A Pilot Study on the Development of Incontinence Panty for Senior Women

  • 본 연구는 50대 이상 여성을 대상으로 요실금 증상 및 요실금 제품에 대한 착용실태 및 구매실태, 선호도 등을 조사 분석하고자 하였다. 연구는 설문지법으로 이루어졌으며, 분석에는 SPSS 26.0 프로그램을 활용하였다. 요실금 패드와 팬티 구입 시 방수기능, 흡수속도 등이 중요한 것으로 인식되었다. 요실금 제품의 추가 희망 기능은 샘 방지 기능이 가장 많았고, 선호 색상은 살색이 많았다. 요실금 횟수가 많을 때 흡수기능, 요실금 양이 적을 때는 치수나 맞음새가 중요하였지만, 요실금 양이 많아지면 방수기능이 중요한 것으로 나타났다. 요실금 발생빈도가 높고 요실금 양이 많을 때 샘 방지 기능이 추가되기를 희망하였고, 팬티형 패드를 선호하였다. 분만 횟수가 많을수록 요실금의 양도 많았으며, 요실금 증상도 빈번하게 나타나는 것으로 분석되었다. 폐경 연령에 따라서도 요실금 양에 유의미한 차이를 나타내 조기 폐경 시 요실금 양이 많은 것을 알 수 있다.

  • Purpose: This study was performed to investigate the effects of lower urinary tract symptoms on depression and quality of life among women in urban areas. Methods: The participants in this study were 306 women in P city. Data were collected using a questionnaire from May 1 to June 6, 2008. Lower urinary tract symptoms (LUTS) were measured with BFLUTS-SF and categorized into voiding, filling and incontinence. The data were analyzed through t-test, one-way ANOVA, Fisher exact test, and multiple regression. Results: Age, filling and incontinence, and all of the subcategories of LUTS explained 16% of variance in depression. Older women, LUTS with incontinence, filling and incontinence, and all the problems including filling, voiding and incontinence lowered the quality of life. Conclusion: In order to develop nursing interventions for women with urinary tract symptoms, comprehensive understanding of LUTS is needed.

  • Purpose: This study was done to examine the effect of a urinary incontinence management program on physical symptoms, daily life problems, and sexual life problems for women with mixed urinary incontinence. Methods: This study was conducted using a one group pre-test and post-test design. For the survey, 28 women having mixed urinary incontinence were chosen by convenience sampling and agreed to participate in this study. Their physical symptoms (urinary symptoms, maximum vaginal contraction pressure, and duration of vaginal contraction), daily life problems, and sexual life problems were measured. For descriptive statistics and Wilcoxon signed-rank test were used with SPSS Win 14.0. Results: There were statistically significant differences in the mean scores of physical symptoms (urinary symptoms and maximum vaginal contraction pressure), daily life problems, and sexual life problems. Conclusion: This urinary incontinence management program showed improvements in physical symptoms, daily life, and sexual life of women in the sample. The study results indicate that for effective urinary incontinence management programs nurses should recognize the importance of continual treatment and also develop coping strategies that have sensitivity and are appropriate for patients' daily and sexual problems.

복합성 요실금과 복압성 요실금의 특성: 하부요로증상과 요역동학 검사결과의 관계 (Characteristics of Mixed Urinary Incontinence and Stress Urinary Incontinence: Relationship between Lower Urinary Tract Symptoms and Urodynamic Parameters)

  • Purpose: The purpose of this study was to analyze the relationship between lower urinary tract symptoms and urodynamic parameters to investigate the characteristics of mixed urinary incontinence (MUI) and stress urinary incontinence (SUI). Methods: The subjects were 318 women with MUI and 128 women with SUI. Data were collected retrospectively from electronic medical records including Bristol Female Lower Urinary Tract Symptoms-Scored Form (BFLUTS-SF), Incontinence Quality of Life Instrument (I-QOL), voiding diaries, and urodynamic parameters. Results: Compared with the SUI group, the MUI group was older and showed lower I-QOL and more severe urinary tract symptoms. The MUI group had more urinary frequency, more nocturia, and a higher urgency score than the SUI group. In the correlation analysis, the greatest difference between the two groups was that urgency was associated with Qmax, maximal cystometric capacity, and detrusor overactivity only in the MUI group (r = -.175, p= .004; r = -.281, p< .001; r= .232, p< .001, respectively). Conclusion: As a result of this study, we propose that a customized management program that emphasizes the control of urgency for the MUI group, and one that effectively strengthens the weak pelvic floor for the SUI group.

