THE STATUS OF ORAL HEALTH BEHAVIORS AND DENTAL SERVICES IN THE KOREAN DISABLED (전국 시설이용 장애인의 구강보건의식행태에 관한 조사연구)

  • The purposes of this study were 1) to investigate the oral health problem of the disabled according to the handicapped types, 2) to collect the empirical data for developing and establishing the oral health policies for the handicapped, and 3) to find out the major obstacles against the dental services and oral health promotion. The handicapped subjects were collected by designed cluster sampling and interviewed with structured questionnaire in order to measure the oral health knowledge and behaviors. The subjects were attending the special schools and private or public welfare institutions for crippled disorder, encephalopathy disorder, mental retardation, visual disturbance, hearing defect, and developmental disorder. Oral examination was conducted by 13 trained dentists. 1,476 of handicapped people were finally surveyed. The results were as follows; The experiences of visiting dental office during the last one year in 18~64 year-old disabled people were 48.86% in crippled disorder, 52.50% in mental retardation, 58.24% in visual disturbance, 39.29% in hearing defect, respectively. To improve this challenging situation, we should find out the obstacles against the dental service and oral hygiene maintenance by the types of handicap, and develop the oral health policies which could support and advocate the Korean disabled.

Effects of Extracorporeal Shock Wave Therapy with Myofascial Release Techniques on Pain, Movement, and Function in Patients with Myofascial Pain Syndrome (근막통증 증후군 환자에게 체외충격파와 근막이완술 병행 치료가 통증, 움직임, 기능에 미치는 영향)

  • Purpose: The study investigated the effects of extracorporeal shock wave therapy with myofascial release techniques (ESWT+MFR) on pain, movement, and function in patients with myofascial pain syndrome. Methods: Forty participants with upper trapezius trigger points were recruited and randomly allocated to two groups: an experimental group (n = 20) and a control group (n = 20). The experimental group performed the ESWT+MFR, and the control group performed only myofascial release techniques. Each group was treated for 15 minutes, twice a week for four weeks. Pain was assessed using a visual analogue scale and a pressure pain threshold measure. Movement was assessed by cervical range of motion, and cervical and shoulder function were assessed on the Constant-Murley Scale and the Neck Disability Index before and after treatment. Results: The results indicate statistically significant improvements in the two groups on all parameters after intervention as compared to baseline (p < 0.05). As compared to the control group, the experimental group showed statistically significant improvements on the visual analogue scale and pressure pain threshold, cervical range of motion (except rotation), and on the Neck Disability Index (p < 0.05). Conclusion: The ESWT+MFR is more effective than myofascial release techniques for pain, movement, and function in patients with myofascial pain syndrome and would be clinically useful for physical therapists treating myofascial pain syndrome.

Effects of the Embedding Acupuncture Treatments for Chronic Low Back Pain Patients (척추세움근 매선침치료가 만성요통환자에 미치는 효과)

  • Objectives To investigate clinical effects of needle embedding acupuncture treatments for chronic low back pain patients. Methods 30 patients with chronic low back pain were recruited and randomized into two groups-the embedding acupuncture group or the placebo. At baseline, the age, height, weight, visual analogue scale (VAS), Oswestry disability index (ODI) scores were measured. And surface electromyography (SEMG) data of both erector spinae at L2, L4 level were also measured on both groups and asymmetry index (AI) were calculated. The embedding or placebo acupuncture treatment was performed on the erector spinae according to SEMG values; immediately after the first evaluation and 48 hours after the first visit. After 96 hours of intervention, the VAS, ODI score and SEMG of both erector spinae were measured again. Statistical significance was determined using the Wilcoxon signed ranks test or the Wilcoxon rank sum test. Results The mean VAS, ODI score after treatment was decreased significantly compared with baseline on both groups. And the VAS, ODI score and AI of the embedding acupuncture group was more decreased significantly than the placebo (p<0.05). Conclusions The results suggest that embedding acupuncture for chronic low back pain patients was effective on the VAS pain score, ODI score and AI of the erector spinae.

