  • As part of the digital era, a digital twin that simulates the weak part of a product by performing a stress test that reduces the lifespan of some expensive equipment that cannot be done in reality by accurately moving the real world to virtual reality is being actively used in the manufacturing industry. Due to the development of IoT, the digital twin, which accurately collects data collected from the real world and makes it the same in the virtual space, is mutually beneficial through accurate prediction of urban life problems such as traffic, disaster, housing, quarantine, energy, environment, and aging. Based on its action, it is positioned as a necessary tool for smart city construction. Although digital twin is widely applied to the manufacturing field, this study proposes a smart city model suitable for the 4th industrial revolution era by using it to smart cities and increasing citizens' safety, welfare, and convenience through the proposed model. In addition, when a digital twin is applied to a smart city, it is expected that more accurate prediction and analysis will be possible by real-time synchronization between the real and virtual by maintaining realism and immediacy through real-time interaction.

Triage Accuracy of Pediatric Patients using the Korean Triage and Acuity Scale in Emergency Departments (한국형응급환자분류도구를 적용한 응급실에서 소아 환자의 중증도 분류 정확성)

  • This retrospective study investigates the accuracy of triage procedures for pediatric patients in emergency departments (EDs) using the Korean Triage and Acuity Scale (KTAS). The study includes 250 randomly selected initial nursing records and clinical outcomes of pediatric patients who visited one regional ED or a local ED from October 2016 to September 2017. The collected data were analyzed by a qualified expert to determine the true triage score. The accuracy of triage was defined as the agreement between the triage score of the emergency nurses (ENs) and the true triage score as determined by the expert. Based on expert comments, the cause of the triage error was analyzed and the KTAS score was compared with the discharge, length of stay (LOS), and medical cost. The results showed that the degree of agreement in the triage score between the experts and the ENs was excellent (weighted kappa=0.77). Among the causes of triage discordance, the most frequent was the incorrect application of vital signs to the KTAS algorithm criteria (n=13). Patients with high severity KTAS levels 1 and 2 were discharged less often (${\chi}=43.25$, p<0.001). There were significant differences in the length of stay (F=12.39, p<0.001) and cost (F=11.78, p<0.001) between KTAS scores when adjusting for age. The results of this study indicate that KTAS is highly accurate in EDs. Hence, the newly developed triage tool is becoming well established in Korea.

Occupational Therapy for Community Mobility in Stroke Patients : Systematic review (뇌졸중 환자의 지역사회이동을 위한 작업치료 중재: 체계적 고찰)

  • Objective : The purpose of this study was to analyze occupational therapy intervention on the community mobility for stroke patients, and to provide evidence of intervention in the clinical fields. Methods : A systematic review was executed according to the PRISMA checklist. The accessed database was PubMed, EMBASE, Cochrane Library (CENTRAL), ProQuest Dissertations & thesis (PQDT), RISS, and KoreaMed. We included the articles published from 2005 to September 2018. RoBANS checklist was used to evaluate the quality of the articles. Included articles, totally eight, were categorized according to the type of intervention. Results : The study design of the literature was varied from two-group randomized trial, quasi-experimental study, case-control trial, one group pre-post comparison study, and cross-sectional study. In the evidence level, 6 articles were included in level II (75%). The percentage of low risk of bias in each article ranged from 52.5%~87.5%. Four studies (50%) provided intervention based on virtual reality or virtual environment. The three (37.5%) provided intervention based on the protocol, and the other (12.5%) did wheelchair training. All studies reported significant effects of the intervention. Conclusion : This systematic review provided evidences to use proper intervention in the clinical fields. Various type of studies should be conducted to prove the effect of occupational therapy intervention for community mobility.

A study on forecasting provinces-specific fertility for Korea (시도별 출산력 예측에 대한 연구)

  • The Korean fertility rate has been declining rapidly since 2000 with the fertility rate among provinces following a uniform tendency. In particular, the province-specific fertility rate is an essential tool for local governments to prepare local policies for low fertility aging policy, education and welfare policies. However, there is limitation on how to reflect different trends on the province-specific fertility rate because the KOSTAT's (2017) province-specific fertility rate projection estimates information use the national average birth rate date of vital statistics for the last 10 years (5 years). In this study, we propose an improvement plan that simultaneously considers important stable pattern maintenance and provincial fertility rate differentiation for an annual birth rate estimation. The method proposed in this study (proposal 1 and 2) can reflect birth rate changes from past to present and national and provincial differences by age that use time series data of the annual fertility rate. Proposal 3 also reflects the unique fertility rate trend from the past to the present by age according to province regardless of the relationship with the national trend. Therefore, it is preferable to use a relationship to the national rate when predicting the birth rate, as in proposals 1 and 2 because the national and the provincial fertility rate pattern are similar. These proposals show improved stability in terms of age-specific fertility rates.

