Rupture of the spleen is relatively common, both immediately and in a delayed fashion following significant blunt abdominal trauma. However, atraumatic splenic rupture rarely occurs. Multiple underlying pathologies have been associated with splenic rupture without trauma, including hematological, neoplastic, inflammatory and infectious conditions. In our case, a 21-year-old male without prior medical history visited the hospital with left upper quadrant abdominal pain that had started one day earlier. He had no history of trauma. An abdominal computed tomography (CT) scan found a collection of perisplenic fluid, accompanying a splenic rupture. Due to the patient's stable vital signs and lack of clinical progression of hemorrhage, he underwent conservative treatment. The patient was discharged at day 14 without complication. Rupture of a normal spleen without a history of trauma is not often reported, and it has long been a subject of debate. Ruptures of normal spleen almost always follow some kind of trauma, such as a car accidents or a fall from significant heights. Here, we report a case of spontaneous rupture of a normal spleen in the absence of other medical pathologies or triggering factors.
Aorticopulmonary window is a rare anomaly among congenital heart disease. Various terms have been suggested including A-P window, A-P fenestration, fistula, aorticseptal defect etc. The defect lies usually between the left side of the ascending aorta and right wall of the pulmonary artery just anterior to the origin of the right main pulmonary artery. We have experienced one case of aorticopulmonary septal defect which was diagnosed as V5D with pulmonary hypertension in 1 4/12 year old, 7.2 Kg, male patient. Operation was done under the hypothermic cardiopulmonary bypass using 5t. Thomas cardioplegic solution. Vertical right ventriculotomy over the anterior wall of RVOT revealed no defect in the ventricular septum, and incision was extended up to the main pulmonary artery to find the source of massive regurgitation of blood through MPA. Finger tip compression of the aorticopulmanary window was replaced with Foley bag catheter balloon, and the $7{\times}10$ mm aorticoseptal defect located 15mm above the pulmonic valve was sutured continuously wih 3-0 nylon suture during azygos flow of cardiopulmonary cannula which was located distal to the window resulted massive air pumping systemically, and temporary reversal of pumping was tried to minimize cerebral air embolism. Remained procedure was done as usual, and pump off was smooth and uneventful. Postoperatively, patient was attacked frequent opistotonic seizure with no recovery sign mentally and p.hysically. Vital signs were gradually worsen with peripheral cyanosis and oliguria, and cardiac activity was arrested 1485 minutes after operation. Autopsy was performed to find the sutured window and massive edema of the brain.

Anxiety and fear is two main factors that keep patients from going to dental clinic. Especially, patients may feel implants operations are more traumatic. Intravenous conscious sedation for dental treatment can make patient comfortable and relaxable. Midazolam is more popular for sedation for dental treatment, but target-controlled infusion (TCI) of propofol and remifentanil is gaining wide popularity. A 54-year-old female patient who had severe dental phobia was referred to our dental hospital. She had past history of 2 times of hyperventilation and syncope during dental treatment. The patient showed a lot of dental anxiety and fear to dental treatments and stress reduction protocol was needed. We administered intravenous conscious sedation using target controlled infusion system with remifentanil and propofol. During sedation, we monitored the status of consciousness with bispectral index and vital signs. Dental treatment could be finished successfully without any problems.

In this paper, we propose a sleep efficiency measurement algorithm based on IR-UWB radar sensor in distance. Among the vital signs which can be measured by the IR-UWB radar sensor such as breathing rate, heartbeat rate, and movement, we analyzed correlation between the movement and the sleep efficiency, and based on the result, we propose a sleep efficiency measurement algorithm. In order to verify the performance of the proposed algorithm, we applied the algorithm to three polysomnography patients in hospitals and obtained the performance of an average absolute error within 3.9%.
Spontaneous hemopneumothorax is a rare disease, and it can cause life threatening condition. It is characterized by the accumulation of more than 400 mL of blood and air in the pleural cavity without any other apparent causes. A previously healthy 22-year-old female patient presented with acute chest pain and dyspnea. Chest X-ray and computed tomography revealed a massive hemopneumothorax in the left hemithorax. The images showed a completely collapsed left lung with right-sided tracheal deviation, several pleural adhesion bands, and fluid collection with air-fluid level. We emergently performed a closed thoracostomy, and then 560 mL of fresh bloods were initially drained. We considered an emergent video-assisted thoracoscopic surgery for pulmonary wedge resection and bleeding control because of the massive hemothorax. However, the patient's vital signs were stabilized after blood transfusion and supportive cares for re-expansion pulmonary edema. The patient discharged from the hospital on 11th in-hospital day after removal of the chest tube, and there had not been any recurrence of the pneumothorax for 10 months. We suggest that treatment strategy should be decided upon individually based on the patient's condition and clinical course of the disease.

