDT Personnel. To realize this Japan must play a leading role with the cooperation of expanded FENDT member countries including China and Russia. As the continued prosperity expected in this region, FENDT should continue its role for the next centry. We, the FENDT countries, must move forward for internationalization(國際化). However' we must realize that it all start from SELF IDENTI-FICATION. We can identify ourselves by actively participating in FENDT. I would like remind you the concluding remark that I have made in the congratulatory speech at JSNDI 40th Anniversary Conference and FENDT'92. JSNDI is too precious to be Japan's sole possession Staling from the Far East, JSNDI shall become one of the loaders of the world. I sincerely hope that JSNDI take initiatives and leaderships in the future activities. of FENDT countries. I would like to close my talk with "We must hang together, or We shall be hang seperately." • This study investigates the characteristics of the Gross Moist Stability (GMS) over the tropics. The GMS summarizes the relationship between large-scale entropy forcing due to radiation and surface fluxes and the response of smaller-scale convection. The GMS is able to explain both to where moist entropy is advected by the atmospheric circulation and how deep the moisture flux convergence is in the tropical region. In the deep convective region, positive GMS appears over the warm pool region due to the strong column-integrated moisture convergence and the ensuing export of moist entropy to the environment. The vertical advection of moist entropy dominates over the horizontal advection in this region. Meanwhile, over the eastern tropical ITCZ region, which is characterized by shallow convective area, import of moist entropy by horizontal winds is dominant compared to the vertical moist entropy advection. Future changes in the GMS are also examined using the 22 CMIP5 model simulations. A decrease in the GMS appears widely across the tropics, but its increase occurs over the western-central equatorial Pacific. It is evident that the increased GMS region corresponds to an increased region of precipitation, implying that strengthened convection in the future due to increased entropy forcing exports the enhanced moist energy to stabilize the environment.

  • Background: One of the most important reasons for the high mortality rates of cancer is the low level of awareness, which can lead to a late diagnosis and treatments starting too late. Therefore, it is necessary for individuals, especially at younger age, to gain awareness and integrate taking the necessary precautions into their lifestyle in order to prevent cancer and ensure early diagnosis. The aim of this study was to assess the levels of awareness of factors for major cancers among students studying in two different campuses of a university in the Western Black Sea Region. Materials and Methods: This descriptive/cross-sectional study was performed between January-March 2014. Students studying in the 2013-2014 academic year in a university in the Western Black Sea Region in two separate campuses were the subjects. Results: The mean age was $21.01{\pm}3.63$. While female students (51.2%) were the majority in the health sciences campus (74.8%), male students (48.8%) were the majority in the social sciences campus (76.5%). Some 9.6% of the students from the health sciences campus and 12.4% of the students from the social sciences campus thought that cancer was communicable, while most of the students from both campuses knew that smoking caused cancer. It can also be seen that the rate of answering questions regarding cancer correctly was higher among students studying in the health sciences campus. Conclusions: It was determined that students who do not study at the health sciences campus have insufficient information on cancer, cancer symptoms, and the possibility of breast cancer in males and approximately half of them regarded obesity as a risk factor for cancer.

  • The comparison of prediction errors in geopotential height, temperature, and precipitation forecasts is made quantitatively to evaluate medium-range forecast skills between Global Seasonal Forecasting System version 5 (GloSea5) and Unified Model (UM) in operation by Korea Meteorological Administration during 2014. In addition, the performances in prediction of sea surface temperature anomaly in NINO3.4 region, Madden and Julian Oscillation (MJO) index, and tropical storms in western north Pacific are evaluated. The result of evaluations appears that the forecast skill of UM with lower values of root-mean square error is generally superior to GloSea5 during forecast periods (0 to 12 days). The forecast error tends to increase rapidly in GloSea5 during the first half of the forecast period, and then it shows down so that the skill difference between UM and GloSea5 becomes negligible as the forecast time increases. Precipitation forecast of GloSea5 is not as bad as expected and the skill is comparable to that of UM during 10-day forecasts. Especially, in predictions of sea surface temperature in NINO3.4 region, MJO index, and tropical storms in western Pacific, GloSea5 shows similar or better performance than UM. Throughout comparison of forecast skills for main meteorological elements and weather extremes during medium-range, the effects of initial and model errors in atmosphere-ocean coupled model are verified and it is suggested that GloSea5 is useful system for not only seasonal forecasts but also short- and medium-range forecasts.

  • Given the significant social and economic impact caused by heat waves, there is a pressing need to predict them with high accuracy and reliability. In this study, we analyzed the real-time forecast data from six models constituting the Subseasonal-to-Seasonal (S2S) prediction project, to elucidate the key mechanisms contributing to the prediction of the recent record-breaking Korean heat wave event in 2018. Weekly anomalies were first obtained by subtracting the 2017-2020 mean values for both S2S model simulations and observations. By comparing four Korean heat-wave-related indices from S2S models to the observed data, we aimed to identify key climate processes affecting prediction accuracy. The results showed that superior performance at predicting the 2018 Korean heat wave was achieved when the model showed better prediction performance for the anomalous anticyclonic activity in the upper troposphere of Eastern Europe and the cyclonic circulation over the Western North Pacific (WNP) region compared to the observed data. Furthermore, the development of upper-tropospheric anticyclones in Eastern Europe was closely related to global warming and the occurrence of La Niña events. The anomalous cyclonic flow in the WNP region coincided with enhancements in Madden-Julian oscillation phases 4-6. Our results indicate that, for the accurate prediction of heat waves, such as the 2018 Korean heat wave, it is imperative for the S2S models to realistically reproduce the variabilities over the Eastern Europe and WNP regions.

