하악 소구치용 post and core systems의 치근 내부 응력분산 효과에 대한 유한요소법적 연구 (FINITE EIEMENT ANALYSIS OF STRESS DISTRIBUTIONS OF DENTIN BY POST AND CORE SYSTEMS)

  • The purpose of this study was to analyze the stress distribution in the dentin and post structures by the various post core materials and the amount of remaining coronal tooth structures. The 2-dimensional finite element models of mandibular 2nd premolars was divided into seven types according to the various amount of remaining coronal tooth structures. All types were modeled using equal length, diameter and shape of the post. 2 types of post and core materials were used : 1) cast gold post and core 2) stainless steel post and compsite resin core 10 Newton force was applied as follows 1) vertical force on occlusal fossa 2) $45^{\circ}$ oblique force on buccal surface of buccal cusp tip The results were as follows : 1. There was no apparent difference in the pattern of stress distribution according to the amount of remaining coronal tooth structure. 2. There was no apparent difference in the pattern of stress distribution within the dentin according to the post and core materials. A cast gold post and core generated lower dentin stress than a stainless steel post and resin core. 3. Max. dentinal stress resulting from vertical force was observed in the lingual side of dentin around the crown margin.This stress resulting from oblique force was observed in the lingual root surface of alveolar bone crest level.

The structural changes of pharyngeal airway contributing to snoring after orthognathic surgery in skeletal class III patients

  • Background: Two-jaw surgery including mandibular and maxillary backward movement procedures are commonly performed to correct class III malocclusion. Bimaxillary surgery can reposition the maxillofacial bone together with soft tissue, such as the soft palate and the tongue base. We analyzed changes of pharyngeal airway narrowing to ascertain clinical correlations with the prevalence of snoring after two-jaw surgery. Methods: A prospective clinical study was designed including a survey on snoring and three-dimensional (3D) computed tomography (CT) in class III malocclusion subjects before and after bimaxillary surgery. We conducted an analysis on changes of the posterior pharyngeal space find out clinical correlations with the prevalence of snoring. Results: Among 67 subjects, 12 subjects complained about snoring 5 weeks after the surgical correction, and examining the 12 subjects after 6 months, 6 patients complained about the snoring. The current findings demonstrated the attenuation of the largest transverse width (LTW), anteroposterior length (APL), and cross-sectional area (CSA) following bimaxillary surgery given to class III malocclusion patients, particularly at the retropalatal level. The average distance of maxillary posterior movements were measured to be relatively higher (horizontal distance 3.9 mm, vertical distance 2.6 mm) in case of new snorers. Conclusions: This study found that bimaxillary surgery could lead to the narrowing of upper airway at the retropalatal or retroglossal level as well as triggering snoring in subjects with class III malocclusion. Based on the current clinical findings, we also found that upper airway narrowing at retropalatal level may contribute to increasing the probability of snoring and that polysonography may need to be performed before orthognathic surgery in subjects with class III malocclusion.

  • Statement of the problem: In cases of low bone level in maxilla followed by extraction due to severe periodontitis or enlarged maxillary sinus, crown-root ratio of implant prosthesis will increase. The prognosis of these cases is not good as expected. Purpose : The purpose is to compare stress distribution due to crown-root ratio and effect of splinting between two implants in maxillary molar area under different loads Material and methods: Using ITI($4.1{\times}10$ mm) implant. two finite element models were made(model S: two parallel implants, model A: one of two is 20 degree inclined). Each model was designed in different crown-root ratio(0.7:1, 1:1, 1.25:1) and set cement type gold crown to make it splinted or non-splinted clinical situations. After that, 300 N force was loaded to each model in four ways.(load 1 : middle of occlusal table, load 2 : middle of buccal cusp, load 3 : middle of lingual cusp, load 4 : horizontal load to middle of buccal cusp), and stress distribution was analyzed. Results: On all occasions, stress was concentrated on neck of implant near cortical bone. In the case of inclined implant, stress was increased compared with parallel implants. Under load 1, 2, 3, stress was not increased even when crown-root ratio increases, but under load 4, when crown-root ratio increases, stress also increased. And more stress was concentrated under load 1 than load 2, 3. When crown-root ratio was same, stress under load 1, 2, 3 decreased when splinting, but under load 4, stress did not really decrease. Conclusion: Under vertical load, stress distribution related to crown-root ratio did not change. But under horizontal load, stress increased as crown-root ratio increases. Under vertical load, splinting decreased stress but under horizontal load, effect of splinting was decreased as condition of implant changes for the worse such as increase of crown-root ratio, inclined implant.

