Acoustic Analysis of Normal and Vocal Pathologic Voice Using Dr. Speech Science (Dr. Speech Science를 이용한 정상 및 후두질환 환자의 음향분석)

  • Background : For example, aerodynamic study, vibratory study, acoustic study, neuro-muscular test and psychoacoustic evaluation, a number of objective methods are now available for assessing pathologic voice change. They help to differentiate pathologic condition from normal condition and to monitor pathologic and aging change. These laboratory analyses are used commonly to monitor speech therapy and to follow a patient's recovery after surgery. Objectives : We investigated the values of jitter, shimmer and NNE of normal person and hoarseness patients in Korea. The values of Jitter and shimmer might be meaningful parameters distinguishing pathologic vibration from normal and recovery after surgery. Materials and Methods : Statistical significance between normal control and 48 subjects taken microlaryngeal surgery were compared with Dr. speech science program that is computerized system for acoustic analysis of voice production employed to determine vocal characteristics of pitch perturbation(jitter) and amplitude perturbation(shimmer). Results : The mean normal values of jitter and shimmer were 0.226${\pm}$0.110(%), 2.200${\pm}$0.421(%) in male and 0.164${\pm}$0.060(%), 2.063 ${\pm}$0.575(%) in female. In patients with vocal nodule, the preoperative and postoperative values of jitter and shimmer were valueless. In patients with vocal polyps, the preoperative and postoperative values of jitter and shimmer were valuable. Conclusion : Dr. speech science program was effective to monitor laryngeal disease and aging changes.

Vocal sulcus accompanying with benign vocal cord disease (양성 성대질환을 동반하는 성대구 (Vocal Sulcus))

  • A retrospective review of 158 patients who had microlaryngeal surgery for benign vocal cord disease, at the Department of Otolaryngology, Kang-Nam St. Mary Hospital, over 2-year period from Mar. 1991 to Feb. 1993, was completed. Of 158 patients, 16 patients(10.1%, male 8 & female 8, most common in fifth decade) had concomitant vocal sulcus unilaterally (10) or bilaterally (6). Vocal sulcus was accompanied with 7.5% (7/93) of vocal polyp, 15.8% (6/38) of vocal nodule, 11.1% (1/9) of Reinke's edema and 8.3% (1/12) of vocal cyst. Vocal sulcus located above the associated disease in 13(59.1%), below the disease in 7(31.8%) and in the lesion in 2(9.0%). Vocal sulcus was found mainly on the midportion of the membranous vocal cord and the length was less than half of membranous vocal cord in 16(72.7%), more than half in 6(27.3%). In 12 cases (54.5%), vocal sulcus was deep enough to attach to the vocal ligament. We thought it is important to consider the concomitant vocal sulcus during the microlaryngeal surgery for benign vocal cord disease.

The Efficiency of Voice Therapy for the Patients with Vocal Nodules (성대 결절 환자를 대상으로 한 음성치료의 효과)

  • Vocal nodule due to vocal hyperfunction is one of the representative chronic diseases of vocal folds, and it can be cured by surgical movement, and/or voice therapy. The present study is, focusing on the latter, to compare the acoustic and aerodynamic results of the pretreatment with those of posttreatment, and then to investigate the objective date on the efficiency of the voice therapy for the patients with vocal nodules. 11 females(age : 7-49) and 5 males(age : 8-40), total 16 patients wi vocal nodules treated by voice therapy were participated as subjects. Six measurements and comparisons of pretreatment and posttreatment of the results were performed : litter, shimmer, and noise-to-harmonic ratio as acoustic analyses ; maximum phonation time, mean flow rate, and the subtraction of mean flow rate from maximum flow rate as aerodynamic analyses. As a result, 14 of 16 subjects showed improvement at more than 4 of 6 measurements, and in group data, every measurements of posttreatment was improved significantly than the pretreatment. On the whole, the improvement of aerodynamic aspects was more statistically significant than that of acoustic ones.