  • The Bismarck Sea represent a unique region in the equatorial western Pacific where one can explore the relationship between tectonic and magmatic processes associated with back-arc opening. The sea, located north of Papua New Guinea and just south of the equator, formed during the final stages of a long, complex geological development of the Melanesian Borderland. The development resulted from the Cenozoic convergence between the Australian and Pacific- Caroline Plates and the opening of back-arc basins. At present, the Bismarck Sea straddles two oppositely facing trenches, the inactive Manus trench and the active New Britain trench, and covers two basins, the New Guinea Basin (NGB) to the west and the Manus Basin (MB) to the east. The two basins are separated by the shallow Willaumez-Manus Rise (WMR), which trends roughly from WNW to ESE. The origin of these major structural units and their relationship with the presentday zone of major seismicity along the Bismarck Sea Seismic Lineation (BSSL) remains unclear and is the main focus of our study.

  • Aim: To evaluate the predominant colorectal polyps in the Almadinah region of Saudi Arabia. Materials and Methods: In this iretrospective study, we analyzed pathology reports of colonoscopies performed in King Fahad Hospital, Madinah, Saudi Arabia during the period 2006 to 2013. Data based on patient age, gender, size, site and type of polyps and the degree of dysplasia were analyzed by software SPSS 17 and compared with other published studies from different geographic regions of the world. Results: During these years, 224 patients had colonic polyps, of whom 149 (66.5%) were men and 75 (33.5%) were women. The most common types of polyps were adenomatous (166), followed by hyperplastic polyps (24), juvenile (18), inflammatory (13), lipomatous (2) and one patient with Peutz-Jegher polyps. Tubulovillous adenoma was the commonest adenomatous polyp (102), followed by tubular (41) and villous (23) types. The sigmoid colon was the most commonly involved region (36.6%). Dysplasia was significantly associated with female patients who had large size tubulovillous polyps located in the left colon. Conclusions: The type and distribution of colorectal polyps in Saudi Arabia is very similar to Western countries. Patient gender, and size, histological type and location of polyps are closely related to dysplastic change in colonic polyps.

  • This study confirms that a decadal variation of the SST (Sea Surface Temperature) in the WNP (Western North Pacific) has an influence on the genesis and passage ofa Tropical Cyclone. The decadal mode was obtained by calculating the SST anomaly on the domain $150^{\circ}E-190^{\circ}E$, and $5^{\circ}S-5^{\circ}N$. Such decadal variation was subsequently analyzed to confirm that it is a dominant mode in central Pacific region. Next, after classifying the years into relatively positive years and relatively negative years, the characteristics of Tropical Cyclone in each year, such as a genesis and passage frequency, were investigated. Compared to the relatively negative years, during the relatively positive years, the location of Tropical Cyclone genesis was biased toward South-Eastern region, while the characteristics of the cyclone were more distinct during late season of the year trom September to December than in mid season from June to August. Examining the movement passage through the observation of passage fiequency, there was a significant difference between positive year and negative year in their passages at a 90% confidence level. Moreover, the number of Tropical Cyclone, maximum wind, and life time also showed higher values in positive years than in negative years. These features were confirmed by examining the 850hPa cyclonic flow field, vorticity field, and vertical wind shear field, all of which contribute to the genesis of a Tropical Cyclone.

  • The geography, circulation pattern, and ecology show that the semi-closed seas of Northwest Pacific be managed as one complete system. Ongoing multilateral cooperative efforts relevant to marine environmental protection in the Northwest Pacific area, include the Working Group for the Western Pacific (WESTPAC) established under the auspices of WNESCO's Intergovernmental Oceanographic Commissions, the UNDP/GEF Programme on Prevention and Management of Marine Pollution in East Asian Seas (PEMSEA), the North Pacific Marine Science Organization(PICES), and the United Nations Environment Programme(WNEP)'s Northwest Pacific Action Plan(NOWPAP). The present report firstly describes the current situations on the existing regional cooperative regimes for marine environmental conservation in the Northwest Pacific region, with a special respect to the Northwest Pacific Action Plan(NOWPAP) which was adopted in 1994 by Japan, People's Republic of China, Republic of Korea and Russian Federation. Then, problems of the existing regimes are also discussed, together with the suggestion of the possible solutions, focusing on NOWPAP. Suggestions include: 1) the Northeast Asian countries should understand the importance of legally-binding regional convention, and should build up any legally-binding instrument which can function as a big umbrella for real regional cooperation without prejudice to the rights of the States, 2) At present stage, it will be possible to make a regional convention flexible without prejudice to the sovereign right of the States or territorial issues; 3) taking into account that the region often faces many generic political problems that often inhibit the effective collective actions on environmental issues, the leadership from UNEP or other international organizations is required; 4) strong institutional and financial framework should be made, and 5) multilateral efforts to respond to the new marine environmental threats should be taken at the regional level in order to protect the coastal and marine environments in the Northwest Pacific.