임상가를 위한 특집 3 - 치주판막 절개의 기본원리 (Basic Rules of Incision in Periodontal Flap Preparation)

  • The periodontal flap surgery is the most widely utilized surgical procedure to reduce the pocket depth and to access the subgingival root surfaces for scaling and root planing. The diagnosis of the periodontal lesion and the objective of the surgery will dictate the type of flap procedure which will be utilized to obtain the best result. The incisions, type of flap and the selection of suturing design must be planned and executed to fit the problem. Periodontal flaps are designed to preserve gingival integrity and to gain access to root surfaces for residual calculus removal and to thoroughly remove granulation tissue so bone defects can be visualized and treated. Gentle and efficient procedures result in optimum healing and minimal postoperative pain. When flaps need to be repositioned apically or less often, coronally, then the flaps must sit passively at the appropriate level before suturing. To ensure this, buccal and lingual flaps need to be elevated beyond the mucogingival junction so the elasticity of the mucosa allows for flap mobility. Sometimes it may be necessary to extend the flap elevation apically with a split incision approach to minimize the effect of the less elastic periosteum. Vertical incisions can aid in flap positioning by allow ing the clinician to suture the flap at a different level to the adjacent untreated gingiva. In osseous periodontal surgery, flaps are apically positioned to minimize postoperative pocket depth. In regenerative periodontal surgery including implant surgery, soft tissue cove rage of bony defects, graft materials, membranes, and bio logic agents is important so sulcular incisions and light suturing techniques are crucial.

Alveolar ridge preservation with an open-healing approach using single-layer or double-layer coverage with collagen membranes

  • Purpose: The aim of this prospective pilot study was to compare alveolar ridge preservation (ARP) procedures with open-healing approach using a single-layer and a double-layer coverage with collagen membranes using radiographic and clinical analyses. Methods: Eleven molars from 9 healthy patients requiring extraction of the maxillary or mandibular posterior teeth were included and allocated into 2 groups. After tooth extraction, deproteinized bovine bone mineral mixed with 10% collagen was grafted into the socket and covered either with a double-layer of resorbable non-cross-linked collagen membranes (DL group, n=6) or with a single-layer (SL group, n=5). Primary closure was not obtained. Cone-beam computed tomography images were taken immediately after the ARP procedure and after a healing period of 4 months before implant placement. Radiographic measurements were made of the width and height changes of the alveolar ridge. Results: All sites healed without any complications, and dental implants were placed at all operated sites with acceptable initial stability. The measurements showed that the reductions in width at the level 1 mm apical from the alveolar crest (including the bone graft) were $-1.7{\pm}0.5mm$ in the SL group and $-1.8{\pm}0.4mm$ in the DL group, and the horizontal changes in the other areas were also similar in the DL and SL groups. The reductions in height were also comparable between groups. Conclusions: Within the limitations of this study, single-layer and double-layer coverage with collagen membranes after ARP failed to show substantial differences in the preservation of horizontal or vertical dimensions or in clinical healing. Thus, both approaches seem to be suitable for open-healing ridge preservation procedures.

골격성 III급 부정교합자의 치열궁 폭경에 관한 연구 (A Study on Basal and Dental Arch Width in Skeletal Class III Malocclusion)

