Evaluation of Effects of a Clinical Reasoning Course among Undergraduate Nursing Students (시뮬레이션을 적용한 임상추론 교과목의 적용효과: 일 대학의 예를 중심으로)

  • Purpose: To evaluate undergraduate nursing students' ability in clinical competence, critical thinking, and problem solving following enrollment in a clinical reasoning course. Methods: A clinical reasoning course utilizing a human patient simulator and scenarios was offered to 22 senior students at a College of Nursing in Seoul. Students' clinical competence was measured with a checklist of 15 items by analyzing students' performance recorded on video tapes for eight scenarios. Critical thinking disposition and problem solving were measured by a self-administered questionnaire before and after the course. Data were analyzed using descriptive statistics and Wilcoxon signed-rank test. Results: The high scored items of clinical competence were: 'obtain relevant subjective/objective data', 'interpret vital signs', 'communicate with healthcare providers', and 'utilize standard precautions including handwashing.' Students' critical thinking and problem solving scores following the course were increased with statistical significance. Conclusion: A clinical reasoning course utilizing a human patient simulator creates a realistic clinical environment for nursing students and provides the opportunity to obtain clinical competence, critical thinking, and problem solving skills.

  • Now a day, Wireless Sensor Networks (WSNs) are being widely used in different areas one of which is healthcare services. A wireless medical sensor network senses patient's vital physiological signs through medical sensor-nodes deployed on patient's body area; and transmits these signals to devices of registered medical professionals. These sensor-nodes have low computational power and limited storage capacity. Moreover, the wireless nature of technology attracts malicious minds. Thus, proper user authentication is a prime concern before granting access to patient's sensitive and private data. Recently, P. Kumar et al. claimed to propose a strong authentication protocol for healthcare using Wireless Medical Sensor Networks (WMSN). However, we find that P. Kumar et al.'s scheme is flawed with a number of security pitfalls. Information stored inside smart card, if extracted, is enough to deceive a valid user. Adversary can not only access patient's physiological data on behalf of a valid user without knowing actual password, can also send fake/irrelevant information about patient by playing role of medical sensor-node. Besides, adversary can guess a user's password and is able to compute the session key shared between user and medical sensor-nodes. Thus, the scheme looses message confidentiality. Additionally, the scheme fails to resist insider attack and lacks user anonymity.

A Case Report of Acute Nicotine Poisoning from Subcutaneous Injection of Nicotine Solution for Electronic Cigarette (전자담배 용액의 피하 주사에 의한 급성 니코틴 중독 1례)

  • Nicotine-poisoning related to the electronic cigarettes (e-cigarette) is increasing worldwide. Moreover, the American Association of Poison Control Centers has advised the public to use caution with e-cigarette devices and highly concentrated liquid nicotine after a surge in related poisonings. We report here the first case of nicotine poisoning from self-injected e-cigarette fluid in Korea. A 17-year-old male patient subcutaneously injected himself with 0.5 ml of nicotine solution for an electronic cigarette via the dorsum of his hand, after which he complained of nausea, vomiting, dizziness and dyspnea. His vital signs were within the normal range, but his mental status was drowsy. He was admitted for observation and the symptoms disappeared the following day. Sinus bradycardia with a rate of 45/min was observed on the third hospital day, but improved after 6 hours. He was discharged without complications.

The Effects of Hand Massage in Patients Who Underwent Transradial Percutaneous Coronary Intervention (요골동맥을 통한 경피적 관상동맥 중재술 환자의 손마사지 효과)

  • Purpose: This study aimed to determine the effect of hand massage in patients who underwent transradial percutaneous coronary intervention. Methods: This was a quasi-experimental study with a nonequivalent control group and non-synchronized design. The study included 30 patients in the experimental group and 30 in the control group. Hand massage was performed 2 times for 5 minutes each in the experimental group and the control group only received usual nursing interventions. Pain, level of discomfort, and vital signs were defined as key outcome measures, and the data were analyzed using the chi-square test, an independent t-test, Mann-Whitney U test, repeated-measures analysis of variance, and Friedman test. Results: Significant differences were observed between the 2 groups in the pain score (F=7.91, p=.003), discomfort score (F=18.15, p<.001), pulse (F=12.92, p<.001), and respiration rate (${\times}^2=19.35$, p<.001). Conclusion: Hand massage can be a helpful nursing intervention for transradial percutaneous coronary intervention by reducing pain and discomfort to a considerable degree.

Study on Intravenous Drip Anesthesia of Ketamine Hydrochloride in Dogs (개에 있어서 Ketamine Hydrochloride의 정맥내 점적마취에 관한 연구)

  • This study was carried out to investigate the effects of intravenous drip with ketamine hydrochloride and its application for control depth and maintenance of anesthesia in dogs. Changes of blood pressure, vital signs, blood gas and anesthetic state were observed in this study. The obtained were summerized as follows ; 1. Changes of blood pressure and heart rate after intravenous drip anesthesia with ketamine hydrochloride were observed with significant increase in all group ; group II (0.135m81k9/min), group III (0.269mg/kg/min) and group IV(0.538mg/kg/min). These conditions were maintained unchangeably until 160 minutes after administration in all group. This may be indicated that there were no side effects on account of ketamine accumulation. 2. There were irregular respiration, pain reflex, Jaw tone reflex and vomition probability in the anesthetic conditions of group II The anesthetic conditions of group III were rarely shown as mentioned above. Awakening time and recovery time of group H were more prolonged 21 minutes and 27 minutes respectively than those of group III. These experimental data suggested that the optimal dosage of intravenous drip anesthesia of ketamine Hcl was 0.269mg/kg/min.

