원발성 부갑상선 기능항진증 -11예 보고- (A Clinical Analysis of Primary Hyperparathyroidism -A Report of II Cases-)

  • Primary hyperparathyroidism is still uncommomn disease in Korea. However the frequency of this disease has been slowly increased with routine measurement of serum calcium and increasing awareness of hyperparathyroidism in recent years. The diagnosis is established by a persistent elevation of serum calcium and parathyroid hormone and by clinical evaluation. This is a report of eleven patients with primary hyperparathyroidism treated with surgical operation during a period from 1983 to 1997 at Department of Hospital. Authors analyzed the cases to evaluate clinical characteristics and outcome of surgical treatment retrospectively. The result was as follows. 1) In sex distribution, female patients were eight and three were male, the age distribution ranged from 18 to 67 years. 2) The presenting clinical manifestations were renal and urinary stone in eight, bone pain or fracture in six, muscle weakness in four, neurologic symptoms in four, neck mass in three, hypertension in two, and G-I symptoms in one. 3) All patients showed hypercalcemia and elevated serum parathyroid hormone level. 4) Preoperative localization study was performed with computerized tomography, ultrasonography, MRI, arteriography and thyroid scaning. 5) The tumor locations were left lower in eight, left upper in one, right lower in one, and right upper location was one case. 6) Histopathologic findings disclosed adenoma in all cases. 7) All patients were treated by surgical excision and postoperatively transient hypocalcemia occurred in six patients, but no other complication was developed.

칼슘과 철 보충제의 과다섭취가 빈혈모델 흰쥐의 체내 철 이용성 및 간과 신장기능에 미치는 영향 (Effect of Excess Calcium and Iron Supplement on Iron Bioavailability, Liver and Kidney Functions in Anemic Model Rats)

  • This study examined the effects of excess intake of calcium(Ca) and iron(Fe) supplements on iron bioavailability, liver and kidney functions in anemic model rats. Seven-week-old female rats were first fed and Fe-deficient diet for ten weeks, and then fed one of nine experimental diets for an additional eight weeks, containing three levels of Ca, normal (0.5%) or high(1.5%) or excess (2.5%) and three levels of Fe, normal(35ppm) or high(210 ppm) or excess(350ppm). In anemic model rats, serum Fe, total iron binding capacity(TIBC), hemogolin(Hb), hematocrit(Hct) and liver Fe contents were significantly decreased. Apparent Fe absorption significantly increased with increasing dietary Fe levels, and decreased with increasing dietary Ca levels. serum Fe concentration significantly increased in rats fed a high- and excess-Fe diet, and decreased in rats fed a excess-Ca diet. TIBC was decreawed in rats fed a excess-Ca diet, and transferrin saturation(%) increased in rats fed ahigh- and excess-Fe diet. Hb and Hct were decreased in rats fed an excess-Ca diet regardless of dietary Fe levels. Fe and thiobarbituric acid reactin gsubstance(TBARS) Contents of liver significantly increased in rats fed a high- and excess0-Fe diet, and decreased in rats fed a high- and excess-Ca diet. Fe content of the spleen showed similar results. Urinary creatinine and GFR increased in rats fed an excess-Ca diet regardless of dietary Fe levels. GOT, GPT and LDH were not significantly affected by dietary Ca and Fe levels. These results suggest that excess intake of Fe may increase liver Fe deposits and TBARS, and excess intake of Ca may decrease Fe bioavailability and kidney function leading to potential health problems in anemic model rats.

신증후군 환아에 대한 Deflazacort의 효과와 안전성 (Efficacy and Safety of Deflazacort in Korean Children with Nephrotic Syndrome)

  • Deflazacort, an oxazoline derivative of prednisolone, has been claimed to have anti-inflammatory effects with fewer side effects compared to prednisone. The objectives of the study were to evaluate efficacy and safety of deflazacort in children with nephrotic syndrome. Eligible Patients were the children with nephrotic syndrome who were treated with deflazacort from October. 1994 to April. 1999. Nephrotic syndrome was defined as having albumin level of less than 2.5 mg/dL and 24-hour urinary protein excretion of greater than $40\;mg/m^2/hr$. The primary parameters evaluating the efficacy of deflazacort were response rate, time to respond and relapse frequency. The safety profiles were the impact on children's growth, calcium sparing effect, glucose metabolism, lipid profile and adverse drug reactions. As results, total of 60 children were evaluated (47 boys, 13 girls). Response rate was $95\%$ (57/60) for initial and late responders. Median time to respond was 12 days (range 7-110 days) and median relapse frequency was one time (range 0-6). Weight/height ratio increased from $22.05\pm3.47\;to\23.20\pm3.44\;kg/m$ (p<0.001) and plasma calcium level, from $7.55\pm3.86\;to\;9.98\pm3.77\;mg/dL$ after treatment (p<0.001). Change of fasting glucose level was not statistically significant $(91.92\pm3.53\;vs.\;98.19\pm4.78\;mg/dL,\;p=0.072)$, while change of total cholesterol was significant $(362.3\pm12.0\;vs\;251.4\pm11.5\;mg/dL$, p<0.001). In conclusion, patients on deflazacort showed similar efficacy in treatment of nephrotic syndrome as reported for prednisone with less impact on growth inhibition and metabolic side effects of hyperglycemia and hyperlipidemia.

