  • The osteochondroma is a cartilage-capped exostosis resulting from an error in the regulation of normal chondrocyte proliferation and maturation that leads to a normal bone growth. Although exostoses are benign lesions, they are often associated with characteristic progressive skeletal deformities and may cause clinical symptoms. Surgery can prevent progression and provide correction for certain deformities. We experienced a rare case of solitary osteochondroma in a 21-year-old male which caused the valgus deformity of the ankle.

  • Purpose: The aim of this study was to evaluate the result of combined first metatarsal and calcaneal osteotomy for static cavovarus deformity of the foot. Materials and Methods: We performed a dorsal closing wedge $1^{st}$ metatarsal osteotomy and a lateral and upward displacement calcaneal osteotomy for 9 patients, 12 feet (6 male and 3 female). The mean age at the time of operation was 37 years and the mean followup period was 27 months. The causes of deformity were 2 poliomyelitis, 1 cerebral palsy, 1 Charcot-Marie-Tooth disease and 5 idiopathic type. Five lateral ligament reconstructions of the ankle and six percutaneous Achilles tendon lengthenings were added. The surgical results in terms of pain, function and alignment of the foot were evaluated by means of AOFAS ankle-hindfoot score and talo-$1^{st}$ metatarsal, calcaneus-$1^{st}$ metatarsal and calcaneal pitch angles were checked with weight bearing radiographs in lateral projection. Results: Talo-$1^{st}$ metatarsal and calcaneal pitch angles were reduced from the mean preoperative values of $21^{\circ}$ and $25^{\circ}$ to $12^{\circ}$ and $19^{\circ}$, respectively, at last followup. Also, calcaneus-$1^{st}$ metatarsal angle was increased from the mean $114^{\circ}$ to $114^{\circ}$. The mean AOFAS score was improved from 44.5 points preoperatively to 89.2 points at followup. There were 1 metatarso-cueiform joint nonunion, 1 sural nerve injury and 3 remaining symptomatic claw toes. Conclusion: Combined first metatarsal and calcaneal osteotomy appears to be an effective procedure for the treatment of adult static cavovarus foot.

  • Purpose: As the traditional treatment of the Wassel's type I or II of bifid thumb, Bilhaut-Cloquet, has always been the standard method despite several disadvantages such as tearing of the finger nail, injuries of the growth plate, joint instability, and long visible scarring. To overcome these drawbacks, we applied a modified Bilhaut-Cloquet Method. Methods: The subjects used for the this study were 10 of 20 patients evaluated. The patients underwent modified methods under every type of Wassel's classification. We designed a central wedge Zig-Zag incision and removed the nail and bony tissues in the remaining digit, but not soft tissue if possible, and transferred the ligaments, tendons, and soft tissue to the remaining thumb from the extra digit. We evaluated the patients' lack of extension, the total ROM of the MP and IP joints, the ROM of IP joints, and the lateral deviations of the reconstructed thumb. Results: The results were encouraging, with all patients showing a good functional and aesthetic outcome. Conclusion: The modified method proved a very effective procedure in the treatment of bifid thumb in all types, especially types I or II.

  • A 42 year old male patient complained of increasing pain and mass around the right ankle with 4 months duration. Simple Roentgenogram and CT showed ill-defined osteolysis and cortical perforation in the neck of the right talus. The pathologic findings showed high grade osteoblastic osteosarcoma. The tumor mass of the ankle increased and pulmonary metastasis was found in spite of administrating of two cycles of preoperative chemotherapy. Below-knee amputation and open wedge pulmonary resection were performed for primary lesion and multiple pulmoanry metastasis, respectively. However, the patient died with multiple pulmonary metastasis in spite of three cycles of postoperative chemotherapy at ten months after the operation.

  • Twelve dogs weighing less than 10 kg underwent unilateral TTO to stabilize the stifle joint with cranial cruciate ligament rupture. Surgical findings, intra-operative and post-operative complications were recorded. Radiographic examinations were performed for 8 weeks following surgery. Postoperative outcome was evaluated using a visual analogue lameness scoring system. Mean preoperative PTA (the angle created by the intersection of the tibial plateau extrapolation line and the patellar tendon) was 103.8 degrees. Mean tibial wedge angle was 16.6 degrees. Mean postoperative PTA was 92.1 degrees. Intraoperatively, fracture through the caudal tibial cortex occurred in all dogs, through the distal tibial crest cortex in 2 dogs, through the lateral tibial cortex in 2 dogs and through the fibula in 1 dog. Four-week postoperative radiographs demonstrated evidence of progressive bone union at osteotomy site and complete unions were identified at 8 week in 10 dogs. All dogs were healed in 11 weeks. Most of dogs revealed weak lameness in 4 weeks and normal ambulation in 8 weeks postoperatively except for only one dog returned in 11 weeks. Despite frequent minor complication, it appears that the TTO is an alternative procedure for management of cranial cruciate ligament rupture in small breed dogs.

