영유아 보육시설의 영양관리 시범사업(I): 유아의 식생활 현황 (Baseline Dietary Behaviors of Children for Nutritional Management Programs at Child Care Centers in Korea)

  • The aim of this investigation was to collect the baseline data about growth, dietary behaviors, nutrient intakes and dietary quality of children at child care centers for planning nutritional management program. The dietary intakes were measured by weighing method for intakes at child care centers and by dietary records by children\\`s mothers for intakes at home. The weight and height were measured and calculated z-scores by using standard of the same age groups. The children were 163 boys and 168 girls and the mean age were 55.6 months(12-78 months) and 56.0 months(16-78 months). respectively. The Z-score for height(0.33) and weight(0. 11) showed that the growth of children were above average. Nutrients intakes such s energy, protein, fiber, iron, sodium, potassium, vitamin A, vitamin B$_1$, vitamin C, niacin of boys from lunch and snack at day care centers were significantly higher than those of girls, and the meals at child care centers provided 20-37% of RDAs. The daily energy intakes were 1332.2kcal(83.1% RDA), protein 44. 26g(109.3% RDA), calcium 437.73mg(72.4% RDA), iron 7.24mg(72.4% RDA), vitamin A 459.6RE(113.1% RDA),vitamin B$_1$ 0.74mg(93.4% RDA), vitamin B$_2$0.79mg(79.7% RDA), niacin 7.66mg(69.9% RDA) and vitamin C 56.84mg(142.1% RDA). The nutrients which more than 50% of subjects ate less than 75% RDA were calcium, iron, niacin, and vitamin B$_2$.The average mean adequacy ratio was 0.80 and mean dietary variety score was 22.42, and those are positively correlated to % RDA of nutrient intakes. In summary, the children using child care centers consumed most nutrients below level of RDA and the nutritional management program at child care center is required to enhance the nutritional status of them. 한국 성인 남녀에서 시리얼 섭취 여부에 따른 영양섭취상태 평가 : 2013 ~ 2016 국민건강영양조사 자료를 이용하여 (Cereal intake status and nutritional status of adults: results from the Korean National Health and Nutrition Examination Survey, 2013 ~ 2016)

  • 본 연구에서는 시리얼 판매 회사의 홈페이지 내 영양정보를 사용하여 시중에 판매되고 있는 24종의 시리얼에 함유된 영양성분 평가를 진행하였다. 또한 2013 ~ 2016년 국민건강영양조사 자료의 식품섭취빈도조사 결과에 근거하여 사용하여 우리나라 19 ~ 64세 성인 12,199명 (남자 5,127명, 여자 7,072명)에서 시리얼의 섭취 빈도, 1회 평균 섭취량을 파악 후 시리얼 섭취 여부에 따른 기본 특성 비교, 영양소 섭취 상태를 비교하였다. 그 결과, 시리얼 24종에서 시리얼 30 g의 평균 열량 함유량은 118.29 kcal였으며, 시리얼 30 g과 흰우유 200 mL를 함께 섭취할 경우 30 ~ 49세 남성은 열량 필요추정량 대비 10.35%, 30 ~ 49세 여성은 13.07%를 섭취할 수 있는 것으로 나타났다. 우리나라 19세 이상 성인의 20.4%가 1달에 1회 이상 시리얼을 섭취하고 있었으며, 1회 섭취량은 $1{\frac{1}{2}}$대접이 54.6%로 대부분의 대상자가 1회 평균 1대접 이상의 시리얼을 섭취하는 것으로 나타났다. 또한 성별 시리얼 섭취 여부에 따른 기본 특성을 분석한 결과 시리얼을 섭취하는 군에서 체질량지수 $25kg/m^2$ 이상인 비만 비율이 비섭취군에 비해 유의적으로 낮았으며, 아침식사 빈도가 유의적으로 높았고, 영양의 질적 지수가 1 이상인 영양소의 개수가 더 많은 결과를 보여, 우리나라 성인에서 시리얼의 섭취와 영양섭취 상태와의 관계에 대한 의미있는 기초결과를 도출하였다. 본 연구결과를 토대로 앞으로도 시리얼의 섭취가 영양 및 건강에 미치는 영향에 대한 체계적인 연구가 함께 수반되어야 할 것으로 생각된다

