  • Nasal valve compromise (NVC) is a distinct cause of symptomatic nasal obstruction, yet there are several ambiguities surrounding the diagnosis and management of this disease. Understanding of nasal valve anatomy with critical assessment of the site of obstruction is essential to effective nasal valve management. Technique selection should be individualized to the type of valve dysfunction. This case report presents a 56 year old man with nasal valve dysfunction due to narrow middle vault, concave lower lateral cartilage and swollen septal body which was diagnosed by various techniques including navigation system and treated by spreader graft, alar batten graft and reduction of septal body.

Limit analysis of seismic collapse for shallow tunnel in inhomogeneous ground

  • Shallow tunnels are vulnerable to earthquakes, and shallow ground is usually inhomogeneous. Based on the limit equilibrium method and variational principle, a solution for the seismic collapse mechanism of shallow tunnel in inhomogeneous ground is presented. And the finite difference method is employed to compare with the analytical solution. It shows that the analytical results are conservative when the horizontal and vertical stresses equal the static earth pressure and zero at vault section, respectively. The safety factor of shallow tunnel changes greatly during an earthquake. Hence, the cyclic loading characteristics should be considered to evaluate tunnel stability. And the curve sliding surface agrees with the numerical simulation and previous studies. To save time and ensure accuracy, the curve sliding surface with 2 undetermined constants is a good choice to analyze shallow tunnel stability. Parameter analysis demonstrates that the horizontal semiaxis, acceleration, ground cohesion and homogeneity affect tunnel stability greatly, and the horizontal semiaxis, vertical semiaxis, tunnel depth and ground homogeneity have obvious influence on tunnel sliding surface. It concludes that the most applicable approaches to enhance tunnel stability are reducing the horizontal semiaxis, strengthening cohesion and setting the tunnel into good ground.

  • Masonry constructions exhibit uncertain behaviors under dynamic effects such as seismic action. Complex issues arise in the idealization of structural systems of buildings having different material types and mechanical properties. In this study, the structural behavior of a vaulted masonry building constructed using full clay brick and lime-based mortar and sitting on consecutive arches was investigated by experimental and numerical approaches. The dimensions of the structure built in the laboratory were 391 × 196 cm, and its height was 234 cm. An incremental repetitive loading was applied to the prototype construction model. Along the gradually increasing loading pattern, the load-displacement curves of the masonry structure were obtained with the assistance of eight linear displacement transducers. In addition, crack formation areas, and relevant causes of its formation were determined. The experimental model was idealized using the finite element method, and numerical analyses were performed for the area considered as linear being under similar loading effect. From the linear analyses, the displacement values and stress distribution of the numerical model were obtained. In addition, the effects of tie members, frequently being used in the supports of curved load-bearing elements, on the structural behavior were examined. Consequently, the experimental and numerical analysis results were comparatively evaluated.

사두증 환자를 위한 맞춤형 헬멧 몰드 디자인 시스템 개발 (Development of a Customized Helmet Design System for Patients with Plagiocephaly)

  • This study developed a three-dimensional helmet mold design software that can design helmets for treating the infant plagiocephaly (flat head syndrome) using three-dimensional head scan data. For this, the three-dimensional head data of sixth SizeKorea body measurement project as well as the data produced by a head modeling software were used to prepare the head shape data of plagiocephaly patients. A total of 14 landmarks and 10 dimensions of heads required for helmet mold shape design and plagiocephaly diagnosis were automatically measured using an anthropometric analysis software. Using the software developed in this study, plagiocephaly can be diagnosed not only visually by three-dimensional head data visualization but also quantitatively by calculating the medically defined indices such as cranial index, which measures the proportions of the head, and the cranial vault asymmetry index, which measures the asymmetry of the head. The basic shape of the helmet mold was automatically generated based on the head scan data. Additionally, it is possible to fine tune the shape of the mold to reflect individual characteristics by using a free form deformation technique. Finally, the designed helmet mold was converted into the data that can be printed on a three-dimensional printer for generating the actual prototype.

A new proposal for controlled recycling of decommissioning concrete waste as part of engineered barriers of a radioactive waste repository and related comprehensive safety assessment

  • As an alternative to conventional management options for a lot of concrete waste from decommissioning of nuclear power plants, a set of scenarios for controlled recycling of decommissioning concrete waste as engineered barriers of a radioactive waste repository was proposed, and a comprehensive safety assessment model and framework covering both pre-and post-closure phases was newly developed. The new methodology was applied to a reference vault-type repository, and the ratios of derived concentration limits to unconditional clearance levels of eighteen radionuclides for controlled recycling were provided for three sets of dose criteria (0.01, 1, and 20 mSv/y for the pre-closure and 0.01 mSv/y for the post-closure phases). It turns out that decommissioning concrete waste whose concentration is much higher than the unconditional clearance level can be recycled even when the dose criterion 0.01 mSv/y is applied. Moreover, a case study on ABWR bio-shield shows that the fraction of recyclable concrete waste increases significantly by increasing the dose criterion for the radiation worker in the pre-closure phase or the duration of storage prior to recycling. The results of this study are expected to contribute to demonstrating the feasibility of controlled recycling of a lot of decommissioning concrete waste within nuclear sectors.

