Objectives: This study was intended to check if the "Creating Clean Wards" project, which is an innovative reinforced campaign activity targeting infection control strategies and active surveillance cultures for VRE (vancomycin resistant enterococci) high-risk patients to be admitted in the NS (neuro-surgery) wards, would be reduced the incidence rates of VRE acquisition, transmission rates. Methods: 75 subjects of the VRE high-risk patients were surveyed by carrying out active surveillance cultures of VRE colonization 11 times from January to March, 2012. And the retrospective study was conducted dividing them into two groups. Results: The incidence rates of VRE acquisition was reduced to 3.67 cases per 1,000 patients day in the control group and to 2.88 cases in experimental group, which was not statistically significant (p = .753). VRE transmission rates of 0.0015 per day before the project tended to increase to 0.0019, although not statistically significant (p = .650). As a result of multivariate analysis with regard to using glycopeptide antibiotics in order to find out risk factors of VRE colonization, the patients who had been treated with glycopeptide until VRE colonization showed 274.41 times higher rate. Conclusion : For effective VRE infection control in NS wards, We should carry out active surveillance culture regularly, especially patient of using glycopeptide. And block the spread of VRE by strengthening infection control through the strict isolation and the changed mind-set of members motivated by the "Creating Clean Wards" campaign.
Objectives: We treated a stroke patient with vancomycin-resistant enterococci (VRE) colonization using Gami-sipjeondaebo-tang. Methods: A patient diagnosed with intracranial hemorrhage with VRE colonization was treated with herbal medicine and acupuncture. We checked general conditions every day and performed stool VRE culture once a week. We evaluated the improvement of symptoms by change in VAS grade, general weakness, and stool VRE culture results. Results: After a treatment of Gami-sipjeondaebo-tang combined with acupuncture therapy, improvement of the general condition was observed. Also, VRE colonization was no longer detected in the stool culture. Conclusions: This case report proved the effect of Korean medicine for a stroke patient with VRE colonization, but further study is needed.
Hospital-acquired bacterial infections, including vancomycin-resistant enterococci (VRE) infection (MIC: 32 mcg/ml), are common. We describe the case of a 63-year-old female patient with subarachnoid hemorrhage and VRE infection treated with Jashin-bowon-tang and Samhuang-sashim-tang for 57 days and 22 days, respectively. The therapeutic effect was assessed weekly via culture, color and viscosity of pus from a coccyx sore, and C-reactive protein (CRP). Vital signs were checked four times a day. Seventeen days after treatment, VRE was not colonized in patient and color and viscosity of pus, CRP and vital sign were improved. This case report suggests that Jashin-bowon-tang and Samhuang-sashim-tang might be an alternative option for VRE infection patients reducing the need for extended isolation periods and speeding up recovery times.
The intestinal microbiota are important to the host with regard to resistance they impart against bacterial infections and their involvement in mediating metabolic functions. Lactic acid producing bacteria such as Lactobacillus play an important physiological role in these matters. The aim of the present study was to isolate Lactobacillus sp. that inhibits enteric pathogens. Initially, 17 isolates from healthy Koreans were collected on Lactobacillus selective medium. Resistance of the isolates to antibiotics including rifampicin, streptomycin, clindamycin and vancomycin was measured. One of the isolate was identified as Lactobacillus ruminus on the basis of bacterial cell morphology, cultural characteristic and biochemical characteristics, 16S rRNA sequence analysis and PCR-RAPD. Antimicrobial activity of the bacterium against Vancomycin Intermediate Resistant Staphylococcus aureus (VISA) and Vancomycin-Resistant Enterococci (VRE) was measured. About $10^4$ cells of VISA or VRE were mixed with 1, 5, and 9 mL of L. ruminus SPM 0211 and the final volume was adjusted to 10 mL with brain heart infusion (BHI) broth. The cell suspension was incubated for 3, 6, 9, and 24 h, serially diluted and then plated on BHI agar plates. As numbers of L. ruminus SPM 0211 were increased, viable cell count of VISA and VRE decreased. The strongest antimicrobial activity of SPM 0211 was observed after 9 h incubation in any mixture, almost completely inhibiting the growth of these two bacteria. The results suggest that the freshly isolated L. ruminus SPM 0211 may be used as a pro-biotic microbe that prevents the colonization of enteric pathogens and can thereby promote good gastrointestinal health.
