Comparison of Vessel Elasticity according to Risk Factors for Coronary Artery Disease, and the Mediating Effects of Treatment Compliance among Patients with Percutaneous Coronary Intervention (관상동맥질환 위험정도와 혈관탄성의 관계에서 치료지시이행의 매개효과: 경피적 관상동맥 중재술 환자 대상)

  • Purpose: This study aimed to provide basic data for vascular health of patient who underwent percutaneous coronary intervention (PCI) by verifying the mediating effect of compliance in the relationship between risk level of coronary artery disease (CAD) and blood vessel elasticity. Methods: This is a descriptive study with 115 patients, who underwent the PCI a year ago and visited in the cardiology department from January to March, 2015. The risk level of CAD, blood vessel elasticity and the compliance were measured. For data analysis, SPSS/WIN 21.0 and AMOS (IBM) 21.0 were used. Results: There were a positive correlation with blood vessel elasticity score (i.e. inelasticity of the blood vessel wall) (r=.189) and a negative correlation with compliance (r=-.658) in mediating effect of risk level of CAD. There was a negative correlation between compliance and blood vessel elasticity (r=-.482). The direct effect (${\beta}=-.226$), indirect effect (${\beta}=.415$) and total effect (${\beta}=.186$) of mediating effect of risk level of CAD on blood vessel elasticity were significant. Compliance had a partial mediating effect of risk level of CAD on blood vessel elasticity. Conclusion: The results of this study suggest that managing and preventing moderating effect of risk level of CAD on compliance is helpful in restoring blood vessel elasticity.

Effects of a Four-week Clothing Program for Improving Vascular Compliance on Heat Tolerance (혈압 관리를 위한 4주간의 착의훈련이 고혈압 전단계자의 내열성에 미치는 영향)

  • This study was performed on 5 prehypertensive male participants to investigate the effects of the clothing program for improving the vascular compliance on heat tolerance. The clothing program means the alternate stimulation of the temperature using clothes. The participants wore two different garments with $1.5^{\circ}C$ difference in the temperature inside clothing in a climatic chamber ($18.8{\pm}0.2^{\circ}C$, $38{\pm}3%RH$) alternately for 4 weeks. Heat tolerance tests were conducted in the climatic chamber of $35.2{\pm}0.5^{\circ}C$, $54{\pm}3%RH$ before and after the clothing program. The results were as follows. The $\overline{T}$sk, Tr and heart rate were lower in the post test than in the pre test (p<.01). The whole body and local sweat rates as well as systolic and diastolic blood pressures had reduced the tendencies in the post test. Participants felt less wet and more comfortable in the post test than in the pre test(p<.01). These results showed that the clothing program through the alternate stimulation of the temperature positively affected the improvement of heat tolerance.

The Effect of Social Support on Compliance with Sick Role Behavior in Hypertensive Clients and Duration-of the Effect for up to 6 months (사회적 지지가 고혈압환자의 역할행위 이행에 미치는 영향과 지지요법 효과의 지속에 관한 연구 I)

  • This study was done using a Quasi-experimental research design to determine the effects of social support on compliance with sick role behaviors in hypertensive patients and to know if the effect of the social support on compliance lasted for at least 6 months. The subjects consisted of 81 hypertensive patients who were registered in the Cardio-Vascular OPD at Chonnam National University Hospital. They were divided by random sampling into 42 people for the experimental group and 39 for the control group. Data were gathered from June 3, 1996 to June 10, 1997 through individual interviews using a structured questionnaire. The results of the study were summarized as follows : 1. Compliance with sick role behaviors in hypertensive clients was significantly increased in the experimental group who received social support from the nurse as compared to the control group who did not receive social support(t=15.99. p<.001). 2. The effect of social support on compliance with sick role behaviors in hypertensive clients lasted for 6 months(t=7.99, p<.001). 3. Four of six people stopped smoking in experimental group after the intervention of social support, but none of the five in control group were able to stop smoking. Fisher's Exact test showed a significant difference between the experimental and control group(x²=4.385. p< .05). Mantel Haenszel test showed that the effect of social support on stopping smoking in the experimental group lasted for six months because there were no significant differences between one month after the social support and six months after, in the number of subjects who stopped smoking(x²=1.154, P>.05). Finally, social support was effective on compliance with sick role behaviors and stopping smoking in the hypertensive clients, and the effect of social support on compliance lasted for 6 months.

