Uremia in slaughtered cattle

  • Uremia was diagnosed in a slaughtered 20-month old bull. It had no special clinical signs, but many lesions associated with uremia were observed at postmortem inspection. It had a lot of ascites. Kidney, urinary bladder and ureter were enlarged, congested and hemorrhagic, and the incised section of those slightly smelled nasty. However, the precise cause of uremia in this case could not be completely identified.

An Extract from Hydrolyzed Normal Human Urine which Induces Drug Binding Defects (정상인뇨의 가수분해에 의한 의약품결합 저해유도인자의 추출)

  • Uremia is associated with defective protein binding of weakly acidic drugs, whereas the protein binding of basic drugs tends to be normal. The exact chemical nature of compound(s) and mechanism for these changes as yet is unknown, and has not been defined. Organic solvent extraction of pooled normal human urine following hydrolysis by hydrochloric acid produced an extract, which when added to normal human serum, was capable of inducing binding defects similar to those in uremia. Binding defects were observed with the weakly acidic drugs such as nafcillin, salicylate, sulfamethoxazole and phenytoin while the binding of the basic drugs such as trimethoprim and quinidine were unaffected. The binding defects induced by the hydrolyzed urine extract could readily be corrected by same organic solvent extraction of acidified serum and the defects could be transferred to the normal human serum using the organic solvent layer at the physiologic pH (7.4). Followed by reacidification ind extraction of the binding defects induced serum with the same solvent, separated several fractions were obtained on thin-layer chromatography. One of these fractions could reinduce the binding defects and this factor(s) is apparently weakly acidic compounds and tightly bound to serum at physiologic pH, but extractable at acidic pH, and its molecular weight range is approximately 500 or less similar to those seen in uremia. These findings strongly support the hypothesis that the drug binding defect in uremia is due to the accumulation of endogenous metabolic products which arc normally excreted by the kidneys but accumulate in renal failure.

  • A 47-year-old male who had hypertension and diabetes mellitus for 7 years suddenly developed bradykinesia, generalized limb muscular rigidity, dysarthria and dysphagia. Uremia developed 5 months prior to this and he had been on hemodialysis. A T2-weighted brain MRI showed extensive hyperintensity over the bilateral basal ganglia, extending to the adjacent periventricular white matter. In T1-weighted images the lesions were hypointense. Supportive treatments were given and his symptoms improved. Exacerbation of glucose utilization failure or vasogenic edema is suggested as the etiology of basal ganglia lesions, but the exact underlying pathophysiology is unknown.

  • The kidney, heart and lung were examined of cats fallen sick lower urinary tract disease(LUTD). There cases typically clinical findings were urinary retention, hematuria, dehydration, cytoplegia and uremia. It is believed that these histopathological and clinical findings resulted from the struvite crystal thrombus in ureter. But we have many factors of an unsolved problem on LUTD. Accordingly, this LUTD demands to process of pathological diposition.

Clostridium perfringens type A associated enteritis in a Shitzu dog

  • Acute hemorrhagic enteritis was diagnosed in a seven-month-old male Shitzu dog dying of blood stained diarrhea and vomiting. Clinical findings were anorexia, dullness and sudden death after massive bloody diarrhea. At necropsy, main lesion was the hemorrhage in small intestine, mainly duodenum and jejunum. Microscopically, Gram positive long bacilli were massively detected on the mucose epithelial cells and necrotic debris of small intestine. Coagulative necrosis of epithelial cells and thrombosis of small intestine were also identified. However, there was no lesion of crypt epithelium. Mineral infiltration in both gastric mucosa and renal tubules was detected and proliferation of fibrous tissue was also shown in corticomedullary regions. In bacterial examination, C perfringens was isolated in anaerobic culture and it was confirmed to type A by multiplex PCR. Therefore, the dog was diagnosed as C perfringens type A associated enteritis with uremia.

