Background: Acute Respiratory failure which is developed after extubation in the weaning process from mechanical ventilation is an important cause of weaning failure. Once it was developed, endotracheal reintubation has been done for respiratory support. Noninvasive Positive Pressure Ventilation (NIPPV) has been used in the management of acute or chronic respiratory failure, as an alternative to endotracheal intubation, using via nasal or facial mask. In this study, we evaluated the usefulness of NIPPV as an alternative method of reintubation in patients who developed acute respiratory failure after extubation. Method: We retrospectively analyzed thirty one patients(eighteen males and thirteen females, mean ages $63\pm13.2$ years) who were developed acute respiratory failure within forty eight hours after extubation, or were extubated unintentionally at medical intensive care unit(MICU) of Asan Medical Center. NIPPV was applied to the patients. Ventilatory mode of NIPPV, level of ventilatory support and inspiratory oxygen concentration were adjusted according to the patient condition and results of blood gas analysis by the attending doctors at MICU. NIPPV was completely weaned when the patients maintained stable clinical condition under 8 $cmH_2O$ of pressure support level. Weaning success was defined as maintenance of stable spontaneous breathing more than forty eight hours after discontinuation of NIPPV. Respiratory rate, heart rate, arterial blood gas analysis, level of pressure support, and level of PEEP were monitored just before extubation, at thirty minutes, six hours, twenty four hours after initiation of NIPPV. They were also measured at just before weaning from NIPPV in success group, and just before reintubation in failure group. Results: NIPPV was successfully applied to thirty-one patients of thirty-two trials and one patient could not tolerated NIPPV longer than thirty minutes. Endotracheal reintubation was successfully obviated in fourteen patients (45%) among them. There was no difference in age, sex, APACHE III score on admission at MICU, duration of intubation, interval from extubation to initiation of NIPPV, baseline heart rate, respiratory rate, arterial blood gas, and $PaO_2/FiO_2$ between the success and the failure group. Heart rate and respiration rate were significantly decreased with increase $SaO_2$ after thirty minutes of NIPPV in both groups(p<0.05). However, in the patients of failure group, heart rate and respiratory rate were increased again with decrease in $SaO_2$ leading to endotracheal reintubation. The success rate of NIPPV treatment was significantly higher in the patients with COPD compared to other diseases(62% vs 39%) (p=0.007). The causes of failure were deterioration of arterial blood gas without aggravation of underlying disease(n=9), aggravation of undelying disease(n=5), mask intolerance(n=2), and retained airway secretion(n=l). Conclusion: NIPPV would be a useful therapeutic alternative which can avoid reintubation in patient who developed acute respiratory failure after extubation.
The aim of this study was to investigate the efficacy of Lactobacillus spp. W44 and J124 strain isolated from Kimchi on the growth rate, average daily gain (ADG), feed conversion and change of intestine microflora in the weaning piglets. In the experiment 1, growth rate was significantly increased to $28.5\;{\pm}\;4.3\;kg$ and $27.6\;{\pm}\;3.3\;kg$ after oral administration of W44 and J124 strain as feed additives, respectively (p<0.05). ADG and feed conversion were also significantly improved after administration during 44 experiment days (p<0.05). In the analysis of intestinal microflora, the number of Lactobacillus spp. in the experiment groups was significantly increased 100 to 1,000 times compared to those of control group. In the experiment 2, the efficacy of W44 and J124 strains on the growth rate, ADG, feed conversion and change of intestine microflora were reconfirmed significantly (p<0.05). However, there were no significant differences on feed conversion in the W44 bead and J124 bead groups. In summary, our results suggest that W-44 and J-124 stains from Kimchi have a significant effect on the weight gain and feed conversion, and it may be useful probiotic strains for the weaning piglets as feed additives.
