Analysis on the Relating Factors of Profitability of Korean Public Corporation Medical Centers(KPCMCs) (지방공사 의료원의 수익성 관련요인 분석)

  • The objective of this study is to analyze a current trend of and relating factors on profitability of the Korean Public Corporation Medical Centers(KPCMCs, hereinafter, hospitals) in Korea. There are 34 hospitals in Korea as of 2004. Among these hospitals some are red ink hospitals, others are black inks in terms of profitability. Data were collected by Korea Health Industry Development Institute(KHIDI) Statistics for Hospital Management 2000-2002 and Ministry of Health and Welfare(MOHW) financial data of public hospitals which was planned to coordinate public health care services roadmap in the long run. The samples are 32 hospitals. Profitability was measured in the aspect of profit rate with normal profit to total assets, and normal profit to gross revenues as dependent variables in respective. Independent variables were classified by general factors, i.e., location, intern/resident training, period of opening, number of beds, and managerial factors(current ratio, fixed ratio, liability to total assets, total assets turnover, personnel costs, materials cost, administrative cost), and finally factors related to patient treatment(average length of stay, bed occupancy rate, admission ratio of outpatients). The methods of analysis are correlation and multiple regression analysis. This study shows firstly, a lot of hospitals are optimal current ratio. Hospitals in upper 100% current ratio are 81.2%. And the personnel cost in total costs are high. Secondly, the trend of normal profit to gross revenues of hospitals are deteriorating gradually. And lastly, as a result of multiple regression analysis, the factors had on significant effect on normal profit to total assets are fixed ratio(+), liability to total assets(-), bed occupancy rate(+), admissions of outpatients(+), etc. And the factors had on significant effect on normal profit to gross revenues are current ration(+), fixed ratio(+), personnel cost(-), administrative expenses(-), admissions of outpatients(+), etc. In conclusion, to improve the profitability of hospitals, the efforts to reduce personnel cost and average length of stay might be needed. And also beds utilization rate need to be increased.

The Relationship between Autonomous Supervision Performance and Role Performance by Kindergarten Teacher (유치원 교사의 자율장학 수행유형과 역할 수행유형과의 관계)

  • This study was designed to investigate the difference in the autonomous supervision and role performance according to the socio-demographic factors of the teachers. Also, the relation of the autonomous supervision and the role performance was examined. This research was conducted using the questionnaire to 394 teachers of national, public, and private kindergarten in Chung-buk province. Data were analyzed with the SPSSWIN program by the frequency, percentage, t-test, ANOVA, Scheffe post-hoc test and Pearson's correlation analysis. The results were summarized as follows: First, levels of autonomous supervision of kindergarten teachers were high in general and there were significant differences according to the institution, age, scholastic ability, and the career. The accomplishment in the public kindergarten was higher than that in the private one. Moreover, the higher the teacher's age, career, and scholastic ability, the higher the accomplishment. Second, levels of role performance of the kindergarten teachers were excellent, but the levels of role performance were higher in the public kindergarten than in the private one because the public teachers who did not transfer much had more chances of continuous education and higher responsibility. The role performance of teachers was found to be relatively higher in the kindergarten in countries than in cities because there were more public kindergartens than private ones in countries. Third, there were significant correlations between the lower factors of autonomous supervision and the role performance of the kindergarten teachers. Because the various types of the autonomous supervision can help the teacher's role performance, various types of the autonomous supervision which fit the developmental stages of the teachers will be desirable.

Housing Welfare Policies in Scandinavia: A Comparative Perspective on a Transition Era

  • It is commonplace to refer to the Nordic countries of Sweden, Norway, Denmark, Finland and Iceland as a distinctive and homogenous welfare regime. As far as social housing is concerned, however, the institutional heritage of the respective countries significantly frames the ways in which social housing is understood, regulated and subsidized, and, in turn, how housing regimes respond to the general challenges to the national welfare states. The paper presents a historical institutionalist approach to understanding the diversity of regime responses in the modern era characterized by increasing marketization, welfare criticism and internationalization. The aim is to provide outside readers a theoretically guided empirical insight into Scandinavian social housing policy. The paper first lines up the core of the inbuilt argument of historical institutionalism in housing policy. Secondly, it briefly introduces the distinctive ideal typical features of the five housing regimes, which reveals the first internal distinction between the universal policies of Sweden and Denmark selective policies of Iceland and Finland. The Norwegian case constitutes a transitional model from general to selective during the past quarter of a decade. The third section then concentrates on the differences between Denmark, Sweden and Norway in which social housing is, our was originally, embedded in a universal welfare policy targeting the general level of housing quality for the entire population. Differences stand out, however, between finance, ownership, regulation and governance. The historical institutional argument is, that these differences frame the way in which actors operating on the respective policy arenas can and do respond to challenges. Here, in this section we lose Norway, which de facto has come to operate in a residual manner, due to contemporary effects of the long historical heritage of home ownership. The fourth section then discusses the recent challenges of welfare criticism, internationalization and marketization to the universal models in Denmark and Sweden. Here, it is argued that the institutional differences between the Swedish model of municipal ownership and the Danish model of independent cooperative social housing associations provides different sources of resistance to the prospective dismantlement of social housing as we know it. The fifth section presents the recent Danish reform of the governance model of social housing policy in which the housing associations are conceived of as 'dialogue partners' in the local housing policy, expected to create solutions to, rather than produce problems in social housing areas. The reform testifies to the strategic ability of the Danish social housing associations to employ their historically grounded institutional relative independence of the public system.

