Serum Vitamin D Status in Iranian Fibromyalgia Patients: according to the Symptom Severity and Illness Invalidation

  • Background: This study was designed to assess serum vitamin D status (25-OHD) in the fibromyalgia (FM) patients and to compare it with a healthy control group. It also aimed to investigate the correlation of serum vitamin D level with FM symptom severity and invalidation experiences. Methods: A total of 74 consecutive patients with FM and 68 healthy control participants were enrolled. The eligible FM patients completed the Illness Invalidation Inventory (3*I), the Revised Fibromyalgia Impact Questionnaire (FIQR) and a short-form health survey (SF-12). Venous blood samples were drawn from all participants to evaluate serum 25-OHD levels. Mann-Whitney tests and multiple logistic regression analyses were performed and Spearman's correlations were calculated. Results: 88.4% of FM patients had low levels of serum 25-OHD. FM patients had significantly higher level of serum 25-OHD than the control group ($17.24{\pm}13.50$ and $9.91{\pm}6.47$ respectively, P = 0.0001). There were no significant correlations between serum 25-OHD levels and the clinical measures of disease impact, invalidation dimensions, and health status. Multiple logistic regression analyses revealed that an increased discounting of the disease by the patient's spouse was associated with a 4-fold increased risk for vitamin D deficiency (OR = 4.36; 95% CI, 0.95-19.87, P = 0.05). Conclusions: This study showed that although high rates of vitamin D insufficiency or deficiency were seen among FM patients and healthy non-FM participants, but it seems there was no intrinsic association between FM and vitamin D deficiency. Addressing of invalidation experience especially by the patient's spouse is important in management of FM.

Low serum 25-hydroxyvitamin D levels, tooth loss, and the prevalence of severe periodontitis in Koreans aged 50 years and older

  • Purpose: Vitamin D deficiency may cause bone loss and increased inflammation, which are well-known symptoms of periodontal disease. This study investigated whether serum 25-hydroxyvitamin D (25(OH)D) levels are associated with periodontal disease status and tooth loss. Methods: Cross-sectional data from 5,405 individuals aged ≥50 years (2,253 males and 3,152 females) were obtained from the 2008-2010 Dong-gu study, a prospective cohort study of risk factors for chronic diseases. Periodontal examinations were conducted to evaluate the number of remaining teeth, the periodontal probing depth (PPD), the clinical attachment level (CAL), and bleeding on probing. The percentages of sites with PPD ≥4 mm and CAL ≥4 mm were recorded for each participant. The severity of periodontitis was classified using the Centers for Disease Control and Prevention and the American Academy of Periodontology case definitions. Serum 25(OH)D levels were classified as reflecting severe deficiency, deficiency, insufficiency, or sufficiency. Multivariate linear regression analysis was performed to assess the associations of serum 25(OH)D levels with periodontal parameters and the number of remaining teeth after adjusting for confounders including age, smoking status, alcohol consumption status, month of blood collection, and physical activity. Multivariate logistic regression was used to evaluate the association between serum vitamin D levels and severe periodontitis. An overall statistical analysis and a stratified analysis by sex were performed. Results: Overall, the rates of severe deficiency, deficiency, insufficiency, and sufficiency were 6.5%, 67.9%, 22.4%, and 3.2%, respectively. After adjustment for confounders, vitamin D levels were directly associated with the number of remaining teeth, an association that was significant in males, but not in females. Sufficient serum 25(OH)D was associated with a low frequency of severe periodontitis. Conclusions: This population-based cross-sectional study indicates that low serum 25(OH) D is significantly associated with tooth loss and severe periodontitis in Koreans aged 50 years and older.

여대생의 혈중 철 영양 상태에 영향을 미치는 요인에 관한 연구 (A Study of the Factors Affecting Blood Iron Status in Female College Students)

  • This study was performed to investigate the iron status and its related factors in female college students residing in Gyeongnam. The prevalence of iron deficiency among subjects ranged from 3.4% in mean corpuscular hemoglobin concentration (MCHC) to 43.7% in ferritin. Weight, lean body mass (LBM) and body mass index (BMI) were positively correlated with ferritin concentration, but negatively correlated with total iron binding capacity (TIBC). Among the nutrients, vitamin A and B2 were major predictors of elevated iron status. Meal regularity was positively correlated with Fe and ferritin concentration, and meal number with transferrin saturation (TS), meal quantity with red blood cell (RBC) and hematocrit (Hct). Consumption of fruit, meat, fish and poultry showed strong positive correlation with hematological indices. Therefore, increasing vitamin A, B2, C, and iron intakes as well as maintenance of a healthy weight may be helpful in preventing iron deficiency in female college students.

