A Literature Review on Obesity in Elderly (노인비만의 임상적 의의에 대한 고찰;2002년 이후 국내 및 국외 논문을 중심으로 분석)

  • Objectives : Obesity causes serious medical complications and impairs quality of life in older persons. We review the literature on the clinical issues related to obesity in elderly and suggest appropriate weight-management guidelines for obese older patients. Methods : PubMed Database and RISS were searched for articles relating to obesity in the elderly. We classified articles according to clinical characteristics, assessments, medical complications, and therapy. Results and Conclusions : The current data show that obesity in elderly is associated with increased mortality, metabolic abnormalities, arthritis, pulmonary abnormalities, urinary incontinence, cancer, and impaired quality of life. Therefore, clinical studies are needed to demonstrate the effectiveness andsafety of weight-loss treatments in obese elderly.

A Comparison of Sward Types on the Intake and Nutrients Utilization of Herbage by Korean Native Goats (산양에 의한 초지유형별 목초의 섭취량 및 영양가치 이용성 비교)

  • The objective of this experiment was to compare the dry matter intake, nutrients digestibility, nitrogen and energy utilization of hehage among the orchardgrass(OG) pure, orchardgrass(OG)-red clover(RC) mixture and complex rnixtures(C. mix) by Korean native goat. The results obtained were surnmerized as follows: 1. Crude protein(CP) content was higher in OG-RC and C. Mix diets than in OG pure diets(P<0.05). NDF content was higher in OG pure diets than other diets(P<0.05). But ADF, Lignin contents and gross energy did not differ among all diets. 2. Dry matter intake by Korean native goat tend to be high in OG-RC diets, and low in OG pure diets. But there was no significant difference statistically. Dry matter and cellular constituents digestibilities were higher for OG-RC and C. Mix diets than OG pure diet, but CP and NDF digestibilities were no significant difference among diets. 3. The amount of nitrogen consumed amount by Korean native goat in the OG-RC diet was higher than other diets. But, due to largely relative urinary and fecal nitrogen loss, Apparrently digested nitrogen and retained nitrogen percents were not significant difference among diets. 4. The amount of energy consumed by Korean native goat in OG-RC diet was higher than other diets. But, due to largely relative urinary and fecal energy loss, digestable energy percent was lower than C. Mix diets. But.metabolic energy percent was no significant difference among diets. Therefore, there was no significant differences among sward types for the utilization of nitrogen and energy by Korean native goats.

  • Bartter syndrome (BS) is an autosomal recessively inherited rare renal tubular disorder characterized by hypokalemic metabolic alkalosis and hyperreninemic hyperaldosteronism with normal to low blood pressure due to a renal loss of sodium. Genetically, BS is classified into 5 subtypes according to the underlying genetic defects, and BS is clinically categorized into antenatal BS and classical BS according to onset age. BS type I is caused by loss-of-function mutations in the $SLC12A1$ gene and usually manifests as antenatal BS. This report concerns a male patient with compound heterozygous missense mutations on $SLC12A1$ (p.C436Y and p.L560P) and atypical clinical and laboratory features. The patient had low urinary sodium and chloride levels without definite metabolic alkalosis until the age of 32 months, which led to confusion between BS and nephrogenic diabetes insipidus (NDI). In addition, the clinical onset of the patient was far beyond the neonatal period. Genetic study eventually led to the diagnosis of BS type I. The low urinary sodium and chloride concentrations may be caused by secondary NDI, and the later onset may suggest the existence of a genotype-phenotype correlation. In summary, BS type I may have phenotype variability including low urine sodium and chloride levels and later onset. A definitive diagnosis can be confirmed by genetic testing.

Relationship between Zinc Status and Obesity of Type 2 Diabetic Women ($\cdot$노년 당뇨병여성의 아연영양상태와 비만도와의 관련성)

  • Zinc is known to have important effects on insulin activity and to increase the body fat deposition. The purpose of this study was to investigate the zinc status and obesity in 50 type 2 diabetic women visiting public health center and hospital. The mean age was 57.9 $\pm$ 6.9 years old. The mean of diabetic duration was 8.0 $\pm$ 6.5 years. Body mass index (BMI) of diabetes was $23.2 \pm 2.3 kg/m^2$. There were no significant differences of mean age, anthropometric indices, and insulin level other than fasting blood sugar (p < 0.001) and insulin resistance (p < 0.00l) between diabetes and control group. The obesity ratio of diabetes was $20\%$, $66\%$ and $84\%$ for BMI, waist circumference and waist/hip ratio (WHR), respectively. Plasma zinc level was not significantly different between diabetes and control group. However, urinary zinc excretion of diabetes was approximately twice of control group (p < 0.001). Urinary zinc loss was fivefold higher in the hyperglycemia ($HbA_{lc}> 10\%$) than in normal blood glucose (p < 0.001). Anthropometric indices were decreased in hyperglycemia. On the other hand, there were the tendency of increased urinary zinc in obese group for waist circumference and percent of body fat. These results suggested that controlled normal blood glucose could improve hyperzincuria and anthropometric changes in type 2 women diabeties.

