Intramuscular epidermal cyst in the masticator space: a case report

  • An epidermal cyst, also known as an epidermoid cyst or epidermal inclusion cyst, is the most prevalent type of cutaneous cyst. This non-cancerous lesion can appear anywhere on the body, typically presenting as an asymptomatic dermal nodule with a visible central punctum. In the case presented herein, an epidermal cyst with uncommon features was misdiagnosed as a lymphatic malformation based on preoperative magnetic resonance imaging (MRI). A 61-year-old man came to us with a swollen left cheek that had been present for 11 months. The preoperative MRI revealed a 3×3.8×4.6 cm lobulated cystic lesion with thin rim enhancement in the left masticator space. The initial differential diagnosis pointed toward a lymphatic malformation. We proceeded with surgical excision of the lesion via an intraoral approach, and the specimen was sent to the pathology department. The pathological diagnosis revealed a ruptured epidermal cyst, indicating that the initial diagnosis of a lymphatic malformation based on preoperative MRI was incorrect. Epidermal cysts located under the muscle with no visible central punctum are uncommon, but should be considered if a patient presents with facial swelling.

  • Automatic control makes the air-handling unit go into operation and determines the functions of high-efficient and energy-saving machines. Yamatake, an automatic control system manufacturer, have expanded fault detection and diagnosis, and data volumes so as to achieve higher technology in control by developing a sensor which makes field data visible, an actuator and Intelligent Conponent. This study, thus, focuses on applications for saving energy with Intelligent Conponent and goes in for easing global warming by creating future field data-based applications.

On Definition of Dong(動) based on Literal Meaning, Literature written by Todo Yoshimasu and Case Reports (동(動)의 의미에 대한 고찰 - 사전적 의미, 문헌적 의미, 임상사례를 중심으로-)

  • Objective : The purpose of study is to define the term Dong(動) in diagnosis view. Method : To define term Dong(動), I refer to Korean, Chinese, Japanese dictionary and Todo Yoshimasu (吉益東洞)'s writing, Yakucho(藥徵) Houkyoku(方極). Next, I search for the diagnosis definition of Dong(動) in 11 case reports. Results : The diagnosis definitions of Dong(動) are two meanings. One is objective symptoms which are doctor can examine, trembling, movement, etc. The other is subjective symptoms which are patient's feeling, emotional problems, etc. It seperates 6 meanings concretely. Conclusions: (1) visible moving of a focus, (2) movement of a focus, (3) appearance of abnormal secretion, (4) abnormal change of focus, (5) feel like moving, (6) change of emotion beyond patient's control. 1~4 are objective symptoms which are doctor can examine, and 5~6 are subjective symptoms which are patient's feeling.

  • Orthopaedic medicine was developed and published by James Cyriax, a British Orthopaedic Physician on 1929. Orthopaedic medicine is concerned with the diagnosis and treatment of soft tissue lesions. These disorders affect a substantial proportion of all patients in general and in particular, physiotherapy and sports clinics. In broad terms these disorders em trace conditions. such as arthritis. rheumatism. fibrositis. backache. lumbago, sciatica, frozen shoulder, tennis elbow, strained wrist, sprains, aches, inflanmation and sports injuries generally. The soft Moving tissues share one thing in common - they are all radiotranslucent and the tissues in question are the joint capsule, the ligaments, the fasciae, dural sheath. These structures can cause pain but none of them is visible on the radiograph. Dr. Cyriax divided all soft tissues into two basic types : 1) Contractile tissue, 2) Inert or noncontractile tissue. The mechanism of diagnosis is tension applied manually. The physician subjects each tissue about the incriminated joint to tension in turn which they call 'Selective tension' with Cyriax's assessment, a more definitive diagnosis can be obtained and proper treatment can be implemented.

  • Criteria for insulating oil condition of pole transformer were suggested by applying the UV-Visible spectrophotometric method. Aging of insulating oil caused the change in absorbances of oil as well as the deterioration of electrical properties of oil. By comparing the electrical properties such as tan$\delta$ and dielectric breakdown voltage with the UV-Visible absorbances of oil, following criteria for absorbance values determining oil condition were established, fair condition is below 0.580, need attention is 0.580-0.900 and poor condition is above 0.900. This W-Visible spectrophotometric method may be applied in aging analysis of oil and expected to replace the current visual method. It is necessary to develop cheap, light and portable UV-Visible spectrophotometer in near future for field application.