농촌지역 알코올 의존자들의 비타민 $B_1$, $B_2$ 영양상태 (Thiamin and Riboflavin Nutritional Status of Subjects with Alcohol Dependency in Rural Area)

  • The prevalence of alcoholism among elderly population is reported to be high in rural areas in Korea. Chronic abuse of alcohol can lead to the development of vitamin B deficiency through inadequate intake, altered absorption and metabolism, and increased excretion. The present study was conducted to assess vitamin B1 and B2 status in seventeen alcohol dependent subjects who do not exhibit any clinical neurological symptoms. Vitamin B1 and B2 nutritional states were determined enzymatically by measurement of transketolase and glutathione reductase activities in erythrocytes, respectively. And dietary intakes of nutrients were determined by a 24-hr recall method. The mean percent activation of erythrocyte transketolase was significantly higher in alcoholics than in alcoholics than in control (p<0.05). The proportion of subjects with a low and borderline status of vitamin B1, was significantly higher in alcoholics than in control (p<0.05). The mean percent activation of erythrocyte glutathione reductase was not different between alcoholics and control. And the proportion of subjects with low and borderline status of Vitamin B2, was higher in alcoholics than in control (p<0.1). Vitamin B1 and B2 status were significantly decreased in alcoholics who were smoking cigarettes compared to non-smoking and non-alcoholic subjects(p<0.05). Whether vitamin supplementation improves the vitamin status of alcohol dependent subjects remains to be researched.

모유수유아에서의 철결핍 빈혈과 비타민 D 결핍 (Iron Deficiency Anemia and Vitamin D Deficiency in Breastfed Infants)

  • 목 적: 철결핍성 빈혈(iron deficiency anemia, IDA)은 영양상태의 개선에도 불구하고 여전히 발견되는 영양질환이다. 저자들은 영유아기에 IDA가 발생할 수 있는 위험 인자에 대해 알아보았고, IDA 환아의 영양 분석을 통하여 철분 및 비타민 D 영양 결핍에 대하여 알아 보았다. 방 법: 2006년 3월부터 2010년 3월까지 저자들의 병원에 내원한 6~36개월의 IDA 환아 103명을 대상으로 하였고, IDA가 없는 같은 연령의 영유아 123명을 비교군으로 혈액검사와 설문 조사를 하였다. IDA가 진단된 6~12개월 환아 중 식이력 작성이 구체적인 11명에 대해서는 Canpro를 이용하여 영양 분석하였다. 결 과: IDA군에서 모유수유 87.4%, 비교군에서는 모유수유 40.7%였다. 이유 시작 시기는 IDA군은 평균 6.4${\pm}$1.8개월이었고 비교군은 평균 5.9${\pm}$1.3개월이었다. 이유식을 잘 먹게 된 시기는 IDA군에서 4주 이내는 46.4%, 비교군에서 4주 이내는 53.5%였다. IDA군의 병원 방문 이유는 호흡기 질환이 36.2%로 가장 많았고, 빈혈 증상으로 방문한 경우는 18.6%였다. IDA 환아 11명의 Canpro 분석에서 철분은 권장섭취량의 40% 미만이었고, 비타민 D 섭취는 30% 미만이었다. 결 론: 모유수유를 하는 영아는 4~6개월에 이유식을 시작해야 한다. 위험 인자가 있는 영아에서 철결핍 빈혈과 비타민 D 결핍이 있을 수 있으므로, 선별검사와 비타민 D 영양 평가가 필요하다. 또한 철분강화 분유, 비타민 D 강화 분유나 이유식을 통한 영양 공급이 필요하다.