부산 시내 일부 저소득층 유아원 원아의 영양실태에 관한 연구 I. 영양섭취실태 및 건강실태 조사 (Nutrition Survey of Children of a Day Care Center in the Low Income Area of Pusan I. A Study on Nutrient Intake and Nutritional Status)

  • 부산시 남구 감만동 소재 어린이집 원아 99명을 대상으로 1992년 7월 1일부터 7월 14일까지 실시한 영양 조사의 결과를 요약하면 다음과 같다. 1) 신체 계측 결과를 Kaup가 제시한 분류 기준에 따라 구분하면, 6세의 경우는 8.3%가 영양불량으로 나타났고, 3세의 21.9%, 4세의 55.6%, 5세의 42.8%, 6세의 50%가 가벼운 영양부족으로 나타났다. 2) 헤마토크릿치는 38.9%, 혈청 철함량은 53.5$\mu\textrm{g}$/100m1이었으며, 빈혈이 의심되는 어린이가 17%이었다 혈청 총단백 함량은6.7g/100$m\ell$, 혈청 알부민 함량은 3.5g/100$m\ell$이었는데, 이중 단백질 결핍이 의심되는 어린이가 27%정도되었다. 혈청Complement $C_3$는 152.4mg/100$m\ell$로 정상 범위에 속했다. 3) 열량섭취량은 권장량의 68.6~87.5%를 나타내었으며, 탄수화물, 지방, 단백질의 구성비율은 63.1~67 2 : 18.6~21.7 : 13.5~l5.2이었다. 단백질은 94.1~114.1%의 섭취량을 보였으나, 칼슘, 철, 비타민 A, 비타민 B$_1$, 비타민 B$_2$, niacin, 비타민 C 등은 권장량보다 낮은 섭취율을 보였다 4) 점심을 제외하고는 매끼 10%정도의 어린이가 결식하는 것으로 나타났고, 1끼 평균 3.5~5.5가지의 식품을 섭취하고 있었다. 5) 해당 영양소의 결핍이 우려되는 수준인 영양권장량의 75% 미만을 섭취하고 있는 어린이의 비율이, 철분의 경우 68%, 비타민 C 60.2%, 칼슘 58.2%, niacin 58.3%, 비타민 B$_1$37.8%, 비타민 B$_2$37.9%로 매우 높음을 알 수 있었다.

경북 성주지역 장수노인의 영양상태(I) -영양섭취상태- (Nutritional Status of the Long-lived Elderly People in Kyungpook Sung-Ju Area(I) -Estimation of Nutrients Intakes-)

  • The purposes of this study were to estimate nutritional intakes of the long-lived elderly and to obtain the data for establishing dietary guidelines that may be recommended for the general population for the sake of longevity. The subjects of the study were 300 elederly people of age over 85 years living in Kyungpook Sung-Ju area who had no problem in daily living. Four times of food consumption survey were carried out seasonally by the repeated 24-hr recall method for one year. Mean daily energy intakes and RDA percentage of energy intakes of the male and female subjects were estimated as 1222 kcal(67.9%) and 1047 kcal(65.4%) respectively. Mean daily intakes of nutrients were estimated as 38.3g for protein, 287mg for calcium, 5.8mg for iron, 314R.E. for vitamin A, 0.6mg for vitamin B1 and 0.43mg for vitamin B2 The mean RDA percentages of nutrients intake were 64.4% and 59.8% for protein in male and female, 39.8% for Ca, 48.3% for Fe, 44.9% for vitamin A, 60% for vitamin B1, and 35.8% for B2. The average PFC ratio of energy-yielding nutrients throughout the year in male and female were 15.1 : 15.2 : 69.7 and 13.8 : 13.2 : 73.0 respectively. The mean daily intakes of energy and most of nutreints were significantly high in winter season. The contribution of plant food sources to nutrient intakes were over 60% for protein and fat, 50% for calcium, and 70% for iron. Long-lived elderly people an Sung-Ju, Kyungpook showed considerably smaller physiques compared to the avergae Korean elderly, however their average BMI fell in normal range. The subjects were consuming much less energy and nutrients compared to the present Korean RDA for the elderly over age 75. The subject showed relatively good health state in spite of low intakes of energy and nutrients. Therefore it seems to be necessary to establish a set of new RDA for the elderly over age 8.5.