A Meta-analysis of the Developmental Effect of Play in Early Childhood (유아 놀이의 발달적 효과에 대한 메타분석)

  • Objective: The purpose of this study was to systematically arrange previous ones on developmental effects of play in early childhood through a meta-analysis. Methods: For this purpose, the researchers searched for variety of databases and analyzed 110 studies, which were 90 graduate theses and 20 journals from 2005-2016. Results: First, the total developmental effect size of infant play was 1.21, with the effect size of the experimental group being 38.7% higher than that of the control group. The total developmental effect size was .81, however when inserted effect size was calculated, so it was supposed that the current effect size might be decreased, if missing studies were included. Second, effect size appears in all developmental areas, though actual effect size of sub-factors of child development tends to be mostly decreased: The fall in physical development fell down from 1.28 to .95 that emotional development was 1.42 to .86. The fall in social development was considerable from 1.13 to .85 that cognitive development was 1.19 to 1.07. In language development, it didn't change much. It stayed about the same as 1.30. The fall In creativity development stood at just below from 1.00 to .69. Third, effect size by moderating variables was examined to show that there were statistically significant difference in measurement, age, total number of training and types of activities between two groups. Conclusion/Implications: The results of this study have an implication, in that the study verified that infant play reflects and promotes child development and functions as a tool for developmental change, by illuminating effects of it child development.

A Preliminary Study on the Correlation Between ICF and Functions of Upper Limbs of Chronic Stroke Patients : ICF Activities, Participations, and Environmental Factors (만성 뇌졸중 환자의 상지 기능과 ICF와의 상관관계 예비 연구 : ICF 활동, 참여 및 환경영역 중심으로)

  • Purpose: This study provides a treatment for central nervous system development in patients with chronic stroke by investigating changes in the upper limb function over time. The correlations among the activities, participation, and environmental factors of the international classification of functioning (ICF), disability and health are also examined. Methods: The subjects of this study are 18 patients with chronic stroke who were hospitalized and treated at 00 hospital in the Chungcheongbuk-do province. Their upper extremity functions are evaluated using the manual function test (MFT). The activities, participation, and environmental factors are evaluated using the ICF generic form. The correlations between the total scores of the affected and unaffected sides and the ICF items are analyzed using the Pearson correlation analysis. The significance level is p<0.05. Results: When the correlations between the activities and participation areas of ICF and the total score of the affected side of MFT were examined, significant correlations (p<0.05) were found in the following items: changing basic body position (D410), lifting and carrying objects (D430), moving around using equipment (D465), using transportation (D470), washing oneself (D510), caring for body parts (D520), and dressing (D540). When the correlations between the activities and participation areas of ICF and the total score of the unaffected side of MFT were examined, significant correlations (p<0.05) were found among writing (D170), speaking (D330), eating (D550), and drinking (D560). In addition, when the correlation between the environment area of ICF and the total score of the unaffected side of the MFT were examined, significant correlations (p<0.05) were found between products and technology for personal use in daily living (E115) and immediate family (E310). Conclusion: The MFT of patients with chronic stroke is closely correlated with the activities, participation, and environmental factors of ICF. This result suggests that ICF can be used as a useful tool to comprehensively evaluate the abilities of the patient, including the upper extremity function.

Psychological Characteristics and Life Satisfaction of the Elderly -Focusing on Gwangju Metropolitan City- (노인의 심리적 특성과 삶의 만족도 분석 -광주광역시를 중심으로-)

  • The purpose of this study is to identify the psychological characteristics of the elderly and to investigate the variables that affect the life satisfaction of the elderly. To achieve this purpose, psychological characteristics such as self-esteem, alienation, loss, and depression were selected. A survey was conducted on the elderly living in Gwangju Metropolitan City. A total of 218 copies were collected and 203 copies were analyzed, excluding poor responses. To draw the results of the research, quantitative analysis was conducted through questionnaires, and SPSSWIN 21.0 statistical program was used as an analysis tool. The analysis methods were frequency and ratio analysis, technical statistical analysis, bivariate correlation analysis, and linear regression analysis. The results of the analysis are as follows: First, the relationship between the psychological characteristics of the elderly showed significant correlations among the self-esteem, alienation, loss, depression, and life satisfaction of the elderly. Second, for the elderly, alienation and depression had a negative effect on life satisfaction, and self-esteem had a positive effect on life satisfaction. However, loss did not have a significant effect on life satisfaction. In conclusion, it suggests that it is necessary to develop and operate policies and programs to improve self-esteem and reduce alienation and depression in order to improve the quality of life of the elderly.

