Case Study of Application on ADIM of Patient With Chronic Low Back Pain for Contraction Training of Transversus Abdominis (만성요통환자의 복횡근 수축훈련을 위한 ADIM 적용사례)

  • Background: The purpose of this study was to identify the effects of the abdominal drawing-in maneuver (ADIM) for contraction training of transversus abdominis (TrA) to the patient with chronic low back pain (CLBP). Methods: A 37-year female patient with CLBP participated in study. This study was fix to method of ADIM that maintain to draw in lower abdomen in sitting position on chair and fix 1 time protocol that perform rest for 1-min and ADIM for 1-min during 10-min. Hereunder the subject performed 2 times protocol during 30-min. Measurement method were visual analogue scale (VAS), Korean version of Oswestry disability index (KODI) and using ultrasound imaging. Results: VAS was decreased 5cm to 1cm after intervention 3 weeks, KODI was decreased 22% to 9% after intervention 3 weeks and changes in thickness of the TrA were increased 32% to 68% during ADIM. Conclusions: These findings suggest that performed the ADIM took effect for decreased pain and functional disability level with increased changes in thickness of the TrA. Therefore, patients with CLBP should be use for training of deep abdominal muscles apply to the ADIM.

Effect of Dangguisoo-san plus Yuhyangjeongtong-san (Dangguixu-san plus Ruxiangdingtong-san) in the Traffic Accidents Patients with Night Pain (당귀수산합유향정통산(當歸鬚散合乳香定痛散)이 야간통을 동반한 교통사고 환자에게 미치는 영향)

  • Objectives This study investigated the effect of korean medicine treatment with Dangguisoo-san plus Yuhyangjeongtong-san (Dangguixu-san plus Ruxiangdingtong-san) on traffic accidents patients with night pain. Methods 77 traffic accidents patients with night pain were treated with Dangguisoo-san plus Yuhyangjeongtong-san (Dangguixu-san plus Ruxiangdingtong-san), acupuncture, cupping, physical therapy. VAS (Visual Analogue Scale), NDI (Neck Disability Index), ODI (Oswestry disability index), total sleep time, the number of bed time arousal were checked to evaluate the effectiveness of the treatment. Results VAS, NDI, ODI of patients treated with Dangguisoo-san plus Yuhyangjeongtong-san (Dangguixu-san plus Ruxiangdingtong-san) were all improved and there were statistical significance (p<0.05). Total sleep time, the number of bed time arousal of patients treated with Dangguisoo-san plus Yuhyangjeongtong-san (Dangguixu-san plus Ruxiangdingtong-san) were all improved and there were statistical significance (p<0.05). Conclusions According to the study, Korean complex therapy with Dangguisoo-san plus Yuhyangjeongtong-san (Dangguixu-san plus Ruxiangdingtong-san) might effective in traffic accidents patients with night pain.

How to Effects of Manual Therapy : Chronic Low Back Pain Patients (도수치료가 만성요통환자의 기능회복에 미치는 영향)

  • Purpose : To evaluate effects of joint mobilization on the range of motion of chronic low back pain patients. Methods : The subjects were consisted of sixty patients who had non specific chronic low back pain(10 females. 10 males; mean aged 36.5). All subjects received modalities treatment with therapeutic massage for 10minutes and joint mobilization or manipulation for 10minutes per day and three times a week during 4 weeks period. The Multilevel Roland-Morris Disability Questionnaire(MR-MDQ) was used to measure functional disability level. Visual Analogue Scale(VAS) was used to measure subjective pain level. Remodified Schober test(RST) was used to measure forward flexion range of motion of lumbar segment. Finger-to-Floor test(F-T-FT) was used to measure forward flexion range of motion of full spine of low back pain patients. All measurements of each patients were measured at pre-treatment and 4 week post-treatment. Results : The MR-MDQ, VAS, RCT and F-T-FT were significantly different within-subjects(p<.05), Conclusion : The manual therapy included therapeutic massage and joint mobilization found that improved chronic low back pain patients. Further studies are needed to including more subjects on long-term outcomes.