  • 본 연구는 전후방적인 부조화가 폭경에 영향을 미치는지 알기 위해 I급 정상교합군과 III급 부정교합군을 비교하였고, III급 부정교합군내에서 수직적인 부조화가 폭경에 영향을 미치는지 알기 위해 Hyperdivergent군과 Neutral군으로 나누어 폭경의 차이를 비교하였다. 부산대학교병원 치과교정과에 내원한 골격성 III급(ANB< 0$^{\circ}$) 부정교합자 중측모 두부방사선 계측사진상에서 하악 평면각(SN-Mandibular plane; 정상인의 평균 32 ${\pm}$ 5$^{\circ}$)이 1.5 SD이상(39.5$^{\circ}$이상)인 환자 37명 (남자 18명, 여자 19명 )을 hyperdivergent군(Group B)으로, 하악 평면각이 정상치의 0.5 SD이내인(32 ${\pm}$ 2.5$^{\circ}$) 40명 (남자 20명, 여자 20명)을 neutral군(Group C)으로 분류하였고 11개의 치과대학 교정과에서 선별한 정상교합과 정상적인 안모를 가진 성인 24) 중 41명(남자 20명, 여자 21명)을 정상교합군(Group A)으로 하여 이 세 군을 대상으로 연구하여 다음과 같은 결론을 얻었다. 1. 골격성 III급 부정교합군내에서 수직적인 양상에 따른 폭경 비율은 유의할 만한 차이가 없었다. 2. 정상교합군과 III급 부정교합군의 상, 하악 기저골에 대한 치아간 폭경 비율의 비교에서 상악에서는 III급 부정교합군의 비율이 더 컸고(p<0.001), 하악에서는 제 1대구치 부분을 제외한 모든 항목에서 III급 부정교합군의 비율이 작아서(p<0.001) III급 부정교합군이 정상교합군에 비해 기저골에 대해 상악 치아가 더 협측으로 경사되고, 제1대구치를 제외하고 기저골에 대해 하악 치아가 설측 경사되어 횡적인 치성보상이 잘 되어 있는 것으로 나타났다(p<0.001). 3. 정상교합군과 III급 부정교합군의 상악에 대한 하악의 치아간 폭경 비율의 비교에서 모든 부분에서 유의할 만한 차이가 없었다. 4. 정상교합군과 III급 부정교합군의 상악에 대한 하악의 기저골 폭경 비율의 비교에서 모든 부위에서 III급 부정교합군이 정상교합군보다 더 커서(p<0.0001) III급 부정교합군의 상악 기저골 폭경이 하악보다 더 좁게 나타났다.

한강하류지형면의 분류와 지형발달에 대한 연구 (양수리에서 능곡까지)

