New Drug Listing Process and Reimbursement Standard Management (약제의 신규등재 절차와 급여기준 관리)

  • The Ministry of Health and Welfare of Korea has implemented various social security programs to ensure a basic standard of living and raise overall quality of life for all citizens. The Korean social security system provides social insurance, public assistance, and social welfare services. To achieve adequate drug benefits, the Drug Management Department of Health Insurance Review and Assessment Service (HIRA) implement drug management duties including drug listing, upper price limit setting, scope of benefits, and post-factum management. When a manufacturer or an importer wants to apply for National Health Insurance (NHI) coverage of the drug that has obtained safety and efficacy approval, the pharmaceutical benefit assessment committee of HIRA evaluates the drug's clinical efficacy and cost-effectiveness to determine whether or not to include the drug into the benefit package. The benefit standards for a listed drug (ingredient) are set either for the whole permitted range or a part of range with conditions. To increase the coverage rate for new drugs, the listed drugs are regularly reviewed for their value. The status of listed drugs can be adjusted or eliminated from the benefit package if the clinical efficacy turns out to be insignificant. Therefore, through these pharmaceutical management procedures, high-quality drugs are provided at reasonable prices, which save healthcare expenditure by price determination and selective coverage in consideration of economic evaluation.

A Cost-Benefit Analysis for the Installation of Piezoelectric Energy Harvester (압전에너지 하베스터 설치사업의 경제성 분석에 관한 연구)

  • The piezoelectric energy harvester is recently being developed and catching on as a way to achieve low carbon green growth. The practical application of the piezoelectric energy harvester is expected to contribute not only to the reduction of greenhouse gas emissions but also to the improvement of residents' welfare. This paper conducted a cost-benefit analysis for the installation of piezoelectric energy harvester on the highway focusing on its impacts on the public. The results showed that the installation of piezoelectric energy harvester on the gyeongbu highway is economically feasible in that it could increase the social welfare for the residents. Finally, this paper suggests policy direction for the practical use of the piezoelectric energy harvester, based on the results obtained.

Money's Worth Analysis of National Pension : Are Returns on National Pension' Contributions Fair? (국민연금에 대한 수익분석 : 국민연금급여는 과연 보험료에 대한 공평한 수익인가?)

  • The main source in financing the National Pension benefits is the contribution raised from the insured's earnings. So, Most of the insured take a great interest in the questions of what return on the payment of contribution National Pension benefits provide and whether there be the difference in return according to earnings level. The Purpose of this study is to assess money's worth of National Pension and to answer the above questions. There are two basic types of money worth analysis, empirical and hopothetical. This study basically belongs to the former in terms that it is based on actual earnings and insured term. For performing money's worth analysis, four different measures which are referred as the "break-even period", the "benefit/tax ratio", the "net lifetime transfer", the "internal rate of return" are used and they all involve the way in which the relationship between the present value of contributions and the present values of benefit is present. The results which evaluate average money's worth of accrued rights before 1999 are le as follows. Break-even period is about 43 months, benefit/tax ratio being 4.9, net lifetime transfers being about 37 mil1ion won, internal rate of return being 33.2%. This verifies that money' worth of National Pension is much higher than actuarially fair. In the mean while, money' worth is proved to be very different according to earnings level. The progressivity relationship between earnings level and rate of return is found in all measures but net lifetime transfer.

  • This study examines the costs and benefits of Korean Self-Support Program of the National Basic Livelihood Security Act. A cost-benefit analysis is used to evaluate the efficiency of the Self-Support program. According to the analysis, the program has net value for society as a whole as well as for the average participant although the result is sensitive to the assumption of forgone earnings. The results of an alternative perspective also show that the increased exit rates (and employment rates) of participants will increase the benefits of the program substantially.

  • Korea Electrical Safety Cooperation(KESCO) have provided the electrical safety speed-call service from 2007 year. Purpose of the service is to reduce discomfort of electricity use and to prevent electrical accident like as electrical fire and shock accident by providing emergency treatment service on fault of the residential electrical facilities notified in the specific house like as a lower-income group and a social welfare facility. But efficiency and economic evaluation of the electrical safety speed-call service is impossible because analysis on the quantitative effect of the service is difficult. This paper presents cost-benefit analysis method and result of the electrical safety speed-call service. The presented cost-benefit analysis method has a two-step process: the first step is to measure quantitative electrical fire prevention effect of the service by using electrical accident statistics and developing outcome analysis logic model of the service effect, and the second step is to analysis cost-benefit(B/C)of the service by calculating quantitative benefit analysis on the measured quantitative electrical fire prevention effect. The results showed that cost-benefit(B/C)of the electrical safety speed-call service is over 4 after 2010 year.

