한국 후천적 시각장애인의 우울과 영향요인에 관한 연구 (Examining depressive symptoms of visually impaired adults in Korea)

  • 본 연구의 목적은 후천적 시각장애인에 대한 우울 실태와 이들의 우울에 영향을 미치는 요인을 규명하는 것으로 이를 위해 본 연구에서는 장애인실태조사 2014 자료를 활용하여 분석을 실시하였다. 연구대상자는 출생 1년 이후부터 시각장애를 가지게 된 20~60세 사이의 성인이며 최종 분석에 포함된 대상자 수는 235명이다. 다중로지스틱회귀분석 실시 결과, 후천적 시각장애인의 약 20%가 우울증상을 경험한 것으로 나타났으며, 우울증상 경험을 예측하는 요인으로는 인구사회학적 요인 중에서는 연령, 성별, 결혼여부가 유의한 요인으로 나타났으며 건강 관련 요인으로는 흡연여부, 일상생활 스트레스 수준, 주관적 건강상태가 유의한 요인으로 파악되었다. 장애관련 요인으로는 장애기간 및 일상생활 수행능력 수준이 유의한 예측요인으로 밝혀졌다. 종합적으로 본 결과는 국내 거주하는 후천적 성인 시각장애인의 많은 수가 우울과 같은 심리적 어려움에 노출되어 있으며, 이를 위한 예방 및 개입 방안 마련이 시급함을 알려준다. 아직까지 장애인 복지 및 장애인 대상 서비스 분야에서 이들에 대한 관심 및 서비스 마련이 미흡한만큼 추후 중도에 실명 혹은 시각손실을 경험하게 된 장애인에 대한 제도적인 고려가 요구된다.

독서보조기기를 이용한 시각장애인 대체자료 이용 활성화 방안 연구 (A Study on the Utilization of Alternative Materials for the Visually Impaired Using Reading Aids Devices)

  • 본 연구는 시각장애인들의 독서보조기기를 이용한 정보접근에서 발생하는 한계와 문제점 등을 확인하고, 이를 기반으로 독서보조기기를 이용한 대체자료 이용 활성화 방안을 제안하는데 주된 목적을 두고 있다. 이를 위해 시각장애인 성인을 대상으로 설문조사를 수행하였으며, 설문 분석 결과를 통해 독서보조기기의 이용을 활성화하는데 있어서의 제한요인을 여섯 가지 측면으로 도출하였다. 도출한 제한요인을 중심으로 독서보조기기 이용 활성화를 위한 방안을 정책적, 운영적 측면으로 구분하여 제안하였다. 정책적 측면에서는 저가의 독서보조기기 개발, 독서보조기기 지원 정책에 대한 인지도 제고, 독서보조기기 적용성 향상을 위한 웹 표준 지침 준수 방안을 제안하였으며, 운영적 측면에서는 시각 이외에 청각 활용 기능의 보강, 독서보조기기 활용 관련 정보서비스 확충, 독서보조기기의 휴대성 강화 등을 제안하였다.

재가 방문구강관리 중재에 의한 시청각장애 노인의 구강건강상태 변화 (Changes in the oral health status of older adults with visual and hearing impairments through home oral health care intervention: a case report)

  • Objectives: This case study was conducted to assess the changes in the oral health status of older individuals with hearing and visual impairments through home oral health care based on community care. Methods: The participants were two older adults with hearing and visual impairments. Through home visits, an oral health intervention program, including oral hygiene care and training on strengthening of oral function, was conducted once a week for 5 months. Dental hygienists performed special oral health interventions such as dental plaque control through individual tooth brushing and interdental care, training on strengthening of intraoral and extraoral muscle function, and denture care for the individuals with visual-hearing impairments. Results: The overall periodontal health status and oral muscle function improved in older adults with hearing and visual impairments. In the case of the visually impaired individuals, changes in the oral health status were oral mucosal moisture (30.1 and 37.2 points before and after intervention, respectively), salivary secretion (3.5 and 4.0 cm before and after intervention, respectively), and maximum tongue pressure (20.5 and 26.2 kPa before and after intervention, respectively). Changes in the oral health status of the hearing impaired individuals increased from 28.3 points before the intervention to 38.4 points after the intervention, and the maximum tongue pressure increased from 1.85 kPa to 23.5 kPa after the intervention. Conclusions: Oral health intervention activities contributed to improving the periodontal health and oral function of older adults with hearing and visual impairments. To improve their overall and oral health, it is necessary to prepare measures to activate customized oral health intervention programs.

The State of Housing of the Elderly with Vision Impaired and the Its Impacts on Quality of Life

  • People have experienced physical problems including vision problems with aging; some are normal and some are abnormal. When they have abnormal and impaired vision, they have a lower level of life satisfaction than their sighted peers. For the elderly with sight loss, well-designed housing could be an important indicator for their quality of life. Housing adjustment and modification could help the elderly cope with sight loss, do daily activities more independently, and lead to better quality of life. The purpose of this study is to investigate housing conditions for the elderly with vision impaired, state of housing adjustment or modifications, and the effects of housing conditions on the wellness of the elderly in Korea. For this study, 18 housing cases were investigated and a total number of 65 visually impaired people aged over 55 participated. Housing condition of those participated was poor. Most observed home hazards are level differences in living areas, obstacles in the hallway, confined space that is not allowed structural modification, swing doors from wardrobe or cabinets, color contrast, poor lightings, and etc. Majority of households participated in this study did not make modification or refurbishments, rather many of them tend to adapt themselves to their residential environments. Older adults living in better housing condition and under less difficult environments are likely to have higher life satisfaction. The effects of housing condition on wellness could be more influential when individual capabilities (e.g. health condition) are weaker. The findings underline that with intervention of individual characteristics, the effects of housing and physical environment on negative outcomes of vision loss would get ameliorated or reduced.

Association between Visual Impairment and Nutritional Risk among Older Adults with Diabetes: A Population-Based Cross-Sectional Study

  • Purpose: Despite the high prevalence of visual impairment caused by diabetic retinopathy and nutritional problems among older adults with diabetes, evidence regarding factors related to nutritional risk in this population is limited. Therefore, this study aimed to identify the correlates of nutritional risk among older adults with diabetes, focusing on visual impairment. Methods: This study was a secondary data analysis of the 2020 National Survey of Older Koreans aged 65 years and above. The sample comprised 2,376 older adults with diabetes, and complex sample ANOVA and Rao-Scott chi-square tests were used to compare the groups according to visual impairment. Complex-sample logistic regression analyses were conducted to verify the association between visual impairment and nutritional risk. Results: Older adults with diabetes, who also have severe visual impairment, are more likely to have nutritional risk status than those without impairment after controlling for covariates (odds ratio [OR] = 2.44, 95% confidence interval [CI] 1.16~5.13). Among the covariates, depression (OR = 3.58, 95% CI 2.60~4.94), dependent activities of daily living status (OR = 2.79, 95% CI 1.60~4.86), and experience of hospitalization during the past year (OR = 2.51, 95% CI 1.57~4.03) were strongly associated with nutritional risk. Conclusion: Severe visual impairment increases the nutritional risk among older adults with diabetes. Therefore, it is essential to prevent visual impairment due to exacerbation of diabetes through appropriate management. Additionally, tailored nutritional interventions for visually impaired older adults with diabetes that consider visual characteristics are required.