The Effects of Stretching and Isometric Exercise for Chronic Neck Pain Patient in Strength and Pain (스트레칭과 등척성 운동이 만성 경부통증환자의 근력과 통증에 미치는 영향)

  • Purpose : The purpose of this study was to assess more effective exercise prescription for the people with chronic neck pain by comparing stretching exercise and isometric exercise about strength and pain. Method : The research design of the study was that 20 young adults with mild neck disability (5-14 out of 50 in Neck Disability Index) were randomized into 2 groups and underwent stretching exercise(10 peoples) and isometric exercise(10 peoples) at 3 times a week for 5weeks. Measures of pain scale (Visual Analog Scale) and Muscle strength of deep neck flexor (Pressure Biofeedback) were assessed before and after intervention. The pre and post exercise results were compared using paired t-test and the effectiveness of intervention of two groups were compared using Independent t test. Results : The results of this study were that pain reduction was seen after stretching and isometric exercise and significantly difference. There was significantly improvement of cervical flexor strength after stretching and isometric exercise. But, there was no significant difference between stretching and isometric exercise groups. Conclusion : The stretching and isometric exercise for chronic neck pain patients may use according to preference of patient because the effects of treatment was same. Further studies are needed to analyzed psycho social factors, cost effects, group approach.

Effects of Lumbar Stabilization Exercise with Kinesio Taping on Pain, Muscle Strength, and Oswestry Disability Index in Patients with Chronic Low Back Pain

  • Objective: The purpose of this study was to examine the effects lumbar stabilization exercise with kinesio taping on pain, muscle strength, and oswestry disability index (ODI) in patients with chronic low back pain. Design: Two groups pre-post randomized controlled design Methods: Thirty-two subjects were randomly divided in two groups; 1) lumbar stabilization exercise with kinesio taping group (Experimental group, n=16), 2) lumbar stabilization exercise with sham kinesio taping group (Control group, n=16). The intervention was conducted in each group for thirty minutes a day, 5 times a week, for 4 weeks. Both group did 30 minutes of lumbar stabilization application. Evaluations were performed before the commencement of training and again 4 weeks after training was initiated. Visual analog scale (VAS) was used to evaluate pain level of patients with chronic low back pain. Distal muscle test was used to evaluate muscle strength of trunk extension. In addition, ODI was used to evaluate activity daily life of low back pain. Results: After training, the VAS, muscle strength of trunk extension and ODI were significantly more improvement in Experimental group than in the control group (p<0.05). Conclusions: We confirmed that the effects of lumbar stabilization exercise with kinesio taping group on pain, muscle strength, and ODI in patients with chronic low back pain.

Effects of Cervical traction and Muscle Energy Technique on Pain, Neck Disability Index, Function, Range of Motion in Patients with Cervical Radiculopathy (목 견인과 근 에너지 기법이 목 신경뿌리병증 환자의 통증, 목 기능장애지수, 관절가동범위에 미치는 영향)

  • Background: This study examined the effects of the muscle energy technique and cervical traction after applying conservative physical therapy in patients with cervical radiculopathy. Methods: Patients were randomly divided into two groups muscle energy technique (8 subjects) and cervical traction (8 subjects). Each group performed their exercise 70 minutes per day, three times week for four weeks. Pain intensity was measured with a visual analogue scale (VAS). Function was measured with the neck disability index (NDI). Cervical range of motion (ROM) was measured with a cervical range of motion (CROM) goniometer. Results: After four weeks of therapy, VAS (p<.05) and NDI (p<.05) significantly decreased, and ROM significantly increased in both groups (p<.05). There were also significant differences between the two groups for these three measures (p<.05). Conclusion: The muscle energy technique and cervical traction are more effective than cervical traction alone in reducing VAS and NDI and increasing ROM in patients with cervical radiculopathy.

Effect of Lower Trapezius Strengthening Exercise Program on Unstable Support Surface for Neck Pain Patients with Forward Head Posture: A Randomized Controlled Trial

  • Objective: The purpose of this study was to investigate the effect of a lower trapezius strengthening exercise program on an unstable support surface on pain, neck dysfunction, psychosocial factors, and postural alignment in neck pain patients with forward head posture. Design: A randomized controlled trial Methods: A total of 36 neck pain patients participated in this study. Screening tests were performed and assigned to experimental group (n=18) and control group (n=18) using randomization program. Both groups performed the lower trapezius strengthening exercise program. In addition, in the experimental group, an exercise program for lower trapezius muscle strengthening was performed on an unstable support surface. All interventions were performed 3 times a week, for a total of 5 weeks. quadruple visual analogue scale (QVAS), neck disability index (NDI), short form (SF)-12, and postural alignment were measured before and after the intervention to compare their effectiveness. Results: Both groups showed significant differences in QVAS, NDI, SF-12, and postural alignment before and after intervention (p<0.05). In addition, the experimental group showed significant differences in NDI and postural alignment compared to the control group (p<0.05). Conclusions: The lower trapezius strengthening exercise program on unstable support surfaces is an effective intervention method with clinical significance in improving neck disability and postural alignment in patients with neck pain with forward head posture.