We developed a simple and rapid method for detecting vancomycin resistance genes, such as vanA and vanB, using loop-mediated isothermal amplification (LAMP). To identify not only vancomycin resistance genes, but also the genus Enterococcus, primers were designed for vanA, vanB, and 16S rRNA. Screening for vancomycin susceptibility in Enterococcus was performed using Etest (bioMérieux Inc). The results of the LAMP assay were compared to those of real-time RT-PCR. The optimal conditions for the LAMP assay were 65℃ for 60 min. The detection limits of the LAMP assay for vanA, and vanB were 2 × 102 copies/reaction. Compared to RT-PCR, the sensitivities and specificities of LAMP for 16S rRNA, vanA, and vanB were 100/100%, 100/100%, and 100/100%, respectively. The vanA genotype-vanB phenotype accounted for 57.5% (46/80) of the vancomycin-resistant Enterococci samples collected from 2016 to 2019. In conclusion, the LAMP assay developed in this study showed high sensitivity and specificity for vancomycin-resistant genes. Moreover, due to the simplicity and rapidity of the LAMP assay, its use can be very useful in clinical microbiology laboratories.
목 적 : 1986년 vancomycin 내성 장구균(이하 VRE)이 보고된 이후 10여년간 빈도는 급격한 증가 추세에 있다. 이에 저자 등은 본 병원에 입원한 소아환자들의 임상검체에서 분리된 VRE의 양상과 해당 환자들의 사용형태에 대한 조사를 시행하여 VRE의 증가양상에 적절히 대처할 수 있는 방안을 모색하고자 본 연구를 시행하였다. 방 법 : 1998년 1월부터 2000년 12월까지 연세의료원 세브란스병원에 입원한 소아 환자에서 분리된 VRE 36주를 대상으로 하여 검체 종류 및 균종, 항균제 감수성과 사용양상을 조사하였고, ICU 환자군과 Non ICU 환자군으로 분류하여 두 군간에 있어서 수술이나 중재적 시술을 시행한 경우와 내과적인 투약처치만을 시행한 경우 등의 분리 전 처치형태를 조사하였고 항균제 감수성을 파악하였으며 중복감염의 여부가 ICU군과 Non ICU군간에서 차이를 보이는지 조사하였다. 결 과 : 본 연구에서, VRE가 분리된 수가 급속히 증가함을 알 수 있었고, ICU 환아군과 수술이나 중재적 시술을 한 경우에서 높은 빈도를 보였으며, E. faecium이 E. faecalis 보다 많이 분리되었다. VRE 항균제 감수성 검사에서 tetracyclin을 제외한 항생제에 높은 내성을 지니고 있었고, 검출된 cephalosporin의 투여비율이 가장 높았으며 검출 후에는 teicoplanin의 투여비율이 증가함을 볼 수 있었고, teicoplanin에 대한 항균제 감수성 결과에 따른 투여비율의 차이는 없었다. 또한, ICU 환자군이 Non ICU 환자군에 비해 중복감염의 경우가 많았다. 결 론 : 증가하고 있는 VRE의 집단 발생과 전파에 이은 합병증을 막기 위해서, 본 연구에서 ICU 단위로 분류하였던 병동과 같은 병원내 VRE의 상재 위험지역에 대한 집중 감시조사와 이의 관리를 위한 여러 행정적 문제의 해결, 적절한 항생제의 사용과 철저한 관리가 필요하리라 사료된다.