Impact of Pulmonary Vascular Compliance on the Duration of Pleural Effusion Duration after Extracardiac Fontan Procedure (수술 전 폐혈관 유순도가 심장 외 도판을 이용한 Fontan 수술 후 늑막 삼출 기간에 미치는 영향)

  • Background: Preoperative risk analysis for Fontan candidates is still less than optimal in that patients with apparently low risks may have poor surgical outcome; prolonged pleural drainage, protein losing enteropathy, pulmonary thromboembolism and death. We hypothesized that low pulmonary vascular compliance (PVC) is a risk factor for prolonged pleural effusion drainage after the Fontan operation. Material and Method: A retrospective review of 96 consecutive patients who underwent the Extracardiac Fontan procedures (median age: 3.9 years) was performed. Fontan risk score (FRS) was calculated from 12 categorized preoperative anatomic and physiologic variables. PVC $(mm^2/m^2{\cdot}mmHg)$ was defined as pulmonary artery index $(mm^2/m^2)$ divided by total pulmonary resistance $(W.U{\cdot}/m^2)$ and pulmonary blood flow $(L/min/m^2)$ based on the electrical circuit analogue of the pulmonary circulation. Chest tube indwelling time was log-transformed (log indwelling time, LIT) to fit normal distribution, and the relationship between preoperative predictors and LIT was analyzed by multiple linear regression. Result: Preoperative PVC, chest tube indwelling time and LIT ranged from 6 to 94.8 $mm^2/mmHg/m^2$ (median: 24.8), 3 to 268 days (median: 20 days), and 1.1 to 5.6 (mean: 2.9, standard deviation: 0.8), respectively. FRS, PVC, cardiopulmonary bypass time (CPB) and central venous pressure at postoperative 12 hours were correlated with LIT by univariable analyses. By multiple linear regression, PVC (p=0.0018) and CPB (p=0.0024) independently predicted LIT, explaining 21.7% of the variation. The regression equation was LIT=2.74-0.0158 PVC+0.00658 CPB. Conclusion: Low pulmonary vascular compliance is an important risk factor for prolonged pleural effusion drainage after the extracardiac Fontan procedure.

A Simultaneous Fluid -Structure Modeling of the End-to-End Vascular Graft Anastomosis (인조혈관의 단단문합에 대한 유동과 구조의 동시해석)

  • 수축절정기에 0.2mm, 0.4mm 두께의 소구경 인조혈관의 내경3.2mm의 동맥에 문합한 단단문합 주위의 유동과 구조해석을 동시에 수행하였다. 유동해석 결과 속도분포 및 벽단전률은 타 연구결과와 매우 유사하며 이론해와 매우 유사함을 보였다. 유동에 의해서 문합부 주위에 작용하는 응력은 문합부에 집중되며 이는 수십만 pa에 달했다. 또한 인조혈관과 동맥에 작용하는 원주방향의 응력을 이론해와 비교한 결과 서로 유사함을 보였으며 두께가 얇은 인조혈관을 사용하는 경우 문합부의 compliance mismatch 는 개선되나 반대로 응력은 더 많이 받음을 알 수 있었다. 본 연구를 통해서 유체에 의하여 혈관이 영향을 받는 심혈관계 현상을 연구하는데 있어서 이와 같은 유체-구조 상호작용을 고려하여 동시에 해결 하는 방법은 매우 유용할 것으로 생각된다.

Prediction Equation for Post-Cessation Weight Gain in Men (남성에서 금연 후 체중 증가 예측을 위한 공식)

  • The purpose of this study is to develop a formula for predicting weight gain after six months of smoking cessation in men. The subjects are 412 men who succeeded in quitting smoking for 6 months at public health center smoking cessation clinic. They have been undergone nicotine patch therapy and weekly counseling for 8 weeks. The final success of smoking cessation has been confirmed by urinalysis. I have measured body composition and vascular compliance before and after the program. Weight(0.98) and BMI(0.85) have shown high positive correlation. The prediction is as follows. Post Weight(kg) = 1.04636 * Pre-Weight - 0.19535 * Pre-BMI + 4.43528. The explanatory power of this estimation equation is 82.46%(<.0001). Based on these results, it is necessary to develop education and programs for effective counseling of the smoking cessation clinic. In addition, research on women is needed.

