급성기 의료기관의 일혈 발생 현황 및 임상간호사의 일혈 예방 및 관리에 대한 지식과 태도에 관한 연구 (Incidence of Extravasation in Acute Care Hospitals and Nurses' Knowledge and Attitude toward Extravasation Prevention and Management)

  • Purpose: Extravasation of diagnostic and therapeutic materials might occur when the intravascular solution leaks into the surrounding tissues. Injury associated with extravasation depends on various factors. It may range from mild skin reaction to severe necrosis. However, the incidence rate for extravasation is largely unknown because of the limited reporting in Korea. Therefore, this study was conducted to identify the incidence of extravasation and nurses' attitude and knowledge of extravasation for providing high-quality nursing care. Methods: Three acute care hospitals were surveyed to estimate the occurrence of extravasation. Knowledge and attitude toward extravasation were investigated from 793 nurses working in six hospitals. Results: The incidence rate of extravasation was 0.5%. Extravasation commonly occurred in elderly patients aged 66 or older (59.9%) and internal medicine (48.2%), and it happened 13.73±20.68 days after hospitalization on average. It mostly occurred in the forearm site (52.9%) and was mainly caused by parenteral nutrition (33.6%). The mean scores of nurses' knowledge and attitude were 14.63±2.86 and 28.91±36.00, respectively. There was a significant negative correlation between the subjects' knowledge and attitude (r=-.11, p=.002). Conclusion: It is necessary to have a reporting system that can accurately monitor the occurrence of extravasation for patient safety management. In addition, it is necessary to develop a protocol that can be applied to clinical practice and a nurse education program.

병원내 안전관리 향상을 위한 항목 및 지침 선정 (Selecting QA Items & Guidelines for Hospital Safety Management)

  • Background : The goal of this study was to establish the QA items and guidelines for preventing and improving of safety management. Therefore we investigated the nurses' recognition and knowledge of the safety and risk procedures and policies, and the agreement between the nurses beliefs on the degree of importance of those procedures and policies, with actual implementation in hospitals. Method : The subjects of this study were 201 nurses who participated in a program called continuing education for nurses, which held in December, 1993. Result: The results of this study were as follows: 1. Among 18 types of hospital risks, the items that scored highest or the need of closer attention in safety management was the needle stick, medication errors, falling, and bed sores. 2. In most questions of the 18 incidences, the nurses showed that the estimated result would have positive signs except for hospital infections, burns, and bed sores. 3. Even though the survey shows that incidences and types of occurences varies according to the person's age and the time of incident, they mostly occur between midnight to 6AM. Falls and bed sores can be seen more in the elderly. Medications errors, hospital infections and burns are frequently found between the ages of one through twenty. 4. There was a higher mean score for recognizing the importance of those items than the importance of implementing them. Conclusion : In summary, nurses did perceive the need of safety management but the hospital policy for proper safety management was not established. So we recommended that the hospital administration would undertake an early detection and proper management system for hospital precautions, based on QA items & guidelines presented in this study.

간호사 노동시장의 수요독점에 대한 연구 - 종합병원을 중심으로 - (Monopsony Power of General Hospitals in Nurse Labor Market)

  • Nurses are medical personnel, who play a key role in supporting patient care, so it is important to supply them adequately in balance with ever increasing medical demand. But there appears severe shortage of nurses in some hospitals because of their uneven distribution, especially in small sized-hospitals and rural-hospitals. As nationwide distorted distribution of nurses in Korea is just like what monopsony model(a kind of market structure model) tells us, it is attempted to explain this situation of nurse labor market in Korea on the basis of monopsony model and presented in this paper. Specifically, determinants of nurse wage and the level of their relative employment were examined, and monopsony impact on their wage and the level of relative employment controlling those determinants were studied. Major results of this study arc as follows. The most important determinant of nurse wage level in this study was the wage level of a local community where each hospital located Hospital owner's characteristics an educational function of each hospital were also important factors. With these factor controlled, it was found that monopsony power of each hospital was negativel associated with nurse wage level as expected. 1% increase in monopsony power of hospital(measured by Herfindah-Hirschman Index) reduced nurse wage by $5,674{\sim}19,19$ won(in Korean currency). With regard to the level of relative employment, the most important determinant wa the capacity for supplying nurses of the local community. Again, hospital owner characteristics and educational function of each hospital were also important. With these factors controlled, it was found that monopsony power of each hospital was negative associated with the number of nurses per bed, as expected. 1% increase in monopsony power of each hospital(again measured by Herfindah-Hirschman Index) reduced the number of nurses per 100 bed as much as $0.46{\sim}0.67$. In conclusion. structural factors of nurse labor market influence the instability of nurse labor supply in Korea. Further consideration for these market structural characteristics needed for policy making related to nurse resource allocation.

