Evaluation of new American Academy of Pediatrics guideline for febrile urinary tract infection

  • Purpose: To evaluate the practical applications of the diagnosis algorithms recommended by the American Academy of Pediatrics urinary tract infection (UTI) guideline. Methods: We retrospectively reviewed the medical records of febrile UTI patients aged between 2 and 24 months. The patients were divided into 3 groups: group I (patients with positive urine culture and urinalysis findings), group II (those with positive urine culture but negative urinalysis findings), and group III (those with negative urine culture but positive urinalysis findings). Clinical, laboratory, and imaging results were analyzed and compared between the groups. Results: A total of 300 children were enrolled. The serum C-reactive protein level was lower in children in group II than in those in groups I and III (P<0.05). Children in group I showed a higher frequency of hydronephrosis than those in groups II and III (P<0.05). However, the frequencies of acute pyelonephritis (APN), vesicoureteral reflux (VUR), renal scar, and UTI recurrence were not different between the groups. In group I, recurrence of UTI and presence of APN were associated with the incidence of VUR (recurrence vs. no recurrence: 40% vs.11.4%; APN vs. no APN: 23.3% vs. 9.2%; P<0.05). The incidence of VUR and APN was not related to the presence of hydronephrosis. Conclusion: UTI in febrile children cannot be ruled out solely on the basis of positive urinalysis or urine culture findings. Recurrence of UTI and presence of APN may be reasonable indicators of the presence of VUR.

  • An optic module system is developed adopting multiple colorimetry units for the measurement of multi-pad urinalysis dipsticks. Multiple photometry system instead of moving mechanisms has the advantages of system reliability and simplicity as well as economic aspects due to the recent development of economic color light emitting diodes and stable photo sensors. An integration amplifier with programmable integration time, a current source circuit with selectable and stable current settings were connected through analog multiplexers to thirty light emitting diodes for illumination and ten photo transistors for reading each strip pad. All the circuits are controlled by a microprocessor through a simple set of serial communication commands. The detect ability is eighteen times better than the minimum color difference of the test grading which is 0.013 in urobilinogen in the color space defined in this paper.

  • Oligmeganephronia is congenital hypoplasia of kidney with renal pathology showing very small number of nephrons with compensatory hypertrophy of the remaining glomeruli. A 7-year-old girl was referred to our nephrology clinic due to hematuria detected on school screening urinalysis and diagnosed as chronic renal failure and oligomeganephronia on renal biopsy. We are reporting the clinical and histomorphometric changes for the four years follow-up with review of literatures.

Serum Fructosamine for Assessment of Glycemic Control in a Cat with Diabetic Ketoacidosis (고양이에서 혈중 Fructosamine을 이용한 인슐린 의존성 당뇨병성 케톤산 혈증 치료 예)

  • A ten-year-old, male cat presented with recent loss of body weight, depression, vomiting, anorexia, polydipsia, and polyuria. General physical findings included depression, weakness, severe dehydration and a strong acetone odor on the breath. A complete blood count and serum biochemical profiles were leukocytosis, hyperglycemia (286 mg/dl), hypokalemia (2.6 mEq/L), hyponatremia, and high serum fructosamine (600 $\mu$mol/L). In blood gas analysis the cat had acidosis (pH 7.127, p$CO_2$26.7 mmHg). In urinalysis glycosuria and ketouria were appeared. On the basis of clinical signs, serum chemistry, blood gas analysis and urinalysis, diabetic ketoacidosis was diagnosed. Treatment included subcutaneous administration of protamine zinc insulin (0.75 U/head) and intravenous administration of 0.9% saline. Potassium phosphate and sodium bicarbonate was added to the fluids. Serum fructosamine for assessment of glycemic control was measured on occasion calls. On day 296, the patient improved clinically and did not experience any problems resulting from diabetic ketoacidosis.