A Study on the Nutrient Intakes of Hospitalized Elderly Dementia Patients Receiving a Regular Diet

  • The purpose of the present study was to examine the nutrient intake status of elderly dementia patients. We surveyed the dietary intake of 50 demented elderly patients receiving a regular diet, who were hospitalized in a geriatric hospital in Yongin, Gyeonggi-do. The average age of the subjects was $79.6{\pm}6.5$ for the males and $80.5{\pm}6.3$ for the females. The average heights were $169.5{\pm}6.0\;cm$ (males) and $154.6{\pm}5.2\;cm$ (females), and the average weights were $58.5{\pm}7.4$ (males) and $51.7{\pm}8.9\;kg$ (females). Depending on the type of dementia, the male patients showed significant differences in their intakes of vegetable protein, fiber, total iron, and non-heme iron between the Alzheimer's disease (AD) and the vascular dementia (VD) groups, and the female patients showed significant differences in their intakes of total protein, phosphorus, zinc, and vitamin $B_6$ between the two groups. The male patients showed significant differences in their intakes of animal-source calcium and carotene according to ability to self-feed. According to physical activity, the male patients showed significant differences in vegetable fat and vitamin A intake, and the females showed significant differences in their intakes of total protein, animal protein, fiber, ash, total calcium, vegetable-source calcium, animal-source calcium, phosphorous, total iron, non-heme iron, heme iron, sodium, vitamins $B_1$, $B_2$, $B_6$, niacin, vitamin C, folate, and potassium. We found that the patients consumed excessive protein, but low amounts of calcium, vitamin $B_2$, and folate. Also, the patients' physical activity abilities appeared to affect their nutrient intakes.

채식선호자와 육식선호자의 식사의 질 및 비타민 K 섭취 비교 연구 (A Study for Comparison of Dietary Quality and Vitamin K Intake of Vegetarians with Carnivores)

  • The prevalence of chronic diseases have been rising in the developing countries because of their increased animal foods consumption and Western lifestyle. Lately, vegetarian diet that exclude animal products get public attention. The purpose of this study was to evaluate the nutritional status and dietary quality of vegetarians, and their consumption of vitamin K and was also assessed. Vegetarians including strict vegan and lacto-ovo-vegetarian consumed their diet at least over 6 months. Carnivores were gender and age matched with vegetarians and they consumed over 50% of protein and fat from animal sources. Current nutrient intakes and dietary quality were assessed using 3-day food records and intake of vitamin K was calculated from the data base of 'Provisional Table on the vitamin K contents of foods, USA'. Blood sample were collected and biochemical parameters and plasma phylloquinone concentrations were analyzed. Anthropometric data from vegetarian and carnivore were not significantly different. The intake of calories, protein, vitamin $B_2$, Ca and Zn of the vegetarians were remarkably lower than RDA for each nutrient. Moreover, index of nutritional quality and nutrient adequacy ratio of vegetarians were lower than those of carnivore. Vegetarian consumed less fat and the ratio of n-6/n-3 fatty acid was lower in vegetarian. The intake of essential amino acids in vegetarian was significantly lower than that of carnivore. The vitamin K consumption and plasma phylloquinone concentration of vegetarian were significantly higher than those of carnivore (p<0.05). The dietary vitamin K consumption was positively correlated with plasma phylloquinone levels in vegetarian (p<0.01).