This experiment was carried out to investigate effects of honeybee venom treatment on the body weight gain, feed conversion and growth rate' in pigs. One hundred twenty nine piglets(LY) from 12 sows were allocated into two groups; honeybee venom-treated group (66 piglets from 6 sows) and non-treated control group (63 piglets from 6 sows). Natural honeybee venom was administrated at 0, 3, 14, 30, and 74 days after birth. The acupoints were Hai-men(ST-25), Du-kou(CV-8) and liao-chao(GV-I) points at 0 day, the regions of castration and tail ampution at 3 days, liao-chao(GV-1) and Bai-hui(GV-20) points at 14(weaning), 30(move into piglet stall) and 74(move into grower stall) days after birth. Control group was injected 1 ml of saline to the same site. Average body weight at 150 days after birth was 98.44 kg in bee venom treated group and 86.24 kg in control group, respectively. Average body weight of treated group was significantly increased by 14.15%(P < 0.0001). Average daily gain of bee venom treated group and control group were 649 g and 569 g, respectively, increased by 14.06%(P< 0.0001). Feed conversion of the treated group was 2.21 and control group was 2.49, increased by 11.25% in control group(P < 0.001). Survival rate at weaning was 95.5 % in treated group and 92.0% in control group, at growing(74 days after birth) was 92.4% in treated group and 85.7% in control group and survival rate at finishing(l50 days after birth) was 89.5 % and 79.4 %, respectively. Collecting together, the results in this study showed that growth performance and feed conversion were increased by treatment of natural honeybee venom to pigs. These results suggested that the treatment of bee venom could be used effectively for the increase productivity of livestock industry.
Background: The phenomenon known as the "weekend effect" impacts various medical disciplines. We compared outcomes between regular hours and off hours to investigate the presence of the weekend effect in extracorporeal cardiopulmonary resuscitation (ECPR). Methods: Between January 2018 and December 2020, 159 patients at our center were treated with veno-arterial extracorporeal membrane oxygenation (ECMO) for cardiac arrest. We assessed the time required for ECMO preparation, the rate of successful weaning, and the rate of in-hospital mortality. These factors were compared among regular hours ("daytime": weekdays from 7:00 AM-7:00 PM), off hours on weekdays ("nighttime": weekdays from 7:00 PM-7:00 AM), and off hours on weekends and holidays ("weekend": Fridays at 7:00 PM to Mondays at 7:00 AM). Results: The time from the recognition of cardiac arrest to the arrival of the ECMO team was shortest for the daytime group and longest for those treated over the weekend (daytime, 10.0 minutes; nighttime, 12.5 minutes; weekend, 15.0 minutes; p=0.064). The time from the ECMO team's arrival to ECMO initiation was shortest for the daytime and longest for the nighttime group (daytime, 13.0 minutes; nighttime, 18.5 minutes; weekend, 14.0 minutes; p=0.028). No significant difference was observed in the rate of successful ECMO weaning (daytime, 48.3%; nighttime, 39.5%; weekend, 36.1%; p=0.375). Conclusion: In situations involving CPR, the time to arrival of the ECMO team was longer during off hours. Furthermore, ECMO insertion required more time at night than during the other periods. These findings warrant specific training in decision-making and emergent ECMO insertion.
Background: Rabbit breeding with high performance imported ones would be of benefit for genetic diversity and improvement of performance in domestic rabbit breeds. The rearing of more productive rabbit breeds could be pathway to improve the productivity and reduce the production cost. Maternal nutritional status exert a great influence on reproductive functions of does, which may expand from conception, through gestation and parturition and development of kits to puberty. Methods: Four breeds of rabbit were evaluated for their parturition, weaning and pubertal differences among the rabbit population in Ibadan, Nigeria. The breed consist of Fauve De Bourgogne (FDB), Chinchilla (CHA), British Spot (BS) and New Zealand White (NZW) rabbits. A total of 60 bucks and 360 does consisting of 15 bucks and 90 does per breed were mated in 6 mating cycles, three each of natural mating and artificial insemination. All does were synchronized for estrus with 20 IU pregnant mare serum gonadotropin 48 h prior to mating. The does after parturition were assessed for milk yield (g) and kit survival rate (%) till weaning, weight changes of kits from birth to puberty. At puberty, the pubertal age (days) and weight (g) of the offspring were assessed. Result: Results obtained reveals that British Spot doe had highest milk yield among the breeds which significantly increased growth of kit and weight at weaning in British Spot rabbits. Survival rates of Chinchilla kits were significantly (p < 0.05) higher than Fauve de Bourgogne, British Spot and New Zealand White kits. Puberty attainment of the rabbits indicates that British spot does and Fauve de Bourgogne bucks are early maturing. Conclusion: Chinchilla shows high kit survivability and British spot has highest milk yield among the four breeds of rabbit.