도시거주 저소득층 노인들의 골지표 및 영양소섭취와 골밀도와의 상관관계에 관한 연구 (Association between Bone Mineral Density and Bone Nutrition Indicators in Elderly Residing in Low Income Area of the City)

  • 본 연구는 부천시에 거주하는 외견상 건강하면서 65세 이상 노인 138명 (남: 38명, 여: 100명)을 대상으로 하여 골밀도와 관련된 각 요인을 분석한 결과 다음과 같은 결론을 얻었다. 여자 노인은 남자 노인에 비해 칼슘, 비타민 C를 제외한 모든 영양소의 섭취량이 유의하게 낮았다(p<0.001∼p<0.05). 남녀 모두 칼슘과 비타민 D 섭취량은 각각 331.0 mg, 1.89 $\mu\textrm{g}$과 308.6 mg, 1.21 $\mu\textrm{g}$으로서 RDA의 50% 미만이었다. 남자노인의 경우 요추골밀도, 대퇴부 골밀도는 에너지 섭취량(p<0.05), 칼슘 섭취량(p<0.05), 비타민 D 섭취량(p<0.05)과 양의 상관관계를 보였다. 여자노인의 경우 요추골밀도, 대퇴부 골밀도는 에너지, 당질, 단백질, 지방, 칼슘, 비타민 D 섭취량 모두와 양의 상관관계를 보였다(p<0.01∼p<0.05). 여자 노인의 혈청 osteocalcin과 소변의 DPYR/CR값이 남자에 비해 유의하게 높아(각 p<0.01, p<0.001)남자 노인에 비해 골 교체율과 골 흡수가 높은 것을 알 수 있었다. 25(OH)VitD$_3$가 10 ng/mL미만으로서 비타민 D 결핍을 보인 노인이 남자 23.7%, 여자 35.0%였다. 혈청 25(OH)VitD$_3$는 척추, 대퇴 전자부 골밀도와 유의한 정의 상관관계 (p<0.05)를 보였으나 혈청 alkaline phosphatase, 소변의 DPYR/CR, 소변의 P/CR의 비는 대부분의 부위에서 골밀도와 음의 상관관계를 보였다(p<0.001∼p<0.05). 다단계 다중회귀분석을 실시한 결과 요추골밀도는 활동량, 혈청 ALP, 체중, 비타민 D 섭취량에 의해 골밀도 변이의 47.6%를 설명할 수 있었으며 그 중에서도 비타민 D 섭취량(p<0.001), 체중(p<0.001), 활동량(p<0.05)이 감소할수록 골밀도는 감소했고 혈청 ALP는 증가할수록 골밀도는 감소하였다(p<0.01). 대퇴전자부 골밀도는 연령, 혈청ALP, 체중에 의해 골밀도 변이 의 45.9%를 설명할 수 있었고 나이가 많을수록(p<0.01), 혈청 ALP가 높을수록(p<0.01), 체중이 적을수록(p<0.001) 골밀도가 낮았다.

Zinc status and dietary quality of type 2 diabetic patients: implication of physical activity level

  • The purpose of this study was to analyze the relationships among zinc status, diet quality, glycemic control and self-rated physical activity level of type 2 diabetic patients. Dietary intakes for two non-consecutive days were measured by 24-hour recall method for seventy-six diabetic patients. Fasting blood glucose and HbA1c were measured for the assessment of glycemic control. We evaluated the extent of dietary adequacy by the percentage of subjects with a dietary intake of a nutrient less than the estimated average requirement(EAR), the dietary diversity score(DDS) and the dietary variety score(DVS). Zinc status was assessed from serum levels and urinary excretion. Dietary inadequacy was serious for five nutrients: riboflavin, calcium, thiamin, zinc and vitamin C. Dietary intakes from the meat, fish, and egg food groups and the milk food group were below the recommended level.   • The present study was aimed at elucidating the mode of inhibitory action of tricyclic antidepressants on the smooth muscle. Effects of amitriptyline on the isolated detrusor muscle strips of the urinary bladder of the rabbit were examined. The spontaneous rhythmic movement of the muscle preparation was frequently observed, which was decreased or abolished by addition of amitriptyline $(10^{-5}{\sim}10^{-3}\;M)$. The muscle preparation responded with contraction dose dependently to carbachol, of which dose response curve shifted to the right in the presence of either amitriptyline or atropine. However, amitriptyline produced a nonparallel shift, whereas atropine caused a parallel one. In calcium free medium, the contraction response to carbachol was markedly decreased, which was resumed by the addition of $CaCl_2$ (2.5mM), but not in the presence of either amitriptyline or nifedipine. KCI (60 mM) produced a potent contraction, which was abolished in the presence of amitriptyline or nifedipine. These results suggest that amitriptyline, unlike atropine, not only acts as a noncompetitive antagonist at cholinergic muscarine receptors but also inhibits Ca-influx through the muscle cell membrane.