Effects of acute heat stress on salivary metabolites in growing pigs: an analysis using nuclear magnetic resonance-based metabolomics profiling

  • Heat stress (HS) causes adverse impacts on pig production and health. A potential biomarker of HS is required to predict its occurrence and thereby better manage pigs under HS. Information about the saliva metabolome in heat-stressed pigs is limited. Therefore, this study was aimed to investigate the effects of acute HS on the saliva metabolome and identify metabolites that could be used as potential biomarkers. Growing pigs (n = 6, 3 boars, and 3 gilts) were raised in a thermal neutral (TN; 25℃) environment for a 5-d adaptation period (CON). After adaptation, the pigs were first exposed to HS (30℃; HS30) and then exposed to higher HS (33℃; HS33) for 24 h. Saliva was collected after adaptation, first HS, and second HS, respectively, for metabolomic analysis using 1H-nuclear magnetic resonance spectroscopy. Four metabolites had significantly variable importance in the projection (VIP > 1; p < 0.05) different levels in TN compared to HS groups from all genders (boars and gilts). However, sex-specific characteristics affected metabolites (glutamate and leucine) by showing the opposite results, indicating that HS was less severe in females than in males. A decrease in creatine levels in males and an increase in creatine phosphate levels in females would have contributed to a protective effect from protein degradation by muscle damage. The results showed that HS led to an alteration in metabolites related to energy and protein. Protection from muscle damage may be attributed to the alteration in protein-related metabolites. However, energy-related metabolites showed opposing results according to sex-specific characteristics, such as sex hormone levels and subcutaneous fat layer. This study had shown that saliva samples could be used as a noninvasive method to evaluate heat-stressed pigs. And the results in this study could be contributed to the development of a diagnostic tool as a noninvasive biomarker for managing heat-stressed pigs.

A Study on the Effect of Individual Development of Job Rotation on Job Satisfaction - Focused on the Mediating Effect of Work Efficiency and the Moderating Effect of Work Overload - (직무순환의 개인발전이 직무만족도에 미치는 영향에 관한 연구 -업무효율성의 매개효과와 업무과부하의 조절효과를 중심으로-)

  • This study conducted a survey of social welfare facility workers in Mokpo, Jeollanam-do from May 21 to May 24, 2019 to empirically analyze the effect of job rotation on work efficiency and job satisfaction. Individual development of job rotaion (independent variable), job satisfaction (dependent variable), work efficiency (mediating variable), and work overload (moderating variable) were set as variables of the research model. For the questions of all variables, the Likert 5-point scale was used, the SPSS version 24.0 was used as an empirical analysis tool to verify the hypothesis, descriptive statistics and reliability analysis were performed, and the mediating effect and moderating effect were analyzed using SPSS PROCESS macro 4.0. The results of this study are as follows. First, it was analyzed that the individual development of job rotation did not significantly affect job satisfaction. Second, it was found that work efficiency fully mediated the relationship between individual development of job rotation and job satisfaction. Finally, it was found that the interaction term between work efficiency and job overload had a negative effect on job satisfaction and had a moderated mediating effect.

Validity and reliability of Korean version of quality of life questionnaire related with music perception and engagement of the elderly (난청노인의 한국어판 음악지각과 참여와 관련된 삶의 질 설문지의 타당도와 신뢰도)

  • The purpose of this study is to develop the Korean version of Music-Related Quality of Life (K-MRQoL) for the elderly. The K-MRQoL consisted of Musical Ability, Attitude, Activity Frequency (PART 1) and Musical Ability, Attitude, Activity Important (PART 2). Each subcategory consists of Music Perception with 11 items and Music Engagement with 7 items. The validity and reliability of K-MRQoL were measured with Pearson's and Cronbach's alpha correlation coefficients and Independent t-test in total 30 elderly with normal hearing and 30 elderly with hearing loss from local welfare Centers and nursing homes. The correlation coefficients between total scores and PART 1 and PART 2 ranged from .701 to .948 and from .598 to .926, respectively. The internal consistency between total and Part 1 and Part 2 ranged from .846 to .931 and from .838 to .918, respectively. The test-retest correlations were .979, .970, and 979 for total, PART 1, and PART 2, respectively. The correlation between K-MRQoL and Quality of Communication Life Scale was .449. There were significant differences in total, PART 1, and PART 2 between the elderly with normal hearing and hearing loss. This indicates that the K-MRQoL can be used as a useful clinical tool to evaluate Music-related Quality of Life in the elderly with normal hearing or hearing loss.
