A Case of Vertigo Patient after Cerebellar infarction improved by Oriental medical Treatment (현훈(眩暈)을 주소(主訴)로 하는 소뇌경색(小腦梗塞) 환자의 한방치료 호전 1례)

  • Patients with cerebellar infarction often complain of many symptom-vertigo, gait disturbance, nausea, vomiting. Especially, Vertigo is the main symptom, and patients cannot achieve normal activity in daily life. When we prescribed Banhabakchulchunma-tang and Gastrodiae Rhizoma extract to a 61year-old female stroke patient with vertigo, we could observe that the symptom was improved rapidly after treatment. Therefore we suggest that Banhabakchulchunma-tang which increase Chunma in quantity is very useful for vertigo after stroke. Further case studies of herbal treatment of this ailment are needed.

Korean Medicine Interventions for Benign Paroxysmal Positional Vertigo: A Systematic Review of Clinical Studies Published in Korea (양성 돌발성 체위성 현훈(Benign Paroxysmal Positional Vertigo)에 관한 임상 문헌 고찰 - 국내에 출판된 한의약 임상 논문을 중심으로 -)

  • Objectives: This study provides evidence for clinical practices by analyzing present clinical Korean medicine articles about benign paroxysmal positional vertigo (BPPV). Methods: Articles published up to July, 2017 were found by searching "BPPV", "Benign positions", and "Vertigo" as keywords in three domestic electric databases (i.e., OASIS, NDSL, RISS). Results: The initial search resulted in 228 articles; however, only 20 articles satisfied the selection criteria and were analyzed. Of these, 40% were published in the Journal of Korean Oriental Internal Medicine, and 35% were published in 2003. When diagnosing BPPV patients, VAS scores are primarily used for clinical assessments rather than objective diagnostic tools. Banhabaekchulcheonmatang was the most frequent herbal medicine used to treat BPPV, and the most commonly used therapeutic acupuncture point was Jok-samni (ST36). The most common diagnosis of BPPV was dam-eum according to nine studies. Conclusions: Systematic and large-scale research for the standardization of BPPV treatment is needed.

  • Objective : This Study is attempted to analyze characteristics of personality by Minnesota Multiphasic Personality Inventory responses in Vertigo patients. Method : 24 Vertigo patients and 28 control group were tested by MMPI and QSCCII. It is analyzed by one-way ANOVA, chi-square test, t-test, Duncan method is used as ex-post test. Result & Conclusion : 1. 24 Vertigo patients were classified into three constitutions by QSCCII. There were 6 of Soyangin(25.0%), 8 of Soeumin(33.3%) and 10 of Taeumin(41.7%) 2. In the analysis of personality disorder scale of MMPI, BDL scale score of Soyangin group was higher than that of other groups, and STY, AVD scale score of Soeumin group was higher than that of other groups. 3. In the analysis of personality disorder scale of MMPI, vertigo patients group showed higher scores than control in CPS, PAG, PAG, DEP, STY, SZD scale.

Short-term Outcome of Benign Paroxysmal Positional Vertigo : Pilot Study (양성돌발성두위현훈의 단기적인 예후 : 예비연구)

  • Background: Benign paroxysmal positional vertigo (BPPV) is a relatively common disorder, and is characterized by episodic vertigo and nystagmus which was provoked by head motion. However, little is known about the short and long-term outcome and the prognostic factors for recurrence of BPPV. In this pilot study, we tried to identify the prognostic factors of BPPV for short-term outcome. Methods: We analyzed clinical features of 32 patients (men=21, mean $age=60.4^{\circ}{\pm}12.6y$) with BPPV that was diagnosed by typical nystagmus induced by positioning maneuver. The induced nystagmus was recorded using video-oculography (VOG). According to the semicircular canal involved, BPPV patients were classified into horizontal, posterior, or anterior canal type. Univariate analysis for age, sex, and history of vertigo, and Kaplan-Meier analysis for each canal type were performed. Results: Horizontal (n=21, 65.6%) semicircular canal type BPPV was more common than the posterior one (n=11, 34.4%). Median follow-up period was 113 day (from 34 to 216 days). Four patients with horizontal canal type BPPV had recurrent attacks. Age, history of vertigo, and days prior to diagnosis were not different between canal type. Overall recurrence rate of horizontal canal type BPPV by Kaplan-Meier estimation was 19% at 60 days (p=0.13). Conclusions: Horizontal canal type BPPV was more common and recurred more frequently than posterior canal type in the present study. However, we did not find prognostic factors for recurrence of BPPV.

  • Fighter pilots are taking 'Advanced Pilot Training' courses to perform their missions perfectly even under adverse conditions. However, there are accidents that fall due to problems with the human body's equilibrium in the acceleration of flight, falling into the 'Vertigo, Spatial disorientation' phenomenon. As such, accidents that fighters fall due to spatial disorientation frequently occur not only in Korea but also abroad. In this study, we implemented the 'Vertigo' warning function in the fighter. First, we analyzed the aircraft's mission computer and the currently implemented warning functions. And we studied the coordinate system to utilize the aircraft attitude information. Based on this, we wanted to provide a visual warning to the HUD when the fighter flies over a certain time in the inverted flight position. Implementing this feature is expected to improve pilot flight safety. In addition, based on the results of this study, we propose a method to implement warning functions through linkage with other subsystems.