25 Vitamin D3 측정에 있어서 화학발광미세입자 측정법과 화학발광면역 측정법 간의 비교 및 고찰 (Comparison between the method of the measurement 25 Vitamin D3)

  • Purpose Vitamin D to Anti- Rickets both steroid compounds showing activity, By acting on bone tissue secretary and the key to maintain serum Ca homeostasis. The blood level of vitamin D is the largest in D3 that the concentration of the metabolite is reflected in the holding state of vitamin D in vivo. Sunlight to change the 7-dehydrocholesterol in the skin and through the skin to D3, In the liver in combination with the D2 and D3 D4 changes. The Radioimmunoassay(RIA) method is measuring the D 3, the sensitivity can be measured also difficult trace substance to measure the normal test because it is very sensitive, but recently, a check is possible, for the Total D3 in Chemimicroparticle immunoassay(CMIA) or Chemiluminescent immunoassay(CLIA) measuring using microparticle RIA and CMIA(Architect i2000SR) / use the CLIA(DXI-800) method to compare and evaluate the correlation between the tests in the same test items. Materials and Methods Commissioned from January 2014 to March 2015 patients were enrolled in a total of 273 people. 29 out of 273 people conducted by RIA were compared with CMIA, 244 patients were compared with CLIA. Using reagents and equipment were used RIA(Diasource), CMIA(Architect i2000SR, Abbott Diagnostics) / CLIA( Unicel DXi-800, Beckman coulter). Results Correlation of the RIA and CLIA was a R2 = 0.1844 (y = 0.7303x + 3.9005), and the correlation of RIA CMIA is R2 = 0.2762 (y = 0.8862x + 4.56) respectively. (According to statistics, during the same period RIA is Deficiency 4.31%, Insufficiency 90.53%, Sufficiency 5.16%, was Excess 0%, CLIA / CMIA is Deficiency 17.02%, Insufficiency 75.91%, Sufficiency 7.03%, indicating the distribution of 0.03 % Excess) Conclusion Serum vitamin D and parathyroid hormone that show an inverse relationship, the level above which are not parathyroid hormone and vitamin D reduced the increase. The density is different for each study, at most 20 is reported to be the maximum between 30 ng / ml. In Korea it requires a proposed standard of vitamin D deficiency, reference to the WHO lack the case more than 10ng/ml, 20ng/ml and defined by the lack of, if not more than, the IOM, but looking at 12ng/ml or less to the normal to lack, at least 20ng/ml, the reference do not match the deficit under 20ng/ml, 21-29ng/ml relative lack between, was also defined as a sufficient condition for more than 30ng/ml. Although not statistically is between RIA and CLIA two ways to vitamin D levels change according to season match, when seasonally seen in summer as commonly known (April to September), winter (October to March) relative to the increase measured than it was found. Finally, the study on the correlation between the two methods have been expected to result in a consistent and apply the same view high reference value on the graph is difficult. However, there may be differences between the test equipment and methods, and could be especially the case of RIA method using an organic solvent is difficult to compare different methods and correlated view similar trend in vitamin D deficiency and quarterly aspect ratio.

Studies on Serum Micro-mineral, Hormone and Vitamin Profile and Its Effect on Production and Therapeutic Management of Buffaloes in Haryana State of India

  • A survey was conducted in certain parts of Haryana to record the prevalence of micro mineral deficiency in buffaloes. The prevalence of soil Cu, Co, Zn and Fe deficiency was 55.26%, 6.9%, 59.12% and 7.89% respectively. While that of fodder Cu, Co, Zn and Fe was 60.64%, 6.7%, 61.22% and 11.37% respectively. The overall prevalence of serum Cu, Co, Zn and Fe deficiency in Haryana was 59.2%, 19.1%, 59.2% and 19.9% respectively. The correlation co-efficient of Cu, Co, Zn and Fe in soil, fodder and serum was significant in most of the cases the values were above 0.8. Blood examination revealed significant decrease in haemoglobin and TEC level. However, no variation in level of TLC were observed in mineral deficient buffaloes. In micro mineral deficiency, thyroid hormone (T3 and T4) levels were decreased in buffaloes. Marginally lower concentration of vitamin A and E were observed in mineral deficient buffaloes in Haryana. The highest deficiency of micro minerals was 61.76% in copper at Ambala followed by 65.86% in zinc at Rhotak. For therapeutic studies a mineral mixture was prepared according to defiency obtained and fed to three groups of animals. Observation was recorded on 0, 15, 30, 45 and 60 days. Group A consist of normal healthy animals and group B mineral deficient animal untreated and group C mineral deficient animal, treated with prepared mineral mixture. 25 gram of mineral mixture was fed daily along with normal ration. There was increase in body weight, milk yield, haemoglobin concentration and total erythrocyte level in group C animals when compared to group B animals. The milk yield in group C animals increased to 6.970${\pm}$0.41 after 60th day of supplement in comparison to 0 day where it was 5.910${\pm}$0.37, similarly the body wt. of group C animals increased from 129.42${\pm}$01.13 (at 0 day) to 159.31${\pm}$03.61 at 60th day of treatment.

