중소병원의 간호인력 수급 논쟁: 인력난 vs 임금난 (Supply and demand of nursing manpower for small and medium hospitals in rural area: nursing shortage versus wage disparity)

  • Recently, small and medium-sized hospitals which are located in rural areas have many difficulties in securing high quality nurses. That is because working environments for nurses in small and medium-sized hospitals in rural areas are poor compared with those of big hospitals in urban. As a result, the migration of nurses from small and medium-sized hospitals in rural areas to big hospitals in urban is continuously happening. In general, big hospitals provide nurses with high level of salary and fringe benefits. To prevent the migration of nurses, chief executive officers of small & medium hospitals in rural areas have been interested in improving nurses' working conditions including wages. Also, they have raised nurses' salary and improved working conditions. But, basically these individualized efforts have some limit. In connection with this, medical interest groups have produced various voices in terms of interpretation and solutions for these issues. However, from the future perspectives, it seems evident that two approaches for both manpower supply and demand plans of nurses are necessary. They should contain not only accurate estimation of the supply-demand of nursing manpower but also the improvement of working conditions and wages of nurses. Estimation of nursing manpower supply-demand depends on the standards and criteria being used. Supply and demand may be met or not in accordance with the points emphasized on the decision. In the articles, issues regarding nursing manpower, levels of salary, other working conditions and social support system for child care are discussed. According to Joe's report (2005), most health institutions did not meet the guidelines of nurse staffing in Medical Law. The wages of nurse vary on every hospital and there is a big difference in wages' range. The average starting salary for a nurse is 22 million won a year. In case of tertiary hospitals, it reaches up to 30 million won a year. Nurse as a profession should have a strong responsibility and should take care of the patients for 24 hours with three working shifts. Also, most of them are female who have the burden of child rearing. Therefore, it is suggested to increase the salary, to provide comfortable working conditions, and to have social support system for nurses with household affairs.

  • Because of the radical changes in the domestic and foreign economic circumstances Koreaa fisheries is confronted with difficulties. Along the end of the UR marine products of other nations are rushing into Korea. Also migration of labor to other industries and rise of wage level in Korean fisheries deteriorate managerial conditions. But in Korea which has little natural resourses fisheries is still more important. That is \circled1 creating job opportunites \circled2 increase of income \circled3 supply of foodstuffs and animal protein \circled4 acquisition of foreign currency \circled5 enlargement of domestic market for industrial products \circled6 development of other interrelated industries \circled7 rational use of domestic resourses \circled8 diversification of population and production activites \circled9 contribution to balanced growth of national economy by the developing regional economy. These roles of fisheries in national economy mentioned above are to be excuted forward. In spite of the radical change in the economic circumstances at the end of the UR fisheries if crucial in Korea as a industry. For this our fisheries is to be made to compete with foreign fisheries. Through the cheap price and high quality our fisheries must be came to compete with foreign fisheries and meet the people's needs for marine products. For this it is necessary to maintain high productivity and competitive power. Now with the exception of a portion of the deep - sea fishing, our fisheries is generally paltry, Especially inshore fishery which is the main stock in our fisheries is very paltry and so productivity and competetive power are very low. So to develop our fisheries which has s comparative disadvantage active polices that follows below are to be promoted on a large scale. \circled1 improvement of structure \circled2 augmentation of productivity in fishing ground by making fisheries resourses \circled3 enlargement of finantial and monetary assistance \circled4 effective administration of fisheries cooperative union \circled5 activation of R&D etc These polices which need to be scientific and comprehensive are very valuable. Especially without making fishieries resources we cannot expect economy of scale, promotion of productivity and development of fisheries. And we do also endeavour to gather the results of the study and investigation about fisheries domestic and foreign and do ceaselessly put these to practical use systimatically.

미국-멕시코 국경지대의 산업화 과정 (Industrialization Process in the Border Area of The U.S. and Mexico)

  • 본 연구는 미국과 멕시코의 경제적 상호관계를 미국-멕시코 국경지대의 산업화 과정을 중심으로 파악하고, 그에 따른 지역적 영향을 살펴보았다. 미국-멕시코 국경지대의 산업화는 멕시코지 마낄라도라에 대한 미국의 투자에 의해 직접적인 영향을 받았으며, 그에 따라 양국의 국경지대에서는 고용확대, 인구증가, 서비스산업의 발달이 이루어 졌다. 세계의 다국적 기업들은 표준화된 노동집약적 산업뿐만 아니라 첨단시설의 자동화 생산방식의 산업까지 저임금 노동력을 활용하기 위해 멕시코에 진출하였으며, 반면에 미국에는 관리, 연구개발 및 시장분배기능을 가진 기업들이 배치됨으로서 양국간의 공간적 분업이 이루어지고 있다. 멕시코 국경지대는 다국적 기업의 단순조립 공장들이 주로 입지한 위성산업지구이지만, 고용의 확대로 주민들의 경제적 지위가 향상되었으며 멕시코 경제 전반에 걸친 파급효과도 크다. NAFTA의 원산지 규정은 국경지대 기업간의 국지적인 연계에도 영향을 미쳐서 다국적기업의 분공장들은 부품 협력업체들과 동반진출을 추진하면서 부품공급의 현지화에 노력을 하고 있다.