  • Purpose : Asian Quality Assurance Survey program(AQuAS) is and activity of Asian Network for Clinical Laboratory Standardization and Harmonization(ANCLS) which had begun her first colloquium in 1999 in Jakarta, Indonesia by laboratory physicians led by Professors Noriyuki Tatsumi and Yoshinori Funahara with themes of standardization and harmonization of clinical laboratories in Asia. Methods : AQuAS had begun its first survey in July. 2001 and done November and March, three times per year in the fields of Chemistry, Hematology. Coagulation and Urinalysis. As of June 2006 the 15th survey have been completed. Korean Association of Health Promotion(KAHP) had participated in it since July, 2004 in the fields of chemistry, hematology and urinalysis.ゝ⨀

  • In this paper, we have developed smart-phone App(application) for screening Dementia using bionic sensors, urine test and questionnaire. Since small amounts of urinary protein strongly predict faster cognitive decline in the elderly, smart-phone based urinalysis is adopted to screen dementia more accurately as well as bionic sensors such as $SpO_2$ and HRV(Heart Rate Variability). Firstly, DI(Dementia Index) is calculated from urinalysis, two bionic sensors and electric questionnaire, and then compared to the threshold from clinical test. Finally the results of Dementia screening is shown in your smart-phone and useful information to prevent or relieve Dementia is also given. We performed simple testing on persons aged over 60 and found out the proposed application can be useful devices for screening Dementia easily and quickly.

  • System to urinalysis using FT-IR instruments is presented based on fuzzy logic knowledge. Linguistic expressions of the possibility of infection and the importance were quantified and membership functions were determined based on general quantitative criteria. Diseases considered were Diabetes Mellitus, Proteinuria, Microalbuminuria. Glucose, Protein, Albumin, Creatinine in 30 samples were analyzed by the present system, which resulted in 74% accuracy. The simple mathematical formulation of present system would enable an easy implementation in commercial analysis instruments. Also, the identical fuzzy logic can be applied to similar diagnostic environments in general.

Biological Monitoring of Human Exposure to Volatile Halogenated Hydrocarbons Using Urinalysis with Capillary GC-ECD

  • For the risk assessment of human exposure to volatile halogenated hydrocarbons, a dynamic purge trap/on-column cryofocusing method using capillary gas chromatograph-$^{63}Ni$ electron capture detector and thermal desorption unit was applied to analyze the free forms, metabolites of 1, 1, 2-trichloroethylene and 1, 1, 2, 2-tetrachloroethylene. The urine sample was diluted with distilled water, hydrolyzed and sealed. Then the inert gas was infused to purge out free 1, 1, 2-trichloroethylene, free 1, 1, 2, 2-tetrachloroethylene and urichloroethanol. These compounds were trapped to $Tenax^R$ / GC-gas trap device throughout clean up tube. Being undertectable to gas chromatograph directly, trichloroacetic acid was methyl esterificated and trapped in the manner above mentioned. The optimal incubation time to get best recovery of methyl ester was 4 hours at $60^circ$C. The concentrations of free volatile halogenated hydrocarbons and their metabolites in urine were obtained of free volatile halogenated hydrocarbons and their metabolites in urine were obtained from 5 healthy volunteers. This analytical method is expected to make the biological monitoring more precise and convenient.

Classification of UTI Using RBF and LVQ Artificial Neural Network in Urine Dipstick Screening Test (RBF와 LVQ 인공신경망을 이용한 요(尿) 딥스틱 선별검사에서의 요로감염 분류)

  • Dipstick urinalysis is used as a routine test for a screening test of UTI (urinary tract infection) in primary practice because urine dipstick test is simple. The result of dipstick urinalysis brings medical professionals to make a microscopic examination and urine culture for exact UTI diagnosis, therefore it is emphasized on a role of screening test. The objective of this study was to the classification between UTI patients and normal subjects using hybrid neural network classifier with enhanced clustering performance in urine dipstick screening test. In order to propose a classifier, we made a hybrid neural network which combines with RBF layer, summation & normalization layer and L VQ artificial neural network layer. For the demonstration of proposed hybrid neural network, we compared proposed classifier with various artificial neural networks such as back-propagation, RBFNN and PNN method. As a result, classification performance of proposed classifier was able to classify 95.81% of the normal subjects and 83.87% of the UTI patients, total average 90.72% according to validation dataset. The proposed classifier confirms better performance than other classifiers. Therefore the application of such a proposed classifier expect to utilize telemedicine to classify between UTI patients and normal subjects in the future.