갱년기 여성의 폐경관리와 관련된 건강요인에 대한 연구 (Health Factors Related to Management of Menopause among Climacteric Women)

  • Purpose: This study was to identify the correlations between management of menopause, urinary incontinence and sexual satisfaction in climacteric women. Method: This study was conducted using a convenient sampling method in and around Jinju city, and each participant gave consent for the research. A total sample of 250 middle-aged women aged 40 to 60 years were chosen for the study, of which 157 women completed the questionnaires used for the analysis. Data was collected from Jan 1st to Mar 31st, 2004 by structured questionnaires (questionnaires for management of menopause, urinary incontinence and sexual satisfaction). Result: The characteristic variables significantly related to management of menopause were occupation and religion. The characteristic variable significantly related to urinary incontinence was age. The characteristic variables significantly related to sexual satisfaction were religion, economic status, and frequency of delivery. The relationship between management of menopause and sexual satisfaction was statistically significant with a positive correlation. Conclusion: It is hoped that the above findings will stimulate health promoting programs for climacteric women.

다요인적 프로그램이 농촌 지역 여성노인의 요실금, 자아존중감 및 자기효능감에 미치는 효과 (Effect of the Multifactorial Program on Urinary Incontinence, Self-esteem and Self-efficacy for Older Women in Rural Communities)

  • Purpose: The purpose of this study was to examine the effect of a multifactorial program for urinary incontinence, self-esteem and self-efficacy in older women. Methods: A one-group pretest-posttest with no control group was used. Nineteen women completed weekly 90-min group sessions for eight weeks. The group received a multi-factorial program comprised of Kegel exercise, education and psychosocial programs over 8 weeks. Data on urinary incontinence, self-esteem and self-efficacy were collected twice: before the intervention and 8 weeks after the intervention. Data were analyzed using paired t-test by non-parametric statistics. Results: Total scores for the International Consultation on Incontinence Questionnaire decreased significantly after the intervention (pretest $7.95{\pm}5.48$, posttest $5.15{\pm}4.72$, p<.05). The scores for self-esteem and self-efficacy improved significantly. Conclusion: These results indicate that a multifactorial program for older women is feasible to prevent urinary incontinence in women living in the community.

노년기 요실금 여성의 수면의 질, 일상생활 수행능력과 우울 (The Relationship of Quality of Sleep, Depression, Late-life Function and Disability (LLFDI) in Community-Dwelling Older Women with Urinary Incontinence)

  • Purpose: The purpose of this study was to investigate the relationships among quality of sleep, depression, late-life function and disability in community-dwelling older women with urinary incontinence. Methods: A stratified random sampling method was conducted to recruit participants from May 10 to August 17, 2007. Data were collected by questionnaires, which were constructed to include lower urinary tract symptoms, quality of sleep, depression, and late-life function and disability in 128 community-dwelling older women. Results: The major findings of this study were as follow: 1) 56.3% of participants belonged to urinary incontinence group. 2) There were significant relationships between depression and sleep latency, sleep duration, daytime dysfunction, quality of sleep, function component, frequency dimension, and limitation dimension. 3) Depression was significantly associated with frequency dimension, limitation dimension in capability which explained 44% of variance in depression. Conclusion: These results may contribute to a better understanding of sleep quality, depression, late-life function and disability in the community-dwelling older women with urinary incontinence. Therefore, health programs for prompting older women's health should be planned based on results of the study.

도서지역여성의 요실금 체험 (Lived Experience of Women체s Urinary Incontinence in Small Island)

