Background: Squatting is one of the best functional exercises to strengthen the quadriceps muscle in independent gait and activities of daily living. Although the use of a gym ball during squat exercise is the most common way of strengthening the vastus medialis oblique (VMO) muscle, published data on this subject are limited. Objects: The purpose of this study was to compare the sequential muscle activation of the VMO and vastus lateralis (VL) muscles during squat exercise using a gym ball at different knee flexion angles. Methods: Forty healthy adults were randomly divided into experimental (squat exercise using a gym ball) and control (squat exercise alone) groups, in which squats were performed at $45^{\circ}$ and $90^{\circ}$ knee flexion. Electromyographic (EMG) activity data were collected over 10 seconds under the 2 angles of knee flexion ($45^{\circ}$ and $90^{\circ}$). Results: There was significant group and time interaction effect for VMO and VL muscle activation at $45^{\circ}$ knee flexion. This was similarly demonstrated at $90^{\circ}$ knee flexion. No significant group main effect and time main effect for VMO and VL muscle activation were noted at $45^{\circ}$ knee flexion, respectively. In contrast, there was significant group main effect and time main effect for VMO and VL muscle activation at $90^{\circ}$ knee flexion. These significant differences were demonstrated through two-way analysis of variance over repeated measurements, suggesting that the EMG activity of the VMO muscle during squatting with a gym ball showed remarkable improvement compared to that of the VL muscle. Conclusion: This research suggests that squat exercise using a gym ball may be more beneficial in improving the activity of VMO than of the VL muscle at both $45^{\circ}$ and $90^{\circ}$ of knee flexion, respectively. We highly recommend squat exercises with a gym ball for selective strengthening of the VMO muscle in knee rehabilitation.
Purpose: The purpose of this study was to evaluate the electromyographic (EMG) activity of vastus medialis oblique (VMO) and vastus lateralis (VL) muscles on foot position and knee angle for hemiplegia patients. Methods: Ten stroke subjects (10 males) participated in the study. Subjects were all right-hemiplegic patients. All subjects did $0^{\circ},\;20^{\circ}$ and $40^{\circ}$ knee flexion while maintaining the foot in a neutral position, or at $30^{\circ}$ adduction or at $30^{\circ}$ abduction. Surface EMG data were collected for VMO and VL muscles on the non-hemiplegic side and hemiplegic side. Collected data were analyzed using two-way ANOVA. Results: VMO and VL activities for the non-hemiplegic and the hemiplegic sides were highest for $40^{\circ}$ knee flexion while maintaining the three foot positions. There were no significant differences in EMG activity of the VMO and VL muscles with different foot positions. There were significant differences between VMO and VL activity for knee flexion angle while maintaining the foot in neutral (p<0.05), at $30^{\circ}$ adduction (p<0.05), or at $30^{\circ}$ abduction (p<0.05). Conclusion: Foot position does not influence VMO and VL activities. But, knee flexion exercise in a closed chain can increase VMO and VL muscle activity for hemiplegic patients. In particular, VMO and VL activities for both the non-hemi side and the hemi side were highest for $40^{\circ}$ knee flexion.

PURPOSE: Femur and tibia alignment in the knee joint is important to the biomechanics of lower limb movement. The purpose of this study was to compare vastus medialis oblique (VMO) and vastus lateralis electromyographic muscle activities according to tibial rotation taping methods. METHODS: Twenty-nine healthy subjects (13 males and 16 females) in the 20s, without knee joint-related diseases or disorders, participated in our study. After identifying each subject's dominant foot, the maximal voluntary isometric contraction (MVIC) was determined using a manual muscle tester. The activity of each target muscle was measured at 50% MVIC in isometric muscle contraction and at a $30^{\circ}$ knee flexion position before and after applying internal and external rotation taping by the Mulligan concept and in the neutral position. Non-elastic tape was used to stabilize the tibia rotation position. RESULTS: In the males, VMO muscle activity was significantly increased in the tibia internal rotation position ($47.2{\pm}14.6$, $mean{\pm}SD$) than in the neutral position ($39.3{\pm}14.9$) (p<.05). CONCLUSION: The results of this study indicate that when applying tibia internal rotation taping in healthy males, VMO muscle activity significantly increases during isometric extension of the knee. Therefore, this study provides a basis for selecting the appropriate taping method, in consideration of the available treatments in clinical practice for patients with knee problems.