2016년도 국내 150병상 이상 의료기관의 감염관리간호사 현황 및 감염관리 활동 영향 요인 (The Status of Infection Control Nurses and Factors Affecting Infection Control Activities in Healthcare Facilities with more than 150 Beds in 2016 in KOREA)

  • Purpose: The purpose of this study was to describe the status of infection control nurses (ICNs) and their activities, and to identify the factors affecting the level of infection control activities. Methods: Data were collected from 199 hospitals from June 24 to July 26, 2016. The structured questionnaires included status of infection control nurses, type and level of infection control activities. Results: Most participating hospital were advanced general hospital (20.1%) and general hospital (67.8%). Among the hospitals, 86.4% had an infection control department (ICD). The average hospital work experience of ICNs was 14.62 years, and their average infection control career was for 4.94 years. Among the ICNs, 85.6% worked in full time and the average number of beds per ICN was 311.21. There were significant differences in the existence of ICD, infection control activities including surveillance, outbreak investigation, negative pressure room, hand hygiene monitoring, disinfection, and sterilization according to hospital size. The level of infection control activities was higher with more number of ICNs, ICN employment as full time, and healthcare institution accreditation status. The explanatory power was 37.5%. Conclusion: These results of this study which reflect infection control status of healthcare facilities with more than 150 beds in 2016 will provide baseline data to establish infection control system in small to medium sized hospitals after the Middle East Respiratory Syndrome outbreak in 2015.

병원간호사의 욕창간호 지식수준 (Knowledge Level of Pressure Ulcer Among Hospital Nurses)

  • The purpose of this study was to evaluate the knowledge level of pressure ulcer among hospital nurses through a cross-sectional survey by using the pressure ulcer knowledge assessment tool of Beitz et al.(1998). The total of subjects was 160 voluntary participants (60 were from Hospital A and 100 were from Hospital B) working at adult patients' units in two university hospitals located in the same city. None of the hospitals had expert nurses of pressure ulcer nor provided a regular pressure ulcer education program during the past one year. The survey tool consisted of 32 true-false items which were grouped into the risk factors knowledge category(13 items), the wound assessment knowledge category(4 items), and the treatment methods knowledge category(15 items). An internal consistency reliability test of the tool yielded an overall coefficient of 0.72; the coefficient for the risk factors knowledge category was 0.40, that for the wound assessment knowledge category was 0.33; that for the treatment knowledge category was 0.54. The main findings of the study are as follows. 1) Demographic characteristics of the two hospital nurses were similar except for the educational level(p=0.029) and the work units(p=0.001). These observations were maintained even if Hospital A and B were separately compared. 2) The knowledge level of the subjects about pressure ulcer in general was low as indicated by 53.3 points(of 100 possible points) on average. The knowledge level about treatment methods of pressure ulcer was the lowest as indicated by 47.0 points on average. 3) No correlation between the knowledge level of the pressure ulcer and the demographic characteristics of nurses was found except that the knowledge level of I.C.U. nurses was significantly higher(p=0.0003) than that of nurses in other units(p=0.2926) in the case of Hospital A. 4) The knowledge level of nurses in Hospital B was higher than for nurses in Hospital A. The reason was not identified, but it seems that it is related to the role of the I.C.U. or some other factors. The study results confirmed the existing literature that knowledge level of nurses about pressure ulcer is low regardless of age, educational level, or work experience. However, the working place(unit) affected the knowledge level. Further research on the exact reason for the differences in the knowledge level is needed in the future.

한방병원에 근무하는 간호사의 정신건강에 대한 연구동향: 국내연구를 중심으로 (Scoping Review of Research on Mental Health of Nurses Working in Korean Medicine Hospitals)

  • Objectives: The aim of this scoping review was to analyze research trends about mental health of nurses working in Korean medicine (KM) hospitals in Republic of Korea. Methods: Searches were conducted using four electronic databases including Oriental Medicine Advanced Searching Integrated System, Korean Studies Information Service System, Research Information Sharing Service, Korea Citation Index to collect relevant studies. The search date was March 4, 2021. All studies published up to the search date were considered. Observational studies reporting mental health outcomes of nurses working at KM Hospital were included. Results: A total of 11 cross-sectional observation studies were included. Four of them compared mental health of nurses working on KM and Western medicine (WM). Commonly reported outcomes related to mental health were job satisfaction, role conflict, and work stress. As a result of meta-analysis based on a 5-point Likert scale, the following factors were related to mental health of KM nurses: job satisfaction (2.844±0.067 points), role conflict (3.678±0.058 points), work stress (3.142±0.021 points), turnover intention (3.483±0.028 points), and burnout (3.180±0.033). Compared to WM nurses, KM nurses had significantly less work stress (p=0.000), role conflict (p=0.039), and job satisfaction (p=0.000). Conclusions: Mental health problems of nurses are known to be very common. Although improving them is an important social task, studies on mental health of KM nurses remain insufficient. Based on findings of this study, more cooperation between nurses and KM doctors should be made to improve the mental health of KM nurses, especially their job satisfaction in the future. The Korean Society of Oriental Neuropsychiatry needs to pay more attention to this topic.