The data on reproductive performance of 1065 purebred Landrace, 1254 purebred Yorkshire. 553 purebred Duroc and 327 purebred Pietrain sows bred to farrow purebred litters and data from 3413 Landrace $\times$ Yorkshire, 487 Duroc $\times$ Pietrain and 3191 commericial litters were udsed in this study. The data were collected form litters farrowed form January 1986 to December 1988 in Holiday Hills Stock and Breeding Farms, San Prero, Laguna. The crossbred sows and litters were better in most traits associated with sow productivity than their contemporary purebred sows and litters. The Duroc $\times$ Pietrain crossbred litters showed 16.67% improvement in litter size at weaning, 10.11% improvement in survival rate form birth to weaning and 16.21% more pigs per sow pre year than the averages of their parental breeds. The litters from the Landrace $\times$ Yorkshire crossbred sows sired by either Duroc or Duroc $\times$ Pietrain boars had 11.01% bigger litter size at birth, 20.74% increase in litter size at weaning and 22.86% more pigs weaned per sow per year than the averages of the four parental breeds. Very little improvement in most traits evaluated were observed form the Landrace $\times$ Yorkshire crossbred litters compared with their parental breeds. Year of birth of litter had litter influence on sow productivity. However, the reproductivity performance declined when sows farrowed in June up to September. The best Months of farrowing were found to be from January to April. The breed $\times$ year, breed $\times$ month and year $\times$ month had no significant effects on all traits evaluated.
The animal production sector in Jordan is characterized by shortages of locally produced feedstuffs derived from rangeland, forage plants or from human food crops as by products. This is exacerbated by insufficient rainfall, overgrazing, early grazing and high stocking rate. Thus, subject to these constraints, other technological improvements are highly desirable to meet the needs of crop growth and animal production. Alternative adapted technologies are also desirable in order to meet the increased demand for red meat in relation to population growth along with the changes in the price subsidy for feedstuff. The technologies are those, which have been introduced to the animal production sector, obtained in agricultural research stations besides on-farm demonstrations. They include technologies suited for increasing birth and twining rates, synchronizing the mating period, introducing the early weaning method, and animal feed and sheep production. Economic assessments conducted in this study demonstrate promising results of hormonal and nutritional practices in improving production efficiency of Awassi sheep in Jordan. Jordanian published data between 1991 and 1998 were used. The examined practices were: 1) use of PMSG in estrus synchronization in ewes, 2) introduction of early lamb weaning program, 3) supplementation with $AD_3E$ for ewes and 4) the use of agro-industrial feed block as a feed supplement for grazing lambs. Production data were then subjected to partial budgeting for economical evaluation. The use of PMSG outperformed the control groups in fertility and net returns per ewe by US$ 8.36/ewe. The early weaning of lambs increased the net returns by US$ 3.90/lamb. The injection with vitamin $AD_3E$ showed an average additional net return of US$ 5.66/ewe. Feeding agriculture by-product blocks improved weight gain in the feed block groups and resulted in additional net returns of US$ 3.5/lamb. The economic viability and reproductive performance indicators demonstrate that efforts should be undertaken to disseminate these new practices in the development program.
