뇌졸중(腦卒中) 전조증상(前兆症狀)으로서의 현훈(眩暈)에 대한 설문조사분석(設問調査分析) (A Questionaire Analysis for the Vertigo as the prodrome of Stroke)

  • Objective : This study has been made to analysis of clinical type of vertigo prodromic Cerebral Vascular Disease in stroke patients who admitted to sangji oriental hospital department of circulatoty internal medicine in the period from July. 1999 to October 1999. Methods : A Questionaire was done after explanationin details to patients and agreement of patients. The inquire was done as follows. Sex and age, risk factor of stroke, type of stroke, existence of vertigo last three years before stroke attack, the beginning age of vertigo and period of prevalence, frequency, continuance time of vertigo, predilection season and time, factor of causing vertigo, accompanying symptoms of vertigo, severity of vertigo. Result and Conclusion : The existence of Rotation sense(Rotation group and non-Rotation group) and the type of Stroke is no difference significantly but the Rotation vertigo group and non-Rotation vertigo group is difference significantly in distribution. The sixties in age of vertigo attack, previous vertigo before 3 year Stroke attack, irregular in predilection season of vertigo, have a high frequency significantly in distribution and previous period of vertigo has no significantly difference in distribution. One or two per a month in frequency of vertigo, below 5 minute in continuance time of vertigo, irregular in predilection time of vertigo, stand up and sit down in causing vertigo position have a high frequency significantly in distribution.

자발현훈 (Spontaneous Vertigo)

  • Vertigo is an illusion of rotation, which results from an imbalance within the vestibular system. This review focuses on two common presentations of spontaneous vertigo: acute prolonged spontaneous vertigo and recurrent spontaneous vertigo. Common causes of acute prolonged spontaneous vertigo include vestibular neuritis, labyrinthitis, and brainstem or cerebellar stroke. The history and detailed neurological/neurotological examinations usually provide the key information for distinguishing between peripheral and central causes of vertigo. Brain MRI is indicated in any patient with acute vertigo accompanied by abnormal neurological signs, profound imbalance, severe headache, and central patterns of nystagmus. Recurrent spontaneous vertigo occurs when there is a sudden, temporary, and largely reversible impairment of resting neural activity of one labyrinth or its central connections, with subsequent recovery to normal or near-normal function. Meniere's disease, migrainous vertigo, and vertebrobasilar insufficiency (VBI) are common causes. The duration of the vertigo attack is a key piece of information in recurrent spontaneous vertigo. Vertigo of vascular origin, such as VBI, typically lasts for several minutes, whereas recurrent vertigo due to peripheral inner-ear abnormalities lasts for hours. Screening neurotological evaluations, and blood tests for autoimmune and otosyphilis are useful in assessment of recurrent spontaneous vertigo that are likely to be peripheral in origin.

말초성 현기증의 감별진단 (Differential diagnosis of peripheral vertigo)

  • Dizziness can be classified mainly into 4 types: vertigo, disequilibrium, presyncope, and lightheadedness. Among these types, vertigo is a sensation of movement or motion due to various causes. The main causes of peripheral vertigo are benign paroxysmal positional vertigo (BPPV), acute vestibular neuritis (AVN), and Meniere's disease. BPPV is one of the most common causes of peripheral vertigo. It is characterized by brief episodes of mild to intense vertigo, which are triggered by specific changes in the position of the head. BPPV is diagnosed from the characteristic symptoms and by observing the nystagmus such as in the Dix-Hallpike test. BPPV is treated with several canalith repositioning procedures. AVN is the second most common cause of peripheral vertigo. Its key symptom is the acute onset of sustained rotatory vertigo without hearing loss. It is treated with symptomatic therapy with antihistamines, anticholinergic agents, anti-dopaminergic agents, and gamma-aminobutyric acid-enhancing agents that are used for symptoms of acute vertigo. Meniere's disease is characterized by episodic vertigo, fluctuating hearing loss, and tinnitus. It is traditionally relieved with life-style modification, a low-salt diet, and prescription of diuretics. However, diagnosis and treatment of the peripheral vertigo can be difficult without knowledge of BPPV, AVN, and Meniere's disease. This article provides information on the differential diagnosis of peripheral vertigo in BPPV, AVN, and Meniere's disease.