The Effect of Dietaty Protein Level on the Ca and Bone Metabolism in Ovariectomized Rat (난소를 절제한 흰쥐에서 식이 단백질 수준이 체내 Ca 및 골격 대사에 미치는 영향)

  • To study the effect of menopause and dietary protein level on Ca metabolism, ovariectomy (OVAX) and sham operations were performed in 16-week old female rats. Each treatment group was fed for 16 weeks either 5%(L) or 50%(H) casein diets, forming SH, SL, OH, OL groups. High protein groups(SH, OH) showed higher Ca and hydroxyproline excretion in urine. Urinay hydroxyproline was also higher in OVAX, which tells the possibility of increased bone resorption by OVAX and by high dietary protein. At 16th wee, however, urinary Ca and hydroxyproline of SH caught up with OH group, whereas those of OL remained higher than SL. Therefore it seems that high dietary protein overrides the effect of OVAX with time. Urnary protein measured at 8th week was higher in high protein groups, especially in OH. GFR was not differ significantly among groups at 8th week. At 16th week, however, high protein groups showed twice the GFR value of low protein groups. Therefore the increase of urinary Ca and hydroxyproline in SH and OH groups can be explained partly by the increased GFR. The tendency of increased GFR and urinary excretion of protein, Ca, and hydroxyproline was most obvious in OH group. It seems that the effect of high protein diet is likely to accelerated by ovariectomy. The effect of Ovax and dietary protein on the composition of femur, scapular, and lumbar bones, was not pronounced. However, when only the high protein groups were compared, OVAX resulted in the reduction of bone weight, ash and Ca contents, especially in femur. The reason that was no significant effect on bone might be due to the short experimental period to induce that was no significant effect on bone might be due to the short experimental period to induce the changes on bone composition and dietary Ca content used in this experiment may have been high enough to prevent bone loss.

Calcium and Phosphorus Balance Study by Soy Isoflavone Intake in Ovariectomized Rats (난소절제 흰쥐에서 대두 이소플라본의 섭취수준에 따른 Ca과 P의 평형 연구)

  • This study was performed to determine the effect of three different concentrations of soy-isoflavones on calcium and phosphorus balance in either sham-operated or ovariectomized female rats. Seventy-two 16-week old Sprague-Dawley rats underwent sham operation or bilateral ovariectomy. They were provided diets containing different levels of soy iso-flavones for 6 weeks: 50 ppm (Jow isoflavone intake; LI) , 250 ppm (medium isoflavone intake; MI) and 500 ppm (high isoflavone intake; HI). The subsequent fecal and urinary excretions of calcium and phosphorus were then measured. In the sham-operated rats, body weight gains and food efficiency ratio of the MI and HI groups were significantly lower than the control group while food intake was not different. However, there was no significant difference in the ovariectomized rats. The fecal excretion of calcium was significantly lower in the LI, MI and HI groups than the control group in sham operated rats, and significantly lower in the HI group than the control group in ovariectomized rats. Also, apparent ab-sorption rate of calcium and phosphorus did not show any significant difference among groups. Urinary excretion of calcium and phosphorus was significantly lower in the HI group than the LI group in the sham-operated rats. Urinary excretion of calcium was significantly higher in the control ovariectomized rats than in the control sham-operated rats. Retention of calcium and phosphorus did not show any significant difference in both groups. From the above result, we see that isoflavone intake increases calcium retention through an increase in calcium absorption and also suppresses the increase of calcium excretion in urine in ovariectomy. Therefore, it is suggested that isoflavone intake is recommended for menopausal women who experience sharp bone loss due to the decrease in estrogen honnone.

