Foreign Body in the Gastrointestinal Tract in Children (소아의 위장관 이물질에 대한 임상적 고찰)

  • Background: The accidental swallowing of foreign body is a common problem in the children. Ingested foreign bodies may be managed by endoscopy, observation, or surgery. So we analyzed the methods of removal, type, location and complications of foreign bodies. Materials and Methods: This report reviewed 37 cases of ingested foreign body in the gastrointestinal tract at the Department of Pediatrics, Yeungnam University Hospital between January 1997 and April 2001. Results: The age ranged from 8 months to 8 years. The most prevalent age group was between 1 year and 2 years of age(19%). The male to female ratio was 2.1:1 with 25 male and 12 female patients. The type of foreign bodies were coins in 20 cases(54%), nail in 4 cases(11%), key in 4 cases(11%), pin in 2 cases(5.5%), necklace in 2 cases(5.5%) and others. The locations of foreign bodies were upper esophagus in 12 cases(32.5%), lower esophagus in 4 cases(10.8%), stomach in 16 cases(43.2%), small bowel in 5 cases(13.5%). 4. Presenting symptoms were variable with asymptomatic(59.4%), vomiting(19.0%), epigastric pain(8.1%), dysphagia(5.4%) and others. The methods for removal of foreign bodies included 20 cases of endoscopic removal(54.0%). 3 cases of spontaneous removal(8.1%) and there was no surgical removal. 14 cases(37.9%) did not confirmed removal of foreign body because of no revisit of our hospital. Endoscopic finding of patients were normal(15 cases), ulceration(2 cases), erosion(1 case), inflammation(1 case), mucosal scratch(1 case). Conclusion: It appears that the endoscopic approach is the preferable method for the removal of upper gastrointestinal foreign bodies in the children.

Two Cases of Gastric Metastasis from Small Cell Lung Cancer (소세포 폐암에서의 위 전이 2예)

  • This is a report of gastric metastases secondary from a primary small cell carcinoma of the lung in two men. Blood-borne metastatic involvement of the stomach by cancer is a rare entity. According to the reports in the literature the prevalence of metastasis to the stomach occurs in 0.4% and the most common cell type of the primary lung carcinoma is large cell type(3.7%) followed by adenocarcinoma(2.4%), small cell carcinoma(1.7%) and squamous cell carcinoma(0.7%). The most common tumors that spread to the stomach through the blood stream are malignant melanoma, breast carcinoma and lung carcinoma. Most of the gastrointestinal tract metastases had no specific symptoms because of its submucosal involvement. The prognosis was poor and the mean survival period from the onset of symptoms was 49 days. The first patient was a 56-year-old man who had primary lung carcinoma with brain metastasis. Gastroscopic findings showed two elevated mass lesions in the anterior wall of the mid body with central ulcer and the posterior wall of the fundus with intact surface mucosa. Pathologic examination of stomach tissue revealed small cell type tumor cells infiltrate in the stomach wall segmentally without destruction of the glands. The second patient was a 67-year-old man who had no other evidence of the distant metastasis. Gastroscopic findings showed a huge, oval shaped, ulcerofungating mass with deep penetrating central ulcer coated with dirty exudate in the anterior wall from mid to upper body of the stomach, and thickened elevated rugal folds in the posterior wall of the fundus. Pathologic examination of stomach tissues revealed the small cell type tumor cells showing small smudged nucleus infiltrate into the mucosa of the stomach and the architecture of mucosa intact. We report the two cases of metastatic gastric cancer from the primary small cell lung carcinoma with the literature review.