Purpose : The purpose of this study was to determine the effects of hip adduction using a ball on the activation of the vastus medialis oblique (VMO) and the vastus lateralis (VL) during dynamic semisquat exercises. Methods : Twenty seven participants performed three repetitions of a double-leg semisquat and squeeze semisquat (semisquat with hip adduction using a ball) at $60^{\circ}$ knee flexion. The activation of the VMO and the VL was recorded at dominant leg during both semisquat exercises using surface electromyography (MP 100). EMG data were normalized to the maximal voluntary isometric contraction (MVIC) of the quadriceps produced during seated, isometric knee extension. Results : Results of paired t-test analyses revealed that there were no differences between activity of the VMO and the VL in both double-leg semisquat and squeeze semisquat. The activity of the VMO was significantly increased in squeeze semisquat whereas there was no difference in activity of the VL between double-leg semisquat and squeeze semisquat. Conclusion : Combining hip adduction using a ball with semisquat at $60^{\circ}$ knee flexion preferentially increases the activity of the VMO. We can conclude that semisquat at $60^{\circ}$ knee flexion with hip adduction using a ball selectively recruits the VMO. Rehabilitation for quadricep imbalance should consider these findings when selecting exercises which could preferentially activate the VMO.
This study aimed to investigate the effect of differing heel heights on the electromyographic (EMG) activity in vastus medialis (VM) and vastus lateralis (VL) during stair ascending and descending activities. A total of 26 healthy women volunteered to perform stair-ascending and stair-descending tasks with 3 heel heights: barefoot, 3 cm, and 7 cm. The EMG activities of the VM and VL were recorded during the tasks. During the stair ascending and descending tasks, the EMG activities of both VM and VL significantly changed with differing the heel heights (p<.05). Moreover, the EMG activities of VM and VL during the stair ascending task were significantly higher than the corresponding values during the stair-descending task (p<.05). However, there were no significant differences between the VM:VL EMG ratios for the 3 heel heights (p>.05). The VM:VL EMG ratios between the 2 tasks differed significantly in the 7 cm high heel condition (p<.05). Despite an increase in the EMG activities in both VM and VL during stair ascending and descending tasks, there was no change in the relative EMG intensities of VM and VL, which was measured by calculating the VM:VL ratio this result indicates that no VM:VL imbalances were elicited. The relative EMG intensities of VM and VL during stair descent were lower than the corresponding values during the ascent, suggesting that VM and VL may show an imbalance in the eccentric activation during the weight-acceptance phase. This study provides useful information that will facilitate future research on how heel height affects muscle activity around the knee joint.
Purpose: This study examined the effects of the abdominal drawing-in maneuver (ADIM) on muscle activity in the trunk and legs while subjects walk on a ramp. Methods: The subjects were healthy adult males (n=15) and females (n=8) in their twenties. The subjects were asked to maintain the ADIM contraction for 15 minutes using a pressure biofeedback unit. Their muscle activity was then measured while ascending or descending the ramp with or without the ADIM contraction maintained. Activity in the sternocleidomastoid, splenius capitis, rectus abdominis, external oblique abdominal, transversus abdominis, erector spinae, vastus medialis, and vastus lateralis muscles was measured using surface electromyography (TM DTS, Noraxon, USA). A paired t-test was conducted using SPSS 18.0 (IBM) for statistical data processing. Results: Maintaining the ADIM contraction during ascension led to a significant increase (p<0.05) in muscle activity for the rectus abdominis, transversus abdominis, vastus medialis, and vastus lateralis, but a significant decrease (p<0.05) in muscle activity for the erector spinae, when compared to the same activity without the ADIM maintained. Furthermore, maintaining the ADIM contraction during descent led to a significant increase (p<0.05) in muscle activity for the rectus abdominis, external abdominal oblique, transversus abdominis, vastus medialis, and vastus lateralis, but a significant decrease (p<0.05) in muscle activity for the erector spinae, when compared to the same activity without the ADIM maintained. Conclusion: As a result of this study, it maintains the ADIM and reduces lumbar muscle activity at the waist and increases muscle activity in the legs when walking on a ramp. Therefore, maintaining the ADIM contraction during ramp walking is recommended as training to improve the function of patients' muscular skeleton.