방사성폐기물 표층처분시설 통합 모니터링 시스템 개발 (Development of an Integrated Monitoring System for the Low and Intermediate Level Radioactive Waste Near-surface Disposal Facility)

  • In this study, the function and purpose of the disposal cover, which is an engineering barrier installed to isolate the disposal vault of the near-surface disposal facility for radioactive waste from natural/man-made intrusion, and the design details of the demonstration facility for performance verification were described. The Demonstration facility was designed in a partially divided form to secure the efficiency of measurement while being the same as the actual size of the surface disposal facility to be built in the Intermediate & low-level radioactive waste disposal site of the Korea Radioactive Waste Agency (KORAD). The instruments used for measurement consist of a multi-point thermometer, FDR (Frequency Domain Reflectometry) sensor, inclinometer, acoustic sensor, flow meter, and meteorological observer. It is used as input data for the monitoring system. The 3D monitoring system was composed of 5 layers using the e-government standard framework, and was developed based on 4 components: screen, control module, service module, and DBIO(DataBase Input Output) module, and connected them to system operation. The monitoring system can provide real-time information on physical changes in the demonstration facility through the collection, analysis, storage, and visualization processes.

  • 최근 들어 우리나라에서는 설치의 간편성과 높은 신뢰도를 가진 전력케이블의 사용이 증가하고 있다. 전력케이블은 출고 전에 IEEE std. 404-1993 시험을 거쳐 안정성을 확인하고 있지만 포설시 발생하는 접속부 내부의 결함으로 인하여 많은 문제가 발생하고 있다. 특히 불순물 혼입 또는 공극 발생시 고장율은 증가하게 된다 부분방전 검출은 포설 후 전력케이블의 상태를 관측할 수 있는 유용한 방법이다. 본 연구에서는 부분방전 특성을 평가하고자 케이블 접속재인 EPR과 케이블 절연체인 XPLE 사이에 인공 결함을 발생시킨 후 데이터 취득 시스템을 이용하여 $\Phi-q-n$ 특성을 검출하였으며, 부분방전의 정량적 해석을 위해 필요한 통계량을 계산하였으며, 신경망 모델을 적용하여 패턴 분석을 수행하여 $88\~96\%$의 구별이 가능하였다.

부정교합자의 상악치열궁 및 구개에 관한 연구 (A STUDY ON THE MAXILLARY DENIAL ARCH AND PALATAL VAULT WITH MALOCCULSIONS)

  • This study was based on the study models of 32 subjects with normal occlusion, 40 with Class I malocclusion, 32 with Class II, Division 1 malocclusion and 38 with Class III malocclusion, aged 12 to 20 years (mean age 16.4 years). The purpose of present study was to define the difference between normal and malocclusion groups in maxillary dental arch and palate. On the basis of findings of this study, the following results were obtained. 1. The intermolar widths and the intercanine widths in Class II, Div. 1 malocclusion group were smaller than in normal occlusion group significantly. 2. The arch lengths measured in both Class I and Class II, Div.1 malocclusion groups were larger than in normal occlusion group. 3. The palates in Class I and Class II, Div. 1 malocclusion groups were longer and narrower than in normal occlusion, but the palates in Class III malocclusion group were shorter than in normal occlusion group significantly. 4. The palatal depths measured at level 1 in Class III malocclusion group were significantly higher than in normal occlusion and in Class II, Div. 1 group they were significantly higher than in normal occlusion at level 2 and 3. 5. The measurements of palatal areas at various levels showed no significant difference between malocclusion and normal occlusion groups. 6. The palatal indies 1 (palatal length / palatal width) measured in both Class I and Class II, Div. 1 malocclusion groups were significantly greater than in normal occlusion and the palatal indice 2 (palatal depth at level 1/palatal width) measured in all malocclusion groups are greater than in normal occlusion. 7. It was determined from findings of this study that the measurements of maxillary dental arch and palate were influenced to a considerable extent by the molar relationship.

남매간에 발생한 두개골조기유합증 (Craniosynostosis Occurring between Siblings)

  • Craniosynostosis is a congenital anomaly in which cranial sutures close prematurely and restrict skull growth. In this paper, the case of two siblings, a male and a female, who were both diagnosed as craniosynostosis is reported. They underwent corrective osteotomy for cranial vault remodeling. A 22-month-old female infant who was brought to the department of plastic and reconstructive surgery of the authors' hospital was diagnosed with plagiocephaly. At the same time, her 7-month-old brother was diagnosed with brachycephaly. In the case of the female infant, corrective coronal osteotomy and supraorbital bar advancement were performed. Her brother underwent frontal advancement osteotomy using Tessier's tongue in the groove procedure. After the correction of the craniosynostosis, the two patients recovered in several days later, and the results were good in both cases cosmetically and functionally. They showed normal head circumference increasing curves and no symptom of functional disorder in their last follow-up. Isolated or nonsyndromic craniosynostosis is sporadic but mostly autosomal dominant. This paper presents a case of craniosynostosis with a genetic tendency; and although it occurred between siblings, the affected lesions differed. Thus, appropriate diagnosis and management in patients are needed.

  • Freeman-Sheldon Syndrome(FSS)은 드물게 발생하는 선천성 유전 질환으로 휘파람을 부는 듯한 특징적인 안모로 인해 'Whistling face syndrome'으로 불리워진다. 또한 편평한 안모, 긴 인중(philtrum), 낮은 비연골이 나타나 두드러진 안면이상을 보이고, 내반족(club foot, 內反足), 손가락의 관절구축(joint contracture)으로 인한 풍차 모양의 손을 가지며, 지능은 보통 정상이다. 본원에 내원한 환아는 이 질환의 특징적 양상인 구강 주위 근육의 수축으로 소구증 및 개구제한, 높은 구개 및 부정교합, 치열의 심한 총생을 보여 구강위생이 매우 불량하였고 치과치료에 비협조적이었다. 소아치과적 행동조절 및 구강위생관리, 섭식장애의 상담으로 치과적 문제는 다소 개선되었으나, 안과 및 정형외과, 신경정신과, 교정과, 마취과적 문제로 인해 다양한 협진이 필요하며 계속적인 관찰 및 치료가 요구되기에 이를 보고하는 바이다.