Three Adverse Events Reports of Simple Exanthematous Eruption after Sumsu (Bufonis Venenum) Pharmacopuncture Topical Treatment (섬수약침 국소시술 이후 발생한 단순 피부 발진: 3 이상사례 보고)

  • Three cases of simple exanthematous eruption were suspected during Sumsu (Bufonis Venenum) pharmacopuncture (SP) topical anesthesia for acupotomy. Patients had skin rash with pruritus on both ankle, posterior neck, and left shoulder after 11, 12, and 7 times of SP treatment, respectively. There were no cases of systemic manifestations or changes in vital signs. As a result of using the World Health Organization-Uppsala Monitoring Centre (WHO-UMC) causality assessment, all the cases were evaluated as 'unlikely'. However, the results of using the Korean algorithm for assessing the causality of drug adverse reactions version 2.0 were evaluated as 'possible'. This report is the first case report on adverse events suspected of occurring after SP treatment. Although the causal relationship between suspected intervention and the adverse event is not clear, there was a difficulty in completely excluding the possibility. Additional safety studies will be required to make SP more widely available.

  • In recent years, reductions in infant mortality have mainly been accomplished by improving the survival of premature and low birth weight infants, however premature infants still remain at great risk. The purpose of this study was to review the maternal child health service related to premature infants and to provide a future direction for improving maternal child health (MCH) in Korea. We reviewed two MCH services which are directly related to premature infants: 1) a registry and financial support program for families with a premature infant, and 2) financial support to build neonatal intensive care units in rural public hospitals. Suggestions are made for the development of a national vital signs record system to identify high risk infants and to monitor the trends in infant mortality due to prematurity. Prevention efforts and preconception care for childbearing women is also an important strategy to reduce the rate of preterm births. Finally, we need consider long-term follow-up plans for premature infants for a successful transit to the special education system. Developing MCH policy related to premature infants that decreases the occurrence of premature may decrease infant mortality, and also improve maternal and child health services.

Submasseteric Abscess (깨물근하 농양)

  • Purpose: The masserteric space is an important tissue compartment of the face, but a disease in it is difficult to diagnose and treat. The submasseteric abscess is located between the masseter muscle and mandibular ramus with different appearances such as sepsis, infection, or tumor. Especially the common misdiagnosis of submasseteric abscess is acute or chronic parotitis. The purpose of this report is to pay special attention to the possible diagnosis of submasseteric abscess for the symptoms of unilateral cheek swelling and tenderness that accompany marked trismus. Methods: A 11-year-old boy came to our hospital because of facial swelling, tenderness, and trismus in a history of left cheek swelling and toothache. We diagnosed his case as submasseteric abscess by CT scan and surgical intervention was performed. Under general anesthesia, the abscess was opened by the intraoral incision and considerably massive pus was drained. Results: Swelling, tenderness, and trismus became to subside during postoperative 10 days and general condition and vital signs became stable. After 6 months, CT scan showed that both masseteric muscles were symmetric and there was no periosteal reaction of the mandible. Conclusion: In conclusion, submasseteric abscess is a rare infection with the symptoms of cheek tenderness and marked trismus. A detailed medical history and clinical examination of a patient as well as computed tomography(CT) are important tools in the accurate diagnosis and efficient treatment of the submasseteric abscess. Adequate drainage, removal of cause, and antibiotic infusion are the management of choice.

  • Purpose: Assessment of the trauma severity associated with the prognosis of trauma patients. But we are having a lot of difficulties in assess the severity because of scarcity of current first-aid records resources. Methods: We presumed that Applying the Revised trauma score which consist of vital signs and GCS score will be helpful to assess the sevirity.This study covers the 10069 patient of Ewah womans hospital (2011.1.1.-2014.12.31) who are able to verify the GCS score from fist-aid records. Results: There is no distinctions between prehospital RTS and hospital RTS. And shows high level of correlation between prehospital RTS and ISS. Conclusion: Therefore we conclude that checking the GCS and RTS at prehospital state will be help to assess the severity of trauma patients.

A Case of Transpalpebral Enucleation in Shih-tzu with Chronic Unilateral Glaucoma (편측성 만성 녹내장을 지닌 Shih-tzu의 안구적출술 치유예)

  • A 4-year-old female Shih-tzu with severe episcleral congestion and buphthalmos in left eye was referred to the Veterinary Teaching Hospital, Chonnam National University in October 1998. During two months in local veterinary clinics, the dog had been treated unsuccessfully with tarsorrhaphy and antibiotics. By history taking, cause of the glaucoma was ascertained as accidental nonpenetrated cornea trauma by owner, Ophthalmic examination revealed that the left eye was buphthalmic with severe episcleral congestion, pain and fever. The direct pupillary light response could not be evaluated in the left eye, but right eye had normal reaction. The intraocular pressure was 4.5 mmHg in the right eye and 33 mmHg in the left eye, as measured with a Schiotz tonometer, Ultrasonographic finding revealed hyperechoic vitrous body and central echofree material. The Schirmer tear test showed 20 mm in the left eye and 8 mm in the right eye. The hematologic examination revealed monocytosis. The initial medical treatment was admitted, but there was no response. Transpalpebral enucleation in the left eye under inhalation anesthesia with enflurane 1.2 vol% in oxygen was performed. The day after operation the left eye ravealed no exudate, and pain, and the systemic vital signs were normal. The prognosis is excellent.