Analysis of the Ground Reaction Force Parameters According to the Change of Position and Weights of Bag during Downward Stairs Between Dominant and Non-dominant in Upper & lower limbs (계단내리기 시 우세·비우세 체지의 가방착용과 무게변화에 따른 지면반력 파라미터 분석)

  • The purpose of this study was to analyze of the GRF (ground reaction force) parameters according to the change of positions and weights of bag during downward stairs between dominant and non-dominant in upper & lower limbs. To perform this study, participants were selected 9 healthy women (age: $21.40{\pm}0.94yrs$, height: $166.50{\pm}2.68cm$, body mass: $57.00{\pm}3.61kg$, BMI: $20.53{\pm}1.03kg/m^2$), divided into 2 carrying bag positions (dominant arm/R, non-dominant arm/L) and walked with 3 type of bag weights (0, 3, 5 kg) respectively. One force-plate was used to collect GRF (AMTI OR6-7) data at a sample rate of 1000 Hz. The variables analyzed were consisted of the medial-lateral GRF (Fx), anterior-posterior GRF (Fy), vertical GRF (Fz), impact loading rate and center of pressure (COPx, COPy, COP area, COPy posterior peak time) during downward stairs. 1) The Fx, Fy, Fz, COPx, and COP area of GRF were not statistically significant between dominant leg and non-dominant leg, but non-dominant leg, that is, showed the higher COPy, and showed higher impact loading rate than that dominant leg during downward stairs. 2) In bag wearing to non-dominant arm, Fx, Fz, COPx, COPy, impact loading rate and COP area showed increase tendency according to increase of bag weights. Also, against bag wearing to dominant arm, non-dominant showed different mechanism according to increase of bag weights. The Ground Reaction Force parameters showed different characteristics according to the positions and weights of bag during downward stairs between dominant and non-dominant arm.

Analysis for Species and Lacquer Ware of Wooden Objects Excavated from Sinchang-Dong, Gwangju (광주 신창동 유적 출토 목제품의 수종 및 칠 분석)

  • Wooden objects excavated from Gwangju Shinchang-dong, we analyzed total 40 pieces consisting of 37 pieces for analysis of species of trees, 3 pieces for analysis of lacquer. and especially it was found that in case of lacquerware and living goods like dish, diffuse-porous woods were used. As a result of this, total 7 species of trees used for wooden objects identified are Quercus spp., Cearsus spp., Prunophora spp., Acer spp., Salix spp., Populus spp. were identified. As a result of analyzing lacquered film, though there is a difference of times of lacquering between holed lacquerware and cylider type lacquerware body part 5, it was confirmed that after undercoating, black pigments and lacquer were painted on upper layer in mixture, and foundation platter for with 5 parts lacquer ware was undercoated with mixture of powdered soil, black pigments and lacquer, and then its upper layer was coated with lacquer mixed with black pigments.

Formal Characteristics and Classifications of Pagodas in the Period of Bagan (버강 시기 불탑의 형식적 특성과 분류)

  • This paper examines the pagodas during the Bagan period, which are thought to be most valuable among those in Myanmar. They have not been sufficiently studied, in spite of their high Buddhist status. It considers the contemplation of Pato, their formal characteristics in the Bagan period and the formal categorizations of pagodas in the same period. Thus, following seven kinds of conclusions are derived: first, researchers provide opinions to the Pato which should be regarded as the Buddhist shrine-type of pagoda with a unique Bagan form, symbolically indicating that Theravada Buddhism incorporated Hinduism; second, the terraces were characteristic components in pagodas, during the Bagan period, which were built after the enthronement of King Anawratha and are thought to symbolically express the wish to widely spread Theravada Buddhism; third, Shwesandaw Zedi seems to affect not only Shwezigon Zedi, a representative standard form of pagoda in Myanmar, but also Ananda Pato; fourth, it is thought to be proper to examine the terraces by classifying them into lower, central(from pedestals to Angryeon and Bokryeon) and upper part, if it intends to divide a pagoda with bell-shaped body on the terraces during the Bagan period, into three parts; fifth, the Pato may be identified as a form of pagoda during the Bagan period, and such a form can be classified as that of Sikhara on the rectangular terrace; sixth, forms of Myanmar's pagodas can be classified into fourteen kinds of them, and they may be also grouped into transmitted, general and special type; and seventh, on the basis of the findings, it is thought that the pagodas during the Bagan period may be classified into six forms, and they can be largely categorized into transmitted, general and special type.