The Effect of Myofascial Release and Cervical Traction on Pain, Range of Motion and the Neck Disability Index in Patients with Chronic Neck Pain: A Randomized Controlled Trial (근막이완술과 목 견인이 만성 목 통증 환자의 통증, 관절가동범위, 목 기능 장애지수에 미치는 영향: 무작위 대조 연구 )

  • PURPOSE: This study examined the effects of the myofascial release and cervical traction after applying conservative physical therapy to patients chronic neck pain. METHODS: Patients were randomly divided into two groups, namely myofascial release (7 subjects) and cervical traction (7 subjects). Each group performed their therapy 45 minutes per day, two times a week, for four weeks. Pain intensity was measured using the visual analog scale (VAS). Function was measured with the neck disability index (NDI). The cervical range of motion (CROM) was measured with a cervical range of motion (CROM) goniometer. RESULTS: After four weeks of therapy, the VAS (p < .05) and NDI (p < .05) significantly decreased, and ROM significantly increased in both groups (p < .05). There were also significant differences between the two groups for these three measures, except for neck flexion and neck extension(p<.05). CONCLUSION: Myofascial release and cervical traction are more effective than cervical traction alone for reducing VAS and NDI and increasing ROM in patients with chronic neck pain.

The Study on Functional State, Self Efficacy, and Life Satisfaction in the Elderly with Decreased Visual Acuity (시력저하노인의 기능상태, 자기효능감, 삶의 만족에 관한 연구)

  • Purpose: This purpose of study was to investigate the relationships among functional state, self-efficacy, and life satisfaction in the elderly with decreased visual acuity. Methods: The subjects were 162 elderly people from the G university hospital. Functional state was measured by Late-Life Function and Disability Instrument (LLFDI) and Minimum Data Set-Home Care version 2.0 (MDS HC 2.0). Self-efficacy and Life satisfaction were measured by the tool of Rho & Lee (2011) and Yoon (2007). Data were analyzed using t-test, ANOVA, Pearson's Correlation Coefficient, and logistic regression. Results: The daily life function was significantly associated with self-efficacy and vision decrease. The regression model with these two variables explained 35.6% of the variance of daily life function. IADL was significantly associated with vision decrease, age, gender, and self-efficacy. The regression model with the three variables explained 52.9% of the variance of IADL. Life satisfaction is significantly associated with self-efficacy, daily life function, vision decrease and IADL. The last regression model with the four variables explained 51.8% of the variance of life satisfaction. Conclusion: The levels of functional state, self-efficacy and life satisfaction in the elderly with decreased visual acuity were low. Self-efficacy was an important factor that influences on the functional state and life satisfaction. Therefore, nursing interventions that can enhance the self-efficacy are required in order to increase the functional state and life satisfaction in the elderly with decreased visual acuity.

The Development and Evaluation of a Motivation-Strengthening Obesity Management Program for Obese Subjects with Visual Impairment (시각장애 비만대상자를 위한 동기강화 비만관리프로그램의 개발 및 효과평가)

  • Purpose: The purpose of this study was to develop and apply a motivation-strengthening obesity management program for obese subjects with visual impairment and evaluate its effects. Methods: The program development and evaluation periods were from March to August 2018. The study design was an experimental study of non-equality, control repeat design. 25 subjects in the experimental group participated in the 12-week obesity management program and the 26 people in the control group did not. Results: The motivation-strengthening obesity management program included stretching exercises for 30 minutes in the morning five times a week. In addition, counseling, education, and group discussions were continued once a week for motivational strengthening. Special custom exercises for blind patients were also completed once a week simultaneously. Both of these activities were continued for 50 minutes a week for a total of 12 weeks. Measurements were recorded periodically to identify the sustained effects of the program. The obese subjects with visual impairment improved their knowledge of obesity, perceived disability, self-efficacy of exercise, and quality of life related to obesity through the motivational obesity management program. Waist circumference, systolic blood pressure, diastolic blood pressure, and fasting blood sugar were also improved in obese subjects. Conclusion: The study findings indicate that this program could be an active intervention for the control of weight gain in participants with visual impairment. Therefore, the motivation-strengthening program based on the ADDIE (Analysis, Design, Development, Implementation& Evaluation) model could be an effective strategy for better health outcomes of obese subjects with visual impairment.