원자력발전소(原子力發電所)가 지역(地域)의 생산(生産), 소득(所得)과 고용(雇傭)에 미치는 효과(效果) 분석(分析) (Regional Production, Income and Employment Impact of Nuclear Power Plant)

  • The present study has quantitatively assessed the regional production, income and employment impact resulting from the construction and operation of nuclear power plant (NPP) upon the domestic local areas by applying the regional input-output analysis model to the case of Wolsong unit-l site. The conclusions regarding the most likely regional economic impacts upon the wolsong site are summarized as follows: 1. The income multipliers are calculated to be 1.563 for the construction phase and 1.500 for the operation phase. These values are relatively high compared with those of other conventional facilities. 2. The level of total employee's wage induced employment associated with the construction phase has been estimated to be 37,000 while that with the operational phase in 1990 to be 5,610. 3. With relation to the aspect of resident welfare it is found that the industrial sector associated with electricity, gas and water supply have remarkably improved with the construction of the NPP. 4. The NPP siting has induced substantial changes in interindustry (input-output) structures of the Wolsong unit-l site which is one of the rural areas where all the domestic NPPs are sited. Such changes are attributed to the industrial recomposition of the region. 5. With the application of other regional economic analysis models and the use of more sufficient regional data, other detailed studies on the economic impact analysis of domestic NPP-related facility sitings are suggested to be carried out further since the influence of NPP sitings is significant to the national economic impact as well as the regional economic impact.

대구.경북지역 병원영양사의 직무특성, 업무수행도 및 조직몰입도가 직무만족도에 미치는 영향 (Effects of Job Characteristics, Job Performance and Organizational Commitment on Job Satisfaction of Hospital Dietitians in the Taegu. Kyungpook Area)

  • The purpose of this study was to identify the effect of job characteristics, job performance and organizational commitment on job satisfaction of hospital dietitians. The first survey was carried out on 74 hospital dietitians to determine their demographic characteristics, job characteristics, organizational commitment, and job satisfaction. The second survey was carried out on 47 hospital foodservice dietitians to determine their job performance. Thirty-two percent of the subjects were aged between 26 and 30, and 62.2% had Bachelor's degrees. The dietitians felt most satisfied by their co-workers, followed by the work-itself, supervision, pay, and promotion possibilities. Dietitians who had a higher education were satisfied with pay and supervision. The longer the length of employment, the more the satisfaction with the work-itself and pay, plus the higher the monthly wage the more the satisfaction with the work-itself, pay, supervision, and co-workers. lob satisfaction was high in the group that scored high on autonomy, feedback, task identity, and friendship of in the job characteristic inventory. The job performances of nutrition counseling(r= 0.469, p < 0.01), nutrition education(r= 0.446, p < 0.01) and management of therapeutic diet(r= 0.394, p < 0.01) were positively correlated with job satisfaction. The $R^2$for the multiple regression model was 0.677, indicating that 67.7% of the variance in job satisfaction could be accounted for by feedback, organizational commitment, nutrition counseling performance, and nutrition education performance. In conclusion, the level of job satisfaction for hospital dietitians would appear to improve with increased feedback, organizational commitment, and opportunity for nutrition counseling and nutrition education.

지역간 여가통행의 시간가치 산정 연구 (Estimating the Value of Travel Time for Intercity Leisure Trips)

  • 주 5일 근무제 시행과 소득수준 향상 등으로 인하여 지역간 통행에 있어서 여가통행의 비중이 높아짐에 따라 철도사업의 합리적 평가를 위한 여가통행의 시간가치 산정 및 적용방안 등의 마련이 필요하게 되었다. 이에 따라 본 연구에서는 업무, 여가, 여가제외 비업무 통행에 대하여 수단선택모형을 구축하여 한계대체율법에 의한 시간가치를 산정하고, 이를 기초로 업무통행 대비 여가통행, 비업무통행의 비율을 도출하여 최종적으로 한계임금율법에 의한 통행목적별 시간가치를 산정하였다. 분석결과 여가통행은 타 비업무목적 통행에 비하여 시간가치가 높은 것으로 분석되었으며, 이를 사례분석에 적용한 결과에서도 비업무통행 시간가치를 적용하는 기존 방법론에 비하여 편익이 다소 증가하는 것으로 나타났다.