The Characteristics of Voice Onset Time of the Korean Stops in the Benign Laryngeal Disorders (후두질환에 따른 자음의 음성발현시간의 특성)

  • Background and Objectives : Voice onset time(VOT) is defined as the time interval from oral release of a stop consonant to the onset of glottal pulsing in the following vowel. VOT is a temporal characteristics of stop consonants that reflects the complex timing of glottal articulation relative to supraglottal articulation. Stop consonants are characterized by creation of a pressure difference across a complete occlusion in the vocal tract, followed by a sudden release 'burst' due to opening that occlusion. The objects of this study is to evaluate a usefulness of voice onset time in the assessment of voice disorderd patients. Subjects : Subjects were 20 adults with normal voice and with benign laryngeal disorders. Subjects with voice disorders represented the following vocal pathologies : vocal polyp, vocal nodule, Reinke's edema and unilateral vocal fold paralysis(UVFP). Control subjects were matched for age (21-40 yews old) and sex(male) with the voice disorders subjects and had normal vocal qualities with no history of voice disorders. Methods : Each voice-disordered and matched control subject read the test passages containing three types of Korean bilabial consonants. VOT measures were made for the initial $/p/p^h/\;and\;/p'/$. VOT was measured using acoustic waveform or wide band spectrogram. Results : For each voiceless stop consonants, there was a significant difference in VOT between the voice disordered and normal subjects. The mean VOTs of the lax stops in UVFP was significantly shorter than those of control subjects in the UVFP. The mean VOTs of the aspirated stops in the vocal polyp and nodule were longer than those of control subjects, but not significant. The mean VOTs of the glottalized in voice disordered groups were longer than those of control subjects, and significant statistically in the UVFP. Conclusions : VOT may be a clinically useful acoustic parameter in the assessment of voice disordered patients, especially in the unilateral vocal fold paralysis.

The Study for Voice Onset Types in Benign Vocal Fold Lesions (양성성대질환에서의 발성시작유형에 관한 연구)

  • Background and Objectives: Benign vocal fold lesions have shown various voice onset types on phonation, however, they have not been documented yet. We studied to know the relationships between benign vocal fold lesions and voice onset types. Materials and Method: 114 subjects were evaluated by using videokymographic examinations. The subjects were classified into three types: normal, contact, and open types according to the patterns of voice onset types on phonation. Benign vocal fold lesions were investigated and voice onset types were compared between normal and disease groups. Voice parameters were obtained from and compared in all subjects to assess acoustic and aerodynamic factors. Results: The normal type among onset types were more than contact type or open type in both normal and disease groups. Disease group showed many contact and open types when. compared with normal group. Vocal nodule and vocal polyp were showed many normal and contact types, however, sulcus vocalis was almost showed open type among voice onset groups. The values of mean flow rate (MFR) of contact type were significantly higher compared to normal type in disease group (p<0.05). Shimmer of contact type was higher than normal type in diseasegroup, but the difference was not significant (p=0.057). Conclusion: Benign vocal fold lesions were related to the various types of voice onset. The various types of voice onset should be considered when benign vocal fold lesions were examined.

Clinical Application of Botulinum Toxin to Contact Granuloma and Vocal Nodule (보툴리눔 독소를 이용한 성대육아종과 성대결절 치료)

  • In the field of otolaryngology-head and neck surgery, botulinum toxins are widely used for the treatment of spasmodic dysphonia and vocal tremors. Recently, the applications of botulinum toxin have gradually expanded with time, to include vocal fold granuloma, mutational falsetto, bilateral vocal cord paralysis, and chemical reduction for arytenoid dislocation as an adjunctive modality. According to a nation-wide multicenter study conducted by the Korean Society of Laryngology, Phoniatrics and Logopedics for treatment modality of contact granuloma, among the various treatment modalities, botulinum toxin injection showed the highest response rate and lowest recurrence rate in both primary and refractory cases. Therefore, botulinum toxin could be reserved as a second-line treatment for contact granuloma in which the first treatment was not effective, but also could be used as a first-line treatment depending on the patient's and institution's situation. For recalcitrant nodules, injection of botulinum toxin into the bilateral thyroarytenoid muscle will reduce glottal contact force and result in a forceful chemical voice rest. In special situations, botulinum toxin injection could be one of the alternative treatment options for recalcitrant vocal nodules.

Acoustic Characteristics of the Smoking Patients in the Voice Disorders (흡연환자 음성의 음향학적 특성에 관한 연구)

  • Background and Objectives: Smoking has been identified as one of the main determinants of negative changes on the larynx histology. The purpose of this study is to investigate the voice characteristics and correlation between voice parameters of the smokers with vocal polyp or nodule or both of them. Materials and Method: MPT, $F_0$, jitter, shimmer, NHR of Korean /a/ vowel from 54 smokers and 50 nonsmokers diagnosed as vocal polyp or nodule were analyzed. A Computerized Speech Lab (4400) was for the analysis of each voice sample and statistical analysis was done by one-way ANOVA and Pearson correlation coefficient. Result and Conclusion: It showed that we can find difference between smokers and nonsmokers group in MPT, $F_0$, jitter, shimmer were different except NHR. each group shows difference in correlation coefficient between MPT, $F_0$, jitter, shimmer.