Effect of Dietary Fiber and Fat on Tumor Incidence and Cell Proliferation of Colonic Mucosa in DMH-Treated Rats (Dimethylhydrazine으로 처리한 쥐에서 식이섬유소와 지방종류가 대장의 종양발생율과 세포증식에 미치는 영향)

  • This study was designed to observe the effect of dietary fiber and fat on colon tumor incidence and cell proliferation. Male Sqraue Dawley rats(n=225) at 7 weeks of age, were divided into 3 groups depending on the type of fat b(beef tallow, corn oil and DHA-rich fish oil) and each group was again divided into 3 groups depending on type of fiber(fiber-free, perctin and cellulose) . The experimental diet containing dietary fat at 15%(w/w) and fiber at 6%(w/w) levels was fed for 25 weeks. At the same time, each rats was intramuscularly injected with DMH two times a week for 6 weeks to geive total dose of 180mg/kg body weight. Cell proliferation was measured by in vivo incroporation of bromodeoxyuridine (BrdU) into DNA. Fish oil decreased the tumor incidence (9.67%) compared with beef talow (33.39%) and corn oil (21.21%). Tumor incidence was decreased in all groups that fed cellulose (11.67%) compared with those of fiber-free(21.74%) and pectic(19.70%). Most of tumors was distributed at the site of the distal colon. The rats fed both fish oil and cellulose significantly decreased th enumber of tumors and tumor incidence compared to other groups. Fish oil was more effective in preventing cell prolofieration by decreasing crypt length and labeling index(LI) compared with beef tallow(p<0.05). Cell proliferation in distal colon was more developed to the upper part of the crypt compared to proximal colon. Overall tumor incidence and cell proliferation were more affected by dietary fat. But the effect of dietary fiber was different depending on type of fat in the experimental diet. These results suggest that a DHA -rich fish oil may has more decisive effect in inhibiting the cell proliferation in colon.

Ergonomic Evaluation of Trunk-Forearm Support Type Chair

  • Objective: The aim of this study is to investigate the effects of trunk-forearm supported sitting on trunk flexion angle, trunk extensor fatigue and seat contact pressure. Background: The relationship between sitting posture and musculoskeletal disorders of the trunk extensor fatigue and seat contact pressure has been documented. The trunk-forearm support type ergonomic chair was devised from the fact that trunk-forearm support has been reported to reduce trunk extensor activity and discomfort. Method: Using three different sitting postures, upright ($P_1$), trunk-forearm supported ($P_2$) and normal sitting ($P_3$), six healthy subjects participated in the study. Motion capture system was used to collect head and trunk flexion angle, and surface electromyography (sEMG) was used to collect myoelectric signal of upper trapezius, lower trapezius, erector spinae, multifidus, and pressure mat system was used to measure seat contact pressure. Results: When trunk and forearm were supported by the ergonomic chair, higher head flexion angle showed upright > trunk-forearm supported > normal in order, and muscle fatigue showed less than upright and normal sitting. Mean seat contact pressure decreased 19% than upright sitting. But muscle fatigue was not affected by each condition. Conclusion: Trunk-forearm supported sitting of the ergonomic chair showed positive effect in respect of trunk and head flexion angle, trunk extensor fatigue, seat contact pressure. To acquire comprehensive understanding of the effectiveness of the ergonomic chair, further studies such as anatomical effects from measurement of external applied loading effect to the body from interface pressure analysis are required. Application: The results of the publishing trend analysis might help physiological effects of trunk-forearm support type chair.

Documentational Study and Observation from the View of Hyungsang Medicine on Bangpungtongseong-san (방풍통성산(防風通聖散)의 문헌(文獻) 및 형상의학적(形象醫學的) 고찰(考察))