  • The risk of pipe-bursting in multi-regional transmission mains consisting of 89 % of singled pipeline is so high that pipeline stabilization project is required such as renewal and replacement, pipe paralleling, emergency ties. Pipeline stabilization projects could be postponed at the step of initial decision-making because effect of this project is intangible benefit like activation of economic, improvement of welfare related to water. This study is to suggest quantified economical feasibility model for intangible benefit presumption to solve above problem. Cost reduction of emergency water supply, leakage, burst restore and energy efficiency improvement was altered and applied. As a result of economic analysis taking into account estimated benefit and cost under discount rate 5.5 %, service life 40 years, sufficient economic feasibility analyzed with B/C 2.45, NPV 317,700 million won, IRR 9.09 %.

Benefit analysis of visiting healthcare in public health centers (보건소 방문건강관리사업의 편익 분석)

  • Objectives: This study is aimed to analyze service user's benefit and perceived-outcomes of visiting healthcare. Methods: Using a questionnaire survey we analyzed the subjective service quality and satisfaction of survey respondents. The sampling was designed with socioeconomic characteristics(age, sex, user group et al.), and each respondent (N=1,000) was presented with double-bounded dichotomous choice questions. To measure the value of visiting healthcare, we employed a contingent valuation method. Results: The respondents were satisfied with service and quality on visiting healthcare. And the acceptance-to-pay of respondent's benefit was 50,458 won for each visit and in totality, service user's benefit was 185.9 billion won. Conclusions: It is necessary to invest in visiting healthcare in public health centers.

Optimal Particle Swarm Based Placement and Sizing of Static Synchronous Series Compensator to Maximize Social Welfare

  • Social welfare maximization in a double-sided auction market is performed by implementing an aggregation-based particle swarm optimization (CAPSO) algorithm for optimal placement and sizing of one Static Synchronous Series Compensator (SSSC) device. Dallied simulation results (without/with line flow constraints and without/with SSSC) are generated to demonstrate the impact of SSSC on the congestion levels of the modified IEEE 14-bus test system. The proposed CAPSO algorithm employs conventional quadratic smooth and augmented quadratic nonsmooth generator cost curves with sine components to improve the accurate of the model by incorporating the valve loading effects. CAPSO also employs quadratic smooth consumer benefit functions. The proposed approach relies on particle swarm optimization to capture the near-optimal GenCos and DisCos, as well as the location and rating of SSSC while the Newton based load flow solution minimizes the mismatch equations. Simulation results of the proposed CAPSO algorithm are compared to solutions obtained by sequential quadratic programming (SQP) and a recently implemented Fuzzy based genetic algorithm (Fuzzy-GA). The main contributions are inclusion of customer benefit in the congestion management objective function, consideration of nonsmooth generator characteristics and the utilization of a coordinated aggregation-based PSO for locating/sizing of SSSC.

Maximizing the Overall Satisfaction Degree of all Participants in the Market Using Real Code-based Genetic Algorithm by Optimally Locating and Sizing the Thyristor-Controlled Series Capacitor

  • The present paper presents a genetic algorithm (GA) to maximize social welfare and perform congestion management by optimally placing and sizing one Thyristor-Controlled Series Capacitor (TCSC) device in a double-sided auction market. Simulation results, with line flow constraints before and after the compensation, are compared through the Sequential Quadratic Programming SQP method, and are used to analyze the effect of TCSC on the congestion levels of modified IEEE 14-bus and 30-bus test systems. Quadratic, smooth and nonsmooth (with sine components due to valve point loading effect) generator cost curves, and quadratic smooth consumer benefit functions are considered. The main aims of the present study are the inclusion of customer benefit in the social welfare maximization and congestion management objective function, the consideration of nonsmooth generator characteristics, and the optimal locating and sizing of the TCSC using real code-based GA to guarantee fast convergence to the best solution.

The Regional Economic Impacts of Taiwan High Speed Rail

  • Starting her business operation on January 5 2007, Taiwan High Speed Rail(THSR) shapes a new time-space frame for Taiwan western corridor, where more than 90% of national population lives around and more than 95% gross domestic product created from. Comparing with the four-hour traveling time by highway before 2007, THSR reduces the time required to one and half hours from Taipei to Kaohsiung. It will not only benefit the communication along the island from north to south, but also change the location advantages/disadvantages for all cities in these regions. Therefore, this paper establishes a spatial computable general equilibrium model(SCGE Model) to simulate the economic effect of High Speed Rail(HSR). This SCGE model divides Taiwan economy into fifteen geographic regions and thirteen industries. Each region has three sectors: household sector, transportation sector, and industries sector. Following the behavior function of economic theories, the general equilibrium can be achieved simultaneously. Thus, gross regional product (GRP), capital formation, employment income and welfare/utility level can be all observed by calculating the different economic result between cases with-/ without-HSR. Besides, this model presents the social welfare benefit from HSR operation, the polarization phenomenon among regions and within certain region, unbalance distribution of welfare along the HSR line, and industries development divergence among regions etc. These major findings should be useful for regional development policy making.