인증 한방병원과 비인증 한방병원 간호사의 감염예방 표준주의지침 수행도의 차이 비교 (A Comparison of Compliance with Standard Precautions for Infection Prevention between Nurses at Accredited Korean Medicine Hospitals and Non-accredited Korean Medicine Hospitals)

  • Purpose: The purpose of this study was to compare compliance with standard precautions of infection prevention between nurses at accredited Korean medicine hospitals and non-accredited Korean medicine hospitals. Methods: Data were collected from a total of 138 participants (69 nurses from 3 accredited hospitals and 69 nurses from 3 non-accredited hospitals) in January of 2021 using structured questionnaires. Descriptive statistics, t-test, one-way ANOVA, and multiple regression analyses were carried using the SPSS Statistics 24.0 Program. Results: Results showed that the scores of nurses' compliance with standard precautions of infection prevention at accredited Korean medicine hospital (40.54±2.74) were significantly higher (p=.002) than the scores of nurses at the non-accredited Korean medicine hospitals (38.94±3.28). After controlling for covariates, the results were same. In addition, we found that scores of compliance with standard precaution for infection prevention in nurses at hospitals belong to university were significantly high compared to those of nurses at private hospitals. We also found that the scores of compliance with standard precaution for infection prevention in nurses with more than 5 years of experience were significantly higher than those of nurses with less than 3 years of experience (p=.039). Conclusion: Nurses working at the accredited Korean medicine hospitals showed higher scores of compliance with standard precaution for infection prevention. Therefore, it is suggested that the participation in the hospital accreditation program should be encouraged for the Korean medicine hospitals.

종합병원 간호사의 다제내성균 감염관리 수행도에 영향을 미치는 요인 (Factors Influencing Performance of MultiDrug-Resistant Organisms Infection Control in Nurses of General Hospital)

  • Purpose: The purpose of this study was to identify factors influencing performance of MultiDrug-Resistant Organisms (MDROs) infection control by nurses in general hospitals. Methods: The research design was a descriptive survey design using convenience sampling. Data were collected from 130 nurses working in 6 general hospitals. Collected data were analyzed using SPSS/WIN 21.0 program for descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and multiple regression analysis. Results: General hospital nurses' MDROs infection control performance was influenced by their awareness of environmental safety, recognition of MDROs infection control, number of beds in the hospital, whether nurses had nursing experience with infection control and guidelines for MDROs infection control. The most important predictors of MDROs infection control performance were awareness of environmental safety and recognition of MDROs infection control. Conclusion: Findings indicate that it is necessary to include content related to awareness of environmental safety and recognition of infection control in developing MDROs infection control education programs for general hospital nurses.

병원간호사의 정기적인 유방암 검진행위 영향 요인 (Factors Affecting Periodic Screening Behaviors for Breast Cancer among Hospital Nurses)

  • Purpose: The purpose of this study was to analyze periodic screening behaviors for breast cancer and factors affecting the screening behaviors among hospital nurses. Methods: A total of 461 nurses were recruited from 15 hospitals located in two southern areas of Korea. The Champion's Health Beliefs Model Scale-Korean version and a structured study questionnaire were used for data collection. Data were collected during July and August 2008. Results: Nurses who performed periodic mammography accounted for 15.6% of the total, while 22.8% performed periodic breast self-examination (BSE). Among married nurses, the rates of periodic mammography and BSE were significantly different by age, menopausal status, delivery experience, family history of breast cancer, and experiences of cervical cancer screening. Significant factors affecting periodic mammography were family history of breast cancer, experiences of cervical cancer screening, age above 40, and performance of periodic BSE. For health beliefs, levels of severity, confidentiality and barrier were significant factors for periodic BSE. Conclusion: Nurses, who are role models for health promoting behaviors, did not adequately performing periodic BSE and mammography. A health promotion program for breast cancer designed for hospital nurses is needed to improve performance rates for periodic screening behaviors for breast cancer.

신규간호사의 직무 스트레스, 자아존중감, 회복탄력성이 현장적응에 미치는 영향 (The Influence of Job Stress, Self-Esteem, and Resilience on the Field Adaptation in Newly Graduated Nurses)

  • Purpose: This study aims to determine the relation among job stress, self-esteem, resilience, and field adaptation of the newly graduated nurses, through which an effective mediation strategy toward field adaptation and basic data on improving nursing quality can be established. Methods: The subjects were 110 newly graduated nurses who have worked at a tertiary hospital and a general hospital for less than 12 months. The data were collected, using self-reported questionnaires, from July 9 to July 31, 2018, and were analyzed by descriptive statistics, t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficient and stepwise multiple regression using an SPSS/WIN 24.0 program. Results: The study result showed that the newly graduated nurses' job stress, resilience, job satisfaction, and desired department influenced their field adaptation, explaining 60.2% of the variance in the field adaptation (F=42.14, p<.001). The nurses' job stress had the biggest influence on their field adaptation, explaining 38.7% of the variance. Conclusion: Human resource management strategy considering those influencing factors may improve the filed adaptation of newly graduated nurses and help them to retain in their work field.