The effectiveness of a synthetic solution of sodium carboxymethy1cellulose (SCMC) and dextran in the prevention of adhesion formation following intestinal anastomosis of intestine in dogs was elucidated and compared with Interceed. Following induction of adhesion on jejunum of dogs by resection and intestinal anastomosis, the dogs were infused with a synthetic solution of 1% SCMC and 10% dextran in saline in the abdominal cavity at the dose of 5ml/kg of body weight or treated with Interceed membrane. Fourteen days after operation, all the dogs showed adhesion of anastomotic sites to surrounding tissues. The average score of adhesion was 0.50, 0.60 and 0.80 in the dogs infused with the synthetic solution, Interceed membrane and saline solution, respectively. The synthetic solution showed better result of adhesion prevention than Interceed or saline solution. No significant change of body weight, vital signs, hematological values and side effects after operation was found in each group. Therefore, a synthetic solution which is combined with 1% SCMC and 10% dextran in saline can be effectively used for the prevention of adhesion formation comparable to Interceed membrane following intestinal anastomosis in the dogs.

This paper presents a user authentication scheme for healthcare application using wireless sensor networks, where wireless sensors are used for patients monitoring. These medical sensors' sense the patient body data and transmit it to the professionals. Since, the data of an individual are highly vulnerable; it must ensures that patients medical vital signs are secure, and are not exposed to an unauthorized person. In this regards, we propose a user authentication scheme for healthcare application using medical sensor networks. The proposed scheme includes: a novel two-factor user authentication, where the healthcare professionals are authenticated before access the patient's body data; a secure session key is establish between the patient sensor node and the professional at the end of user authentication. Furthermore, the analysis shows that the proposed scheme is safeguard to various practical attacks and achieves efficiency at low computation cost.

U-Healthcare service is a real-time service using the vital signs which are continuously transferred from monitoring sensors attached to mobile patients under the wireless network environments. It should monitor the health condition of mobile patients everywhere at any time. In this paper, we have improved two features of the three layered mobile patient monitoring system with load balancing ability. First, the simulation process has been improved by allowing the number of related nodes to be changed. Secondly, we have modified S node to which queue is added to reduce the loss rate of collecting data from patients during the delay of S node process. And the data from the patient with high priority can be transferred to the server immediately through the filtering function. Furthermore, we have solved the problem of redundancy in sharing information among S nodes by differentiating process time to each S node. By performing a DEVS Java-based system simulation, we have verified the efficiency of this improved system.
Purpose: The purpose of this study was to analyze nursing research trends on the complementary and alternative therapies (CAT) for the cancer patients in Korea. Method: Thirty eight articles published in 17 nursing-related journal from 1987 to 2008 were analyzed. Results: Nursing Researches on CAT for the cancer patients have increased since 2000. Among the 38 studies, 34 studies used Quasi-experimental design and 13 studies (33%) were conducted for the breast cancer patients. Mind-body therapy (53%), manual healing therapy (42%), and pharmacologic and biological therapy (5%) were the often used CAT types. Nausea, vomiting, vital signs, immune cell, pain, and fatigue were measured as physiologic outcome variables; anxiety, depression, and hope as psychological outcome variables; and quality of life as social outcome variables. Conclusion: More CAM studies are needed targeting the patients with various cancer types and home-based cancer patients.

Anaphylactic reactions to anesthetic drugs could potentially produce life-threatening immune-mediated crisis. Most published reports are associated with neuromuscular blockers and anaphylactic reactions to inhalation anesthetics are rare. A 25-year-old male patient with no significant medical history and no previous abnormal drug reaction was scheduled for orthognathic surgery under general anesthesia. After uneventful anesthetic induction and nasotracheal intubation, generalized urticaria and erythema were detected during the maintenance period with $O_2-N_2O$-enflurane. No severe changes of vital signs and no ventilation problem were accompanied. The operation was cancelled and the cutaneous lesions were faded away during the recovery with 100% $O_2$. The skin-prick and intradermal tests showed that he was hypersensitive to all halogenated inhalation anesthetics including enflurane and not to intravenous anesthetics and neuromuscular blockers. The re-operation was safely carried out under intravenous anesthesia with propofol-fentanyl-vecuronium. We report this case of intraoperative anaphylactic reaction to enflurane with literature review.