The purpose of this study was to compare the electromyography signal's power spectrum mean and median tendencies appearing in the lower extremity during walking while wearing roller shoes. 9 male subjects volunteered who have no experience riding inline-skate or roller-skate, and have no record of musculoskeletal disorder. Subjects walked on treadmill twice for an hour (Once a week, one trial with the roller on and the other without roller, Walking velocity = 1.39 m/s). Electromyography was measured every 15 minute (0, 15, 30, 45, 60 minutes). Surface electrode sticked muscle at rectus femoris(R.F.), vastus lateralis(V.L.), vastus medialis(V.M.), biceps femoris(B.F.), tibialis anterior(T.A.), gastrocnemius lateralis(G.L.), gastrocnemius medialis(G.M.). At Rectus femoris, Vastus Lateralis, Vastus medialis, and Biceps femoris showed no statistically significant decrease of median frequency or mean edge frequency as time passes. Also, between two treatments (wearing the roller shoes vs not wearing the roller shoes), no statistically significant difference. After 60 minutes, mean edge frequency showed statistically significant decrease at tibalis anterior and after 45 minutes, mean edge frequency showed statistically significant decrease compared to wearing roller shoes without the wheels at gastrocnemius lateralis. At gastrocnemius medialis after 30 minutes, median frequency showed statistically significant decrease, and showed statistically significant difference compared to the control group. Wearing the roller shoes with wheels for a long time resulted in statistically significant decrease of mean edge frequency and median frequency in lower extremity, especially in shank muscles. Increase of wearing time of roller shoes and walking on a bumpy road wearing roller shoes with wheels result fatigue and thus, danger of injury.
The purpose of this study was to investigate the functional changes of the vastus medialis while the neuromuscular electrical stimulation(NMES) was performed in elderly women with knee osteoarthritis. The subjects of this study were 30 women who were diagnosed degenerative arthritis, 15 in the experimental group and 15 in the control group. The experimental group performed conventional physical therapy and neuromuscular electrical stimulation, and the control group received only conventional physical therapy, five times per week for four weeks. Outcome measures were assessed before and after 4weeks using the VAS(pain), quadriceps femoris strength, thigh circumference, and Q-angle. The data of the quadriceps femoris strength and thigh circumference were significantly increased in experimental group(p<0.05). The data the pain was significantly decreased in both the two groups(p<0.05). There were statistically significant differences in quadriceps femoris strength and thigh circumference between the two groups(p<0.05). As the result, the application of neuromuscular electrical stimulation of the vastus medialis could be more effective for decrease pain and improving muscle function in patients with knee osteoarthritis.

Objective: Physiologically, the vastus medialis muscle is the first muscle to undergo muscle atrophy, and it was thought that pain in patients with knee osteoarthritis could be reduced if this muscle could be strengthened and stabilized. The purpose of this study was to prove the effectiveness in knee osteoarthritis using polydioxanone sutures that have been tried in other musculoskeletal areas. Method: Forty knee osteoarthritis patients voluntarily participated in the study, and divided into 30 polydioxanone suture needle (MEST-B2375 produced by Ovmedi Co.) and 10 sham needle (without suture). And the needles were inserted into the vastus medialis muscle. In all patients, safety evaluation including blood tests and ultrasonography as well as efficacy evaluation including isometric maximal contractile strength of quadriceps muscle, weight bearing pain, impression of change, quadriceps angle, rescue drug intake were evaluated up to 30 weeks after the procedure. Results: Isometric maximal contractile strength showed a significant improvement at 4 weeks after the procedure in the polydioxanone suture group, and the weight-bearing pain showed a significant improvement at every visit in the polydioxanone suture group compared with baseline values. Patient global impression of change score showed significant improvement at 20 and 30 weeks, and clinical score showed improvement at every visit. Conclusion: Insertion of polydioxanone sutures showed improvement in muscle strength and knee pain by supporting and fixation of the vastus medialis muscle in patients with degenerative knee osteoarthritis. Insertion of polydioxanone sutures is considered to have a therapeutic effect in knee osteoarthritis patients.

■ Objectives The purpose of the this study was to investigate organization change(pennation angle) of lower extremity muscle in elderly with knee osteoarthritis through ultrasonic image. Also, we examined organizational change of muscle was by influenced after total knee replacement(TKR). ■ Methods Rectus femoris image was taken at 50% of the distance between the anterior superior iliac spine and the superior border of the patella. Vastus medialis image was taken at the most distal insertion level of vastus medialis on the medial border of the patella. measurements included the most distal insertion level of the vastus medialis on the medial border of the patella. ■ Results Change of pennation angle in rectus femoris showed decrease from 5.11° in preoperation to 4.29° in 1 week after operation. Change of pennation angle in vastus medialis showed decrease from 6.94° in preoperation to 5.76° in 1 week after operation. ■ Conclusion We consider decrease of pennation angle after total knee replacement in this study was casued by organizational change of muscle and this pennation angle was influenced force created by muscle. Therefore, we thinks that this pennation angle through ultrasonic image will be available to plane individual muscle exercise program and to confirm muscle mass by muscle strength training.