A Case Report of Improvement of Glomerular Filtration Rate and Anemia in Chronic Renal Failure through Herbal Medication (만성신부전 환자에게 한약투여 후 사구체여과율 및 빈혈 호전 1례)

  • Renal failure means irreversible damage and depression of renal function, represented by glomerular filtration rate(GFR). A renal failure patient with uremia and anemia is presented. Anemia, in this patient, appeared before uremia did. A 55 year-old male patient was treated for renal failure with Weilingtang. Improvement was observed in, not only GFR, blood urea nitrogen(BUN), and creatinine, but also in such symptoms as difficulty in urine excretion, dyspepsia and abdominal swelling. But anemia appeared during treatment, so a mixture of Dangguibuxuetang(當歸補血湯) and Xiongguitang(芎歸湯) was prescribed. After this treatment, improvement in hemoglobin, and hematocrit was observed.

Assessment of Nutritional Status in Elderly Dialysis Patients (노인 투석환자의 영양상태 평가)

  • It is important to evaluate nutritional status of elderly patients receiving dialysis, since wasting and malnutrition are their common problems. This study aims at assessing their nutritional status by the type and duration of dialysis. The nutritional status such as somatic fat and protein storage was evaluated with anthropometric measure including weight/height ratio, triceps skinfold thickness and midarm muscle circumference. It was also measured with albumin, transferrin, C3 and IGF-1 and calorie and protein intakes. The general clinical condition of patients was evaluated with the severity of uremia and metabolic acidosis. which were measured through the levels of serum urea, creatinine and bicarbonateion. The data were analyzed by using t-test, ANOVA, Wilcoxon-rank sums test, Scheffe test, Kruskal-Wallis test and Pearson correlation coefficients. The results are following : 1. There was no significant difference in the calorie and protein intakes by the type and duration of dialysis received. 2. As for the anthropometric measures, no significant difference was found by the type of dialysis in body mass index triceps skinfold thickness and midarm muscle circumference. Yet these anthropometric measures differed significantly by the duration of dialysis in those elderly patients receiving hemodialysis(HD group), but this finding was not found in those receiving continuous ambulatory peritoneal dialysis(CAPD). 3. Regarding the indicators of uremia and metabolic acidosis, blood urea nitrogen(BUN) and creatinine were lower in the CAPD group than in the HD group, whereas bicarbonate ion was higher in the CAPD group than in the HD group, with no statistical significance. In the HD group, creatinine increased significantly with the increase of the duration of dialysis. 4. Serum trasferrin and C3 were significantly higher in the CAPD group than in the HD group. However. each of biochemical indices did not show statistical significance by the duration of dialysis in both HD and CAPD groups. 5. Anthropometric measures were significantly associated with dietary intake. Significant correlations were observed between biocarbonate ion, BUN and creatinine. In addition, the correlations between serum protein and albumin and between transferrin and C3 were statistically significant. Yet, IGF-1 revealed no significant correlation with other nutritional indices. The above findings indicate that there were no difference in nutritional status measured with protein and calorie intakes between the type and duration of dialysis, but CAPD seems to benefit correcting uremia and metabolic acidosis than HD. Studies of dietary management for dialysis patients need to be pursued in order to improved the quality of aged patients receiving dialysis.

Histopathological study of uremia in a juvenile shepherd dog (어린 shepherd 개에서 요독증의 조직 병리학적 연구)

  • A 7 month old male shepherd was presented with anorexia, diarrhea, and salivation, and euthanized due to no clinical improvement. Grossly, ulcers were seen on ventral surface of tongue and stomach. The mineralization was obvious in the intercostal space, lung and inner surface of aorta. Kidney was pale, firm and irregular. Histopathology confirmed uremic pneumonitis, end stage kidney and hyperplasia of parathyroid. This case was the typical uremic condition of juvenile dog, which was characterized the diffuse soft tissue mineralization due to hyperparathyroidism secondary to chronic renal failure.