2014년 건강보험심사평가원 환자표본데이터 분석을 이용한 현훈환자의 의료비용에 관한 연구 (A Study on Medical Costs for Patients with Vertigo Based on 2014 Health Insurance Review & Assessment Service-National Patients Sample Data)

  • Objectives: The purpose of this study was to analyze the medical cost for patients with vertigo and to examine associations between chronic vertigo and mental disorder using 2014 Health Insurance Review & Assessment Service-National Patients Sample (HIRA-NPS). Methods: We analyzed sociodemographic characteristics, medical cost and medical care use pattern for vertigo patients. We used hierarchical multiple logistic regression analysis to examine odds ratio between chronic vertigo and mental disorder. Results: A total of 46,502 people and 118,504 claims data were identified for vertigo cases. Characteristics of vertigo patients have significant differences on proportion of female patients (68.36%), patients' average age (54.98) and proportion of medical assistance (5.76%) compared with non-vertigo patients. Results revealed that Korean medicine are one of frequent methods among total treatments for vertigo patients. Total days of medical care and total costs are 2.78 days and 111,362 won, respectively, and days for outpatients in Korean medical care (mean: 2.26 days) are more than those (mean: 5.05 days) in Western medical care. There is significant difference relative to sex between acute vertigo and chronic vertigo. The odds ratio between chronic vertigo and mental disorder is estimated as 1.34, that means risk of becoming chronic is 34% higher for vertigo patients with mental disorder. Conclusions: This study assessed socio-demographic characteristics, medical care use and expenses related to vertigo, and estimated associations between chronic vertigo and mental disorder. Findings provide a basis for economic evaluation studies on vertigo patients and development of clinical practice guidelines for vertigo patients with mental disorder.

어지럼증의 감별진단 (Differential diagnosis of vertigo)

  • Vertigo and dizziness are common symptoms with various etiologies and pathogeneses. Vertigo is an illusion of motion due to disease of the vestibular system, usually a sense of rotation. Dizziness, a term that represents a wide range of non-vertigo symptoms, is commonly associated with non-vestibular disorders including old age, cardiac syncope, orthostatic hypotension, metabolic disease, anxiety, and drugs. Vertigo should be determined whether the cause is central or peripheral. Peripheral vertigo is usually benign but central vertigo is serious and often require urgent treatment. The careful history and detailed physical examinations(pattern of nystagmus, ocular tilt reaction, head impulse test and positional tests such as Dix-Hallpike maneuver) provide important clues to the diagnosis of vertigo. Most of patients have benign peripheral vestibular disorders - vestibular neuritis, benign paroxysmal positional vertigo (BPPV), and Meniere's disease. BPPV is a leading cause of peripheral vertigo and can easily be cured with a canalith repositioning maneuver. In this review, a focus is on the differential diagnosis of common vestibular disorders with peripheral and central causes.

기훈 환자의 보허음 투여 후 호전 1례 (An Improved Case after Treating Chi-Vertigo Patient with Boheo-eum)

  • The subject was a 46-year-old woman who was suffering from severe vertigo. Her major symptoms were vertigo, headache, neck pain, feverishness, and her minor symptoms were intermittent nausea and tinnitus, mild chest discomfort, facial flushing and sweating. Her symptoms were diagnosed as Chi-Vertigo and the patient was treated with Boheo-eum(Buxu-yin). Next, the patient's vertigo was evaluated with Vestibular disorders activities of Daily Living scale and Dix-Hallpike maneuver per 3days. An improved state in vestibular disorders activities of daily living scale, a frequency and duration of Dix-Hallpike maneuver were observed. In this case Boheo-eum(Buxu-yin) has an effect on a patient with vertigo due to Chi-Vertigo.