Effect of Boxing Aerobic Dance on Body Composition, Blood Component and Vascular Compliance in Obese Middle Aged Women (복싱에어로빅 참여가 비만 중년여성의 신체조성, 혈액성분 및 혈관탄성에 미치는 영향)

  • This study was conducted to test the effects of participation in 12-week program of boxing aerobics by obese middle-aged women on their body composition, blood constituents, and vascular compliance The samples are the middle-aged women in their forties or more, who has 30% or more body fat percentage, but has no medical history in cardiovascular disorders or metabolic diseases. The samples are divided into 8 of exercising group, and 8 of control group by random assignment. The intensity of boxing aerobics was HRmax 50% for the initial 4 weeks, HRmax 60% for the 5th to 8th week, and HRmax 70% for the 9th to 12th week. Each session took 60 minutes. The result is as follows. First, as a result of participating in the boxing aerobics program, body weight, body fat percentage, and muscle showed significant differences depending on the measuring period and the interacting term of the group and measuring period(p<.001), and the result of t-test on the sample matched to each group's measuring time also showed the significant increase or decrease in the exercising group(p<.001). Second, as a result of participating in the boxing aerobics program, the exercising group's TC, TG, HDL-C, and LDL-C, all showed significant differences in accordance with each group, measuring time, and the interaction term between groups and measuring time(p<.01, p<.001), and the result of t-test on each group's samples matched to the measuring time also shows significant increase or decrease in the exercising group(p<.01). Third, as a result of participating in the boxing aerobics programs, the vascular compliance of right hand, left hand, right hand, and left hand showed significant differences in accordance with each group, measuring time, and the interacting term between the measuring time and the group(p<.001), the t-test results of the samples matched to the each group's measuring time also showed significant differences in the exercising group(p<.001). To summarize the results above, it is suggested that the 12-week boxing aerobics program can improve body composition, blood constituents, and the blood circulation, which may prevent or enhance relevant diseases such as cardiovascular disorders.

Early dropout predictive factors in obesity treatment

  • Diet attrition and failure of long term treatment are very frequent in obese patients. This study aimed to identify pre-treatment variables determining dropout and to customise the characteristics of those most likely to abandon the program before treatment, thus making it possible to modify the therapy to increase compliance. A total of 146 outpatients were consecutively enrolled; 73 patients followed a prescriptive diet while 73 followed a novel brief group Cognitive Behavioural Treatment (CBT) in addition to prescriptive diet. The two interventions lasted for six months. Anthropometric, demographic, psychological parameters and feeding behaviour were assessed, the last two with the Italian instrument VCAO Ansisa; than, a semi-structured interview was performed on motivation to lose weight. To identify the baseline dropout risk factors among these parameters, univariate and multivariate logistic models were used. Comparison of the results in the two different treatments showed a higher attrition rate in CBT group, despite no statistically significant difference between the two treatment arms (P = 0.127). Dropout patients did not differ significantly from those who did not dropout with regards to sex, age, Body Mass Index (BMI), history of cycling, education, work and marriage. Regardless of weight loss, the most important factor that determines the dropout appears to be a high level of stress revealed by General Health Questionnaire-28 items (GHQ-28) score within VCAO test. The identification of hindering factors during the assessment is fundamental to reduce the dropout risk. For subjects at risk, it would be useful to dedicate a stress management program before beginning a dietary restriction.

Development of Porous polyurethane Arterial-Venous Shunt by Thermal Control (온도 조절을 통한 다공성 폴리우레탄 동정맥 누관의 개발)

  • A technique or the preparation of porous polyurethane vascular prostheses was investigated. Small-diameter vessels are not in general clinical use due to their limited long-term biocompatibility and low patency rates in experimental trial. These limits are mainly due to the failure of mechanical unction of the vascular grafts. This failure has been suggested to result partially from compliance mismatch. The long-term patency is considered to depend critically on the properties of the material and the fabrication process of the graft. So the control of pores is very important and main points to develop a available vascular grafts. Two-kind polymer sheets was compared. One was the porous PU-sheet made at room temperature by the solvent/non-solvent exchange. And the other was the porous PU-sheet which was fabricated by thermal phase transition and solvent-/non-solvent exchange using the thermal controller. The polymer sheets had a uniform pore size and pore occupation. According to the result of the above experiments, polyurethane solution was injected into a mold designed or U-type tube. The average pore size and pore occupation were easily changed by changing polyurethane concentration, freezing temperature, and methods. This technique can give a proper pore size ($10{\sim}45\;{\mu}m$) or tissue in growth, and suitable compliances or matching with arteries and veins. Besides, the fabrication of more complicated shaped vessels such as the U-type vascular grafts is easily controlled by using the fixed mold. this method might give a desired compliant graft or artificial implantation with the presently valid medical polymers.