일부 농촌 지역 흡연 여고생의 영양 섭취 및 혈액 성상 조사 (A Study on the Dietary Nutrient Intakes and Blood Profiles of Smoking Teenage Girls Living in a Rural Community in Korea)

  • Cigarette smoking has a negative effect for nutrient intakes and nutritional status of antioxidant nutrients including vitamin C. This study has been performed to compare the dietary nutrient intakes, serum vitamin C concentration and serum cotinine concentration of smoking teenage girls(n=104) with those of non-smoking teenage girls(n=791). The subjects were 895 high school girls(15-19 years of age) living in a rural community in Korea. Of the subjects, 11.6% stated that they were currently smoking. The frequency of taking alcohol, soda and coffee was higher in smokers than in non-smokers, respectively. The usage rate of vitamin/mineral supplements was higher in smokers than in non-smokers. The smokers tended to skip breakfast and lunch and to take snack more often than did the non-smokers. The dietary intakes of calorie, protein, vitamin A, vitamin B, vitamin C, calcium and iron were lower in smokers than in non-smokers. Daily mean intakes of these nutrients were less than the 2/3 of the Korean RDAs in smokers while these intakes were similar to or more than the RDAs in non-smokers. The ratio of under weight or lean tended to be higher in smokers. Serum vitamin C concentration was lower in smokers than in non-smokers. These findings show that smokers have unsound eating habits and poor nutirtional status. In particular, nutritional status of vitamin C is significantly poor in smokers, and this tendency can be found more notably in heavy smokers. Therefore, oxidative stress may be present in young teenage girls with short smoking history as evidence by above results. (Korean J Nutrition 34(3) : 338∼347, 2001)

식사형태와 식사능력에 따른 요양병원 여자 노인 환자의 영양섭취조사 (Nutrition Status of Elderly Female Patients in Long-term Care Hospital according to Meal Types and Eating Ability)

  • Objectives: This study was carried out to determine nutritional status of elderly patients in a long-term care hospital according to meal type and eating ability. Methods: Subjects were 47 female patients aged over 65 ($79.3{\pm}7.1$ years) who resided in a long-term care hospital in Seoul. Thirty seven patients who ate diet orally were grouped according to meal type (27 general diet and 10 soft diet) and eating ability (26 eating by oneself and 11 eaten with help) and 10 were on tube feeding. Nutritional status was determined by food consumption and mid-arm circumference. Results: The mean adequacy ratios (MARs) of 12 nutrients (protein, calcium, phosphorus, zinc, vitamin A, vitamin $B_1$, vitamin $B_2$, vitamin $B_6$, niacin, folic acid, vitamin C) were 0.687 for general diet, 0.565 for soft diet, 0.680 for eating by oneself and 0.677 for eaten with help, which were significantly lower than 0.982 for tube feeding (p < 0.05 or p < 0.01). The patients on tube feeding had significantly lower % arm circumference compared to those who ate general diet (84.0% vs. 95.4%, respectively, p < 0.05). Nutrients intakes, nutrient adequacy ratio (NAR) and index of nutritional quality (INQ) were not different between meal types as well as eating ability. The most insufficiently consumed nutrients by the patients on diet were folic acid, vitamin $B_2$, and calcium (NAR 0.334~0.453, 0.515~ 0.539, and 0.516~0.533, respectively). Conclusions: The results suggested that regardless of meal type or eating ability, the subjects who were on diets in this study might have inadequate intake of folic acid as well as vitamin $B_2$, and calcium, which need to be reflected on menu planning. The measurement of mid-arm circumference presented more risk of malnutrition of patients on tube feeding than those on diets, despite apparently better nutrient consumption.