A Study on Job Satisfaction among Physical Therapists in the Public Health Centers of the Seoul Metropolitan Area (서울특별시 공공의료기관 물리치료사의 직무 만족도에 관한 조사연구)

  • Purpose: The purpose of this study was to investigate (1) job satisfaction, (2) general characteristics and (3) working fields of 57 physical therapists at public health centers in the Seoul Metropolitan area. Methods: We distributed questionnaires to 57 physical therapists and collected completed questionnaires from 51 We analyzed the data using frequency analysis, independent t-tests and analysis of variance. Results: The results were as follows; 1) General characteristics of participants were 68.6% female, 64.7% over 35 years old, 43.1% working over 10 years in the public health centers. 2) Most (56.8%) participants treated 20~29 patients a day 66.7% were regular staff members and 33.3% irregular ones. 3) Participants, who worked at the public health center and who worked at the Seoul Metropolitan affiliated organization, showed a sense of dignity about their job. 4) Regular staff members showed a higher satisfaction rate with interpersonal relations than irregular ones. Satisfaction in the work place was greatly increased in those over 40 years old. Conclusion: Because the number of patients who visit physical therapists in Seoul Metropolitan public health centers have been increasing, it is necessary to improve the level of satisfaction of physical therapists so that there will be better physical therapy services.

The Actual Status of Elderly Orofacial-Function Improvement Program in Seoul and Gyenggi-Area

  • Background: Although the orofacial-function improvement exercise (OFIE; oral exercise) was first introduced in Korea 10 years prior, it is still not covered by medical insurance, and no detailed survey on the dissemination of related programs has been conducted. Therefore, this study investigated the actual status of the education and practice of OFIE among the elderly and at elderly welfare institutions in the Seoul and Gyeonggi Provinces. Methods: Senior citizens aged more than 65 years old, public health centers (total of 69) and elderly welfare institutions (including nursing homes and elderly welfare centers, total of 56) per administrative area in the Seoul and Gyeonggi Provinces were targeted. We analyzed 200 elderly people and 93 institutions who agreed to participate in the survey. For the elderly, general characteristics, experience and route, current practice, and necessity regarding OFIE were investigated. For institutions, the history and plan of education programs on OFIE were investigated. Results: Regardless of the general characteristics, both the rate of experience and practice for OFIE were low overall; moreover, although they felt it was necessary, they had insufficient motivation for its implementation. Moreover, only a few institutions which were operating the education about OFIE regardless of the COVID-19 situation. Conclusion: Although OFIE is necessary for the elderly, its distribution remains insufficient. Therefore, further efforts are needed to expand the education and raise the awareness of oral exercise among elderly individuals and senior welfare institutions.

Factors Influencing Safe Nursing Activities of Public Hospital Nurses (공공병원 간호사의 안전간호활동에 미치는 영향 요인)

  • Purpose: In this descriptive study, we aimed to identify factors influencing safe nursing activities of nurses working at public hospitals. Methods: We collected data in March 2021 through an online survey. The subjects were 199 nurses who had worked for at least six months at public hospitals. The self-reported questionnaire consisted of items pertaining to safe nursing activities, perception of patient safety culture, and the general characteristics of the subjects. To examine the differences in the safe nursing activities and perception of patient safety culture by general characteristics, we employed a t-test and ANOVA. We conducted a multiple linear regression analysis to explore the factors affecting safe nursing activities. Results: The mean score for safe nursing activity was 4.59±0.39. Perception of patient safety culture and age group were found to be the main factors affecting the safe nursing activities of nurses working at public hospitals (F=17.24, p<.001, Adj. R2=.40). When the score for perception of patient safety culture increased by 1 point, the score for safe nursing activities increased by 0.58 points. In addition, the score for safe nursing activity in the 40s age group was 0.16 points higher than for those in the 20s age group. Conclusion: Effective management and leadership in public hospitals is needed to improve the culture of patient safety at various levels so as to ensure safe nursing activities.