Relationship between vitamin D levels and intravenous immunoglobulin resistance in Kawasaki disease

  • Purpose: Vitamin D is associated with various pathological conditions such as cardiovascular diseases and cancer. We investigated the relationship between vitamin D and Kawasaki disease (KD). Methods: We performed a retrospective review of the medical records of patients with KD between February 2013 and March 2016 in Daegu Fatima Hospital. Study participants were grouped according to vitamin D serum concentration. Group 1 included patients with 25(OH)-vitamin D ${\geq}20ng/mL$. Group 2 included patients with 25(OH)-vitamin D <20 ng/mL. We analyzed the clinical characteristics and laboratory data of the 2 groups. Results: Of the 91 patients, 52 were included in group 1, and 39 in group 2. Group 1 patients had significantly higher levels of calcium, phosphate, albumin and sodium than group 2 patients did. There were no differences in clinical characteristics, but the proportion of patients with polymorphic rash was significantly higher in group 2. Resistance to intravenous immunoglobulin was more frequent in group 2 (P=0.023). No significant difference in the incidence of coronary artery complications was observed. Conclusion: Low vitamin D levels are associated with resistance to intravenous immunoglobulin therapy in KD. Vitamin D deficiency might be a risk factor for immunoglobulin resistance in KD.

일개 전자제품 제조업 근로자에서 교대근무와 불안증상: 비타민 D와 수면의 질의 매개효과 (Shift Work and Anxiety Symptoms at an Electronic Manufacture Company: The mediating effect of vitamin D and sleep quality)

  • Objectives: The purpose of this study was to compare vitamin D levels, sleep quality, and anxiety symptoms between shift workers and daytime workers and analyze the mediating effect of vitamin D and sleep quality between shift work and anxiety symptoms. Methods: The subjects of the study were 6744 people who received a medical examination while working for an electronics manufacturing company in 2016. We evaluated their shift work, vitamin D levels, sleep quality, and anxiety symptoms with blood samples and questionnaires. Chi-square test, t-test and path statistical analysis were performed. Results: The shift workers had significantly higher symptoms of anxiety than the day workers, significantly lower levels of vitamin D in the blood, and significantly poorer quality of sleep. The results of the pathway analysis confirmed that blood vitamin D and sleep quality had a mediating effect between shift work and anxiety symptoms. Conclusions: Shift workers should be constantly paying attention to mental health effects such as anxiety symptoms. Shift workers should also be alerted of the risk of vitamin D deficiency, encouraged about sun exposure and vitamin D intake, and required to undergo training and counseling on sleep hygiene.

한국인의 비타민 D 부족 유병률에 관한 연구: 국민건강영양조사 2010~2011 분석결과 (Prevalence of vitamin D deficiency in Korea: Results from KNHANES 2010 to 2011)

  • Vitamin D deficiency (VDD) is becoming an epidemic and thereby a global health problem. Further, VDD adversely affects calcium metabolism and skeletal health, and is associated with increased risk of several diseases, e.g., autoimmune diseases, several types of cancers, type 2 diabetes mellitus, cardiovascular diseases, infectious diseases, asthma, psoriatic arthritis, and etc. To evaluate the prevalence of VDD in Korea, and then to evaluate the association of several factors with serum 25(OH)D level, the author analyzed the data of 14,456 individuals who were 10 years of age and over from the Fifth Korea National Health and Nutrition Examination Survey 1 & 2 (KNHANES V-1 & 2) conducted by the Korean Centers for Disease Control & Prevention. As a result, among Koreans (age $${\geq_-}$$ 10years), 65.9% of males and 77.7% of females were below optimum blood serum 25(OH)D (20 ng/mL). VDD is more severe in female than in male at all age groups. In addition, the younger generations had less 25(OH)D level than older generations in Korea. The analysis by complex sample general linear model (CSGLM) suggested that blood 25(OH)D concentration was related with gender (p < .001), residence (p = .030), occupation (p < .001), anemia (p < .001) and physical activity (p < .001). In conclusion, VDD is pandemic and it is more severe in younger generations in Korea. Further, from the results by CSGLM, serum 25(OH)D status is closely related with the life style of Koreans.