The Clinical Studies for Non Surgical Spinal Decompression Treatment on Cervical Disc Herniation (경추추간판 탈출증환자의 감압치료 병행효과에 대한 임상적 연구)

  • Objectives: The purpose of this study is to make a survey of the effectiveness of the non surgical spinal decompression treatment in patients with cervical disc herniation documented on magnetic resonance imaging(MRI). Methods: We studied about 27 cases of cervical disc herniation which were treated with non surgical spinal decompression and other medical treatment (acupuncture, interferential current therapy and ice poultice). Each patient has been treated with spine decompression and other medical treatment. And degree of improvement has been evaluated by short form-McGill pain questionnaire(SF-MPQ), visual analogue scale(VAS) and neck disability index(NDI) before treatment and 5th, 10th times after treatments. Results: 1. The VAS SF-MPQ and NDI scores showed statistically significant improvement after 5th and 10th treatment. 2. The VAS, SF-MPQ and NDI scores after 5th treatment showed better improvement than those of after 10th treatment. 3. The VAS, SF-MPQ and NDI scores in Subacute stage showed more statistically significant improvement among the other stages. Conclusions: As a result, non surgical spinal decompression treatment has clinical effects of pain reduction on patient with cervical disc herniation.

Effects of Pulsed Electromagnetic Therapy with Acupuncture Therapy for Sacroiliac Joint Syndrome (천장관절 증후군에 박동성 전기자장 치료와 병행한 침치료의 효과 연구)

  • Objectives : Although the controversy surrounding the biomechanics of the sacroiliac joint remains unresolved at this time, the clinical importance of this joint in the cause of back pain has been established since 1930's. Recently, there has been renewed interest in the sacroiliac joint. This study was performed to evaluate the effects of pulsed electromagnetic therapy(PEMT) with acupuncture therapy for patients, who were suffering from sacroiliac joint syndrome, and to conduct more researches in the usage of acupuncture therapy for treating sacroiliac joint syndrome. Methods : 25 patients, who were diagnosed as sacroiliac joint syndrome were selected. They were treated twice a week during 3 weeks. They were measured after all the treatment and firs week and fourth week after termination of treatment by using visual analogue scale(VAS) and Roland Morris disability index(RMDI). The VAS and RMDI patterns were analyzed by using 'pared T-test' and 'Kruskal-Wallis' test. Results : 1. Each times of PEMT with acupuncture therapy, there were statistical significance in improvement of VAS(p<0.05) and each times of therapy except 1st one, there were statistical significance in improvement of RMDI(p<0.05). 2. After 4th therapy, there were most significant improvement of VAS with RMDI(p<0.001), when we compared the change in VAS and RMDI before and after the each therapy. 3. There was no statistical significance in VAS and RMDI by onset, sex and age. Conclusions : The results indicate that pulsed electromagnetic therapy and acupuncture therapy had good effect on sacroiliac joint syndrome.

  • This paper was studied on developmental direction of telecommunications for disabilities welfare. Method of this study is delphi method. Persons with disabilities is classed as motor disability, visual handicap, hearing impairment, and language and speech disorders. Persons with motor disability is needs as follow, speed recognition technology, video recognition technology, breath capacity recognition technology. Persons with visual handicap is needs as follow, display recognition technology, speed recognition technology, text recognition technology, intelligence conversion handling technology, video recognition - speed synthetic technology. Persons with hearing impairment and language - speech disorders is needs as follow, speed signal handling technology, speed recognition technology, intelligence conversion handling technology, video recognition technology, speed synthetic technology the results of this study is as follow: first, disabilities telecommunications organization must be constructed. Second, persons with disabilities in need of universal service. Third, Persons with disabilities in need of information education, Fourth, studying for telecommunications in need of support. Fifth, small telecommunications company in need of support. Sixth, software industry in need of new development. Seventh, Persons with disabilities in need of standard guideline for telecommunications.

Comparison of Ultrasound Guided Soyeom Pharmacopuncture Therapy Effect and Unguided Soyeom Pharmacopuncture Therapy Effect on Cervical Facet Joint of Acute Cervical Pain Patient Caused by Traffic Accidents: A Retrospective Study (교통사고로 유발된 급성 경추통 환자의 경추 후관절에 시행한 초음파 유도하 치료군과 비유도 치료군의 소염 약침 치료 효과 비교: 후향적 관찰 연구)