대구지역 유아들의 식사 및 식생활 형태 조사 - 비만도에 따른 영양소섭취 수준 및 식태도 조사 - (Study of Food Intakes and Eating Patterns among Preschool Children in Daegu Area - Nutrient Intakes and Dietary Habits Associated with Body Weight Status -)

  • This study was conducted to investigate the body weight status (by WLI: Weight-Length Index) and dietary habits and to assess the nutritional status among preschool children in the Daegu area. Dietary habits including dietary attitude and behavior were investigated using questionnaire answered by mothers of 680 subjects aged 4 to 6 years old (508 completed). Also, dietary intake survey using 24-hour recall method was performed by mothers of the children. According to WLI, the underweight, normal, overweight and obesity level of subjects were 9.1, 62.0, 19.5, and 9.4%, respectively. As well, the number of girls was higher than boys in underweight, overweight, and obese group. There were significantly different on overeating, eating fast, and preferring eating-out by body weight status, and overweight group got higher score than underweight or normal-weight group (p < 0.01, p < 0.001). As well, there was significantly different on not moving around during mealtime by body weight status, and underweight group have lower score than obese group (p < 0.001). From the 24-hour recall survey, it was found that intakes of all the nutrients were higher than the %KDRI except energy, calcium and folate. The energy intake of underweight group of 4~5-year old (1338.2 kcal) and 6-year old was lower than Koreans %DRI for those age group. Folate and dietary fiber intakes of obese group were significantly lower than underweight (p < 0.01, p < 0.05). For 4~5-year old, vitamin $B_6$ intakes of obese group were significantly higher than obese group (p < 0.05). For 6-year old, obese group showed that intake of vitamin $B_1$ was significantly higher than the other three groups, and intake of vitamin $B_1$ of overweight group was significantly higher than the other three groups. Taken together, these results indicate that there were significant differences in the nutrient intake level and dietary habits of preschool children by body weight status in Daegu area. Therefore, parents (family) and caregivers should be aware of the prevalence of obesity and nutritional status of preschool children, and start nutrition education as soon as possible.

액상철분제제 및 시리얼보충이 사회복지시설 아동의 철분영양상태에 미친 영향 비교 (The Effects of Iron Supplements and Cereal Intake on the Iron Nutritional Status in Children in Social Welfare Institutions)

  • The study was designed to assess the effect of iron and cereal supplementation on children's iron nutritional status in social welfare institutions. Dietary survey was carried out methods of food weighing and record by interview (n=74). A nutritional intervention study was carried out through supplementing iron supplements and cereal for 4 weeks in 4-12 years old children. Children received daily 40 mg elemental Fe as iron protein succinylate (n=23) and 3.6 mg elemental Fe as 100 g cereal (n=24), respectively. Blood samples were drawn before and after supplementation. Nutrients which children's intake was less than two-thirds of the RDA were vitamin A, vitamin B-1, vitamin B-2, calcium and iron. The mean daily intake of iron was 5.1 mg for male and 4.9 mg for female, and 52.3% for male and 45.4% for female of Korean RDA. The proportion of children with iron depletion assessed by TIBC (> 360 ${\mu}g$/dl) and serum ferritin (< 20 ng/ml) were 56.6% and 58.7%, respectively. The proportion of children with the iron deficient erythropoiesis assessed by serum iron (< 70 ${\mu}g$/dl), Hb (< 12 g/dl), Hct (< 36%) were 76.0%, 58.7%, 64.0%, respectively. After iron supplements treatment, Hb (p<0.001), Hct(p<0.001), serum iron (p<0.001), transferrin saturation (p<0.001) and serum ferritin (p<0.Ol) increase significantly and only TIBC decreased slightly. After cereal supplementation, in anemic children, Hct (p<0.001), serum iron (p<0.001) and transferrin saturation (p<0.001) were significantly increased. The effect of iron supplements and cereal supplementation in children with iron deficient erythropoiesis were more effective to improve the iron nutritional status than children with iron depletion. It was concluded that cereal supplementation program in anemic children was also effective to improve iron nutritional status.