  • 미국-멕시코 국경지대는 멕시코의 마킬라도라 산업에 대한 세계 다국적기업들의 투자로 인해 고용확대, 인구증가, 산업발달이 이루어지고, 특히 국경지대의 쌍둥이 도시들은 많은 사회경제적 변화를 겪게 되었다. 1965년에 시작된 멕시코의 마킬라도라 프로그램은 외국기업의 투자를 유치하기 위한 보세가공 산업정책이었는데, 2006년부터는 멕시코인 소유의 수출기업을 포함하는 IMMEX 프로그램으로 대체되었다. 마킬라도라를 포함한 IMMEX 기업들은 2007년 이후 업체수는 큰 변화가 없는 반면 업체당 종사자수는 점차 대형화하고 있다. 업종별로 보면 자동차, 항공기와 관련된 운송장비제조업이 가장 크게 성장하였으며, 과거 마킬라도라 산업에서 큰 비중을 차지하던 섬유·직물·의류제조업은 상대적으로 침체의 길을 걷고 있다. IMMEX의 비제조업 분야에서는 종사자수와 해외매출액에 있어서 농업의 비중이 압도적이다. IMMEX 기업들의 지역적 분포에서는 티화나와 후아레스가 압도적으로 높은 비중을 차지하고 있다.

양질의 산림일자리 평가 기준 및 지표 개발 (Developing Evaluation Criteria and Indicators of Decent Forest Jobs)

  • 일자리의 양과 질, 사회적 가치의 측면에서 산림일자리가 얼마나 양질인가를 평가하기 위한 3대 기준과 11개 지표를 개발하였다. 기준 및 지표의 중요도와 가중치는 산림일자리위원회, 학계 등 일자리 관련 전문가 50명을 대상으로 실시한 설문을 통해 설정하였다. 설문에 응답한 35명의 전문가들은 '일자리 창출 잠재력'을 가장 중요한 기준으로 선택하였으며, '일자리 질'이 '사회공헌도'보다 상대적으로 높은 중요도를 갖는 것으로 인식하였다. 지표의 가중치는 일자리 창출 잠재력의 '정책적 의지'가 가장 높게 나타났고, 일자리 질의 '고용 안전성'을 다음으로 높게 인식하였다. 반면 일자리 질의 '임금 수준'은 가장 낮은 가중치를 나타내었다. 개발된 기준과 지표는 신규 산림일자리는 물론 기존의 산림일자리에 대한 양질의 일자리 평가 수단으로써 활용이 가능하다.

만성질환자에서 실손형 민간의료보험의 가입 관련 요인과 외래 의료이용의 관련 요인 (Factors Affecting the Purchase of Indemnity Private Health Insurance and Related Factors of Outpatient Healthcare Utilization among Chronic Disease Patients)

  • Purpose: The purpose of this study was to investigate determinants of purchasing indemnity private health insurance and its impact on the healthcare utilization among outpatients with chronic disease. Methods: The study analyzed 4,997 chronic ill patients using 2015 Korean Health Panel data. Logistic regression analysis was conducted to analyze the factors affecting the purchase of indemnity private health insurance and multiple regression analysis was conducted to analyze the effect of private health insurance on the number of outpatient visits and outpatient expenditures. Findings: The age, education level, and number of chronic diseases were significant factors affecting the purchasing of indemnity private health insurance among chronic patients. As a result of analyzing the impact of indemnity private health insurance on healthcare utilization, the number of outpatient visits for those who enrolled in the indemnity private health insurance was higher than the number of outpatient visits for those who did not. But there was no statistically significant difference in outpatient medical expenses. Practical Implications: The results of this study shows that indemnity private health insurance may cause inequality in the healthcare utilization of the socially vulnerable. It is necessary to seek ways to strengthen the health security of chronic disease patients and high-risk elderly people who need more healthcare.

경력단절 간호사의 경력단절 원인에 대한 탐색적 연구 (An Exploratory Study on the Causes of Career Interruption in inactive nurses)

  • 본 연구의 목적은 간호사들의 병원 근무 경험과 경력단절 영향요인을 심층적으로 분석하기 위해 시도되었다. 이를 위해 26-55세 경력단절 간호사 20명을 대상으로 간호사로서의 경험, 간호 일의 특성과 퇴직사유의 내용을 인터뷰하였으며 현상학적 방법 중 꼴라쥐(Colaizzi)의 분석방법을 적용하였다. 분석결과 104개의 주요 진술과 17개의 구성된 의미, 6개의 공통 주제군, 2개의 범주가 도출되었다. 2개의 범주는 간호 업무 특성과 근무 환경 특성이며 간호 업무 특성에서는 '시간 압박', '자율성 부재', '신체 감정 노동', '낮은 임금 상승률'의 4가지 공통 주제군이, 근무 환경 특성에서는 '물리적 환경', '인적 환경'의 2가지 공통 주제군이 각각 도출되었다. 따라서 간호사의 경력단절 현상을 개선하기 위해서는 과도한 업무강도를 완화하고 적정수준의 임금인상과 안전한 근무환경을 제공하려는 노력이 필요하다.