  • This study aims to rectify the disfigurement of bowlegged, (which means they have a gap between knees due to knee joint problems) females in their twenties through the development of pants patterns. The contents of the study are as follows: First, two types of basic pattern-making methods were selected. Two mock-ups, which were made by using these pre-selected basic patterns, were worn by three bowlegged participants and evaluated with a fit test. A basic pattern, which had good body fit was selected, which had a bubble or a wedge at in crotch area and roomy or snug upper thigh. Second, the mock-up, which was made by using the selected basic pattern, led to the outward movement of crease lines. Two types of patterns that were adjusted were evaluated through a fit test. As a result, an effective adjustment for moving the crease line to the outside or to the center was to cut along the slash line of a horizontal balance line to, but not through the crotch seam to spread the pattern at the hip. Third, three patterns were readjusted using the chosen alteration pattern, and the crotch seam and spread patterns of 1 cm, 2 cm, and 3 cm at the hip were applied. As a result of the evaluation of the fit test, three participants-the majority-showed a better fit when the spread amount in the pattern was 2 cm, although every participant did have a different gap between the knees. This study has significance in which rectifies the disfigurement of bowlegged individuals through the development of patterns for females in their twenties who are bowlegged.

  • Ankle arthrodesis was performed on a 55-year-old male patient with an active lifestyle who developed severe arthritis in the left ankle. Over the follow-up period, high tibial valgization osteotomy was conducted for painful medial knee joint arthritis with genu varum deformity to correct overall lower limb alignment from varus to valgus with respect to the fused ankle. This study was conducted to investigate how hindfoot alignment would change when the overall alignment of the lower limb shifted from varus to valgus with the ipsilateral ankle in a fused state. Conclusively, while no intrinsic changes in the hindfoot alignment were observed following the alteration of lower limb alignment, the hindfoot naturally adjusted to valgus deviation in response to limb valgus realignment. Moreover, symptoms changed in line with this adjustment. Given the absence of similar case studies or reports, a review of relevant literature is included to contribute to knowledge of this subject.

  • Types of slope failure related to cut slope stability are interpreted through case analyses, and also factors affecting structurally controlled instability investigated, which are developed by geologic structures along a national road No. 25 across the Cheongwon and Boeun-Guns, Chungbuk. Engineering properties such as orientation, persistence, roughness and uniaxial compressive strength of joints are analyzed by square-inventory method in three areas with well-preserved outcrops. The study area is located in Ogcheon folded bet, and are composed of quartz-schist and quartzite in the Midongsan Formation and phyllite in the Ungyori Formation. Flexural beds by folding, schistosity and cleavage besides joints are developed due to slight metamorphism. Various types of joints developed by folding are formed such as strike-parallel, strike-perpendicular, wedge and wrench joint sets by both initially regional and later superposed folding. Factors of slope instability are created by crossing the orientations of joint, cleavage, bedding and slope one another. In the case that the orientation of a slope is coincident with one of beds, factors causing large-scale failure including plane failure are increased greatly. Also in the region that orientations of the slope and bed are crossed each other at high angle, only local and minor failures are shown in the slope.

  • Volcanic activity commonly creates a highly complicated volcanic complex due to the admixture of lava flow and sedimentation of volcanic ash. The Song-Gok site is composed of volcanic rocks that collapsed at the lower part of the slope, in combination with several discontinuities in and around a fault. The results of projection analysis indicated the possibility of plane, wedge, and toppling failure in the failure section. The results of discontinuity modeling using the Distinct Element Method (DEM) revealed a total displacement of 207 mm and a joint shear displacement of 114 mm. The yield surface zone was verified at the fault plane of the failure section. In geotechnical terms, volcanic rock slopes are characteristically vulnerable to failure because of differential weathering among the various rock types, the effect of groundwater based on the permeability of the rocks, and the presence of systematic joints generated by the cooling and contraction of lava. When considering the stability of a volcanic rock slope, it is necessary to consider data such as the geological features of the rock, as obtained through detailed geological survey, and variations in discontinuities and rock blocks.

  • The study shows a method called a square-inventory method, which is a better and faster method than scanline survey and window method for an analysis of slope stability. The study area is located in the Changri area, Boeun-Gun, Chungbuk, and consists of many formations of the Okcheon Supergroup. Various types of failure are observed from the phyllite including the rocks in the study area. The physical properties of meta-sedimentary rocks are that minerals of the rocks are composed of microcrystalline quartz and sericite, which are arranged parallel to bedding (or schistosity) and crenulation cleavage. Therefore, such properties affect geotechnical ones of the rock. The slope stability are analyzed by selecting 3 areas, each of which are divided into 2 or 3 slopes of $1m{\times}1m$ area that represent each of 3 investigation sites. The possibility of wedge and toppling failure is very high in all 3 areas by using square-inventory method. Although possibility of plane failure is weak in the investigation site 2, the plane failures are frequently found from the slope of site 2. The bedding (or schistosity) plane and cleavage, another types of discontinuity coexist in meta-sedimentary rocks uulike igneous rocks, and therefore are important factors to be considered together with joint structures in th ε analysis of slope stability.