현훈 진단에 있어 함정 (Pitfalls in the Diagnosis of Vertigo)

  • Vertigo/dizziness is a common complaint in patients who are seeking a primary health clinic. Vertigo is traditionally attributed to damage of the vestibular system. Many peripheral and central vestibular disorders are usually presented with vertigo. However, patients with benign paroxysmal positional vertigo (BPPV), a leading cause of vertigo, may present with postural lightheadedness, near faint, imbalance rather than true vertigo. On the contrary, patients with orthostatic hypotension may present with true spinning vertigo, not dizziness. Persistent postural perceptual dizziness, a second most common cause of dizziness (after BPPV), is mainly occurred after organic vestibular disorders such as BPPV or vestibular neuritis, and classified as a chronic functional vestibular disorder. This article describes non-vestibular disorders presenting dizziness and/or vertigos, which conditions may be misdiagnosed as structural vestibular disorders.

외상 후 말초성현훈 (Posttraumatic Peripheral Vertigo)

  • Posttraumatic vertigo can be defined as the vertiginous disorder occurred after head and neck trauma without other pre-existing vestibular disorder. Central, peripheral, and combined deficits might cause this condition. Especially, various peripheral vestibulopathies are possible causes of posttraumatic vertigo; benign paroxysmal positional vertigo, temporal bone fracture, perilymphatic fistula, labyrinthine concussion, posttraumatic hydrops, and cervical vertigo. Since the differential diagnosis of the posttraumatic vertigo is often difficult, it is essential to acquire knowledge of their pathophysiology and clinical features. In this review, peripheral vestibulopathy as the possible causes of posttraumatic vertigo were described according to the current literature.

현훈을 동반한 이명 환자들의 임상적 특성 (Clinical Characteristics of Patients with Tinnitus accompanied Vertigo)

  • Objectives : This study is to find out clinical characteristics of patients with Tinnitus and Vertigo. Methods : 28 patients were recruited in this study who were ill with tinnitus accompanied vertigo. They told me the aspects of tinnitus and wrote out the Tinnitus Handicap Inventory(THI). Also, they were divided into central and peripheral vertigo. Through symptoms of tinnitus and vertigo, the discrimination of Heo-sil(虛實) was based on the specific aspect of tinnitus along with Zhuang fu bian zheng(臟腑辨證). Results : In tinnitus, high-narrow tone group is more than low-wide tone group and the rate of Heo symptom(虛證) group is higher than Sil symptom(實證) group. In vertigo, the rate of peripheral group is higher than central group. Conclusions : It is expected that this survey will help patients with both tinnitus and vertigo in their treatment progress and prognostic judgment.

현훈(眩暈)의 형상의학적(形象醫學的) 치료(治療) (Treatment of Vertigo in Hyungsang Medicine)

  • Vertigo is one of the subjective symptoms which appears commonly in so many diseases that it is often neglected. But it needs active medical care because it can be a forewarning of serious illness like cerebral apoplexy. This thesis makes a study of vertigo in view of Hyungsang medicine. The followings are the conclusion drawn in diagnosis and treatment of Vertigo: Excess in the upper and deficiency in the lower part(上實下虛) is a main cause of male vertigo. Deficiency in the upper and excess in the lower part(下虛上實) is a general cause of female vertigo. Vertigo is also caused by the insufficiency of the kidney water(腎水) or reservoir of marrow(髓海). persons of Jung pe(精科) with large check bones , those of Fish type(魚類) with Dig mouth and thick lips ; and those with large heads are mainly attacked by this cause. A fleshy person's vertigo comes from the deficiency of Ki and damp phlegm(氣虛濕症) so it is cured by reinforcing Ki, removing dampness and dispelling phlegm. A slim person's vertigo comes from the deficiency of blood and dryness heat(血虛有火) so it is treated by tonifying the blood, cleaning heat away the heat. In case of womanly shaped male and manly shaped female, the vertigo is due to the phlegm fire. Wind-heat(風熱) can cause vertigo generally 施 the persons of following types : wind type(風人), running animal type(走類),bird type(烏類), and Yangmyung meridian type (陽明形). Vertigo also comes from consumption. The deficiency of Ki can be a cause of vertigo in case of male; persons with pale complexion; and those with pronounced noses. The deficiency of blood can be a reason of vertigo in the persons of Hyul type(血科) and those with pale eyelids and lips. The hypofunction of the splean and stomach brings on the deficiency of Ki and blood, which can give rise to the vertigo for those with large nose and mouth. The old people's vertigo comes from the hypofunction of Ki, blood, Yin and Yang.