  • This study adopts the phenomenological approach in order to explore the experience of urinary felt by the small island women and to find the meaning and structure of their experience, for the further understanding of them. This study succeeded in detecting five topics and three basic structure from eight participants, and followings are the comprehensive statement of them. The five topics include neglect of care after childbirth, unavoidable life in the tidal flat, shame which cannot be expressed even to their husbands, endless anxiety toward the expected future, and sad(dilemmatic) lived experience. The basic structure is that small island women who have urinary incontinence are apt to regard their disease as a natural destiny of women who fail to get adequate care after childbirth, and something to be endured to live in the seashore. They think of urinary incontinence as something so shameful that they cannot reveal it even to their husband and family. They believe that it even changes their personality since they must always stay alert in order to cope with the situation; for example, when it takes place unexpectedly, like too often to go to toilet, to change the underwears, to wake up in the middle of the night to go to toilet, to try not to laugh loudly, or to have showers. In addition, they accept it as a natural process of aging and incurable disease, and they consider themselves already ruined on the way of becoming uglier. They show dilemmatic abandonment: give it up unwillingly but at the same time think it is natural for others too. The unique experience of small island women with urinary incontinence implied in those statement are inseparable with the specific conditions for survival in the island. Unlike other diseases, it is considered the result of traditionally poor care after childbirth. However this misunderstanding that it is a natural phenomena for all the women who experience childbirth and aging and thereby incurable leads to an undesirable attitude toward urinary incontinence. According to the analysis, environmental conditions specific for small islands make the women there have distinct and unique experience concerned with urinary incontinence. Consequently, the future nursing plan for urinary incontinence in the small island area must be made and enforced with the consideration of these specific phenomenological meanings. Modern Korean nursing has basically been centered to hospital or urban areas. Besides, nursing intervention has long depended upon the research of western countries. This research, however, shows how greatly the regional and cultural characteristics influence the understanding of a certain disease, and is expected to make more specific and in-depth nursing approach enable for those who have urinary incontinence in small islands.

중년여성의 요실금 관리를 위한 자가 발 반사자극요법의 효과 (The Effects of Self-foot Reflexology on Urinary Incontinence in Middle-aged Women)

  • Purpose: This study was to identify the effects of self-foot reflexology on urinary incontinence symptoms, vaginal contraction and daily life discomfort of middle-aged women. Method: A quasi-experimental design was used. The subject were 39 middle-aged women with urinary incontinence, who were composed of 18 women in the experimental group, while 21 were in the control group. In the experimental group, self-foot reflexology was applied for 30 minutes, three times a week for 4 weeks. The obtained data were analyzed by using the Mann-Whitney U test of SPSS. Result: 1. Frequency(U=78.00, P=.001), amount(U=65.00, p=.001) and the situation score(U=81.00, P=.002)of urinary incontinence were reduced significantly in the experimental group as compared to the control group. 2. Maximum pressure(U=33.50, p=.000), mean pressure(U=38.00 p=.000) of vaginal contraction were improved significantly in the experimental group as compared to the control group. 3. Daily life discomfort in the experimental group was reduced whereas that of the control group was increased(U=63.00, p=.000) significantly. Conclusion: These findings indicate that self-foot reflexology is an effective method for reducing urinary incontinence symptoms and daily life discomfort and for increasing pressure of vaginal contraction of middle-aged women. Therefore self-foot reflexology can be considered as a independent nursing intervention for urinary incontinence.

도시지역 여성의 요실금, 하부요로증상과 일상 및 성생활과의 관계 (A Study on the Urinary Incontinence, Lower Urinary Tract Symptoms, Lifestyle and Sexual Matters of Women in an Urban Area)

  • Purpose: The purpose of this study was to identify the frequency of urinary incontinence and lower urinary symptoms, to identify lifestyle and sexual matters for UI of adult women in an urban area. Method: The sample consisted of 364 educated women from a women's health education program in a public health center. For data collection, the Bristol Female Lower Urinary Tract Symptoms Questionnaire developed by Jackson, et. al.(1996) and the demographic questionnaire were used. For data analysis, SAS 8.2 program was used. Result: The results were as follows: 1. the prevalence rate of UI was 73.63%. 45.88% of them were stress incontinence and 27.75% of them were urge incontinence. 2. Remarkable differences were found between the type of UI and LUTS in the case of urgency($x^2=42.0585$, P<0.001), unexplained incontinence($x^2=59.0585$, P<0.001), noctural incontinence($x^2=18.8080$, P<0.001) and catetherization($x^2=10.7207$, P= 0.004), burning sense($x^2=27.7400$, P<0.001). 3. Remarkable differences were found between the type of UI and lifestyle matters in the cases as follows fluid intake restriction($x^2=31.0532$, P=0.008), interference in physical activity($x^2=36.7481$, P=0.001), interference in relation with others($x^2=22.2729$, P=0.034). 4. Remarkable differences were found between the type of UI and sexual matters with difficulty of sexual intercourse($x^2=16.1898$, P=0.002), and urine leakage during sexual intercourse($x^2=17.9752$, P=0.001). Conclusion: Based on the results, developing and carrying out an adequate UI care and education program is needed.