  • This study typified the rural centers with myeon locations in Hwasun-gun which showed comprehensive and various class types through settlement class structure analysis. It is also for establishing the awareness of strengthening functions and facilities of the centers and the directions of strengthening it. Subjects of the study could be classified into three types including base type (Neungju-myeon), general type (Nam-myeon) and decline type (Dongbok-myeon) through the analysis of settlement class structure. Neungju-myeon location as the base type could function as the myeon location by itself and tended to serve education, health and welfare functions through the strengthening of central living functions. Nam-myeon location as the general type required sports facilities management based on the vicinity and accessibility to the senior's welfare functions. Dongbok-myeon location as the decline type required the accessibility to public health facilities and the security of vicinity to the facilities because of its high population of the aged.

Theoretical Analysis on Optimal SOC Investment in Urban Planning (도시계획관련 사회간접자본 투자의 적정성 분석을 위한 이론적 고찰)

  • The purpose of this paper is to present the optimal investment conditions of SOC facilities for maximizing regional social welfare in implementing the urban development project in the theoretical fashion. Particularily, SOC facilities are divided into both supply-side($P_s$) and demand-side SOC ($P_d$) in the paper. General equilibrium analysis from the intra-regional viewpoint by utilizing Pareto's Optimal Conditions and by revising Samuleson's Conditions for public goods($P_s$ and $P_d$) results in the optimum pattern of SOC investment. The following are important implications from the analysis. First, rather than the pursue social equity, SOC investment is to resolve the issue of efficiency to activate the regional economy. Second, the marginal rate of transformation (MRT) between $P_s$ and $P_d$ in the region is to play a significant role in structuring SOC investment plant of local government for social welfare maximization. Third, the optimal SOC investment policy based on this regional economy but also to generate the enhancement of soical amenities of the residents.

A Study on the Job Environment for Interns and Residents in University Hospital (일부 대학병원 인턴, 레지던트의 직무환경 분석)

  • This study was conducted to evaluate the satisfaction level of interns and residents on training and job environment of two university hospitals in Pusan. The concept of subject's satisfaction was evaluated in general characteristics, socioeconomic status, working condition, human relationship, and job status and scored from 1 to 5. The newly developed questionnaire for this study(32 items) was revised and modified by the preliminary survey. The internal consistency of questionnaire was 0.73(Cronbach's alpha). The self-administered questionnaire was provided to 218 subjects and collected the answering from March 2 to March 25th, 1997, and statistical significances were tested by $x^2-test$, t-test and ANOVA. The satisfaction level between the residents group was significantly different, but satisfaction level of working condition, human relationship and job status showed no difference. The service division part showed highest(2.73) and the surgical division part showed lowest(2.57). The job satisfaction level between the subjects showed higher satisfaction level in working condition, socioeconomic status, and total satisfaction dimension, and intern group showed higher satisfaction level than resident group. Satisfaction level in human relationship dimension, resident group showed higher satisfaction level than intern group, but showed no statistically difference. To improve the working condition for residents, public welfare facilities was firstly suggested and the next was salary increase, overloaded work, respectively. For intern groups, overloaded work and waste work were firstly suggested and the next was public welfare facilities and salary increase, sequentially. The career gets longer, the satisfaction level on general characteristics of working condition, socioeconomic status, and total satisfaction level were increased. The subjects who have religion and higher socioeconomic status showed higher satisfaction level than the other group. The general satisfaction level of subjects in all dimensions showed lower aver age(3.0) and the overall satisfaction level showed below than average. Thus, in order to improvement of subject's working condition, higher quality of life is necessary rather than economic state. In future, these results might be considered to improve the working condition for interns and residents.

A Exploratory Study on the Determinants of Welfare Attitudes of Korean Youth: Focusing on Old Age Responsibility (한국 청소년의 복지의식 결정요인에 대한 탐색적 연구: 노후복지 책임주체를 중심으로)

  • This study purports to examine the potential determinants of welfare attitudes of Korean Youth. In a sense, this study could be considered as exploratory since the independent variables for model are chosen in a somewhat arbitrary manner. The independent variables are mostly value orientation of Youth. The welfare attitude is measured by the question asking respondent's nonnative idea on the primary care-giver in the old age. Data has been collected by National Youth Policy Institute as a part of research project, named "Korean Youth Development Indicators". The analysis shows that the welfare attitudes of Korean youth is influenced. by a number of factors. such as parent's education, a proxy for the parents' SES, attitude toward equality. and state's responsibility in general. The author contends that although this is an exploratory kind of study, this study contributes not only to the research in the field, hut also to the welfare policy-makers by providing generalizable outcomes on the value and attitudes of Korean youth on welfare issues. After all, policy needs public supports to be sustained.