  • Objectives To compare the therapeutic effect of ultrasound guided soyeom pharmacopuncture therapy and unguided soyeom pharmacopuncture therapy on cervical facet joint of patient who have cervical pain caused by traffic accidents. Methods The study was planned as a retrospective study that analyzes the patient's medical records. We analyzed medical records of 20 patients with acute cervical pain caused by traffic accidents from April 1, 2022 to May 31, 2022. 10 patients (Group A) were treated ultrasound guided soyeom pharmacopuncture therapy on cervical facet joint, 10 patients (Group B) were treated unguided soyeom pharmacopuncture therapy on cervical facet joint. We used visual analogue scale (VAS) and neck disability index (NDI) to evaluate improvements in cervical functions and pain, five point Likert scale to estimate patient's satisfaction. We used the IBM SPSS Statistics version 25 program (IBM Co., Armonk, NY, USA) for statistical analysis. Results Group A showed a significant decrease in the VAS on the 5th day of the soyeom pharmacopuncture therapy (p=0.001). However, there was no statistically significant difference of VAS between two groups (p=0.142). Group A showed significant decrease in the NDI on the 5th day of the therapy (p=0.002). Also, there was statistically significant difference on NDI between two groups (p=0.018). Difference of five point Likert scale between two groups was not statistically significant (p=0.407). Conclusions In patients of acute cervical pain caused by traffic accidents, ultrasound guided soyeom pharmacopuncture therapy on cervical facet joint showed significant efficacy on reduction of neck disability compared with unguided therapy.

Effects of Equipment-Based Pilates Exercises on Visual Analogue Scale Scores, Oswestry Disability Index scores, and Core Muscle Thickness in Patients with Chronic Low Back Pain (기구 필라테스 운동이 만성 요통 환자의 통증 정도, 장애 지수, 복부근 두께에 미치는 영향)

  • Background: This study comparatively evaluated the effects of equipment-based pilates exercises (EPE) and lumbar stability exercises (LSE) in patients with chronic low back pain in terms of their Visual Analogue Scale(VAS), Oswestry Disability Index(ODI), and abdominal muscle thickness. Methods: A total of 30 participants were recruited and randomly assigned to either the EPE or the LSE. The VAS, ODI, and abdominal muscle thicknesses of the participants were measured before and after the intervention. Results: The EPE were more effective in terms of the duration of a sustained reduction in VAS scores. post hoc test revealed that EPE were more efficacious in terms of a sustained improvement in ODI scores. With respect to changes in abdominal muscle thickness, there was a significant difference in the thickness of internal oblique muscles(IO) and the external oblique muscles(EO) between the two groups. Conclusion: In this study, both types of exercise interventions resulted in improvements in the VAS, ODI scores, and abdominal muscle thickness in patients with chronic low back pain. However, EPE were found to be more effective than LPE in terms of longer sustained improvements in VAS and ODI scores. Also, with respect to abdominal muscle thickness, the thickness of EO and IO improved only in the EPE group.

EARLY CARIES DETECTION WITH DIGITAL IMAGING FIBER-OPTIC TRANS-ILLUMINATION (Digital Imaging Fiber-Optic Trans-Illumination을 이용한 초기우식의 탐지)

  • It's important that detect early caries of deciduous and permanent teeth to prevent dental caries and prevserve teeth, especially on proximal surface of deciduous teeth. The reason is that their prominent pulp horn lead to pulp treatment easily due to rapid caries progression. There are conventional exploring, visual inspection and radiographic exam for early caries detection. But, the standard method for diagnosing dental caries is subject and cavitation may be accelerated during exploring procedure. Caries can be diagnosed up to 40% mineral loss with radiograph. $DIFOTI^{(R)}$ (Digital Imaging Fiber-Optic TransIllumination) is diagnostic imaging system for early caries detection using fiber-optic illumination. It is possible that remineralize the tooth surface without tooth preparation and conserve the tooth structure by using $DIFOTI^{(R)}$.

Effects of Interferential Current Treatment on Pain, Functional Ability, and Health-Related Quality of Life in Chronic Stroke Patients with Lumbago; A Randomized Controlled Study

  • PURPOSE: This study examined the efficacy of an interferential current (IFC) treatment on the improvement of pain, disability, and quality of life in stroke patients with lumbago. METHODS: A double-blind, randomized clinical trial was conducted on 40 stroke patients with lumbago. The patients were allocated randomly into two groups: the IFC treatment group (n= 20) and the placebo treatment group (n= 20). The IFC group received 30 minutes of IFC treatment on the lumbar region, while the placebo group received IFC treatment but without real electrical stimulation. The intervention was administered five days a week for four weeks. The primary outcomes of pain intensity were measured using a visual analogue scale. The secondary measurements included the Barthel Index, Oswestry Disability Index (ODI), and health-related quality of life (HRQoL). RESULTS: The measurements were conducted before and after the two-week intervention period. Compared to the placebo treatment group, the IFC treatment group showed significantly greater improvement in the pain intensity (p<.05), ODI (p<.05), and SF-36 (p<.05) at the end of the intervention. No significant differences in the Barthel Index were found between the two groups. CONCLUSION: These findings show that an IFC treatment can improve pain, functional ability, and quality of life, highlighting the benefits of somatosensory stimulation from IFC in stroke patients with lumbago.