Effects of Dietary Methionine and Folate Supplementation in Ethanol-Fed Rats

  • Chronic alcohol consumption is associated with perturbation of hepatic metabolism of sulphur-containing amino acid. The goal of present study was to evaluate the influence of dietary supplementation of methionine or folate to chronically ethanol-fed mts on the metabolism of sulfur-containing amino acids and one-carbon metabolism. Sprague-Dawley male mts were fed Lieber-Decarli liquid diet with 0% ethanol (control), 36% ethanol (E), 36% ethanol combined with methionine supplement (EM) or folate supplement (EF) for 8 weeks. Hepatic S-adenosylmethionine (SAM) and S-adenosylhomocysteine (SAH), plasma folate and homocysteine (Hcy), urinary excretion of folate and formiminoglutamate were investigated after feeding experimental diets. Growth was retarded by 36% ethanol consupmtion (E, EM and EF) (p<0.01). Liver total fat (p<0.05) and plasma ALT (P<0.01) were increased by methionine supplementation (EM), implicating fatty liver and liver injury. Liver folate was increased slightly by folate supplementation (EF) (p=0.077). Urinary folate loss was increased 2.3 fold by ethanol consumption (E) and 17.2 fold by folate supplementation (EF), while decreased by methionine supplementation (EM) (p<0.000l). Plasma Hcy was increased 1.9 fold by methionine supplementation (EM) in ethanol-fed mts (p<0.05), which was related with decreased methionine synthase activity (p<0.05). Hepatic SAM/SAH ratio was depressed by methionine supplementation in ethanol-fed mts (EM) (p<0.05). Urinary formininoglutamate (Figlu) excretion after histidine loading was increased by ethanol ingestion and reduced by methionine supplementation (p<0.00l). Based on these data, methionine supplementation appears to accelerate histidine oxidation. In conclusion, dietary supplementation of methionine to ethanol-fed mts exacerbates alcoholic liver injury possibly by complicating sulphur-containing amino acid metabolism, as while it may have beneficial effects on folate and histidine metabolism.

Urine Analysis in Transgenic Mice Expressing the Growth Hormone-releasing Factor (성장호르몬 방출인자를 발현하는 형질전환 생쥐에서 소변분석)

  • The major urinary proteins(MUPs) of mice that bind hydrophobic molecules known as pheromones are regulated in part by the actions of growth hormone. The expression of the MUPs was therefore investigated in transgenic mice that express a human growth hormone-releasing factor gene from a metallothionein gene promoter(MT-GRF) and as a result have elevated growth hormone levels. MUPs were severely down-regulated in the urine of these animals compared to normal mice or to control transgenic mice expressing another gene(the inhibin a subunit) from the same metallothionein promoter(MT-Inh) and more MUPs disappeared in male mice than female ones. MUPs were also down-regulated in the urine of the UT-GRF-injected mice. In addition, it was observed that the urine of the MT-GRF mice included a high molecular weight protein that co-migrates with the major serum protein albumin, indicating an impairment in glomerular filtration within the kidney. The urinary loss of serum proteins was more severe in male MT-GRF mice than female ones. Thus the overexpression of human GRF mimics changes observed in MUP protein expression and glomerular function in other models of growth hormone hypersecretion with sex-dependent differential effects.

Intake/Balance of Dietary Protein in Korean College Student (한국인 대학생에게 식이성 단백질의 흡수 및 평형)

  • This study was conducted to obtain accurate data on intake, apparent digestibility and nitrogen balance of dietary protein which the korean take in habitually. Subjects were 8 male college students, aged from 20 to 26, and maintained their menu and life pattern as usual during a 4-week study. The same amount of diet that the subjects had consumped, and feces and urine were collected and measured to extract their nitrogen content data by Kjeldahl method. From above data, the apparent digestibility and the body nitrogen balance were estimated by determining daily protein intake and excretion. The daily protein intake measured by Jjeldahil method was 88.3$\pm$ 0.9g(1.45/kg of body weight /day) which marked 8.3% higher than that estimatd by food analysis table. The proportional of animal protein against total protein intake was 50.4$\pm$ 2.3%. Daily fecal protein loss was 14.3$\pm$0.6g and the apparent digestibility was 83.8$\pm$ 0.7%. The urinary nitrogen excretion was 10.30$\pm$0.19g. The nitrogen balance considering nitrogen excretion from feces indicated positive balance of 1.06$\pm$0.20g.

Case Report of the Neurogenic Bladder after Spine Surgery (수술 후 발생한 신경인성 방광 환자에 대한 치험 1례)

  • We treated a 51 year-old female patient of neurogenic bladder. The patient complained of urinary incontinence, lack of urine sense, loss of right leg sense etc. The symptoms developed right after spinal stenosis surgery. The symptoms didn't show any improvement continually. But after admission and treatment in our hospital, symptoms were improved from severe to mild and the score of Assesment of Bladder function got better. We continued to prescribe the herbal medicine over 8 weeks and could observe that symptoms were improved. This result suggests that oriental medical therapy is useful enough to treat the neurogenic bladder. Therefore further study of the approach and application of this treatment for neurogenic bladder would be necessary.