  • The following conclusions were obtained from the studies on Bangpuntongseongsan both from the documents and Hyungsang medicine. Bangpungtongseong-san was created by Yu Wan-so to relieve both interior and exterior of disease from the pathogenic fire, and it cures wind syndrome and dry syndrome. Bangpungtongseong-san is of light herbal combination and it works in the upper part of the body and is mainly applied to skin disease. Perspiration without harming the exterior and purgation without hurting the interior shows that it is not a severe prescription belonging to meditation therapy. It is mostly used for curing the disease of internal heat caused by over drinking and consuming heavy food, and it has special relationship with Yangmyung meridian. It is mentioned in the chapters of spirit, head, face, eye, ear, nose, throat, skin, hair, prescription, wind, dryness, fire, internal damage, epidemic infectious disease, carbuncle and cellulitis, ulceration, and pediatrics of ${\ulcorner}$Donguibogam${\lrcorner}$. It is usually applied to those who belong to Yangmyung type of the six meridian types or wind type, who has excessive heat, people with red complexion, reddened nose, pimples over the face and nose, coarse heel, loss of hair due to wind-heat, and to those who tend to have dandruff. Through examination over the cases treated with Hyungsan medicine, Bangpungtongseong-san was found efficacious in bloodshot eyes, brandy nose, loss of hair, various skin problems, tetanus, acute alcoholism, paralysis of hand and foot, deafness, and tinnitus.

Effects of Alfalfa and Brown Mid-rib Corn Silage and Level of Forage Neutral Detergent Fiber on Animal Performance of Lactating Cows in Michigan

  • Alfalfa silage and corn silage are the major dairy feeds in most dairy operations in Michigan, USA. In recent years, the need to improve digestible fiber and dry matter intake of forages to meet the nutrient requirements of high yielding dairy cows and the willingness to plant corn specifically for silage has led plant breeders to focus on the brown mid-rib (BMR) trait. The objective of this study was to investigate the effect of different ratio of alfalfa to BMR corn silage and ration level of forage neutral detergent fiber (NDF) on animal performance of lactating cows in the Upper Peninsula of Michigan. This study was conducted at the Upper Peninsula Experiment Station of Michigan State University in Chatham, Michigan, USA. Two different ratios of forage type (high alfalfa silage/low BMR corn silage, AS, and high BMR corn silage/low alfalfa silage, BMRCS) and two different dietary NDF contents (27% NDF, 27 = low forage/high grain feeding, and 33% NDF, 33 = high forage/low grain feeding) were used. The experimental design was a $4{\times}4$ Latin Square with 20 milking cows (12 multiparous and 8 primiparous). This trial had four 21-day periods with 14 d adaptation and 7 d data collection. Milk yield and body condition score (BCS) on the AS-27, BMRCS-27 and BMRCS-33 treatments were significantly (p<0.05) higher than on the AS-33 treatment. Dry matter intake of the AS-27 and BMRCS-27 treatments was significantly (p<0.05) higher than for the AS-33 and BMRCS-33 treatments. Milk urea nitrogen (MUN) on the AS-33 treatment was significantly (p<0.05) higher than on the other diet treatments. A key finding of this study was that the BMRCS-33 (higher amounts of brown mid-rib corn silage than alfalfa silage, high forage and low grain feeding diet at 33% NDF) led to the equal highest milk production whilst having the equal lowest dry matter intake. This study demonstrated that the diet with higher ratio of highly digestible NDF forage such as brown mid-rib corn silage to alfalfa silage could lower grain feeding in the ration.

Comparison of Adult Brassier between Korea and Japan - Based on the Information on the Internet -

  • A brassier supports and protects breasts and makes a better shape of the upper half of the body through shaping breasts. A brassier, therefore, is recognized as the key underwear for female. Recently, the distribution structure and channels of the brassiere industry is diversified from conventional type of markets to department stores, convenience stores, and internet shopping mall. Studies on the sales of brassieres via internet, however, is not sufficient even though the market size has been dramatically increased thursdays. The study on the size structure (including the size and the materials fabric of brassieres circulating via internet) is especially rare. Therefore, this study tries to comply with increasing requests of consumers through comparing brassier brands on internet. In depth, this study compares cases of Korea and Japan in terms of availability of website on sales, quality indication like materials and functions, and size. The results indicated that there were three companies in Korea which run a website and make a sale through a website. All three Japanese companies operate a website and make sales on a website as well. In terms of size, Korean companies diversify their size of products in two ways. It varies from A cup to D cup based on cup size, and 65 to 100 according to its entire size. On the other hand, Japanese companies use AA, A to I to measure cup size, and 65 to 100 for entire size.