비급식 농촌 국민학교 아동의 현미 플레이크와 두유 또는 우유 간식 급여의 영양효과에 관한 연구 (Effect of the Feeding of Brown Rice Flake with Soy Milk or Cow's Milk to School Children in Remote Village)

  • To evaluate the effect of the feeding program of brown rice flake with soy milk or cow's milk to school children in rural areas. The survey was conducted twice before and after practice of feeding in March and July of 1990. The survey covered a total of 87 children aged from 7 to 12 in Ypsuk elementary school located in remote village of Sangju county. Kyung Buk province and the branch school of Songmyun elementary school located in remote village of Goisan county. Chung Buk province. Family environment dietary intake anthropometric data and biochemical tests were examined. The results are summerized as follows: The mean balance and food diversity at lunch were significantly lower than those at breakfast and dinner. Mean daily energy and nutrient intakes were below the Korean RDA's except for intakes of niacin vitamin A and ascorbic acid Especially intake of calcium was not reached to 50% of Korean RDA's. Most of anthropometric data of subjects surveyed showed slightly higher results in summer than those in spring except for weight for height and sitting height for height. The percentage of anemic according to hematocrit criterion was not reduced during this period. however urinary urea nitrogen/creatinine ratio was improved. The conbined feeding program of brown rice flake with soy milk or cow's milk was not sufficient to support the children's growth. So the survey recommends the support of goberment to the school lunch program in the remote village.

홍화자·녹용·자하거 약침이 골다공증에 미치는 영향 (A Study on the effects of the Carthami semen·Cervi pomtotrichum cornu·Hominis placenta aquacupuncture on the osteoporosis in the rats)

  • Objectives : This study was designed to evaluate on the effects of the Carthami semen Cervi pomtotrichum cornu Hominis placenta aquacupuncture on the osteoporosis in the rats Materials and Methods : In order to induce estrogen deficient osteoporosis, ovariectomy was done on rats. Then aquacupunctures were injected into the pai shu points corresponding to urinary bladder meridian of human body in experimental groups. And such indexes were measured as the change of body weight, bone mineral density, histological change of tibia, and levels of osteocalcin, bone alkaline phosphatase, calcium, phosphorus in serum. Results : 1. Significant changes of trabecular area % in epiphysis of tibia were shown between control group and experimental group II and between experimental group I and group II. 2. Significant changes of trabecular area % in diaphysis of tibia were shown between control and experimental groups. 3. Significant changes of serum osteocalcin were shown between control group and experimenta group I . 4. Significant changes of levels of serum bone alkaline phosphatase were shown between control group and experimental group I and II. Conclusions : The Carthami semen Cervi pomototrichum cornu Hominis placenta aquacupuncture is effective to estrogen deficient osteoporosis, ovariectomy was done on rats.

체중부하운동이 여대생의 골대사에 미치는 효과 (Effects of Weight-Bearing Exercise on Bone Metabolism in College Women)

  • Purpose: This study was done to identify the effects of weight-bearing exercise(WBE) on bone metabolism. Method: WBE was performed for 12 weeks by healthy college women. Bone-related parameters were measured four times during this period by evaluating the immunoradiometric assay and enzyme immunoassay. Bone mineral densities(BMDs) were measured by dual energy x-ray absorptiometry before and after the WBE program. Data was analyzed using t-test, paired t-test, $x^2$-test, and repeated measures ANOVA. Result: Osteocalcin, a bone formation marker, increased more in the experimental group than in the control group based on the interaction between time and group(F=3.29 p=.024). Little difference between the two groups was found for the other parameters: urinary deoxypyridinoline, insulin-like growth factorI, parathormone, serum calcium, and serum phosphorus without showing any time interaction between the groups. The femoral trochanter BMD rose in the experimental group while that of the control group fell, showing a significant difference for BMD(t=3.06 p=.005). However, there was no significant difference between the two groups for changes in BMD of the forearm, lumbar spine, femoral neck, and femoral ward's triangle. Conclusion: These findings supported the WBE is beneficial for increasing bone formation in college women and long-term application is needed to substantiate the effects of WBE as a intervention in promotion of bone-health.

지방종증에 이환된 한우의 혈청 지질관련 효소치 분석 (Serum Lipid Analyses in Korean Indigenous Cattle with Abdominal Fat Necrosis)

  • Bovine abdominal fat necrosis (lipomatosis) is relatively common disorder in adult Korean indigenous cattle. Thirteen Korean indigenous cattle with bovine lipomatosis and five clinically healthy cattle were selected and serum biochemical profiles were analyzed. Serum free fatty acids level was significantly high, while total cholesterol, serum albumin and total calcium levels were significantly low in bovine lipomatosis group. In a case of necropsy, saponificated adipose masses surrounding colon was observed and hepatic fatty degeneration and fat deposition in the renal tubules were found in a histopathologic examination. These findings indicate that affected cattle have a predisposition to deposit more fat into adipose tissue than normal cattle. Such abnormalities might lead to the development of abdominal fat necrosis with fibroplasia and possibly compress the intestines and urinary organs.