  • Purpose of study; The purpose of this study is specifically classified as two parts. The one is to attempt the chronological annals of Quaternary topographic surface through the study over the formation process of alluvial surfaces in our country, setting forth the alluvial surfaces lower-parts of Han River area, as the basic deposit, and comparing it to the marginal landform surfaces. The other is to attempt the classification of micro morphology based on the and condition premising the land use as a link for the regional development in the lower-parts of Han river area. Reasons why selected the Lower-parts of Han river area as study objects: 1. The change of river course in this area is very serve both in vertical and horizontal sides. With a situation it is very easy to know about the old geography related to the formation process of topography. 2. The component materials of gravel, sand, silt and clay are deposited in this area. Making it the available data, it is possible to consider about not oかy the formation process of topography but alsoon the development history to some extent. 3. The earthen vessel, a fossil shell fish, bone, cnarcoal and sea-weed are included in the alluvial deposition in this area. These can be also valuable data related to the chronological annals. 4. The bottom set conglometate beds is also included in the alluvial deposits. This can be also valuable data related to the research of geomorphological development. 5. Around of this area the medium landform surface, lower landform surface, pediment and basin, are existed, and these enable the comparison between the erosion surfaces and the alluvial surfaces. Approach : 1. Referring to the change of river beds, I have calculated the vertical and horizontal differences comparing the topographic map published in 1916 with that published in 1966 and through the field work 2. In classifying the landform, I have applied the method of micro morphological classification in accordance with the synthetic index based upon the land conditions, and furthermore used the classification method comparing the topographic map published in 1916 and in that of 1966. 3. I have accorded this classification with the classification by mapping through appliying the method of classification in the development history for the field work making the component materials as the available data. 4. I have used the component materials, which were picked up form the outcrop of 10 places and bored at 5 places, as the available data. 5. I have referred to Hydrological survey data of the ministry of Construction (since 1916) on the overflow of Han-river, and used geologic map of Seoul metropolitan area. Survey Data, and general map published in 1916 by the Japanese Army Survbey Dept., and map published in 1966 by the Construction Research Laboratory and ROK Army Survey Dept., respectively. Conclusion: 1. Classification of Morphology: I have added the historical consideration for development, making the component materials and fossil as the data, to the typical consideration in accordance with the map of summit level, reliefe and slope distribution. In connection with the erosion surface, I have divided into three classification such as high, medium and low-,level landform surfaces which were classified as high and low level landform surfaces in past. furthermore I have divided the low level landform surface two parts, namely upper-parts(200-300m) and bellow-parts(${\pm}100m$). Accordingly, we can recognize the three-parts of erosion surface including the medium level landform surface (500-600m) in this area. (see table 22). In condition with the alluvial surfaces I have classified as two landform surfaces (old and new) which was regarded as one face in past. Meamwhile, under the premise of land use, the synthetic, micro morphological classification based upon the land condition is as per the draw No. 19-1. This is the quite new method of classification which was at first attempted in this country. 2. I have learned that the change of river was most severe at seeing the river meandering rate from Dangjung-ni to Nanjido. As you seee the table and the vertical and horizontal change of river beds is justly proportionable to the river meandering rate. 3. It can be learned at seeing the analysis of component materials of alluvial deposits that the component from each other by areas, however, in the deposits relationship upper stream, and between upper parts and below parts I couldn't always find out the regular ones. 4. Having earthern vessel, shell bone, fossil charcoal and and seaweeds includen in the component materials such as gravel, clay, sand and silt in Dukso and Songpa deposits area. I have become to attempt the compilation of chronicle as yon see in the table 22. 5. In according to hearing of basemen excavation, the bottom set conglomerate beds of Dukso beds of Dukso-beds is 7m and Songpa-beds is 10m. In according to information of dredger it is approx. 20m in the down stream. 6. Making these two beds as the standard beds, I have compared it to other beds. 7 The coarse sand beds which is covering the clay-beds of Dukso-beds and Nanjidobeds is shown the existence of so-called erosion period which formed the gap among the alluvial deposits of stratum. The former has been proved by the sorting, bedding and roundness which was supplied by the main stream and later by the branch stream, respectively. 8. If the clay-beds of Dukeo-bed and Songpa-bed is called as being transgressive overlap, by the Eustatic movement after glacial age, the bottom set conglomerate beds shall be called as being regressive overlap at the holocene. This has the closest relationship with the basin formation movement of Seoul besides the Eustatic movement. 9. The silt-beds which is the main component of deposits of flood plain, is regarded as being deposited at the Holocene in the comb ceramic and plain pottery ages. This has the closest relationship with the change of river course and river beds.

Dual Plane Augmentation Genioplasty Using Gore-Tex Chin Implants

  • Background: The chin shape and position is important in determining the general shape of the face, and augmentation genioplasty is performed alone or in combination with other aesthetic procedures. However, augmentation genioplasty using osteotomy is an invasive and complex procedure with the potential to damage mentalis muscle and mental nerve, to affect chin growth, and prolonged recovery. Our aim was to present our experience with a modified augmentation genioplasty procedure for hypoplastic chins using a Gore-Tex implant. Methods: Two vertical slit incisions were made at the canine level to create a supra-periosteal pocket between the incisions, preserving the periosteum and mentalis muscle. Minimal sub-periosteal dissection was performed lateral to the incisions along the mandibular border. The both wings of implant were inserted under the periosteum to achieve a stable dual plane implantation. Results: In total, 47 patients underwent dual plane chin augmentation using a Gore- Tex implant between January 2008 and May 2013. The mean age at operation was 25.77 years (range, 15-55 years). There were 3 cases of infection; one patient was treated with antibiotics, the others underwent implant removal. Additionally, two patients complained of postoperative parasthesia that spontaneously improved without any additional treatment. Most patients were satisfied with the postoperative outcomes, and no chin growth problems were observed among the younger patients. Conclusion: Dual plane Gore-Tex chin augmentation is a minimally-invasive operation that is simple and safe. All implants yielded satisfactory results with no significant complications such as mental nerve injury, lower lip incompetence, or chin growth limitation.