Voice Analysis and Videostroboscopic Findings before and after Laryngomicrosurgery of Intracordal Cysts (성대낭종환자에서의 후두미세수술전후의 음성언어분석비교)

  • Background and Objectives : Dysphonia may be secondary to many different type of benign vocal cord lesions such as vocal polyp, vocal nodule, Reinke's edema, and intracordal cyst. Diagnosis and treatment of intracordal cysts are more difficult than other benign vocal cord lesions. But postoperative voice analysis of intracordal cyst have rarely been reported in the literature. The purpose of this study is to analyze aerodynamic and acoustic results and videostroboscopic findings before and after laryngomicrosurgery. Materials and Methods : We reviewed the pre and post-operative voice analysis results and videostroboscopic findings of 15 surgically treated patients of intracordal cysts at Severance hospital from Jun. 1997 to Nov. 1999 retrospectively. They were diagnosed with videostroboscopic findings, surgical findings, and pathologic reports. Their pre and post-operative speech were analyzed with MDVP(Multi Dimension Voice Analysis Program) of CSL(Computerized Speech Lab) and Aerophone II. Their pre and post-operative mucosal wave of true vocal cord was analyzed with videostroboscopy. In order to compare this results with normal group, 10 of normal persons were evaluated with same methods. Results : After the operation, mucosal wave of true vocal cord was improved in all patients. Postoperative acoustic and aerodynamic results were improved in almost parameters, but they did not reach the normal value. Conclusions : Videostroboscopy was essential in diagnosing intracordal cysts. By comparing the acoustic and aerodynamic results and video-stroboscopic findings before and after the laryngomicrosurgery, postoperative vocal function was defined more accurately and objectively. Almost parameters may be useful in assessing the quantitative changes in vocal quality before and after the laryngomicrosurgery.

VHI, V-RQOL, and vocal characteristics of teacher and singer (교사 및 성악가의 VHI, V-RQOL, 음향학적 특성 비교)

  • The purpose of this study was to investigate VHI, V-RQOL, and vocal characteristics of a teacher and singer. 56 subjects were participated in this study (20 subjects are a teacher with vocal nodule, 20 subjects are a singer with vocal nodule, and 16 subjects are normal speakers). All subjects completed the VHI, V-RQOL, and vocal characteristics were measured using CSL 4500(Kay Pentax. USA). 21 subjects completed the VHI, V-RQOL, and vocal evaluation using CSL 4500 twice to assess test-retest reliability. A statistical analysis was performed using the Statistical Package for Social Sciences 18.0 (SPSS Inc, Chicago, IL). The VHI and V-RQOL showed that the teacher and singer group were significantly higher than those of the control group in functional, physical and emotional aspects(p<0.05). the acoustic analysis using CSL 4500 showed the teacher and singer group were significantly higher than those of the the control group in fundamental frequency related variables, fundamental perturbation related variables, amplitude perturbation related variables, noise related variable, and tremor related variables(p<0.05). Conclusionally, the teacher and singer group recognized their voice problems as a serious physico-functional aspects.

KTP-532 Laser Microlaryngeal Phonosurgery (KTP-532 레이저를 이용한 후두미세음성수술의 임상적 적용 결과)

  • Recently, conservative and bloodless operative procedures have been focused, so that endoscopic laser have been used. However application of endoscopic laser surgery for laryngeal lesion is capable for followings: (1)capability of delivery through an operating microscope, (2) vaporization of ultraspot. $CO_2$ laser which has been used, has limitations for voice improvement because of 700 micron beam spot. KTP-532 laser which is capable of delivery through an operating microscope vapore 200 micron ultraspot has developed and applied to microlaryngeal surgery. We have experienced 60 cases who were contracted with hoarseness(53 cases of benign lesions vocal nodule 13, vocal polyp 13, bilateral diffuse polyposis of vocal cords 11, intracordal cyst 8, vocal papilloma 5, laryngocele 1, laryngeal stenosis 2 and 7 cases of malignant lesions : laryngeal carcinoma stage I and II) since 1991. We operated them with KTP-532 Laserscope(3 Watt/0.05 sec., pulsed or continuous. San Jose, Calif, USA). Forty eigh cases(90.6%) of 53 bengin laryngeal lesions and 4 cases(57.1%) of malignant lesions were significantly improved in their voices. There were a few complications and technical problems, but laser surgery alone had a limitation to eradicate the disease.