In order to investigate the effect of incorporation of thermally processed cereal (maize) grain and differently degradable protein sources in the calf starter, twenty four newly born crossbred $(Bos\;taurus{\times}Bos\;indicus)$ calves were assigned at random to six diets in a $3{\times}2$ factorial design involving three protein sources viz. groundnut meal (GN), cottonseed meal (CS) and meat and bone meal (MB), each along with two differently processed grain, namely ground raw (R) and pressure cooked (P) maize. The corresponding calf starters with green oats (Avena sativa) were given free-choice from 14 d onwards till the end of the 90 d experimental feeding. A restricted milk diet was fed till the age of weaning at 60 d. Total DM intake was not affected by cereal or protein sources. However, daily intake of DM (59.23 vs 66.45 g) and CP (12.38 vs 14.10 g) per kg $W^{0.75}$ was reduced (p<0.05) due to cereal processing. Better (p<0.05) feed and protein efficiencies after weaning and during entire period in calves fed processed maize resulted in a trend of higher $(p{\leq}092)$ growth rate especially when GN was the source of protein. In comparison among protein sources, calves fed MB diets tended to grow faster $(p{\leq}098)$ concurrent with a higher CP intake before weaning. It is thus evident that thermal processing of maize in the calf starter seems to improve calf performance. Moreover, results indicated that feeding of protein and starch sources of matching ruminal degradability may prove beneficial for early growth of crossbred calves.

The aim of this study was to analyze post-weaning reproductive performance of Large White sows in relation to season, parity and their lactation performance under tropical conditions in Guadeloupe (French West Indies, $16^{\circ}$ Lat. N, $61^{\circ}$ Long. W.). This work was based on data recorded in the experimental unit of INRA from January 1993 to December 2003. Two seasons were determined a posteriori from climatic parameters recorded continuously in a station close to the experimental unit. Mean ambient temperature was higher during the hot season than the warm season ($26^{\circ}C$ vs. $24^{\circ}C$) but relative humidity was comparable for both seasons (i.e. 87% on average). Season had a significant effect on all reproductive parameters analyzed. Primiparous sows weaned in the hot season had a higher probability of a prolonged weaning to estrus interval, WEI (odds ratio was 4.1; p<0.01) but multiparous sows were not affected. A higher probability of a prolonged weaning to conception interval, WCI (odds ratio >2.5, p<0.01) and a lower subsequent farrowing rate (-10%, p<0.01) were found for sows weaned in the hot season. A higher daily feed intake during lactation reduced the probability of a prolonged WEI (p<0.05). Body weight and average back-fat thickness at farrowing affected WEI and WCI (p<0.05), whereas body weight and average backfat thickness change in lactation did not. This study confirms the negative effects of the hot season on primiparous reproductive performance. It also indicates that lactation performance influences sow non-productive period.
Between May, 1994 and December, 1995, 122 adult cardiac surgical procedures requiring cardiopulmonary bypa s were performed at Kang Dong Sacred Heart Hospital, including 18 cases(14.8%) that were associated with preoperative(n:9), intraoperative(n=7), postoperative(n:2) use of an IABP (intra-aortic balloon pump). The reasons for IABP were low cardiac output and PTCA(percutaneous transluminal coronary angioplasty) failure in preoperative period, CPB(cardiopulmonary bypass) weaning difficulty in intraoperative period, and intractable arrhythmia in postoperative period. The mean age of the IABP patients was 61.8 $\pm$ 6.9 years(range, 39 to 75years). The overall hospital mortalities in patients with preoperative and intraoperative IABP insertion were 3 and 42.9% respectively. Two patients with postoperative IABP insertion are alive. The rate of IABP weaning is 66.7% for preoperative group, 85.7% for intraoperative group and 100% for postoperative group . In conclusion, if there were no irreversible myocardial damages, IABP could be used safely and emergently at any perioperative period for hemo ynamic stability, CPB weaning, and to overcome low cardiac output syndrome.