  • In order to extend the life time of building and civil infra-structure, nowadays, patch type fibrous composite materials are widely used. Repaired concrete columns and beams gain the stiffness and strength, but they lose toughness and show brittle failure. Usually, the cracks of concrete structures are visible with naked eyes and the status of the structure in the life cycle is estimated with visible inspection. After repairing of the structure, crack visibility is blocked by repaired carbon sheets. Therefore, structural monitoring after repairing is indispensible and self diagnosis method with optical fiber sensor is very useful. In this paper, peel-out effects is detected with optical fiber sensors and the strain difference between main structure and repaired carbon sheets when they separate each other.

  • The facility diagnosis technique of past subjective judged the technical expert and interpreted stably mainly. The facility the periodical civil official becomes indispensability consequently with the law and the system and objectivity inspection which relates with a this and development of diagnosis technique were necessary. Currently the evaluation elements which relate with an inspection and diagnosis technique depended to the subjectivity of each facility quality, data statistics, diagnosis know-how, various circumstance and the technical expert and the evaluation item came to decide. But the evaluation score distribution of marks of each item depended plentifully in data analysis result and experience. The past method is weak in logically because the objection theoretical background and basis are insufficiency. Consequently in order to apply a data analysis result and an experience logically, it used AHP(Analytic Hierarchy Process) method from this research. It is visible with the fact that the application result reconfirmation and complement process will be necessary the technique which is suitable in the multi facility management. Also it is expected with the fact that the application characteristic from the next relation technique will be excellent.

  • Medical image segmentation, which is essential in diagnosis and 3D reconstruction, is performed manually in most applications to produce accurate results. However, manual segmentation requires lots of time to segment, and is difficult even for the same operator to reproduce the same segmentation results for a region. To overcome such limitations, we propose a convenient and accurate semiautomatic segmentation method. The proposed method initially receives several control points of an ROI(Region of Interest Region) from a human operator, and then finds a boundary composed of a minimum cost path connecting the control points, which is the Live-wire method. Next, the boundary is modified to overcome limitations of the Live-wire, such as a zig-zag boundary and erosion of an ROI. Finally, the region is segmented by SRG(Seeded Region Growing), where the modified boundary acts as a blockage to prevent leakage. The proposed User-steered balloon method can overcome not only the limitations of the Live-wire but also the leakage problem of the SRG. Segmentation results of thigh muscles of the Visible Human are presented.

Challenges in fibromyalgia diagnosis: from meaning of symptoms to fibromyalgia labeling

  • Fibromyalgia (FM) is a contested illness with ill-defined boundaries. There is no clearly defined cut-point that separates FM from non-FM. Diagnosis of FM has been faced with several challenges that occur, including patients' health care-seeking behavior, symptoms recognition, and FM labeling by physicians. This review focuses on important but less visible factors that have a profound influence on under- or over-diagnosis of FM. FM shows different phenotypes and disease expression in patients and even in one patient over time. Psychosocial and cultural factors seem to be a contemporary ferment in FM which play a major role in physician diagnosis even more than having severe symptom levels in FM patients. Although the FM criteria are the only current methods which can be used for classification of FM patients in surveys, research, and clinical settings, there are several key pieces missing in the fibromyalgia diagnostic puzzle, such as invalidation, psychosocial factors, and heterogeneous disease expression. Regarding the complex nature of FM, as well as the arbitrary and illusory constructs of the existing FM criteria, FM diagnosis frequently fails to provide a clinical diagnosis fit to reality. A physicians' judgment, obtained in real communicative environments with patients, beyond the existing constructional scores, seems the only reliable way for more valid diagnoses. It plays a pivotal role in the meaning and conceptualization of symptoms and psychosocial factors, making diagnoses and labeling of FM. It is better to see FM as a whole, not as a medical specialty or constructional scores.