목 적 : 병원 입원환자의 임상 검체에서 vancomycin-resistant enterococci(VRE)의 출현은 1980년대 후반부터 증가하기 시작했으나 국내에서 신생아에 대한 보고는 드물다. 최근 본 병원 신생아 중환자실에서 신생아 대변내 VRE 검출이 증가되어 연도별 발생추이와 위험인자를 알아보고자 하였다. 방 법 : 2000년 1월부터 2004년 12월까지 전남대학교병원 신생아 중환자실과 소아과 병동에 입원한 교정연령 4주 미만의 신생아들 중 대변 배양 검사가 시행된 환아 294명에 대해 병력 기록지를 후향적 조사하였다. VRE의 연도별 분리빈도를 조사하였고 대변 배양검사상 VRE 양성인 군을 연구군, VRE 음성인 군을 대조군으로 하여 VRE 집락형성의 위험인자 및 예후 등을 조사하였다. 결 과 : 전체 신생아 환아 중 대변 내 VRE 검출률은 2000년 0명, 2001년 1명(3.7%), 2002년 74명(60.7%), 2003년 52명(38.2%)으로 최근 들어 증가하는 추세이며 2004년에는 2명(0.8%)으로 줄어들었다. 평균 재태연령은 VRE군 $237.9{\pm}28.7$일, Non-VRE군 $265.7{\pm}18.7$일로 VRE군에서 의의있게 짧았다(P<0.001). 출생체중은 VRE군이 $2,124.9{\pm}893.8g$으로, Non-VRE군 $3,023.8{\pm}718.0g$에 비해 유의하게 작았고(P<0.01), 입원기간도 VRE군이 $30.5{\pm}26.5$일로 Non-VRE군의 $12.3{\pm}11.0$일에 비해 길었다(P<0.001). Vancomycin 사용 빈도는 VRE군이 7.0%로 Non-VRE군 1.2%에 비해 높았고(P<0.05), cepalosporin 사용 빈도는 VRE군이 68.5%로 Non-VRE군 48.5%에 비해 더 높았다(P<0.001). VRE 감염으로 인한 직접적인 사망례는 없었다. 결 론 : 신생아 대변내 VRE 감시 배양 결과 연도별로 VRE 검출이 증가하는 추세였는데, 재태연령이 짧을수록, 출생체중이 작을수록, 3세대 cephalosporin 사용 또는 vancomycin 사용이 잦을수록 의의있게 증가하였다. VRE의 발생을 막기 위해서는 극소 저출생 체중아에 대한 무균적 간호와 불필요한 항생제 남용을 줄여야 할 것으로 사료된다.
Purpose: The purpose of this study was to develop a web-based education program on control vancomycin-resistant enterococci (VRE) infections and to identify the effects of the program on knowledge and performance of nursing students. Methods: The web-based VRE infection control education program was developed by using the network-based instructional systems design model. The nursing students in the experimental group could access this web-based education program at any time, and as many times as they wanted, during the clinical training period. Effects were evaluated by assessing knowledge and performance of VRE infection control measures during the clinical training period. Results: The contents of the education program included diagnosis, transmission, and treatment of VRE, contact precautions, hand washing, personal protective equipment, environment management, and quizzes. The lecture portion was filmed in a virtual screen studio using flash animation, video, and sound effects, and it was uploaded on an internet site. The knowledge and performance scores of the experimental group after using the education program were significantly higher than those of the control group. Conclusion: The results suggest that the web-based VRE infection control education program is an effective educational method to enhance knowledge and performance of VRE infection control measures.
Objectives: An ambulance can be a potential source of contagious or droplet infection of a community. We estimated the prevalence of positive carriage of tuberculosis (TB), methicillin-resistant $Staphylococcus$$aureus$ (MRSA), and vancomycin-resistant $Enterococci$ (VRE) in patients transported by ambulance. Methods: This was a retrospective observational study. We enrolled all patients who visited a tertiary teaching hospital emergency department (ED). Blood, sputum, urine, body fluid, and rectal swab samples were taken from patients when they were suspected of TB, MRSA, or VRE in the ED. The patients were categorized into three groups: pre-hospital ambulance (PA) group; inter-facility ambulance (IA) group; and non-ambulance (NA) group. Adjusted odds ratio (OR) and 95% confidence intervals (CI) were calculated using a multivariable logistic regression model for the prevalence of each infection. Results: The total number of patients was 89206. Of these, 9378 (10.5%) and 4799 (5.4%) were in the PA and IA group, respectively. The prevalence of TB, MRSA, and VRE infection were 0.3%, 1.1%, and 0.3%, respectively. In the PA group, the prevalence of TB, MRSA, and VRE were 0.3%, 1.8%, and 0.4%. In the IA group, the prevalence of TB, MRSA, and VRE were 0.7%, 4.6%, and 1.5%, respectively. The adjusted ORs (95% CI) of the PA and IA compared to the NA group were 1.02 (0.69 to 1.53) and 1.83 (1.24 to 2.71) for TB, 2.24 (1.87 to 2.69) and 5.47 (4.63 to 6.46) for MRSA, 2.59 (1.78 to 3.77) and 8.90 (6.52 to 12.14) for VRE, respectively. Conclusions: A high prevalence of positive carriage of TB, MRSA, and VRE in patients transported by metropolitan ambulances was found.