Modified toe pulp fillet flap coverage: Better wound healing and satisfactory length preservation

  • Background Amputation is commonly performed for toe necrosis secondary to peripheral vascular diseases, such as diabetes mellitus. When amputating a necrotic toe, preservation of the bony structure is important for preventing the collapse of adjacent digits into the amputated space. However, in the popular terminal Syme's amputation technique, partial amputation of the distal phalanx could cause increased tension on the wound margin. Herein, we introduce a new way to resect sufficient bony structure while maintaining the normal length, based on a morphological analysis of the toes. Methods Unlike the pulp of the finger in the distal phalanx, the toe has abundant teardrop-shaped pulp tissue. The ratio of the vertical length to the longitudinal length in the distal phalanx was compared between the toes and fingers. Amputation was performed at the proximal interphalangeal joint level. Then, a mobilizable pulp flap was rotated $90^{\circ}$ cephalad to replace the distal soft tissue defect. This modified toe fillet flap was performed in 5 patients. Results The toe pulp was found to have a vertically oriented morphology compared to that of the fingers, enabling length preservation through cephalad rotation. All defects were successfully covered without marginal ischemia. Conclusions While conventional toe fillet flap coverage focuses on the principle of length preservation as the first priority, our modified method takes both wound healing and length into account. The fattiest part of the pulp is advanced to the toe tip, providing a cushioning effect and enough length to substitute for phalangeal bone loss. Our modified method led to satisfactory functional and aesthetic outcomes.

The Effect of Hounsfield Unit Value with Conventional Computed Tomography and Intraoperative Distraction on Postoperative Intervertebral Height Reduction in Patients Following Stand-Alone Anterior Cervical Discectomy and Fusion

  • Objective : The most common complication of anterior cervical discectomy and fusion (ACDF) is cage subsidence and maintenance of disc height affects postoperative clinical outcomes. We considered cage subsidence as an inappropriate indicator for evaluating preservation of disc height. Thus, this study aimed to consider patients with complications such as reduced total disc height compared to that before surgery and evaluate the relevance of several factors before ACDF. Methods : We retrospectively reviewed the medical records of 40 patients who underwent stand-alone single-level ACDF using a polyetheretherketone (PEEK) cage at our institution between January 2012 and December 2018. Our study population comprised 19 male and 21 female patients aged 24-70 years. The minimum follow-up period was 1 year. Twenty-seven patients had preoperative bone mineral density (BMD) data on dual-energy X-ray absorptiometry. Clinical parameters included sex, age, body mass index, smoking history, and prior medical history. Radiologic parameters included the C2-7 cobb angle, segmental angle, sagittal vertical axis, disc height, and total intervertebral height (TIH) at the preoperative and postoperative periods. Cage decrement was defined as the reduction in TIH at the 6-month follow-up compared to preoperative TIH. To evaluate the bone quality, Hounsfield unit (HU) value was calculated in the axial and sagittal images of conventional computed tomography. Results : Lumbar BMD values and cervical HU values were significantly correlated (r=0.733, p<0.001). We divided the patients into two groups based on cage decrement, and 47.5% of the total patients were regarded as cage decrement. There were statistically significant differences in the parameters of measuring the HU value of the vertebra and intraoperative distraction between the two groups. Using these identified factors, we performed a receiver operating characteristic (ROC) curve analysis. Based on the ROC curve, the cut-off point was 530 at the HU value of the upper cortical and cancellous vertebrae (p=0.014; area under the curve [AUC], 0.727; sensitivity, 94.7%; specificity, 42.9%) and 22.41 at intraoperative distraction (p=0.017; AUC, 0.722; sensitivity, 85.7%; specificity, 57.9%). Using this value, we converted these parameters into a bifurcated variable and assessed the multinomial regression analysis to evaluate the risk factors for cage decrement in ACDF. Intraoperative distraction and HU value of the upper vertebral body were independent factors of postoperative subsidence. Conclusion : Insufficient intraoperative distraction and low HU value showed a strong relationship with postoperative intervertebral height reduction following single stand-alone PEEK cage ACDF.