Electro-Acupuncture for Cervical Vertigo: A Systematic Review and Meta-analysis (경추성 현훈에 대한 전침 치료: 체계적 문헌고찰 및 메타분석)

  • Objectives The objective of this study was to appraise the effect of electro-acupuncture (EA) for the treatment of cervical vertigo. Methods We performed a literature search using nine electronic databases up to the end of November 2020. We included randomized controlled trials (RCTs) which appraised the effect of EA in the treatment of cervical vertigo. The risk of bias was evaluated from the Cochrane risk of bias tool. Results Thirteen appropriate RCTs were included and analyzed. The efficacy rate of the EA group was statistically higher compared with the control group treated with western medicine (p<0.00001). EA treatment significantly improved efficacy rate when combined with western medicine than western medicine alone (p<0.0001). Also EA used with combined therapy (traction, traditional chinese medicine, injection) significantly improved efficacy rate compared with combined therapy alone (p=0.02). Meta-analysis of thirteen studies demonstrated positive results for using EA for the treatment of cervical vertigo. Conclusions Based on results, EA treatment could be effective in cervical vertigo. However, it should be noted that the studies included in this research are regionally biased and small in number. Further well-designed research should be required.

  • Vertigo is one of the most common complaints encountered by physicians and the patients are steadily increasing. These patients are exposed to the risk of secondary accidents such as falls due to vertigo. There are two ways to improve this symptom: medication and rehabilitation. Although temporary symptomatic improvement may be expected in patients treated with medication, vertigo may recur and medication can delay central compensation. In contrast vestibular rehabilitation exploits central mechanisms of neuroplasticity to increase postural stability and enhance visual-vestibular interactions in situations that generate conflicting sensory information. However, vestibular rehabilitation may be compromised by incorrect performance of exercises, and there is a need for active effort and interest from the patient during rehabilitation. To solve these problems, we decided to apply FOVE HMD for eye-tracking and Unity3D to create virtual reality. The proposed eye-tracking based algorithm calculates the concentration of users with eye tracking data and calculates the motion width of the patient with nystagmus, thus the severity of the patient according to the score can be determined. According to our experimental test against healty group and patients group, this result showed the meaningful data to use define the contents result.

  • The main symptoms of the Wallenberg's syndrome are ataxia of gait, clumsiness of ipsilateral limbs, nausea and vomiting, vertigo, visual disturbance such as difficulty in focusing blurred vision diplopia, numbness, dysphagia, hoarseness, hiccup, nystagmus, Horner's syndrome. The purpose of this paper is to report the patient with the Wallenberg's syndrome who was improved by oriental medical treatment. The vertigo of the Wallenberg's syndrome is classed as the pungwhadam(風火痰) and we prescribed Cheonghunhwadam-tang. The intensity and frequency of vertigo wewe dramayically improved.

Korean Treatment with Samul-tang plus Yijin-tang gamibang in Patient for Migrainous Vertigo : 2 Cases Report (사물탕합이진탕 가미방 투여한 편두통성 현훈을 치료한 치험 2례)

  • This case study reports patients suffering from migraine and vertigo had treated with Samul-tang plus Yijin-tang Gamibang. Two patients had taken Samul-tang and Yijin-tang Gamibang three times a day during hospitalization in oriental hospital. One patient had been hospitalized for 2 weeks and other had been for 3weeks. We control dose of added herbs according to other symptoms like dyspepsia, sleep discomfort, flushing, heating sense and diarrhea for each patients. We measure migraine by migraine disability assessment (MIDAS) and dizziness by korean dizziness handicap inventory (KDHI). Also we measure overall improvement with Visual Analogue Scale(VAS). The main symptoms had been steadily decreased to VAS2. Patient complaing of dyspepsia, heating sense, flushing had been improved with reduction of dyspepsia and patient with diarrhea, abdominal discomofort had been improved following reduction of diarrhea. Two patients didn't agree with study because this paper was written following retrospective study but patients claimed no side effect. Samul-tang plus Yijin-tang Gamibang may be considered as a treatment of migrainous vertigo with digestive problems.

Oriental medical treatment in Meniere's disease (메니에르병의 현훈에 대한 한방치험 례)

  • Meniere's disease is not a life-threatening but life-altering inner ear process characterized by episodic vertigo, fluctuating hearing loss, tinnitus, and ear fullness. The etiology and pathophysiology of the symptoms is still disputed. Thereby the treatment of Meniere's disease is empirical. As yet, no treatment has prospectively modified the clinical course of the condition and thereby prevented the progressive hearing loss. We experienced two cases of Meniere's disease treated with herbal medication and acupuncture. 62 year-old male and 64 year-old female patients had definite Meniere's disease. Banhabaeckchulchunma-tang was administered in both cases. After treatment Vertigo attacks were completely controlled in both, whereas Hearing level remained unchanged. Tinnitus was improved in one patient and unchanged in the other. Therefore, Banhabaeckchulchunma-tang and acupuncture could be a safe and simple therapeutic modality that is effective in controlling the vertigo ; however further studies must be done on hearing preservation and tinnitus.
